3. Lobbying for North and Eastern Syria : the PYD-PKK's strategic framing of the Syrian civil war in the US and the UK

- Abstract
Several studies on ethnic lobbying within the US/UK seek to explain what makes an ethnic lobbying campaign successful (Herner-Kovács, 2013; Kirk, 2008; Reese and Ramirez, 2002) and understand reasons/strategies for the mobilisation of ethnic lobbies (Koinova, 2014; 2013; 2011). They approach framing either as a facilitator in a successful lobbying campaign process or a strategy to explain the mobilisation of ethnic lobbies, however, they tend to ignore the role of framing itself as a dynamic process, and particularly the role of frames. This thesis develops a new perspective regarding the role of frames and framing in ethnic lobbying through a dynamic framing process, which includes the creation and presentation of frames/counter-frames (Kaufman et al., 2013) by considering the developments and the interaction between frames and counter-frames within the concentrated period. The thesis conceptualises three key political opportunities, the outbreak of the 2011 Syrian civil war, the withdrawal of the Syrian government forces from northern Syria in July 2012 with the start of the Aleppo battle, and the rise of ISIS in 2014 within the ongoing process of Aleppo battle for the Syrian Kurds' lobbying efforts through framing. Through content analysis and interviews, it analyses the framing efforts of the Kurdish PYD, and as a significant component of the dynamic framing process, the counter-framing efforts of the Turkish government against the PYD framing in the US/UK through the Syrian civil war from March 2011 to 20th January 2017. The thesis argues that framing success is explained through the responsiveness of the targeted countries towards frames. Findings demonstrate that the US/UK's responsiveness may not be merely addressed through the characteristics of frames, which can play an important role in framing success. These countries' policy stances and policy objectives in Syria, which can also be understood through the dynamic framing process, must be considered while explaining these countries' responsiveness towards the PYD frames. The US/UK's policy stances and policy objectives in Syria are also essential factors to understand to what extent the Turkish counter-frames managed to challenge these countries' responsiveness towards the PYD frames.

Suriye Kürtleri sivil savaşa kadar uluslararası arenada az bilinen bir etnik azınlık konumundadır 2011 yılında patlak veren iç savaş Kürtler açısından bazı fırsatlar ortaya çıkarmıştır. Bu makalenin amacı, terör örgütü Kürdistan İşçi Partisi, PKK’nın Suriye kolu olan Demokratik Birlik Partisi, PYD’nin Suriye’de politik bir aktör haline gelmesine katkıda bulunan sivil savaş kaynaklı fırsatları, “siyasal fırsatlar” kavramsal çerçevesi kapsamında değerlendirmektir. Makale temel olarak, Suriye sivil savaşının ilk beş yılında Kürtler için üç kritik siyasal fırsatın ortaya çıktığını ve terör örgütü PKK’nın Suriye kolu olan PYD’nin bu fırsatlar aracılığıyla ülkede politik bir aktör haline gelme yolunda önemli adımlar attığını ortaya koymaya çalışmaktadır. Bu siyasal fırsatlar sırasıyla Mart 2011’de iç çatışmaların başlaması, Temmuz 2012’de Suriye rejim güçlerinin ülkenin kuzeyinden çekilmesi ve Eylül 2014’te DAEŞ’in Kobani’yi (Ayn al Arab) kuşatmasıdır. Makale, Suriye sivil savaşının ilk beş yılında ortaya çıkan fırsatları siyasal fırsatlar kavramsal çerçevesi kapsamında değerlendirmekte ve bu fırsatların PYD’nin politik bir aktör haline gelmesinde oynadığı rolleri ampirik olarak analiz etmektedir. Bu haliyle makale Türkçe akademik literatürde PYD’nin aktörleşmesiyle ilgili yapılan tartışmalara katkıda bulunmayı amaçlamaktadır., particularly before the Syrian civil war. Following the outbreak of the conflict, the Kurds had some unprecedented opportunities. The purpose of this article is to assess these opportunities, which led the Democratic Union Party (PYD), the Syrian affiliate of the terrorist organisation, the Kurdistan Workers’ Party (PKK), to become a political actor, in the concept of the political opportunities. The article argues that there were three critical political opportunities for the PYD during the first five years of the civil war and by seizing these opportunities, the PYD almost became a political actor in Syria. They were the outbreak of the Syrian conflict in March 2011, the withdrawal of the Syrian government forces from northern Syria in July 2012 and the DAESH siege on Kobane in September 2014. By empirically analysing these developments and conceptualising them in the context of the political opportunities, the article aims to contribute to the academic literature in Turkey, which discusses the reasons how the PYD became a political actor in Syria.

