1. Targeting Cognitive Resilience through Prebiotics : A Focused Perspective

Abstract

This perspective article is a product of the work of an expert group within the Prebiotic Task Force convened by the International Life Sciences Institute Europe, a non-profit organization that brings together experts from academia, industry, and public service to catalyze nutrition science for public benefit. An expert group was conceived in October 2023 to discuss the evidence base on the use of prebiotics to promote cognitive functioning, with a focus on highlighting knowledge gaps and proposing a list of recommendations to guide this specific area of research forward. To address this, we evaluated existing systematic reviews and meta-analyses of human intervention studies that examine the effects of prebiotics on cognitive functioning. These are predominantly conducted in healthy participants under basal conditions and have, to date, revealed limited effects. In this perspective, we propose that prebiotics should be investigated as agents to promote cognitive resilience by testing their effects on cognitive performance under certain cognition-taxing factors that individuals encounter across their lifespan. These include stress, poor sleep outcomes, sedentary behavior, and unhealthy dietary patterns, all of which have been shown to be associated with altered microbiome and impact global cognition or specific cognitive domains. In addition, we recommend identifying vulnerable populations that are either subclinical or that struggle chronically or periodically with 1 or more cognition-taxing factors, to better uncover the boundary conditions for prebiotic effectiveness. By broadening the scope of research to include diverse populations and challenging conditions in daily life or experimental settings, we can expand our understanding of the role of prebiotics not only in cognitive health or impairment, but also as potential preventative agents that may promote cognitive resilience during aging and in response to various lifestyle-related challenges.

3. Stroke etiology and white matter burden in women with and without migraine.

Abstract

Background: Women with migraine, especially with aura (MA), have a higher risk of white matter hyperintensities (WMH) and ischemic stroke. We aimed to assess differences in stroke etiology between women with and without migraine and the impact of migraine on WMH volume in women with stroke. Methods: We included women aged 40–60 years with a history of ischemic stroke, migraine or both. Stroke etiology was categorized using the TOAST criteria. WMH volume was measured using 3D-FLAIR images. Presence or absence of cerebellar WMH was scored. We used regression analysis to assess differences between groups, with adjustments for age, BMI, hypertension and smoking status. Results: We included 55 women with stroke, 55 with stroke and migraine, and 38 with MA. Women with stroke more often had a history of smoking than those with stroke and migraine (74% vs. 46%, p = 0.004). Stroke of undetermined origin was more common in women with both conditions than with stroke alone (49% vs. 27%, p = 0.019). Periventricular WMH volumes were higher in women with stroke with migraine than in those with MA alone (0.55mL vs. 0.42mL, B = 0.21, 95%CI = 0.01–0.41, p = 0.040). There were no differences in deep WMH volume and cerebellar WMH between groups. Importantly, the addition of migraine did not affect WMH volume in women who had experienced stroke. Conclusion: Women with both stroke and migraine more often had undetermined etiology of stroke compared to women with stroke alone, and in women with stroke alone smoking was a more prevalent risk factor. Abstract

Background: Accelerated reproductive aging, in women indicated by early natural menopause, is associated with increased coronary heart disease (CHD) risk in observational studies. Conversely, an adverse CHD risk profile has been suggested to accelerate menopause. Objectives: To study the direction and evidence for causality of the relationship between reproductive aging and (non-)fatal CHD and CHD risk factors in a bidirectional Mendelian randomization (MR) approach, using age at natural menopause (ANM) genetic variants as a measure for genetically determined reproductive aging in women. We also studied the association of these variants with CHD risk (factors) in men. Design: Two-sample MR, using both cohort data as well as summary statistics, with 4 methods: simple and weighted median-based, standard inverse-variance weighted (IVW) regression, and MR-Egger regression. Participants: Data from EPIC-CVD and summary statistics from UK Biobank and publicly available genome-wide association studies were pooled for the different analyses. Main Outcome Measures: CHD, CHD risk factors, and ANM. Results: Across different methods of MR, no association was found between genetically determined reproductive aging and CHD risk in women (relative risk estimateIVW = 0.99; 95% confidence interval (CI), 0.97-1.01), or any of the CHD risk factors. Similarly, no associations were found in men. Neither did the reversed analyses show evidence for an association between CHD (risk factors) and reproductive aging. Conclusion: Genetically determined reproductive aging is not causally associated with CHD risk (factors) in women, nor were the genetic variants associated in men. We found no evidence for a reverse association in a combined sample of women and men.

