Abstract

Abstract Background Overactive bladder (OAB) syndrome is a symptom complex affecting 12–14% of the UK adult female population. Symptoms include urinary urgency, with or without urgency incontinence, increased daytime urinary frequency and nocturia. OAB has a negative impact on women’s social, physical, and psychological wellbeing. Initial treatment includes lifestyle modifications, bladder retraining, pelvic floor exercises and pharmacological therapy. However, these measures are unsuccessful in 25–40% of women (refractory OAB). Before considering invasive treatments, such as Botulinum toxin injection or sacral neuromodulation, most guidelines recommend urodynamics to confirm diagnosis of detrusor overactivity (DO). However, urodynamics may fail to show evidence of DO in up to 45% of cases, hence the need to evaluate its effectiveness and cost-effectiveness. FUTURE (Female Urgency, Trial of Urodynamics as Routine Evaluation) aims to test the hypothesis that, in women with refractory OAB, urodynamics and comprehensive clinical assessment is associated with superior patient-reported outcomes following treatment and is more cost-effective, compared to comprehensive clinical assessment only. Methods FUTURE is a pragmatic, multi-centre, superiority randomised controlled trial. Women aged ≥ 18 years with refractory OAB or urgency predominant mixed urinary incontinence, and who have failed/not tolerated conservative and medical treatment, are considered for trial entry. We aim to recruit 1096 women from approximately 60 secondary/tertiary care hospitals across the UK. All consenting women will complete questionnaires at baseline, 3 months, 6 months and 15 months post-randomisation. The primary outcome is participant-reported success at 15 months post-randomisation measured using the Patient Global Impression of Improvement. The primary economic outcome is incremental cost per quality-adjusted life year gained at 15 months. The secondary outcomes include adverse events, impact on other urinary symptoms and health-related quality of life. Qualitative interviews with participants and clinicians and a health economic evaluation will also be conducted. The statistical analysis of the primary outcome will be by intention-to-treat. Results will be presented as estimates and 95% CIs. Discussion The FUTURE study will inform patients, clinicians and policy makers whether routine urodynamics improves treatment outcomes in women with refractory OAB and whether it is cost-effective. Abstract

Staphylococcus aureus (S. aureus) is a challenging bacterial pathogen which can cause a range of diseases, from mild skin infections, to more serious and invasive disease including deep or organ space surgical site infections, life-threatening bacteremia, and sepsis. S. aureus rapidly develops resistance to antibiotic treatments. Despite current infection control measures, the burden of disease remains high. The most advanced vaccine in clinical development is a 4 antigen S. aureus vaccine (SA4Ag) candidate that is being evaluated in a phase 2b/3 efficacy study in patients undergoing elective spinal fusion surgery (STaphylococcus aureus suRgical Inpatient Vaccine Efficacy [STRIVE]). SA4Ag has been shown in early phase clinical trials to be generally safe and well tolerated, and to induce high levels of bactericidal antibodies in healthy adults. In this review we discuss the design of SA4Ag, as well as the proposed clinical development plan supporting licensure of SA4Ag for the prevention of invasive disease caused by S. aureus in elective orthopedic surgical populations. We also explore the rationale for the generalizability of the results of the STRIVE efficacy study (patients undergoing elective open posterior multilevel instrumented spinal fusion surgery) to a broad elective orthopedic surgery population due to the common pathophysiology of invasive S. aureus disease and commonalties of patient and procedural risk factors for developing postoperative S. aureus surgical site infections.

4. Encapsulated and ‘free’ pig islet xenotransplantation: recent experience and clinical progress

Abstract

Genetically-engineered pigs offer a possible alternative to deceased human donors as a source of isolated islets for transplantation into patients with life-threatening diabetes. We here consider the advantages and disadvantages of ‘free’ pig islet transplantation into immunosuppressed recipients vs. ‘immunoisolated’ pig islet transplantation into non-immunosuppressed recipients. Although hurdles to successful free pig islet transplantation remain, e.g., the instant blood-mediated reaction (IBMIR) and the immune response, we are optimistic that, as new genetically-engineered pigs become available, the remaining barriers may be overcome. In contrast, we have several concerns with regard to the ultimate success of immunoisolation. Without exogenous immunosuppressive therapy, we suggest that immune injury will occur. If immunosuppressive therapy is required, the primary advantage of encapsulation is lost. A key point is that the lack of adequate nutrition and oxygen to the encapsulated islets has not yet been overcome. Furthermore, an optimal site for the placement of the islets has also not been determined. We also very briefly review several other points of importance to islet xenotransplantation, namely (i) Human leukocyte antigens/Swine leucocyte antigens sensitization, (ii) physiological aspects of pig islet xenotransplantation, (iii) the safety of islet xenotransplantation, and (iv) what will be required to initiate a clinical trial.