Introduction: Capecitabine is a pre-metabolite of 5-fluorouracil and is used as a chemotherapeutic agent. Among the common side effects of capecitabine, there are gastrointestinal side effects including nausea, vomiting, and diarrhea, and dermatological side effects including hand-foot syndrome and skin pigmentation change. However, neurological side effects of capecitabine are very rare. We describe herein a patient who developed neurological side effects in the form of agraphia and dysarthria on the 7th day of capecitabine treatment. Case Report: A 34-year-old male patient, who was being followed up with the diagnosis of colon cancer, presented with speech and writing disorder that developed while under capecitabine treatment. Dysarthria and agraphia were detected in his neurological examination. Diffusion-weighted magnetic resonance imaging (MRI) revealed acute diffusion restriction in the splenium of the corpus callosum and at the level of the bilateral centrum semiovale. Brain MRI revealed symmetrical T2-weighted fluid-attenuated inversion recovery (T2-FLAIR) signal increases at the right temporoparietal medial, corpus callosum level, and bilateral white matter level. Management & Outcome: The capecitabine treatment was terminated, and methylprednisolone treatment was administered and plasmapheresis procedure was carried out. Subsequently, significant improvement was observed in the clinical findings and neuroimaging. Discussion: Capecitabine is used as an oral agent; thus, it provides ease of use. Neurological side effects associated with the use of capecitabine reportedly occur very rarely. The findings of this case demonstrated that leukoencephalopathy can be seen during the use of capecitabine, imaging results are very important in the diagnosis of leukoencephalopathy, and improvement can be achieved with the termination of the capecitabine treatment. Objective: Nutritional status assessment, dysphagia evaluation and enteral feeding decision are important determinants of prognosis in acute neurovascular diseases. Materials and Methods: NöroTek is a point prevalence study conducted with the participation of 87 hospitals spread across all health sub regions of Turkey conducted on 10-May-2018 (World Stroke Awareness Day). A total of 972 hospitalized neurovascular patients [female: 53%, age: 69±14; acute ischemic stroke in 845; intracerebral hematoma (ICH) in 119 and post-resuscitation encephalopathy (PRE) in 8] with complete data were included in this sub-study. Results: Gastrostomy was inserted in 10.7% of the patients with ischemic stroke, 10.1% of the patients with ICH and in 50% of the patients with PRE. Independent predictors of percutaneous endoscopic gastrostomy (PEG) administration were The National Institutes of Health Stroke Scale score at admission [exp (β): 1.09 95% confidence interval (CI): 1.05-1.14, per point] in ischemic stroke; and mechanical ventilation in ischemic [exp (β): 6.18 (95% CI: 3.16-12.09)] and hemorrhagic strokes [exp (β): 26.48 (95% CI: 1.36-515.8)]. PEG was found to be a significant negative indicator of favorable (modified Rankin’s scale score 0-2) functional outcome [exp (β): 0.032 (95% CI: 0.004-0.251)] but not of in-hospital mortality [exp (β): 1.731 (95% CI: 0.785-3.829)]. Nutritional and swallowing assessments were performed in approximately two-thirds of patients. Of the nutritional assessments 69% and 76% of dysphagia assessments were completed within the first 2 days. Tube feeding was performed in 39% of the patients. In 83.5% of them, tube was inserted in the first 2 days; 28% of the patients with feeding tube had PEG later. Conclusion: The NöroTek study provided the first reliable and large-scale data on key quality metrics of nutrition practice in acute stroke in Turkey. In terms of being economical and accurate it makes sense to use the point prevalence method. Amaç: Akut nörovasküler hastalıklarda nütrisyonel durum ve disfaji değerlendirmesi ve enteral beslenme kararı önemli prognoz belirleyicilerindendir. Gereç ve Yöntem: NöroTek, 10 Mayıs 2018’de (Dünya İnme Farkındalık Günü) Türkiye’nin tüm sağlık alt bölgelerine yayılmış 87 hastanenin katılımıyla gerçekleştirilen bir nokta prevalans çalışmasıdır. Hastanede yatan ve bu alt çalışma için toplanan verisi tam olan toplam 972 nörovasküler hasta (kadın: %53, yaş: 69±14 yıl; 845’i akut iskemik inme; 119’u intraserebral hematom ve 8’i post-resüsitasyon ensefalopatisi) analiz edildi. Bulgular: Gastrostomi iskemik inmeli hastaların %10,7, intraserebral kanamalıların %10,1 ve post-resusitasyon ensefalopatisi olanların %50’sine uygulanmıştır. Perkütan endoskopik gastrostomi (PEG) gereksiniminin bağımsız belirleyicileri, iskemik inme grubunda kabul NIHSS [exp (β): 1,09, %95 güven aralığı (GA): 1,05-1,14, puan başına] ile hem iskemik hem de hemorajik inmelerde mekanik ventilasyon uygulanmış olmasıdır [iskemik için: exp (β): 6,18, %95 GA: 3,16- 12,09] ve hemorajik inme için: [exp (β): 26,48, 95% GA: 1,36-515,8]. İnme olgularında PEG uygulaması hastane içi mortalite için bağımsız belirleyici değildi [exp (β): 1,731, 95% GA: 0,785-3,829]. Ancak, PEG uygulanmış olması taburculuk esnasında iyi prognoza (modifiye Rankin skoru 0-2) sahip olabilme için anlamlı bir negatif etmen olarak bulundu [exp (β): 0,032, %95 GA: 0,004-0,251]. Hastanede yatan nörovasküler hastaların yaklaşık üçte ikisinde malnütrisyon ve yutma bozukluğu açısından değerlendirme yapılmıştı. Nutrisyonel status değerlendirmesinin %69’u ve disfaji değerlendirmesinin %76’sı ilk 48 saat içinde gerçekleştirilmişti. Tüple enteral nütrisyon uygulama oranı %39’du. Beslenme tüplerinin %83,5’i ilk 2 gün içinde yerleştirilirken beslenme tüpü olan hastaların %28’ine daha sonra PEG açılmıştı. Sonuç: NöroTek çalışması ile Türkiye’de hastanede yatan akut inme hastalarında nutrisyonel uygulamaların temel kalite ölçütlerine ilişkin ilk güvenilir ve büyük ölçekli veri sağlanmıştır. Ekonomik olması ve doğruluğu açısından nokta yaygınlık yönteminin bu tip verilerin temini için daha fazla kullanılması mantıklıdır.

