The relevance of the study is determined by the strategic interests of the Russian Federation dictating the need to achieve a national priority, which is the development of human potential. Given the ongoing reduction in labor force against a background of depopulation processes, finding a solution to the problem seems especially relevant. The purpose of this study is to develop a scientific conceptual framework for an intelligent risk-based innovative system for analysis, prediction and prevention of occupational and work-related health disorders of workers. This will help substantiate keynote sanitary-hygienic and medical-preventive measures aimed at reducing losses in healthy life expectancy and increasing occupational longevity. The main theoretical idea is based on a unique information-intellectual innovative system for analyzing and predicting cause-and-effect relationships between the effects of harmful and hazardous occupational factors and the resulting health problems of workers, occupational diseases included. The developed predictive digital neural network models, trained on retrospective or actual data on working conditions, health status, socioeconomic conditions and lifestyle factors, are the information and analytical basis for carrying out calculations and assessing the evolution of personal and group (occupation, age, and work records) health risks caused by occupational or work-related diseases in workers. This provides solid grounds for making a forecast of a prevented period of reduction in occupational working capacity associated with working conditions, specifically for each examined occupation, age, and work records. Such forecasts are an eligible information basis for developing and making well-grounded managerial decisions including those concerning sanitary, hygienic and medical preventive measures aimed at preserving occupational longevity. This will significantly increase effectiveness of corporate health-preserving policies.

Assessment of occupational health risks for workers with basic mining occupations including those exposed to elevated levels of vibroacoustic factors remains a topical issue. Given that, the aim of this study was to assess degree of exposure to occupational vibroacoustic factors and its effects on mining industry workers in the Arctic zone using occupational risk as a basic criterion. Prior risk assessment relied on results of hygienic assessment of vibroacoustic factors according to data derived by the Special Assessment of Working Conditions (SAWC) and verified by instrumental research. Posterior risk quantification was performed using data on occupational incidence among workers over the last 10 years considering their numbers. Unacceptable prior occupational risks caused by exposure to noise were detected for all analyzed occupations. According to instrumental research, risks levels higher than those identified by SAWC were detected for open face miners and cargo handling machine operators. According to SAWC data, unacceptable prior occupational risks caused by exposure to vibration were identified only for cargo handling machine operators; additional instrumental research identified such levels of occupational risks for them as well but also for blast hole drillers. Occupational risk quantification established that unacceptable (above 1•10-3) levels of occupational risk caused by vibration disease (VD) and sensorineural hearing loss (SHL) were detected in all analyzed occupational groups. The highest risk levels caused by exposure to vibration were identified for such an occupational group as ‘cargo handling machine operators’ and ranked as ‘average’ (1.37•10-2). The highest risk levels caused by exposure to noise were identified for timbermen and were also ranked as ‘average’ (1.32•10-2). Results obtained by quantification of occupational health risks for mining industry workers in the Arctic zone refine results of prior assessment can be applied in planning activities aimed at mitigating health risks for workers caused by exposure to vibroacoustic factors.

The whole complex of air protection activities has been planned in the RF with its aim to reduce levels of ambient air pollution. It is being implemented actively now and as a result the quality of the environment should improve for more than 7 million people. In this study, an algorithm has been suggested for assessing effectiveness of air protection activities. It includes six subsequent stages. The algorithm was tested at heat and power enterprises located in a region participating in the Clean Air Federal project. As a result, it was established that these enterprises were sources of potential public health risks; 70 % of them belonged to high risk categories. Until air protection activities are implemented, heat and power enterprises pollute ambient air in some areas in the city (up to 29.9 single maximum MPC; up to 6.9 average daily MPC; up to 19.0 average annual MPC), create unacceptable health risks (up to 25.8 HI for acute exposure, 22.7 HI for chronic exposure, CRT is up to 3.28∙10-4), and cause more than 87 thousand additional disease cases. Implementation of air protection activities at heat and power enterprises will reduce local levels of ambient air pollution but we still expect hygienic standards to be violated for 10 chemicals up to 3–22 MPC and high health risks are likely to persist (up to 6.5–25.5 HI for acute exposure, 11.9–22.4 HI for chronic exposure, CRT will be up to 3.28∙10-4). Effectiveness of the air protection activities planned at heat and power enterprises corresponds to the target levels of the gross pollutant emissions (reduction by 20.56 % by 2024) set within the Clean Air Federal project but it is estimated as ‘unacceptable’ as per the health harm indicator, which is additional disease cases associated with activities of these enterprises (< 20 %). It is necessary to implement additional air protection activities with respect to 12 pollutants (nitrogen dioxide, particulate matter, carbon (soot), carbon oxide, sulfur dioxide, dihydrosulfide, inorganic dust containing silicon dioxide in %: 70–20, dimethyl benzene, ethyl benzene, benzene, formaldehyde, and kerosene); to use the best available technologies with respect to the most hazardous chemicals; to monitor public health in areas with elevated health risks; to implement complex medical and preventive activities.

