Abstract

Background and aims: One of the objectives of the ESC-EORP EUROASPIRE V survey is to determine how well European guidelines on the management of dyslipidaemias are implemented in coronary patients. Methods: Standardized methods were used by trained technicians to collect information on 7824 patients from 130 centers in 27 countries, from the medical records and at a visit at least 6 months after hospitalization for a coronary event. All lipid measurements were performed in one central laboratory. Patients were divided into three groups: on high-intensity LDL-C-lowering-drug therapy (LLT), on low or moderate-intensity LLT and on no LLT. Results: At the time of the visit, almost half of the patients were on a high-intensity LLT. Between hospital discharge and the visit, LLT had been reduced in intensity or interrupted in 20.8% of the patients and had been started or increased in intensity in 11.7%. In those who had interrupted LLT or had reduced the intensity, intolerance to LLT and the advice of their physician were reported as the reason why in 15.8 and 36.8% of the cases, respectively. LDL-C control was better in those on a high-intensity LLT compared to those on low or moderate intensity LLT. LDL-C control was better in men than women and in patients with self-reported diabetes. Conclusions: The results of the EUROASPIRE V survey show that most coronary patients have a less than optimal management of LDL-C. More professional strategies are needed, aiming at lifestyle changes and LLT adapted to the need of the individual patient.

Abstract

A high-resolution X-ray diffraction method with enhanced surface sensitivity has been used to investigate the effects of various polishing steps on the near-surface region of single crystal substrates. The method involves the study of a highly asymmetric reflection, observable under grazing incidence conditions. Analysis of rocking curve measurements and reciprocal space maps (RSMs) revealed subtle structural differences between the polished substrates. For aluminum nitride wafers, damage induced from diamond sawing and mechanical polishing was readily identifiable by on-axis rocking curves, but this method was unable to distinguish between sample surfaces subjected to various degrees of chemical mechanical polishing (CMP). To characterize sufficiently these surfaces, (10.3) RSMs were measured to provide both qualitative and quantitative information about the near-surface region. Two features present in the RSMs were utilized to quantitatively assess the polished wafers: the magnitude of the diffuse scatter in the omega-scans and the elongation of the crystal truncation rod. Abstract

Chemical surface treatments were conducted on mechanically polished (MP) and chemomechanically polished (CMP) (0001)-oriented single crystalline aluminum nitride (AlN) substrates to determine a surface preparation procedure for the homoepitaxial deposition of AlN epitaxial layers by metalorganic chemical vapor deposition. MP AlN substrates characterized by atomic force microscopy exhibited 0.5 nm rms roughness and polishing scratches, while CMP AlN substrates exhibited 0.1 nm rms roughness and were scratch-free. X-ray photoelectron spectroscopy analysis of MP and CMP AlN substrates indicated the presence of a surface hydroxide layer composed of mixed aluminum oxide hydroxide and aluminum trihydroxide. Wet etching with sulfuric and phosphoric acid mixtures reduced the amount of surface hydroxide. Ammonia annealing at 1250 °C converted the substrate hydroxide layer to AlN and increased the rms roughness of MP and CMP AlN substrates to 2.2 nm and 0.2 nm, respectively. AlN epitaxial layers were deposited at 1100–1250 °C under 20 Torr total pressure with a V/III ratio of 180–300 in either N2 or H2 diluent. High-resolution x-ray diffraction measurements revealed that AlN epitaxial layers deposited on MP substrates were strained due to nucleation and coalescence of AlN grains on the mechanically damaged surfaces. AlN deposited on CMP substrates was epitaxial and strain-free. Thermodynamic models for nitridation and AlN deposition were also proposed and evaluated. [ABSTRACT FROM AUTHOR]

Abstract

A thermodynamic supersaturation model for gallium (Ga) was developed to describe GaN growth characteristics in low-pressure metalorganic chemical vapor deposition. The model takes into account the simplified GaN chemical reaction that occurs at the growth interface, Ga+NH3=GaN+3/2H2. The supersaturation was varied in two ways: (1) by the V/III ratio and (2) by the choice of the diluent gas. Two diluent gases were considered: H2, a commonly used diluent gas, and N2, a reaction inert gas. The choice of the diluent played a role in the degree of Ga supersaturation; since H2 is the product in the GaN formation, the addition of hydrogen significantly lowered the supersaturation. Atomic force microscopy revealed that surface morphology was associated with the different Ga supersaturation and the Burton–Cabrera–Frank model was used to relate it to the observed spiral size and terrace width. In addition to growth morphology, the degree of Ga supersaturation also influenced the carrier compensation level in n-type GaN. Secondary ion mass spectrometry studies identified that the trend of carbon incorporation followed that of the compensation level and inversely that of the Ga supersaturation. For samples intentionally doped with Si, it was found that GaN became semi-insulating when the carbon concentration exceeded that of silicon. In general, it has been shown that two growth processes at seemingly different conditions will be equivalent if the corresponding Ga supersaturation is the same. 34. Avances en el estudio de la regulación de la síntesis de NO en la célula mesangial

