Zusammenfassung Hintergrund Die offene Varizenchirurgie kann sowohl in einem ambulanten als auch in einem stationären Setting durchgeführt werden. Aktuelle Leitlinien zur Therapie der Varikose nehmen zu der Frage, welche Patienten ambulant und welche stationäre zu operieren sind, nicht Stellung. Fragestellung Erarbeitung von Kriterienkatalogen zur Frage einer ambulanten oder stationären Varizenoperation. Material und Methode PubMed-Recherche zur Fragestellung und basierend auf den Literaturdaten Erarbeitung eines Delphi-Konsensus der Autoren. Ergebnisse Zur Frage, ob eine Varizenchirurgie ambulant oder stationär durchgeführt werden kann, existieren lediglich 2 monozentrische retrospektive Analysen sowie eine Registerstudie. In diesen Studien werden ambulante Operationen im Wesentlichen bei Patienten mit Normalgewicht und ASA-Stadium 1 durchgeführt. Gut geeignete Lokalbefunde für eine ambulante Operation sind Primäreingriffe und ein geringeres C-Stadium der CEAP-Klassifikation. Für eine stationäre Varizenoperation sprechen ausgedehnte Befunde, bei denen mehr als eine Crosse saniert werden muss, Rezidiveingriffe in der Leiste oder der Kniekehle, vorangegangene thromboembolische Ereignisse, eine erhöhte Blutungsneigung aufgrund monströser Varizen oder einer Antikoagulation, Hautkomplikationen wie ein Ulcus cruris oder eine schwere Stauungsdermatitis bzw. Dermatosklerose sowie ein BMI über 30 bei Crossektomie. Schlussfolgerungen Basierend auf Literaturdaten und Expertenmeinungen konnten Kriterienkataloge für eine ambulante versus stationäre offene Varizenchirurgie erarbeitet werden.

Summary: Background: Varicosis of the great saphenous vein (GSV) is a common disease. Most of the therapeutic concepts attempt to remove or destroy the truncal vein. However, the absence of the GSV could be harmful for further treatments of artherosclerotic disease as the GSV is often used as bypass graft in lower extremity or coronary artery revascularisations. External valvuloplasty (EV) is one of the vein-sparing treatment options. The aim of this clinical study was to describe the outcome, safety and complications of this procedure in a prospective multicentre trial. Patients and methods: The function of the terminal and preterminal valve was restored by external valvuloplasty. Furthermore, multiple phlebectomies of tributaries were performed. Patients were reinvestigated six weeks after surgery. Primary endpoint was the function of the external valvuloplasty measured by diameter of the GSV and the prevalence of reflux in the GSV. The eligibility of the vein as a potential bypass graft was noticed. CEAP class and VCSS scores were analysed. Results: A total of 359 patients were included in the study. After six weeks 297 patients could be reinvestigated. The function of the external valvuloplasty was sufficient in 284 patients (95.6%). Treatment failed in 8 patients (2.6%) due to an occlusion or junctional reflux despite valvuloplasty. The GSV was estimated as suitable as a bypass graft in 261 patients (87.8%). Memory T cells cause faster and more effective elimination of pathogens during reinfection by the rapid production of effector T cells. Worldwide, the leading cause of death among children and infants is infection, partly due to under-developed T cell memory populations commonly seen in infants. In the context of biomedical research, the specific pathogen-free (SPF) facilities research animals are housed in further the deficit of memory T cells, as it is known that adult mice housed in SPF facilities have few memory T cells, but that exposure to normal microbial experiences (NME) replenishes this population of cells. This phenomenon, though, has not been investigated in infants. Therefore, we aimed to investigate how NME influences infant memory T cell commitment to reveal novel insights into natural mammalian immune development. At various time points, the trajectory of infant immune maturation of age-matched SPF and NME (pups birthed and raised in an environment exposed to NME) mice were examined using flow cytometry. We observed significant increases in both the number and frequency of memory T cell subsets (CD8+ and CD4+ Tcm and Tem cells) at all time points in pups exposed to NME. We also observed increased serum concentrations of cytokines required for memory T cell differentiation and survival (IL-7 and IL-15) in NME pups. These data support the hypothesis that NME accelerates the maturation of the memory T cell compartment in infant mice. We predict this more mature memory T cell compartment may contribute to improved immunity to novel infections. Our ongoing experiments test this hypothesis using a Listeria monocytogenes challenge model.

Die offene und endovenose Chirurgie der Varikosis bietet eine ausgezeichnete Moglichkeit zur Sanierung der Varikosis bei den Patienten. Jedoch gibt es grose Unterschiede in der Durchfuhrung der Techniken. Egal welches Verfahren durchgefuhrt wird, es gibt Standards, die beachtet werden sollten. „State of the art“ der offenen Venenchirurgie mit radikaler Krossektomie ist hinreichend bekannt, wird aber leider heutzutage immer noch nicht uberall korrekt durchgefuhrt. „State of the art“ der endovenosen Venenchirurgie ist bisher leider noch nicht ausreichend dokumentiert, sollte sich aber an den offen chirurgischen Techniken orientieren. Wie der Standard bei beiden Verfahren heute ist (oder sein sollte), wird in dieser Arbeit beschrieben und detailliert erlautert. Eine Rezidivdefinition, die fur beide Techniken gilt, wird aufgestellt.