The development of the Arctic zone as a strategic resource base and its rational use aimed at the economic growth acceleration is a key interest of the Russian Federation in this region. The occupational environment in mining industry is associated with unique working conditions. Underground mining makes workers’ health protection the most significant task. The aim of this study was to perform hygienic assessment of working conditions, occupational incidence and occupational health risks for workers employed in mining industry in the Arctic zone of the Norilsk industrial area. Occupational health risks were assessed using indicators outlined in the Guide R 2.2.2006-05 The Guide on Hygienic Assessment of Factors Related to Working Environment and Work Process. Exposure to occupational noise and considerable work hardness are priority factors in the analyzed industry that cause occupational diseases. Occupational diseases among workers employed in mining industry in the Norilsk industrial area belonged to six nosologies. Vibration disease, stages 1 and 2, makes the greatest contribution to the overall structure of occupational incidence since it accounts for 68.6 % in it. The second place belongs to two-sided sensorineural hearing loss, stages 1 and 2, 13.5 %. Radiculopathy occupies the third rank place accounting for 10.9 %. Unacceptable occupational health risks were identified at 76.8 % of workplaces. A high share of workplaces with unacceptable occupational health risks leads to high occupational incidence among mining workers in the Norilsk industrial area, its levels being higher than the national average.

The review analyzes a range and prevalence of health disorders in workers employed at mining enterprises in the Arctic and exposed to heterogeneous occupational factors. We revealed that working processes typical for basic occupations in under-ground mining involved exposure to a set of heterogeneous harmful and (or) hazardous occupational factors such as intense occupational noise; elevated vibration; aerosols with predominantly fibrogenic effects, dusts and chemicals; high hardness and intensity typical for physical work; non-ionizing electromagnetic radiation. It was shown that diseases of the ear and mastoid, vibration syndrome, diseases of the musculoskeletal system, respiratory diseases and diseases of the nervous system prevailed both in the structure of general morbidity and in occupational one typical for miners. To create proper working conditions and to minimize effects of harmful and hazardous occupational factors as well as occupational health risks, it is advisable to perform comprehensive hygienic assessment of introduced equipment, machinery and mechanisms; to establish levels and doses of occupational factors. Engineering and technical, technological, medical and preventive and treatment and health-improving activities should be developed on this basis with special emphasis on such occupational groups as drift miners, drill-operators, blasters, timbermen, operators of cargo handling machinery, drilling unit operators, miners in mining outputs and faces, repairmen, and electric gas welders. It seems extremely vital to apply risk assessment methodology to assess occupational health risks for workers employed in mining operations in the Norilsk industrial region considering climatic features of the Arctic. This assessment is important for substantiating relevant activities aimed at managing such risks and protecting workers’ health.

The study was conducted due to the necessity to streamline management of ambient air quality in large industrial cities in the country. The relevant tasks were set within the ‘Clean Air’ Federal project and the system for setting emission quotas. The aim was to develop scientific-methodical approaches that would support Rospotrebnadzor in performing its functions and duties as regards management of ambient air quality, including those accomplished within the ‘Clean Air’ Federal project. We took into account that initial data for the whole system for setting emission quotas were represented by aggregated calculation of pollutant dispersion. The study relied on input and output data provided by the ‘Ekolog-Gorod’ software package for calculating ambient air pollution. This software employs methods for calculating emission diffusion in ambient air that are applied as standards in Russia. Calculations were accomplished at points located within residential areas in the analyzed cities and covered not less than 20 major contributions made by emission sources to levels of each chemical at each calculation point. Airborne health risks were assessed in accordance with the valid methodical documents. We applied the following criteria for permissible (acceptable) risks: carcinogenic ones should not exceed 1.0•10-4; non-carcinogenic chronic and / or acute risks should be at a level of a hazard index for chemicals with the same effects equal to 3.0. The brunch and bound method of linear programming was applied to substantiate optimal regulatory impacts aimed at minimizing health risks by reducing emissions into ambient air. We developed a fundamental algorithm for identifying a list of priority pollutants and a list of objects for setting emission quotas, as well as for substantiating optimal regulatory impacts to mitigate airborne public health risks. We suggest ranking chemicals as priority pollutants in case their registered levels are higher than the established hygienic standards and they in total account for not less 95 % of contributions to unacceptable health risks for critical organs and systems at least at one calculation point. Priority objects are those that are responsible for not less than 95 % of unacceptable health risks and violations of the established hygienic standards. The study describes a developed and tested instrument for selecting optimal regulatory impacts as per relevant hygienic indicators, including levels of public health risks. The suggested approaches support the Sanitary Service in its effort to provide proper quality of ambient air. They make it possible to identify priority chemicals and objects for setting emission quotas on the unified methodical basis for any city on the country, including those listed within the ‘Clear Air’ Federal project as priority ones. They also allow estimating whether environmental protection activities are relevant to the essence and levels of public health risks.