Abstract

5 p.-3 fig.-1 tab., En los últimos años, la célula mesangial ha sido objeto de intenso estudio en cuanto a su capacidad para producir óxido nítrico (NO) en determinadas condiciones. Esta sustancia actúa como un importante mediador biológico que regula gran cantidad de funciones en diversos tejidos. Desde 1990 se sabe que las células mesangiales, al igual que las células musculares lisas, son capaces de sintetizar NO tras ser tratadas con citoquinas o lipopolisacárido bacteriano . La vía genéricamente más aceptada para esta síntesis inducida de NO transcurre a través de la expresión de novo de una isoforma inducible de la óxido nítrico sintetasa (NOSi), enzima que cataliza la síntesis de NO a partir de la L-arginina. La expresión de la NOSi produce la liberación de grandes cantidades de NO, que ejerce funciones citotóxicas y citostáticas. Desde el punto de vista fisiopatológico es importante resaltar que el NO sintetizado en el glomérulo ha sido relacionado con la regulación de algunos procesos de gran relevancia homeostática, como la regulación de la hemodinámica renal y glomerular, la secreción de renina, el feedback túbulo-glomerular y la prevención de la trombosis glomerular en situaciones de isquemia o infección. Nuestro interés en el estudio de la regulación de la síntesis de NO y expresión de la NOSi en células mesangiales de rata y humanas deriva de los efectos de esta sustancia sobre la fisiología de las células mesangiales. El NO sintetizado por ellas contribuye al mantenimiento de la tasa de filtración glomerular en condiciones basales y actúa de manera autocrina, regulando la respuesta glomerular en condiciones de inflamación y endotoxemia. Una relación de los efectos del NO sobre la célula mesangial se muestra en la tabla I. Si bien el NO juega claramente un papel en las alteraciones hemodinámicas del shock séptico, la contribución relativa de las células mesangiales desde el punto de vista cuantitativo a la producción total de NO glomerular ha sido discutida por algunos autores, quienes sostienen que las células esencialmente responsables de la síntesis de NO son aquellos macrófagos glomerulares no residentes que infiltran al glomérulo durante el proceso infeccioso. Debido a la implicación del NO en la fisiopatología renal, el estudio de la regulación de la NOSi mesangial resulta imprescindible para así comprender el papel de la síntesis de NO en condiciones tanto fisiológicas como fisiopatológicas. Los problemas principales que hemos abordado son: 1. Caracterización molecular y funcional de la NOSi en células mesangiales de rata tratadas con factor de necrosis tumoral (TNF-a) y lipopolisacárido bacteriano (LPS). 2. Estudio de la inhibición de la síntesis de NO en células mesangiales de rata por antiinflamatorios y antioxidantes y sus mecanismos moleculares. 3. Caracterización de la región promotora del gen de la NOSi de rata. 4. Estudios en células mesangiales humanas. Por evidentes razones de espacio, y al haberse publicado o estar en vías de hacerlo la mayor parte de estos estudios, no podemos hacer una descripción exhaustiva de cada uno de estos apartados, limitándonos a destacar los aspectos más relevantes., Este trabajo ha sido financiado con cargo al proyecto de la DGICYT PB 93-0044.

Abstract

The nanocontact plastic behaviour of single-crystalline Ta (1 0 0), Ta (1 1 0) and Ta (1 1 1) was studied as a function of temperature and indentation rate. Tantalum, a representative body centred cubic (BCC) metal, reveals a unique deformation behaviour dominated by twinning and the generation of stacking faults. Experiments performed at room temperature exhibit a single pop-in event, while at 200 °C, above the critical temperature, a transition to multiple pop-ins was observed. The experimental results are discussed with respect to the orientation as well as temperature and correlated to the defect structures using both anisotropic finite element and MD simulations. The serrated flow observed at 200 °C is related to differences in the quasi-elastic reloading originating from changes in the defect mechanism. [ABSTRACT FROM PUBLISHER]