30. The effect of neutrophyl/lymphocyte ratio and mean platelet volume on short-term prognosis in middle cerebral artery infarctions

Amaç: Bu çalışmada akut iskemik inme tanısı alan ve orta serebral arter (OSA) enfarktı saptanan hastalarda nötrofil/lenfosit oranı (NLR) ve ortalama trombosit hacminin (MPV) kısa dönem prognoz üzerine etkisinin araştırılması amaçlanmıştır.Gereç ve Yöntem: Bir üniversite hastanesi Nöroloji Yoğun Bakım ve İnme Ünitesine akut iskemik inme tanısıyla yatan ve OSA enfarktı saptanan hastalar alınmıştır. Kontrol grubu olarak Nöroloji polikliniğine başağrısı yakınmasıyla başvurmuş, inme öyküsü olmayan, bilinen sistemik hastalığı olmayan yaş ve cinsiyet dağılımı benzer hastalar bu çalışmaya alınmıştır. Hasta ve kontrol grubunun NLR ve MPV değerleri kaydedildi. Ayrıca hastaların yatış sırasında ve taburculuk sırasındaki NIHSS (National Institute of Health Stroke Scale) skoru, taburculuk sırasındaki modifiye Rankin skoru (mRS) kaydedildi. Bulgular: Çalışmaya dahil edilen 72 hastanın % 58,3’ü (n=42) erkekti. Hasta ve kontrol grubu NLR ve MPV ortalamaları açısından karşılaştırılmış ve her iki parametre de hasta grubunda kontrol grubuna göre daha yüksek bulunmuştur. NLR kötü prognoz grubunda iyi prognoz grubuna göre ve mortalite grubunda sağ kalım grubuna göre daha yüksek saptanmıştır.Sonuç: Klinisyenler OSA enfarktı olan hastalarda NLR ile kısa dönem prognoz tayininde kullanabilirler ve bu kolayca belirlenebilir ölçüm, objektif, basit ve kolay ulaşılabilirdir, aynı zamanda herhangi bir ek maliyete de yol açmaz., Purpose: The aim of this study was to investigate the effect of neutrophil / lymphocyte ratio (NLR) and mean platelet volume (MPV) on short-term prognosis in patients with acute ischemic stroke who had middle cerebral artery (MCA) territory infarction.Materials and Methods: The study included patients who were admitted to a University Hospital Neurology Intensive Care and Stroke Unit with the diagnosis of acute ischemic stroke and had MCA territory infarction. The control group consisted of patients with similar age and gender distribution applied to the Neurology outpatient clinic with the complaint of headache, without history of stroke or systemic disease. NLR and MPV values of groups were recorded. In addition, the patients' NIHSS (National Institute of Health Stroke Scale) score at hospitalization and at discharge and the modified Rankin score (mRS) at discharge were recorded. Results: Of the 72 patients 58.3% (n = 42) included in the study were male. The patient and control groups were compared in terms of mean NLR and MPV values, and both parameters were higher in patient group than the control group. NLR was higher in poor prognosis group than the good prognosis group and in mortality group compared to the survival group.Conclusion: Physicians can use NLR for short-term prognosis in patients with MCA infarction, and this easily identifiable measurement is objective, simple and easily accessible, and does not cause any additional costs.