The Federal project entitled “Pure air” is a part of “Ecology”, the National project; its primary goal is to achieve a significant improvement in ambient air quality in cities where at present air contamination and population health risks related to it are the most significant. Activities aimed at improving the ecological situation in these cities are provided with the considerable state investment. Results of systemic instrumental measuring that is performed within state systems of ecological and social-hygienic monitoring are considered to be the most informative and reliable database to assess efficiency of air-protecting activities accomplished within the project. Our research goal was to develop and test methodical approaches to substantiating points and programs for observation over ambient air quality within social-hygienic monitoring. The said approaches were to be tested on concrete examples taking into account the existing ecologic monitoring system. We chose the following objects to test our approaches: two cities out of the priority list, namely Krasnoyarsk and Chita. Systemic observations are performed in both cities. There are data on the structure of emissions from all major contamination sources. In Krasnoyarsk there is an aggregated database that contains data on parameters of emission sources; there was no such database in Chita at the moment our research was accomplished. Given the availability of necessary initial data, we suggested algorithms for creating a system of points where observation posts were to be located and for monitoring programs development. We applied health hazards and health risks as our basic criterion for creating an observation system within social-hygienic monitoring. It was shown that data that were collected at ecologic monitoring posts without any changes in their location could be applied to solve tasks related to assessing and predicting health risks as well as analyzing efficiency of accomplished activities provided that research programs were supplemented with parameters that were priority ones as per health hazards and risks. We developed approaches to selecting points and programs for independent research within social-hygienic monitoring. These approaches involve dividing city territories into specific zones as per potential health hazards (when dispersal is not calculated) or health risk levels (when dispersal is calculated); substantiating a list of priority admixtures taking into account carcinogenic and/or non-carcinogenic hazards and risks as well as results of accomplished instrumental measuring and determining parameters of marker (indicator) enterprises with their emissions making the most significant contribution into air contamination.

It is shown in the article that since the establishment in 1992 and until now the social and health monitoring (SHM) operates as a complex open system of extended collection and processing of heterogeneous data about the parameters of the environment, socio-economic indicators of living of the population of the country’s regions, medical and demographic characteristics of society as a whole and of individual population groups. As part of the SHM system Rospotrebnadzor annually performs hundreds of thousands of instrumental measurements of objects in the environment of the Russian Federation. These federal and regional information assets have tremendous analytical capacity and allow a knowledge-based processing of information in the "health-environment" system for a variety of management tasks of sanitary and epidemiological situation. Changing of the general paradigm of control and supervisory activities of Rospotrebnadzor and giving a fundamentally new status of control measures to researches and measurements of the system of social and hygienic monitoring without interaction with legal entities and individual entrepreneurs were important bases for further improvement of the SHM. The development of SHM in conjunction with the risk-based supervision model can significantly improve analytical capabilities, effectiveness and efficiency of each system. This development requires the adoption of a new edition of The statements of the SHM, the development of a scientific approach to the formation of "risk profiles" of objects of sanitary and epidemiological surveillance; the development of methodological approaches to the selection of points and formation of programs of instrumental studies of air, water and soil as the control activities in the areas of influence of economic entities. Of great current interest are the development and consolidation of regulatory requirements for the registration of the results of sampling and measurement in the course of monitoring activities in the framework of the SHM; the establishment of criteria for associating violations of hygienic standards of air quality, natural and drinking water and soil with the facts of causing threat to life and health of citizens; the development of scientific and organizational approaches to the establishment, registration and proof of cases of harm to human health in violation of the mandatory requirements of the objects of sanitary and epidemiological surveillance.