Objectives External valvuloplasty (eVP) is a reconstructive surgical method to repair the function of the terminal and preterminal valves. We evaluated the 6-month outcomes of eVP regarding the diameter of the great saphenous vein (GSV). Methods Patients from five vein centres were included in this observational study. Follow-up involved detailed duplex sonography of the GSV. The venous clinical severity score (VCSS) and the C class of the clinical, aetiologic, anatomic and pathophysiologic (CEAP) classification were recorded. Results We enrolled 210 patients, with a follow-up rate of 58%; eVP was sufficient in 95.24% of the patients. The GSV diameters decreased significantly from 4.4 mm (standard deviation (SD): 1.39) to 3.9 (SD: 1.12), 4 cm distal to the saphenofemoral junction (SFJ); from 3.7 mm (SD: 1.10) to 3.5 mm (SD: 1.02) at the mid-thigh; from 3.6 mm (SD: 1.14) to 3.3 mm (SD: 0.94) at the knee and from 3.1 mm (SD: 0.99) to 2.9 mm (SD: 0.78) at the mid-calf. VCSS decreased significantly from 4.76 (SD: 2.13) preoperatively to 1.77 (SD: 1.57) 6 months postoperatively. Conclusions GSV function can be restored by eVP; diameters over the total length of the GSV decreased significantly.

Soft nanoimprinted titanium dioxide (TiO2) substrates decorated with methylammonium lead halide perovskite (MAPbI3) crystals were fabricated by controlling the perovskite precursor concentration and volume during spin coat processing combined with the use of hydrophobic TiO2 templates. The patterned growth was demonstrated with different perovskite crystallization methods. We investigated and successfully demonstrated the controlled assembly of two MAPbI3 nanomaterials, one a nanocomposite formed between the perovskite and a hole conducting polymer poly(2,5-bis(N-methyl-N-hexylamino)phenylene vinylene) (BAMPPV), and a second formed from perovskite crystals using common solution based MAPbI3 growth methods (1-step and 2-step processing). Both types of MAPbI3 crystals were fabricated on hydrophobic TiO2 nanotemplates composed of nanowells or grating patterns. Patterned areas as large as 100 μm × 100 μm were achieved. We examined and characterized the substrates using atomic force microscopy, scanning electron microscopy, x-ray diffraction, and energy dispersive spectroscopy. We present the optical properties (i.e. fluorescence and transmission) of soft nanoimprinted nanowells decorated with perovskites demonstrating the successful synthesis of MAPbI3 perovskite nanocrystals. As an example of their use, we demonstrate a two terminal device and show photocurrent response of a perovskite patterned micro-grating. Our method is a nondestructive approach to nanopatterning perovskites, and produces patterned arrays that maintain their photo-electric properties. The results presented herein suggests an attractive route to developing nanopatterned and small area perovskite substrates for applications in photovoltaics, x-ray sensing/detection, image sensor arrays, and others.

Researchers at the Naval Air Warfare Center Weapons Division (NAWCWD) and Wright-Patterson Air Force Base (WPAFB) investigated poly[2-methoxy-5-(2’-ethylhexyloxy)-1,4-phenylene vinylene], (MEH-PPV) for its potential corrosion-inhibition properties on aerospace aluminum alloy AA2024-T3. Solution processing of the polymer, as well as adhesion testing and accelerated weathering tests were performed on MEH-PPV full military aerospace coatings. Wet and dry tape adhesion testing, as well as pencil hardness, impact flexibility and pneumatic adhesion tensile test instrument (PATTI) testing were used to demonstrate the adhesion performance of MEH-PPV on aluminum substrates. The results showed that MEH-PPV had acceptable adhesion characteristics when compared to hexavalent chromium (Cr(VI)) based coatings in all of these tests. Accelerated weathering analysis was performed on MEH-PPV coatings to determine their corrosion protection and weathering resistance capabilities. These tests included neutral salt spray (NSS) exposure and xenon-arc lamp testing. The results showed that while MEH-PPV does not exhibit significant color change after 500 hours of xenon arc lamp exposure, the polymer has poor corrosion protection performance under aggressive salt environments.

Open and endovenous surgery of varicose veins provides an excellent way to treat varicose veins. However, there are great differences in the how the techniques are performed. No matter which procedure is carried out, there are standards that should be observed. The state of the art of open venous surgery with radical crossectomy is well-known, but unfortunately is still not always performed correctly nowadays. The state of the art of endovenous surgery has unfortunately not yet been sufficiently documented, but should be based on open-surgical techniques. How the standard of both methods today is (or should be) is described in detail in this work. A recurrence definition which applies to both techniques, is set up.

In this study, an electroactive polymer (EAP), poly(2,5-bis(N-methyl-N- hexylamino)phenylene vinylene) (BAM-PPV), was tested as an alternative to current hexavalent chromium (Cr(VI))-based Army wash primers. BAM-PPV was tested in both laboratory and field studies to determine its adhesive and corrosion-inhibiting properties when applied to steel and aluminum alloys. The Army Research Laboratory (ARL) tests showed that BAM-PPV combined with an epoxy primer and the Army chemical agent-resistant coating (CARC) topcoat met Army performance requirements for military coatings. After successful laboratory testing, the BAM-PPV was then field tested for one year at the Aberdeen Test Center (ATC). This field testing showed that BAM-PPV incorporated into the Army military coating survived with no delamination of the coating and only minor corrosion on the chip sites.