WOS: 000486138900009 Amyotrophic lateral sclerosis is a neurodegenerative disease with progressive motor neuron degeneration in primary motor cortex, corticospinal pathways, brain stem and spinal cord. The disease may start with asymmetrical weakness in the arms and legs, hyperreflexia, myolysis, cramps, twitching, fatigue, difficulty in speech and swallowing, emotional lability due to upper and lower motor neuron involvement. Sleep disorders may occur generally during the course of the disease in amyotrophic lateral sclerosis. Here a case of amyotrophic lateral sclerosis presenting with excessive daytime sleepiness was presented.

Bu araştırma; Hatay'da su ürünleri yetiştiriciliği gerçekleştiren işletmelerin sahip oldukları teknik imkânlar ve karşılaştıkları sorunları ortaya çıkarmak, elde edilecek bilgilere göre sektörün gelişmesini sağlayacak çözüm önerilerini sunmak amacıyla yapılmıştır.Araştırma bölgede Tarım İl Müdürlüğüne kayıtlı olan toplam 14 adet işletmeden faal olduğu belirtilen 12 adet işletmede anket çalışması yürütülmüştür.Hatay'da avcılık ve yetiştiricilik dahil 2016 yılında toplamda 3202 ton balık üretimi ve 125 000 adet/yıl akvaryum balığı üretimi yapılmıştır. Araştırmaya katılan işletmelerde kullanılan su kaynağı incelendiğinde %50'sinin kaynak suyundan, %25'inin ise göl veya göletten, %17'sinin yer altı suyundan ve %8'inin ise deniz suyundan yararlandığı, işletmelerin %59'u işletmesinde beton havuz kullanırken %17'si gölleri diğerlerinin ise %8 ile cam akvaryum, plastik ve toprak akvaryum kullandıkları görülmüştür.İşletmelerin ulaşım sıkıntısı çekmezken yetkili eleman bulundurmadıkları, kooperatif, depolama sorunları yaşadıkları ayrıca, işletme girdilerinde yem fiyatlarının masrafları arttırdığı sonuçlarına ulaşılmıştır. This work; The technical terms of fisheries aquaculture producers in Hatay province and the suggestions for solutions that will enable the sector to develop in order to determine and obtain the problems they are encountered have been made.A total of 14 enterprises registered in the provincial Directorate of Agriculture in the research area were allowed to conduct surveys in 12 businesses. In Hatay province for 2016 year, amount of caught fish and aquaculture production was 3202 tons per year and amount of production of aquarium fishes was 125 000 fishes per year.When studying the water resources used in the survey, it is seen that 50% of the water resources are used from the source water, 25% from the lake or pond, 17% from the groundwater and 8% from sea water, 59% 17 of them used glass aquariums, plastic and soil aquariums with lakes and 8% others.Businesses have come to the conclusion that they do not have authorized personnel when they do not have transportation problems, cooperatives are experiencing storage problems, and that feed prices have increased costs in business inputs. 67

WOS: 000426503600020 Schwartz-Jampel Syndrome is a rare disease that is characterized with skeletal deformities, joint contractures, and dysmorphic facial appearance. Most patients become symptomatic within the first decades of life. The diagnosis is based on clinical findings. Movement limitation of joints, delay in starting to walk, and walking with support are common preliminary findings. The disease is caused by the mutation in heparan sulfate proteoglycan 2 (HSPC2) gene which encodes perlecan protein. Herein we report a case of Schwartz-Jampel syndrome with the age of 18.