Abstract

Abstract In utero hematopoietic cell transplantation (IUHCT) utilizes fetal immune tolerance to achieve durable chimerism without conditioning or immunosuppression during a unique window in fetal development. Though donor cells have been observed within the nervous system following in utero injection, the timeline and distribution of cellular trafficking across the blood-brain barrier following IUHCT is not well understood. We injected 20 × 106 adult bone marrow mononuclear cells intravenously at gestational age (GA) 12–17 days and found that donor cells were maximally concentrated in the brain with treatment between GA 13–14. Donor cell engraftment persisted within the brain at every timepoint analyzed and concentrated within the hindbrain with significantly more grafted cells than in the forebrain. Additionally, transplanted cells terminally differentiated into various nervous system cellular morphologies and also populated the enteric nervous system. This study is the first to document the timeline and distribution of donor cell trafficking into the immune-protected nervous system and serves as a foundation for the application of IUHCT to treat neurogenetic diseases.

Abstract

Abstract Background This study examines the impact of SARS-CoV-2 (i.e., coronavirus, COVID, COVID-19) using data from a measurement-based care (MBC) system utilized in an outpatient psychiatric clinic providing telemedicine care. A novel Patient Rated Outcome Measure (PROM), the COVID-19 Events Checklist (CEC) was administered in a hospital system based ambulatory clinic beginning April 2020 to track COVID-19-19’s impact on patients’ mental, emotional, and health-related behaviors during the pandemic. The study (1) provides descriptive CEC data, and (2) compares CEC results with PROMs evaluating anxiety (Generalized Anxiety Disorder-7; GAD-7), depression (Patient Health Questionnaire; PHQ-9), and psychological distress (Brief Adjustment Scale-6; BASE-6). Methods This retrospective observational study included patient intake data collected from April 2020 to March 2021. Patient (N = 842) reports on the CEC’s five domain questions were aggregated to calculate average reports of COVID-19 related impacts at intake over the initial 12 months of the pandemic. Trends in COVID-19 related impacts were examined, and non-aggregated scores on the PHQ-9, GAD-7, and BASE-6 were compared to primary dichotomous (yes/no) CEC survey questions via Wilcoxon rand sum testing. Results Results capture the relationship between COVID-19 exposure, COVID-19- related sequelae and behaviors, and psychological symptom severity. Specifically, Wilcoxon rank-sum tests indicate that social determinants of health (SDOH), negative mental health impacts, and positive coping skill use were significantly associated with psychological symptomatology including overall psychological functioning via the BASE-6, anxiety via the GAD-7, and depressive symptoms via the PHQ-9. A global heat flux model based on a fractional derivative of plasma pressure is proposed for the heat transport in fusion plasmas. Abstract

Summary: Background: Post-traumatic stress disorder (PTSD) and depression are common after mild traumatic brain injury (mTBI), but their biological drivers are uncertain. We therefore explored whether polygenic risk scores (PRS) derived for PTSD and major depressive disorder (MDD) are associated with the development of cognate TBI-related phenotypes. Methods: Meta-analyses were conducted using data from two multicenter, prospective observational cohort studies of patients with mTBI: the CENTER-TBI study (ClinicalTrials.gov ID NCT02210221) in Europe (December 2014–December 2017) and the TRACK-TBI study in the US (March 2014–July 2018). In both cohorts, the most common causes of injury were road traffic accidents and falls. Primary outcomes, specifically probable PTSD and depression, were defined at 6 months post-injury using scores ≥33 on the PTSD Checklist-5 and ≥15 on the Patient Health Questionnaire-9, respectively. We calculated PTSD-PRS and MDD-PRS for patients aged ≥17 years who had a Glasgow Coma Scale score of 13–15 upon hospital arrival and assessed their association with PTSD and depression following TBI. We also evaluated the transferability of the findings in a cohort of African Americans. Findings: Overall, 11.8% (219/1869) and 6.7% (124/1869) patients were classified as having probable PTSD and depression, respectively. Abstract