Fur die thermoablativen Verfahren zur Therapie der Stammvarikose der V. saphena magna (VSM) wird eine periprozedural verbesserte Lebensqualitat (LQ) postuliert. Diese These entspricht nicht uneingeschrankt dem klinischen Eindruck in der Praxis. Auch in der Literatur finden sich Hinweise, die diesem Postulat widersprechen. Allerdings sind die meisten Studien statistisch unzureichend. Im Rahmen einer prospektiv randomisierten Interventionsstudie zwischen Krossektomie/Stripping (C/S), dem Laserverfahren (Laser) und einer Kombination beider Prozeduren (Laser/C) soll uber eine Patientenbefragung mit dem standardisierten Freiburger Venenfragebogen (FLQA-v) die phlebologisch assoziierte Lebensqualitat (LQ) untersucht werden. 2 Monate nach dem Eingriff wurde 449 Patienten der FLQA-v vorgelegt. Der FLQA-v umfasst 81 Fragen in den Kategorien korperliche Beschwerden, Alltagsleben, Sozialleben, psychisches Befinden, Therapie, Zufriedenheit und Beruf. Die Anzahl der beantworteten Items der jeweiligen Skala musste mindestens 75 % betragen. Die FLQA-v-Skalen wurden mit Mittelwert, Standardabweichung und 95 %-Konfidenzintervall ausgewertet. Die Behandlungsarten wurden uber die Berechnung der FLQA-v-Skalen als auch des Gesamt-Scores verglichen. 343 Patienten fullten den FLQA-v aus. Die FLQA-v-Skalen weisen mit Mittelwerten kleiner als 2,5 auf eine gute Lebensqualitat hin. Die Patienten der C/S-Gruppe erreichten durchschnittlich einen Gesamtscore von 1,7 ± 0,5 (95 %-KI: 1,6–1,8), die der Laser-Gruppe von 1,7 ± 0,4 (95 %-KI: 1,6–1,7) und die der Laser/C-Gruppe 1,8 ± 0,5 (95 %-KI: 1,7–1,8). Fur keine Skala oder den Gesamtscore gab es statistisch signifikante Unterschiede zwischen den drei Gruppen (p = 0,782, korperliche Beschwerden; p = 0,706, Alltagsleben; p = 0,919, Sozialleben; p = 0,523, psychisches Befinden; p = 0,418, Therapie; p = 0,158, Zufriedenheit; p = 0,307, Gesamtscore). Die Skala „Beruf“ wurde wegen > 25 % fehlender Werte ausgeschlossen. Mit der C/S-, der Laser- und der Laser/C-Methode konnen hervorragende Besserungen der LQ im Vergleich zum praoperativen Zustand erreicht werden. In der perioperativen Phase bis zu 2 Monaten ist die LQ bei den Verfahren ahnlich. Die Ergebnisse decken sich mit der Literatur, in der mit einer Ausnahme keine Unterschiede in der Anderung der LQ zwischen den Methoden gesehen werden konnten, unabhangig von der Art des verwendeten Messinstruments.

Einleitung: Fur die Therapie der Stammvarikose der V. saphena magna (VSM) stehen die Crossektomie mit Invaginationsstripping, die endoluminale Lasertherapie und eine Kombination von beiden zur Verfugung. In dieser Veroffentlichung prasentieren wir bis zu 2 Jahre postoperativ erhobene Daten zu klinischen und sonografischen Rezidiven im Bereich der VSM-Crosse. Material und Methode: In einer offenen 3-armigen prospektiv-randomisierten Multicenterstudie wurden sonografische und klinische Parameter perioperativ, nach 2, 6, 12 und 24 Monaten zwischen Crossektomie und Invaginationsstripping, der Lasertherapie und einer Kombination von beiden Methoden verglichen (Laser: 980 nm, Continuous Mode, 30 W, Biolitec®, Jena). Perioperativ und nach 2 Monaten waren Daten von 449 Patienten verfugbar. Nach 6 Monaten waren es 388, nach 12 Monaten 380 und nach 24 Monaten 332 Patienten. Klinische Rezidive, sonografische Refluxbefunde und refluxive Seitenaste an der Magnacrosse wurden untersucht. ZusammenfassungHintergrund: Multizentrische Langzeitstudien mit hohen Fallzahlen nach Crossektomie und Stripping der Vena saphena magna (VSM) liegen nicht vor. Den in mehreren Studien und Registern erhobenen Daten zu den Rezidivraten der endovenösen Therapieverfahren stehen außerordentlich heterogene Daten der operativen Therapie gegenüber.Methode: In einer von der Arbeitsgemeinschaft für Venenoperationen (VOP-AG) der Deutschen Gesellschaft für Phlebologie (DGP) initiierten prospektiven Multizenterstudie wurden in 12 Zentren 841 Patienten (1 070 Extremitäten) mit einer Stammvarikose der VSM aufgenommen und entsprechend einem streng standardisierten Protokoll operiert. Primärer Endpunkt ist das hämodynamisch relevante saphenofemorale Rezidiv, definiert als eine Vene in der Crossenregion (Durchmesser >5 mm, Reflux >0,5 sec) mit einer Verbindung zur V. femoralis communis Sekundäre Endpunkte sind die Rate duplexsonografisch nachweisbarer pathologischer Crossenrefluxe (Durchmesser 0,5 sec.), perioperative Komplikationen und neu aufgetretene Varizen nach dem REVAS Klassifikation. Die klinischen und duplexsonografischen Kontrollen erfolgten 7 bis 14 Tage, 3 bis 4 Monate und ein Jahr postoperativ. Hiernach sind jährliche Kontrollen vorgesehen.Ergebnis: Die Rate klinischer Rezidive lag 3–4 Monate postoperativ bei 1,1 % und ein Jahr postoperativ bei 6,4 %. Die Rate duplexsonografischer Rezidive im Crossenbereich betrug nach 3–4 Monaten 0,53 % und nach einem Jahr 2,24 %. Davon waren lediglich zwei (0,29 %) ein Jahr postoperativ hämodynamisch relevant (Durchmesser >5 mm).Schlussfolgerung: Die unter standardisierten Bedingungen vorgenommene Crossektomie und Stripping-Operation weist im Verlauf bis zu einem Jahr eine geringe Rezidivrate auf.