WOS: 000438303700004 Objectives: Sudden unexpected death in epilepsy (SUDEP) is attributed to peri-ictal cardiopulmonary abnormalities. In the present study, epilepsy patients on monotherapy (carbamazepine [CBZ], oxcarbazepine [OXZ], valproic acid [VPA]) were compared with a healthy control group to investigate the effects of epilepsy and various antiepileptic drugs on QTmax, QTmin, and QTd. Methods: Seventy eight (51.3% females) epilepsy patients with a mean age of 32.7 +/- 11.4 (range, 17-63) years and 71 (56.3% female) healthy subjects with a mean age of 36.3 +/- 13.6 (range, 18-64) years participated in the study. WOS: 000426503600014 The color change in urine may result from various factors such as drugs, stains, foods, infections, metabolic diseases and structural abnormalities. When Status Epilepticus can not be controlled with the benzodiazepine and at least one antiepileptic drug treatments, it is defined as refractory status epilepticus. Propofol, which is a drug that may be used for the treatment of refractory status epilepticus. We present a 28-year-old male with refractory status epilepticus who experienced a color change in urine to green due to propofol infusion. Green urine discoloration is a benign and very rare side effect of propofol.

In this study, identification of epilepsy prevalence, epilepsypatients’ demographical data and epilepsy risk factors in the Adanacity centre is aimed. Methods: This is a community-basedcross-sectional study, and by using “proportional stratifiedsampling method”, target population of 7052 people was identifiedwith considering Adana city center population. Three districts,reflecting general socioeconomic status, are identified by usingrandom sampling method. One doctor and two pollsters filled surveyforms by face to face interviews. When the target population of 7052people was reached, the study was terminated. Pre-diagnosed epilepsypatients and participants who are more likely to be diagnosed withthe epilepsy in the future, because of their history features, areinvited to our clinic for further surveys and investigations.Results: Out of 7052 participants who were face to faceinterviewed, 3577 of them were male (50,7%) and 3475 were female(49,3%). 52 of the participants were epilepsy patients and activeprevalence of the epilepsy was found as 0,7%. 31 of the 52 patientswere male (59,6%), 21 of the 52 patients were female (40,4%).Prevalence of the epilepsy in the high, medium and low socioeconomicstatus districts were 0,3%, 0,7% and 1,4% respectively. As thesocioeconomic level decreased, the prevalence of epilepsy increasedand the results were statistically significant. Gereç ve yöntem: Çalışma,toplum temelli kesitsel olarak planlandı ve Adana il merkezi nüfusuüzerinden ‘'orantılı tabakalı örneklem yöntemi'' kullanılarakhedef nüfus 7052 kişi olarak belirlendi. Toplumun, sosyoekonomikdüzeyini yansıtacak şekilde rastgele örneklem yöntemi ile üçmahalle belirlendi. Hazırlanan anket formları bir doktor ve ikianketör tarafından yüz yüze görüşme yöntemi ile dolduruldu.Hedef nüfus olan 7052 kişiye ulaşıldığında çalışmasonlandırıldı. Daha önceden epilepsi tanısı almış olan veyaöykü özellikleri ile epilepsi olabilecek olan grup, hazırlanandiğer anket formlarının doldurularak ileri tetkiklerin yapılmasıiçin kliniğimize davet edildi. Bulgular: Anket formudoldurulan 7052 kişinin, 3577'si erkek (%50,7), 3475'i (%49,3)kadındı. Taranan nüfus içerisinde 52 kişi epilepsi olaraktanındı ve aktif epilepsi prevalansı %0,7 olarak bulundu.Epilepsi tespit edilen grubun 31'i (%59,6) erkek 21'i (%40,4) isekadındı. Purpose: The aim of this study was to investigate the effect of neutrophil / lymphocyte ratio (NLR) and mean platelet volume (MPV) on short-term prognosis in patients with acute ischemic stroke who had middle cerebral artery (MCA) territory infarction. Materials and Methods: The study included patients who were admitted to a University Hospital Neurology Intensive Care and Stroke Unit with the diagnosis of acute ischemic stroke and had MCA territory infarction. The control group consisted of patients with similar age and gender distribution applied to the Neurology outpatient clinic with the complaint of headache, without history of stroke or systemic disease. NLR and MPV values of groups were recorded. In addition, the patients' NIHSS (National Institute of Health Stroke Scale) score at hospitalization and at discharge and the modified Rankin score (mRS) at discharge were recorded. Results: Of the 72 patients 58.3% (n = 42) included in the study were male. The patient and control groups were compared in terms of mean NLR and MPV values, and both parameters were higher in patient group than the control group. NLR was higher in poor prognosis group than the good prognosis group and in mortality group compared to the survival group. Conclusion: Physicians can use NLR for short-term prognosis in patients with MCA infarction, and this easily identifiable measurement is objective, simple and easily accessible, and does not cause any additional costs.