Summary: Background: Nuclear factor kappa B (NF-κB) c-Rel is a psoriasis susceptibility locus, however mechanisms underlying c-Rel transactivation during disease are poorly understood. Inflammation in psoriasis can be triggered following Toll-like Receptor 7 (TLR7) signalling in dendritic cells (DCs), and c-Rel is a critical regulator of DC function. Here, we studied the mechanism of TLR7-induced c-Rel-mediated inflammation in DCs. Methods: The overall expression of c-Rel was analysed in skin sections from patients with psoriasis in human transcriptomics datasets as well as the imiquimod-induced psoriasis mouse model. The function of c-Rel in DCs following TLR7 stimulation was determined by c-Rel CRISPR/Cas9 knockout DC2.4 immortalised cells and primary bone marrow derived dendritic cells from c-Rel knockout C57BL6/J mice. Findings: c-Rel is highly expressed in lesional skin of patients with psoriasis and TLR7-induced psoriatic lesions in mice. c-Rel deficiency protected mice from the disease, and specifically compromised TLR7-induced, and not TLR9- or TLR3-induced, inflammation in dendritic cells. Mechanistically, c-Rel deficiency disrupted activating NF-κB dimers and allowed binding of inhibitory NF-κB homodimers to the IL-1β and IL-6 promoters thus inhibiting their expression. This functionally compromises the ability of c-Rel deficient DCs to induce Th17 polarisation, which is critical in psoriasis pathogenesis. Interpretation: Our findings reveal that c-Rel is a key regulator of TLR7-mediated dendritic cell-dependent inflammation, and that targeting c-Rel-dependent signalling could prove an effective strategy to dampen excessive inflammation in TLR7-related skin inflammation. Assisted reproductive techniques might prove themselves useful tools in producing buffaloes free of specific diseases (BFSD), which are in demand in South Africa. Freezing protocols for African buffalo semen must not only result in good post-thaw qualities, but must also be practical. Epididymal sperm from six mature African buffalo bulls was collected, diluted with three different semen extenders and frozen. Pre-freezing equilibration times between 2 and 9 h were tested. Total and progressive motility, longevity and acrosomal integrity were measured and compared. The use of TriladylTM proved to result in better post-thaw parameters than the other two diluents. Equilibration times of between 4 and 9 h did not influence post-thaw sperm qualities significantly. For some of the treatments, exposure to semen extenders before freezing for less than 4 h resulted in inferior post-thaw semen parameters.

En comparaison à d'autres types de traitements d'eaux usées domestiques, les filtres plantés de roseaux à écoulement horizontal exigent une exploitation simple et peu importante. C'est particulièrement le cas en traitement tertiaire, où l'exploitation ne demande que quelques jours par an, par comparaison au traitement secondaire d'effluents ayant subi une simple décantation. Malheureusement, en pratique, cela se traduit souvent par une absence complète d'attention. Les filtres pour le traitement tertiaire ne sont pourtant pas du type installé-oublié, même s'ils sont souvent traités de cette façon et considérés comme trop tolérants. Après un certain nombre d'années, quelques-uns de ces filtres tertiaires sont détériorés à tel point qu'ils ne sont pus aptes à répondre aux objectifs réglementaires. Severn Trent Water Ltd a pris conscience de cette situation et a prévu un budget pour un programme de maintenance des filtres plantés de roseaux. Ce travail a démarré par une enquête sur l'état des filtres, particulièrement pour déterminer les problèmes les plus courants et de là le régime d'exploitation nécessaire. Cinq principaux problèmes reviennent individuellement ou en conjonction : a) accumulation de boues ; b) ruissellements superficiels (partiellement causés par a), c), et d) ; c) dysfonctionnement/colmatage des répartiteurs de flux en entrée ; d) problèmes dans les dispositifs de collecte en sortie/mauvais calages en hauteur et e) infestation de plantes indésirables/parasites.

38. Routine Disruption Predicts Stress During the 2020 COVID-19 Lockdowns in the U.S

Abstract

In early 2020, governments around the world imposed various non-pharmaceutical interventions (NPIs) to slow the spread of COVID-19. NPIs like stay-at-home (or “lockdown”) orders were especially effective in mitigating virus transmission but could also significantly disrupt people's usual activities. Given the hypothesized relationship between routine disruption and stress (e.g., Hou, Lai, Ben-Ezra, & Goodwin, 2020), we predicted that the amount of daily-life disruption people experienced during lockdowns would significantly predict their stress levels. To test this hypothesis, we collected data from over 300 adults living across the United States via an online survey. Analyses revealed that lockdowns did disrupt participants’ daily routines, that most participants experienced several stress symptoms during lockdowns, and that overall life disruption strongly correlated with stress scores (r = .50). Subsequent multiple linear regression analyses revealed that changes in just 4-5 activities were driving this relationship—exercising, relaxing, using social media, eating, and volunteering—and that disruptions to these behavioral routines predicted between 24-32% of the variance in participants’ stress symptoms. Overall, our results showed that many disruptions in daily routines correlated with stress during lockdowns but that some activity changes were more strongly related to stress than others.