Bovine cortical bone was pyrolyzed to produce a network of conductive carbon entwined with native hydroxyapatite that maintains its macroscopic structure during pyrolysis and prevents collapse of the carbon. Self-supporting conductive carbon monoliths were prepared by removing the hydroxyapatite with acid or ethylenediaminetetraacetic acid. The specific surface areas of these monoliths were determined by nitrogen adsorption, and their chemical structure was characterized using Raman spectroscopy. The monoliths exhibit specific surface areas and Raman spectra similar to those of amorphous carbons. Capacitance was assessed using the monoliths as the working electrodes in three-electrode cells, and two-electrode devices in which both electrodes were monoliths. Individual monoliths exhibit specific capacitances of 134 ± 11 F/g in aqueous solutions of potassium nitrate and 108 ± 9 F/g in the ionic liquid 1-ethyl-3-methylimidizolium bis(trifluoromethylsulfonyl)imide. The capacitance of individual 6 mm diameter by 1 mm thick electrodes was typically on the order of 0.2 Farads.

ZusammenfassungEinleitung: Zur Therapie der Stammvaricosis der Vena saphena magna stehen u.a. die Kombination Crossektomie/ Stripping und die endoluminale Lasertherapie zur Verfügung, jedoch bislang ohne statistisch belastbare Daten.Material und Methode: Perioperativ, nach 2 und 6 Monaten, wurden sonographische und klinische Parameter in einer prospektiven randomisierten dreiarmigen Multicenter-Studie zwischen der endoluminalen Lasertherapie, der invaginierend durchgeführten, modifizierten, offen operativen Therapie der Stammvarikosis der Vena saphena magna und der endoluminalen Lasertherapie (Laser-prozedur mit 980 nm Laser (Biolitec), Continuous-Mode-Verfahren mit 30 Watt) vergli-chen. Initial und nach 2 Monaten wurden Daten von 449 Patienten, nach 6 Monaten von 388 Patienten erhoben. Zielparameter waren die sonographischen, inguinalen Refluxbefunde einschließlich durchströmter Seitenäste und deren Bezug zu klinischen Rezidiven.Ergebnisse: In den beiden Lasergruppen wurden signifikant mehr crossennahe Seitenäste gefunden als in der operativen Therapie. Es gab zu diesem Zeitpunkt keinen statistisch signifikanten Zusammenhang zwischen Refluxbefunden und klinischen Rezidiven. Die Re-flux- und Rezidivbefunde zeigten im zeitlichen Verlauf keine intraindividuelle Konstanz. Bezüglich der Endpunkte Ödeme, Lymphödeme, neurologische Sensibilitätsstörungen, Matting und postoperative Einschränkungen gab es signifikant mehr Lymphödeme, lokale Sensibilitätsstörungen und Saphenusschäden in den beiden Lasergruppen.Diskussion: Mit längerfristigen Nachuntersuchungen muss die Bedeutung der crossenahen Seitenäste für die Entwicklung klinischer Rezi-dive geklärt werden. Aufgrund des Nebenwirkungsprofils kann die verwendete Lasertechnik nicht als schonenderes Verfahren gegenüber der klassischen Operation bezeichnet werden.

Congenital anomalies of the kidneys and urinary tract (CAKUT) are among the most common anomalies in newborn infants, and may cause chronic renal disease in newborns. We investigated the sensitivity and specificity of different ultrasound-based screening strategies for CAKUT.Newborns (n = 4331) were analyzed for CAKUT in at least one ultrasound examination as a part of the Survey of Neonates in Pomerania (SNiP), a 7-year population-based study on neonates in Western Pomerania (Germany). Intrauterine ultrasound examinations were compared with early postnatal ultrasound findings (from days 3 - 7 of life) and pathological findings within the first 6 months of postnatal life.Cases of CAKUT were detected in 309 (3.7 %) kidneys in one ultrasound examination at the following points of time at least: (i) prenatally in 56 newborns (18.2 %), (ii) 3 - 7 days postnatally in 201 newborns (65.2 %) and (iii) in 52 newborns (17 %) during the 6-month follow-up. The prevalence was significantly higher in male infants, and hydronephrosis was found to be the most frequent obstructive nephropathy (83.3 %). Significant co-morbidity was observed with CNS malformations. The diagnostic sensitivity was significantly higher in postnatal ultrasound screening (79.6 vs. 18.2 % prenatally), while the specificity was above 99 % at all time points.This study demonstrates a high prevalence of CAKUT and demonstrates the importance of combined prenatal and postnatal ultrasound examinations for early CAKUT diagnosis.ZIEL:: Angeborene Fehlbildungen der Nieren und ableitenden Harnwege (CAKUT) gehören zu den häufigsten neonatalen Anomalien und können zur Entstehung chronischer Nierenerkrankungen und Niereninsuffizienz führen. In dieser Studie haben wir Sensitivität und Spezifität der verschiedenen ultraschallbasierten Screening-Ansätze untersucht.Im Rahmen des über 6 Jahre durchgeführten Survey of Neonates in Pomerania (SNiP) erhielten Neugeborene (n = 4331) mindestens eine Ultraschalluntersuchung der Nieren und ableitenden Harnwege im Rahmen der zweiten Vorsorgeuntersuchung (U2) zwischen dem 4.–7. Lebenstag. Intrauterine Ultraschallergebnisse wurden mit den postnatalen Ergebnissen und eventuellen weiteren Auffälligkeiten in den ersten 6 Lebensmonaten verglichen.Bei 309 (3,7 %) Nieren wurde in einer der folgenden Ultraschalluntersuchungen eine CAKUT diagnostiziert, (i) pränatal bei 56 Neugeborenen (18,2 %), (ii) 4 – 7 Tage postnatal bei 201 (65,2 %) und (iii) während der ersten 6 Lebensmonate bei weiteren 52 (17 %) Neugeborenen. Es zeigte sich eine signifikant höhere Prävalenz bei männlichen Neugeborenen. Eine angeborene Hydronephrose war die am häufigsten gefundene obstruktive Nephropathie (83,3 %). Eine signifikante Komorbidität zeigte sich mit Auffälligkeiten des ZNS. Die diagnostische Sensitivität des postnatalen Screenings war signifikant höher (79,6 vs. 18,2 % pränatal), die Spezifität war zu allen Screeningzeitpunkten über 99 %.Diese Studie zeigt eine hohe CAKUT-Prävalenz und demonstriert die Wichtigkeit eines Ultraschallscreenings zur Früherkennung einer angeborenen Fehlbildung der ableitenden Harnwege.