The analysis of the primary morbidity dynamics in the population of the Krasnoyarsk region is performed for the period 2005–2015. The incidence forecast by classes of ICD diseases, X revision is calculated. It was found that the figures of the first time revealed incidence in the Krasnoyarsk region exceeded the average for the Russian Federation during the years 2011–2014 on neoplasms, nervous system diseases, diseases of the circulatory and the digestive systems, diseases of the musculoskeletal system and connective tissue, endocrine diseases and a number of other systems. The values of the integral health risk indicators are calculated by the methodology of State Sanitary and Epidemiological Surveillance Agency of the year 1995. The article presents the hygienic assessment of water quality in water sources of the Krasnoyarsk region used for centralized drinking water supply. It is shown that the exceeding of hygienic standards was registered at such indicators as iron (iron content is noted at the level to 1.8 mg/dm 3 or 6 MPC); fluorine (up to 6 MPC); ammonia and ammonium nitrogen (up to 2 MPC), nitrates (up to 5 MPC), organochlorine compounds (chloroform, carbon tetrachloride up to 5 MPC), manganese (up to 5.5 MPC), aluminium (up to 2 MPC). In water carcinogenic contaminants are recorded in significant concentrations: benzo(a)pyrene, cadmium, arsenic, nickel, lead. It is determined that the total lifetime carcinogenic risk to public health in the cities and districts of the Krasnoyarsk region due to oral intake from drinking water of chemicals of carcinogenic nature is negligible in 22 areas, and requires no additional measures to reduce. 23 territories of the region have carcinogenic risk ranged 1.0E-6 to 1.0E-5, which meets the criteria of acceptable risk. 9 territories (the town of Borodino, Lesosibirsk, Yeniseisk, Kazachinskiy, Partizansky, Pirovskiy, Rybinskiy, Sayanskiy, Uyarskiy areas) show the level of lifetime individual cancer risk from 1.0E-5 to 2.0E-4, which is unacceptable for the population in general. The main contribution to the risk level (80.8–98.4 %) makes the contents of arsenic in the drinking water. There is an increased risk of district (NI=1.2 and NI=1.17, respectively); bone and teeth from residents of the Sukhobuzimsky district (NI=1.04). High hazard indexes due to the nitrate and fluoride. Providing the population of urban districts and municipal districts of the Krasnoyarsk region with safe drinking water requires a set of various measures with the development and implementation of programs on improvement of water supply of populated areas

This study fulfills the hygienic assessment of ambient air quality in the populated areas of the Krasnoyarsk Region. It is shown that the total number of emission sources in the region is more than 23 600 units, what is higher than in previous years. Around 90.7 % out of them correspond to the set standards of permissible emissions. Air monitoring was carried by the establishments of Roshydromet, Rospotrebnadzor and by other organizations at 94 observation posts in eight urban districts and 2 municipal districts of the region. The status of the ambient air in a sequence of the populated areas of Krasnoyarsk region, namely in the cities Achinsk, Kansk, Krasnoyarsk, Lesosibirsk, Minusinsk, Norilsk, is characterized by the presence of certain pollutants, the level of which exceeds the hygienic standards. Prioritized pollutants are benzo(a)pyrene, suspended solids, nitrogen, and sulfur dioxide, formaldehyde and others. In the settlements the economic entities violate the legal requirements in the field of sanitary and epidemiological welfare of the population. The probability of the population’s health deterioration grows along with the growth of risk factors. The risks of respiratory diseases, immune system, blood and blood-forming organs and the additional mortality are assessed as unacceptable. Ensuring air quality of the urban residential areas and municipal districts of the Krasnoyarsk Territory requires the introducing the complex measures to improve it. The established levels of human health risk associated with exposure to polluted air are an additional criterion for selection of the priority objects when planning the implementation of risk-based model for supervisory activities in the field of sanitary and epidemiological welfare of the population.

In order to improve the control system of sanitary and epidemiological welfare of the population under the control of Rospotrebnadzor in the Krasnoyarsk region, a register of legal entities and individual entrepreneurs, whose activities are subject to sanitary and epidemiological supervision was established. For each of the 21 386 economic entities the number of population was defined (workers, consumers of goods and services), the potential risk of harm to the health of these populations was calculated. The computational methods and expert assessments were used. Testing of objects classification into four classes (1-extremely high, 2-high, 3-moderate, 4 class – low risk) allowed to rank to the class 1 around 0.6% of supervised legal entities and individual entrepreneurs; to object class 2 – about 14%; to class 3 – almost 48%. More than 36.7% of business entities have been referred to the fourth class; the frequency of scheduled inspections of their activity is minimal or can be ruled out at all (in the absence of violations of sanitary legislation for certain time). Using a risk-based planning model has streamlined planning towards increasing the share of inspections in respect of the subjects that are highly relevant and hygienically engaged in providing health care to the population (from 3.9% to 6.8%), education (from 32.8% to 55.6%), services and audits of local governments (from 0.9% to 15.2%).