Abstract

BackgroundIn order to identify an inexpensive yet highly stable SARS-CoV-2 collection device as an alternative to foam swabs stored in transport media, both contrived ("surrogate") CoV-positive and patient-collected spun polyester swabs stored in dry tubes were evaluated for time- and temperature-stability using qPCR.MethodsSurrogate specimens were prepared by combining multiple, residual SARS-CoV-2-positive clinical specimens and diluting to near-LOD levels in either porcine or human mucus ("matrix"), inoculating foam or polyester nasal swabs, and sealing in dry tubes. Swabs were then subjected to one of three temperature excursions: (1) 4°C for up to 72 hours; (2) 40°C for 12 hours, followed by 32°C for up to 60 hours; or (3) multiple freeze-thaw cycles (-20°C). The stability of extracted SARS-CoV-2 RNA for each condition was evaluated by qPCR. Separate usability studies for the dry polyester swab-based HealthPulse@home COVID-19 Specimen Collection Kit were later conducted in both adult and pediatric populations.ResultsPolyester swabs stored dry demonstrated equivalent performance to foam swabs for detection of low and moderate SARS-CoV-2 viral loads. Mimicking warm- and cold- climate shipment, surrogate specimens were stable following either 72 hours of a high-temperature excursion or two freeze-thaw cycles. In addition, usability studies comprised of self-collected patient specimens yielded sufficient material for molecular testing, as demonstrated by RNase P detection.ConclusionsPolyester nasal swabs stored in dry collection tubes offer a robust and inexpensive self-collection method for SARS-CoV-2 viral load testing, as viral RNA remains stable under conditions required for home collection and shipment to the laboratory.

49. Working from home during lockdown: the impact on performance and wellbeing

Abstract

The experience of working from home changed drastically with the arrival of COVID-19. Compared to pre-pandemic experiences, key differences included the vast number of people involved, its involuntary nature, the suddenness of its implementation, its lengthy duration, and the presence of others at home. The demands of this form of remote work during lockdown have partly been mitigated by the resources employees have accessed. This study aimed to investigate the factors impacting employee performance and wellbeing while compulsorily working from home during New Zealand's first nationwide lockdown. We analyzed qualitative data gathered from employees in two organizations. The resulting aggregate dimensions across both demands and resources include organizational factors, furniture and technology factors, and individual factors. Given the ongoing nature of COVID-19 we identify new research directions for investigating remote work, and practical implications focusing on suitable home furniture and technology, plans for future remote work, and supporting employees.

50. Changements observés au Canada dans la consommation de cannabis à des fins médicales pendant la pandémie de COVID-19

Abstract

Introduction La pandémie de COVID-19 a eu des effets négatifs généralisés sur la santé, notamment une perte de sécurité matérielle et une exacerbation des maladies mentales chez les populations à risque. Bien que l’on ait observé une augmentation des cas d’usage non médical de certaines substances, dont le cannabis, dans des échantillons de la population canadienne, aucune étude n’a porté sur les changements dans la consommation de cannabis à des fins médicales au Canada pendant la pandémie de COVID. Méthodologie Les données ont été tirées de l’Enquête canadienne sur le cannabis de 2021, une enquête en ligne réalisée en mai 2021 auprès de personnes autorisées à utiliser du cannabis à des fins médicales et qui ont été recrutées par le biais de l’un des deux producteurs de cannabis à usage médical autorisés au Canada. Nous avons eu recours à des tests de McNemar pour comparer la fréquence de consommation de cannabis à des fins médicales au cours des trois mois ayant précédé la pandémie et la fréquence de consommation de cannabis à des fins médicales pendant la pandémie. Nous avons exploré les corrélats d’une augmentation de fréquence de consommation de cannabis depuis le début de la pandémie dans des modèles logistiques à deux variables et à variables multiples. Résultats Au total, 2 697 répondants (49,1 % de femmes) ont répondu à l’enquête. La consommation quotidienne de cannabis à des fins médicales a augmenté légèrement, mais de façon statistiquement significative, entre la période prépandémique (83,2 %) et la période pandémique (90,3 % au moment de l’enquête; p < 0,001). Les facteurs associés de manière statistiquement significative à l’augmentation de la fréquence de consommation de cannabis à des fins médicales sont notamment le genre féminin, un âge plus jeune, une perte d’emploi liée à la pandémie, la consommation de cannabis principalement pour la gestion de sa santé mentale, l’usage de médicaments sur ordonnance et la consommation de cannabis à des fins non médicales (p < 0,05). Conclusion Depuis le début de la pandémie de COVID-19, il y a eu une tendance légèrement à la hausse de la fréquence de consommation de cannabis à des fins médicales. Comme on ignore quels sont les effets à court et à long terme de la consommation de cannabis sur la détresse mentale en lien avec une pandémie, les cliniciens dont des patients utilisent du cannabis à des fins médicales devraient être à l’affût d’éventuels changements d’habitudes de consommation durant la pandémie.