A novel processing technique was used to solution cast films of poly(benzimidazo benzophenanthroline), (BBL), and the novel ladder polymer poly(4-aza-benzimidazo benzophenanthroline) (Py-BBL), which were used as cathode materials in Type IV electroactive polymer-based electrochemical capacitors (EPECs). This new processing technique involves co-casting the polymer from solution with a room temperature ionic liquid, 1-ethyl-3-methylimidazolium bis(trifluoromethylsulfonyl)imide (EMIBTI). The new processing technique gave polymer films with superior transport properties and electrochemical stabilities, did not require a break-in period, and yielded higher charge capacity than the standard films. Co-cast films of BBL and Py-BBL were each incorporated into separate Type IV EPECs using poly(3,4-propylene dioxythiophene) (PProDOT) as the anode material. It was found that the PProDOT/BBL capacitors store, on average, about 50% more energy than a comparable PProDOT/Py-BBL EPEC. While PProDOT/BBL films have an energy density advantage at rates (power densities) less than 0.01 kW kg −1 , PProDOT/Py-BBL EPECs are capable of delivering higher energy than the BBL EPECs at rates greater than 0.01 kW kg −1 (550 s per cycle). In fact, PProDOT/Py-BBL devices delivered more than ten times the energy density of PProDOT/BBL devices at 0.5 kW kg −1 (50 s per cycle). The PProDOT/Py-BBL EPECs were cycled for 10,000 cycles at 65% depth of discharge and maintained 96% of the initial energy and power density, whereas the PProDOT/BBL EPECs were cycled under the same conditions and lost more than 35% of the initial energy and power density after only 2300 cycles.

Die US-amerikanischen Leitlinien zur Therapie der Varikose bewerten die Crossektomie und Strippingmethode der V. saphena magna mit dem Level 2B und favorisieren die endovenosen Techniken, die sie mit 1B bewerten. In praktischer Konsequenz werden die meisten venenchirurgischen Eingriffe in den USA zurzeit endoluminal vorgenommen. In Deutschland ist die Situation eine andere. Von den uber 300.000 venenchirurgischen Eingriffen im Jahr wird das Gros nach wie vor offen chirurgisch therapiert. In spezialisierten Zentren der BRD ist die postoperative Quote pathologischer Crossebefunde nach Crossektomie gering. Es gibt Hinweise fur eine fehlerhafte Crossektomie in mehreren Vergleichsstudien (RCTs). Nach korrekter Crossektomie sind die Ergebnisse sehr gut.

ZusammenfassungEinleitung: In der vorliegenden Arbeit gilt es, mittelfristige Refluxbefunde nach 2 und 6 Monaten sowie die klinischen und sonografischen Befunde im Bereich der Magnacrosse zu beschreiben, die für eine langfristige Entwicklung von Bedeutung sein könnten.Material und Methode: Die offene multizentrisch prospektiv randomisierte kontrollierte 3-armige Vergleichsstudie zwischen der endoluminalen Lasertherapie und der invaginierend durchgeführten modifizierten offen operativen Therapie der Stammvarikosis der Vena saphena magna (VSM) hat den primären Endpunkt der Beobachtung der mittelund langfristigen Rezidiventwicklung mit einem Ausgangspunkt im Inguinalbereich. (Laserprozedur mit 980 nm Laser (Biolitec), Continous-Mode-Verfahren mit 30 Watt). Um die Bedeutung der Crossektomie in Bezug zur Entstehung eines Crosserezidivs herauszuarbeiten wurde eine dritte Gruppe gebildet, in der die Lasertherapie des Stammvenenabschnitts mit einer Crossektomie der Vena saphena magna kombiniert wurde.Ergebnisse: 449 Patienten wurden perioperativ und nach 2 Monaten untersucht. 388 Patienten konnten nach 6 Monaten nachuntersucht werden. Nach 2 und 6 Monaten zeigten sich signifikant mehr inguinale Refluxe und Seitenäste in den Lasergruppen.Ausblick: Die Befunde werden in einer derzeit laufenden Nachuntersuchung überprüft und auf ihre klinische Relevanz für das Langzeitergebnis getestet.

Bei der Krampfadererkrankung (Krampfaderleiden, primare Varikose) handelt es sich um eine degenerative Erkrankung der Venenwand im oberflachlichen Venensystem der Beine, bei der sich unter dem Einfluss verschiedener Realisationsfaktoren (z. B. Schwangerschaften, Orthostasebelastung) im Laufe des Lebens in unterschiedlicher Auspragung und Schweregrad Krampfadern (Varizen) entwickeln [243]. Von der primaren Varikose ist die sekundare Varikose zu unterscheiden, die auf dem Boden obliterierender Prozesse im tiefen Venensystem entstehen kann. Nach topographischen bzw. morphologischen Kriterien werden im Wesentlichen folgende Krampfadertypen unterschieden: Stammvarizen, Seitenastvarizen, Perforansvarizen [25], Retikulare Varizen, Besenreiservarizen. Zur anatomischen und topographischen Bezeichnung der Venen sollte die, in einem transatlantischen Konsensusdokument erarbeitete, Nomenklatur nach VEIN-TERM verwendet werden [50, 99]. Die hamodynamisch bedeutsame Stammveneninsuffizienz lasst sich nach Hach (1981) in Stadien einteilen [450]. Bei Insuffizienz ab der Mundungsregion wird die Refluxstrecke durch den distalen Insuffizienzpunkt bestimmt (Tab. 1). Tab. 1 Stadieneinteilung (Refluxstrecke) der Stammvenen. B. pudendale, gluteale, pelvine Varikose [2, 122].

SummaryNeovascularisation can compromise the success of high ligation and resection of the greater saphenous vein. Studies using duplexultrasound to classify recurrent groin veins have described rates of neovascularisation as high as 60% and raised the question whether high ligation is actually able to prevent groin recurrences. In the present study, recurrent groin veins were excised and examined histologically in order to prove whether neovascularisation is the main cause for sapheno-femoral recurrences. Patients, methods: 419 patients accounting for 458 legs with clinically symptomatic groin recurrences were included in a country-wide multicenter study. The recurrent groin veins were excised in a standardized fashion and subsequently divided into the different types of recurrence based on histopathological criteria. Results: 427 specimen (93%) were available for histopathological examination. In 69 cases (16.2%) a neovascularisation was found to be the cause of recurrence. 311 specimen (72.8%) contained a long residual stump of the greater saphenous vein, out of which 32 (7.5%) showed additional neovascularisation at the site of the ligation. In 29 cases (6.8%) a venous side branch was found to be the recurrent groin vein. 11 specimen (2.6%) did not contain any evidence of venous material and in another 7 cases (1.6%) it was not possible to clearly identify the cause of recurrence during the histo pathological workup. Conclusion: The high rates of neovascularisation described in several duplex ultrasound studies could not be confirmed in our investigation. Recurrences seem to be mainly caused by a technically incorrect initial operation which leaves a long residual stump of the saphenous vein in place. Following a technically correct high ligation, clinically relevant recurrences appear to be rare. This finding underlines the necessity of a high ligation of the saphenous vein according to current guidelines.

Despite the availability of a large number of alkyl tetrahydrofurfuryl ethers that have a wide range of applications, pure bis(tetrahydrofurfuryl) ether (BTHFE) has not been previously synthesized. Here, we report the synthesis of BTHFE (consisting of the RR, SS, and meso stereoisomers) at greater than 99 % purity from tetrahydrofurfuryl alcohol, using (tetrahydrofuran‐2‐yl)methyl methanesulfonate as an intermediate. Additionally, we demonstrate that BTHFE can be used as a non‐volatile solvent in poly(3,4‐propylenedioxythiophene)‐based supercapacitors. Supercapacitor devices employing solutions of the ionic liquid 1‐ethyl‐3‐methyl‐imidizolium bis(trifluoromethylsulfonyl)imide in BTHFE display similar performances to those prepared by using the neat ionic liquid as an electrolyte, although solution‐based devices exhibit a somewhat higher resistance.

Functional polymeric coatings comprise a diverse array of organic/inorganic functional groups, properties, and applications. This chapter attempts to provide a comprehensive review on the science and engineering of functional polymer coatings for both the novice and experienced chemist/chemical engineer. Functional polymer coatings are coatings that are designed through careful selection of organic/inorganic functional groups and controlled synthesis on the substrate's surface. The design leads to changes in the surface properties (e.g., adhesion, wettability, corrosion resistance, wear resistance, anti-fouling, non-stick, and anti-scratch) of a particular substrate (metal, glass, wood, or plastic). Functional polymeric coatings represent the “next generation” of coatings. These coatings dynamically respond to their environment, as opposed to a static coating that is used as a simple barrier, or for decorative purposes. This review is divided into three sections: (1) functionalization of polymers, (2) polymer properties and surface analytical techniques, and (3) applications.

Zusammenfassung Ziel: Nach endovenöser Hitzeablation der V. saphena magna verbleibt im Bereich der Krosse nahezu regelhaft bis zu 5 cm langer Venenstumpf unverschlossen. Aufgrund fehlender Langzeiterfahrungen ist unklar, ob sich aus dem zurückgelassenen Stumpf im Spätverlauf ein Leistenrezidiv entwickelt. Um Anhaltspunkte für den potenziellen Zeitpunkt der Rezidiventwicklung zu gewinnen, sollte bei Pat. mit histologisch gesichertem Saphenastumpfrezidiv geklärt werden, wie lange es dauerte, bis nach dem Ersteingriff Rezidiv klinisch apperent wird. Methoden: Im Rahmen einer an sieben Zentren durchgeführten Multizenterstudie wurden operativ exstirpierte Leistenrezidive histologisch klassifiziert. Die Pat. mit eindeutigem Saphenastumpfrezidiv wurden anhand eines standardisierten Fragebogens nach den ersten Rezidivzeichen befragt. Anhand der Angaben wurde das beschwerdefreie Intervall ermittelt, das Zeitraum zwischen dem Ersteingriff und dem ersten Auftreten von Rezidivzeichen kennzeichnet. Ergebnisse: Bei 251 Pat. wurde an 278 Extremitäten ein lang belassener Saphenastumpf als Ursache des Leistenrezidivs ermittelt. den meisten Pat. bestand nach dem Ersteingriff ein beschwerdefreies Intervall, das im Durchschnitt 7,4 ± 5,70 Jahre betrug. Rezidivvarizen kündigten nach durchschnittlich 6,3 ± 5,68 Jahre und Stauungsbeschwerden nach durchschnittlich 8,5 ± 5,71 Jahre das Rezidiv an. Schlussfolgerungen: Überträgt man die Erfahrungen aus operativen Therapie auf die unverschlossenen Krossestümpfe nach endovenöser Therapie, so ist frühestens nach 7–8 Jahren mit den klinischen Zeichen eines Leistenrezidivs zu rechnen. Die gegenwärtig vorhandenen Nachbeobachtungszeiten reichen nicht aus, um den Langzeitverlauf nach endovenöser Therapie beurteilen zu können.

A general review of the chemistry and corrosion control properties of electroactive polymers will be presented. These polymers are also known as conductive polymers (CPs), and this term will be used throughout this article. This paper will focus on both the synthesis of applicable CPs used for corrosion protection in various environments and their potential benefits over common organic barrier coatings.

This chapter provides a comprehensive review of both inorganic and organic pretreatment coatings for metals and alloys. Aqueous and nonaqueous corrosion are introduced as well the economic costs associated with corrosion for several industrialized countries. Smart coating strategies currently employed to mitigate corrosion damage are shown as well as future materials and film development. Conversion coatings including chromate and phosphate conversion coatings are discussed as early examples of “smart coatings” for the inhibition of corrosion; and the latest use of lanthanide-based conversion coatings is discussed. The chapter also discusses the corrosion-inhibiting abilities of sol-gel and conductive polymer smart coatings. An examination of new methods to inhibit corrosion highlights self-assembling (SA) molecules, polyelectrolyte (PE) multilayers, the incorporation of “nanocontainers” in a coating formulation, and protective biofilms. These new innovative coatings can provide efficient barrier protection and/or controlled release of corrosion inhibitors, reducing the effects of corrosion via a self-healing mechanism.

Objectives High ligation and stripping was compared to endovenous laser ablation for the therapy of great saphenous vein varicosity. Long-term efficacy was assessed in terms of avoidance of inguinal reflux and mechanisms of recurrence were investigated. Design Multicentre, randomised, three-arm, parallel trial. Materials and methods A total of 449 patients were randomised into three different treatment groups: high ligation and stripping group ( n = 159), endovenous laser ablation group ( n = 142; 980 nm, 30 W continuous mode, bare fibre) or a combination of laser ablation with high ligation (endovenous laser ablation group/ high ligation group, n = 148). Patients were examined clinically and by duplex ultrasound once a year. The primary end point of this study is inguinal reflux at the saphenofemoral junction after 2 years. This paper presents secondary data on sonographically determined inguinal reflux and clinical recurrences in the treated area after up to 6 years of follow-up. Results Median time to follow-up was 4.0 years; the mean time follow-up 3.6 years. Follow-up rates were: 2 years 74%, 3 years 47%, 4 years 39%, 5 years 36% and 6 years 31%. Most reflux into the great saphenous vein appeared in the endovenous laser ablation group (after 6 years: high ligation/stripping versus endovenous laser ablation p = 0.0102; high ligation/endovenous laser ablation vs. endovenous laser ablation p Conclusions Clinical recurrence appears with the same frequency in all three treatment groups, but the responsible pathological mechanisms seem to differ. Most reflux into the great saphenous vein and side branches appears after endovenous laser ablation, whereas more saphenofemoral junction-independent recurrences are seen after high ligation/stripping.

Therapeutic options for the treatment of varicosis of the great saphenous vein (gsv) include endoluminal laser therapy, crossectomy/stripping and a combination of both. In this paper we present data for clinical and sonographic inguinal recurrencies up to two years post operation.In an open multicentre, randomised three-arm trial, sonographic and clinical parameters were compared perioperatively, after 2, 6, 12 and 24 months between endoluminal venous laser therapy, high ligation and invaginating stripping and a combination of both (laser: 980 nm, continuous mode, 30 W, Biolitec®, Jena, Germany). Data of 449 patients were available for the perioperative and 2 months examination. 388 patients were followed up until 6, 380 patients for 12 and 332 patients for 24 months. We compared clinical recurrences, sonographic reflux findings, and reflux side branches in the inguinal region at the saphenofemoral junction.We found significantly more inguinal reflux and reflux side branches in the laser groups (p 0.0001), however, there was no statistically significant relation between clinical recurrences and sonographic reflux. Over time, there were no intraindividual constant refluxes. Regarding the secondary endpoints oedema, lymphatic oedema, local disturbances of sensibility and irritations of the saphenous nerve, lymphatic oedema and irritations of the saphenous nerve were significantly more present in the two laser groups. After two years pain and restrictions in professional life were no longer relevant in all groups.Clinical recurrences developed both in the C/S and in the laser group but reflux into the gsv and into proximal side branches developed significantly more often in the laser group. In a long-term follow-up we have to investigate the importance of reflux side branches for the development of clinical recurrences to reveal differences between the three therapeutic strategies.

Recent results concerning the synthesis of new main-chain syndioregic nonlinear optical polymers are presented. In particular, the synthesis of polymers with extended pi conjugation in the chromophore and chromophores with improved thermal stability are presented. The nonlinear optical coefficient of several of the polymers and the optical loss at 1.3 and 1.55 μm were measured and are discussed. © 2000 John Wiley & Sons, Inc. Using the method of aqueous solution alternating polyelectrolyte deposition (APD), second-order nonlinear optical (NLO) polymer films were prepared for the first time in which both polymers were NLO-active. Films were prepared by alternately coating a solid substrate with an NLO-active side chain polycation and an NLO-active main chain polyanion. This new polyanion was comprised of α-cinnamoyl chromophores in the syndioregic configuration (an “accordion” polymer). The new polycation was derived from polyepichlorohydrin that was completely substituted with new stilbazolium side chain. The films were transparent and had no texture when observed by polarized microscopy. The increase in intensity of the second harmonic (SH) signal generated in the films was quadratic with each molecular layer to 20 layers. Beyond that, the SH signal intensity saturated as more layers were added.

The aim of this workshop was to assess the ability of individual autoantibody (ab) assays and their use in combination to discriminate between type 1 diabetic and control sera. Coded aliquots of sera were measured in a total of 119 assays by 49 participating laboratories in 17 countries. The sera were from 51 patients with new onset type 1 diabetes and 101 healthy control subjects with no family history of diabetes. In the final analysis, data on diabetic sera were restricted to 43 subjects younger than age 30 years. The laboratories were asked to report results for these sera using their currently available anti-islet autoantibody assays. In addition, they were asked to combine information from their assays to classify sera as having high, moderate, or low probability of originating from a patient with type 1 diabetes. Actual strategies for combining assays were determined by each laboratory. There were no significant differences in sensitivity among 19 radioimmunoassays (RIAs) for IA-2 autoantibodies (cytoplasmic islet cell antibody [ICA] 512) using different constructs that included the intracellular portion of the molecule (mean sensitivity 73%). However, an enzyme-linked immunosorbent assay (ELISA) using the extracellular portion of the IA-2 molecule did not discriminate between diabetic and control sera. Among GAD autoantibody assays that achieved sensitivity >70%, 26 were RIAs and one was an ELISA. When the sera were ranked according to their autoantibody levels, the concordance for insulin autoantibodies (IAAs) in different laboratories was markedly less than for IA-2ab and GADab. Using a combination of autoantibody assays, several laboratories achieved excellent discrimination between diabetic and control sera (sensitivity up to 80% with false-positive rate of 0%). A variety of strategies for combining information from different assays were successful (e.g., those including and excluding ICA), and no one strategy emerged as clearly superior. In conclusion, IA-2/ICA512 autoantibodies are a marker of type 1 diabetes and can be measured consistently by most assays. Several different strategies for combining assays achieved high sensitivity with a low false-positive rate.

We report here recent progress toward the goal of processing polymers near room temperature to produce all-polymeric non-linear optical films which may ultimately find application as active waveguides. Polymers designed for spin-casting were found to be processable by the Langmuir–Blodgett–Kuhn (LBK) technique. The T g of the bulk polymer is about 240°C. The temporal stability of non-centrosymmetric order in LBK films at elevated temperatures was followed by measurement of the second harmonic generated (SHG) light intensity. The SHG signal was found to be constant in LBK films heated at 95°C for 10 h.

We present the case of a 14-year-old girl suffering from Maffucci's syndrome, a rare ailment belonging to the group of osteochondrodysplasias. At the age of 6 months, a diffuse swelling first appeared in the girl's right cheekbone region and the periauricular area. Because of recurrent meningitis with massive otoliquorrhea, several surgical revisions were performed, beginning at the age of 4 years. The histological and immunohistochemical diagnosis showed hemangioma and lymphangioma with enchondroma. As a peculiarity of our patient's diagnosis, we found multiple bone defects apparently caused by venous and lymphomatous angiomatosis. There was also enchondromatosis of the skull base and the upper cervical vertebrae, which caused the recurrent otoliquorrhea and rhinoliquorrhea. A fistula closure was undertaken through a retromastoidal, suboccipital approach and fascial graft of the posterior cranial fossa.

Recent results (covering the approximate time period from January 1985 to August 1996) in the area of conducting-polymer synthesis, characterization and applications of conducting polymers are reviewed. Speculation as to the future direction of selected commercial and military applications and material properties required to make a major impact are offered.

The design and synthesis of mainchain chromophore polymers in a syndioregic (head-to-head) configuration is described. Polymers were designed for both electric-field poling and Langmuir-Blodgett-Kuhn (LBK) deposition. LBK deposition gives intrinsically polar films (no E-field poling) as evidenced by nonlinear optical (χ (2) ) properties.