Abstract

Humans often show variable responses to dietary, prebiotic, and probiotic interventions. Emerging evidence indicates that the gut microbiota is a key determinant for this population heterogeneity. Here, we provide an overview of some of the major computational and experimental tools being applied to critical questions of microbiota-mediated personalized nutrition and health. First, we discuss the latest advances in in silico modeling of the microbiota-nutrition-health axis, including the application of statistical, mechanistic, and hybrid artificial intelligence models. Second, we address high-throughput in vitro techniques for assessing interindividual heterogeneity, from ex vivo batch culturing of stool and continuous culturing in anaerobic bioreactors, to more sophisticated organ-on-a-chip models that integrate both host and microbial compartments. Third, we explore in vivo approaches for better understanding of personalized, microbiota-mediated responses to diet, prebiotics, and probiotics, from nonhuman animal models and human observational studies, to human feeding trials and crossover interventions. We highlight examples of existing, consumer-facing precision nutrition platforms that are currently leveraging the gut microbiota. Furthermore, we discuss how the integration of a broader set of the tools and techniques described in this piece can generate the data necessary to support a greater diversity of precision nutrition strategies. Finally, we present a vision of a precision nutrition and healthcare future, which leverages the gut microbiota to design effective, individual-specific interventions.

Abstract

Cardiovascular disease (CVD) is still the leading cause of death in both sexes in Western countries with 17.7 million deaths worldwide in 2015. Sex differences are recognized more often and incidence rates show that women accounted for more new cases of CVD compared to men. Studies also show that women have similar risk factors compared to men, although their impact is different for some in women. Furthermore, female-specific risk factors receive increasing attention in cardiovascular research. This thesis aimed to study whether female-specific risk factors (menopausal status, age at menopause, vasomotor menopausal symptoms and polycystic ovary syndrome) are (causally) associated with CVD, coronary heart disease (CHD) and stroke and to assess the availability and performance of existing CVD risk prediction models for women in the general population and to establish whether the performance of a subset of these models differs between women with and without a history of hypertensive disorders of pregnancy. Our observational findings on menopause support the general believe that women have an unfavourable cardiovascular risk profile around and after menopause and that CHD risk in women increases after menopause. However, our Mendelian Randomization study showed that the association between menopause and CHD risk is likely not causal. Additionally, we did not find associations for vasomotor menopausal symptoms and polycystic ovary syndrome, and thus literature on these risk factors remains inconclusive. Therefore, it will be worthwhile to also conduct MR studies on these and other female-specific risk factors to establish causality. Although the causes and mechanisms underlying CVD risk in women are not fully understood yet, it is evident that also in women CVD is an important cause of morbidity and mortality. Therefore, it is important that we identify women at high risk and implement preventive measures when necessary. In our review we showed that an abundance of cardiovascular risk prediction models is available for the general population and many of them are developed specifically for women or included sex as a predictor. Most of these models have moderately to good performance and it has been thought that these models could improve by adding female-specific risk factors. However, several studies that implemented this method, could not confirm this hypothesis. When applying the well-known CVD risk prediction models in women with hypertensive disorders of pregnancy, they do predict higher absolute cardiovascular risks compared to women with normal pregnancies. Thus, these women might benefit from close monitoring or screening for CVD as is also advised in the American guideline for the prevention of cardiovascular disease in women. We showed that the well-known models used in the general population can also be used in women with hypertensive disorders of pregnancy and no separate model for women with hypertensive disorders of pregnancy is necessary.

Abstract

Aim:To provide a comprehensive overview of cardiovascular disease (CVD) risk prediction models for women and models that include female-specific predictors. Methods:We performed a systematic review of CVD risk prediction models for women in the general population by updating a previous review. We searched Medline and Embase up to July 2017 and included studies in which; (a) a new model was developed, (b) an existing model was validated, or (c) a predictor was added to an existing model. Results:A total of 285 prediction models for women have been developed, of these 160 (56%) were female-specific models, in which a separate model was developed solely in women and 125 (44%) were sex-predictor models. Out of the 160 female-specific models, 2 (1.3%) included one or more female-specific predictors (mostly reproductive risk factors). A total of 591 validations of sex-predictor or female-specific models were identified in 206 papers. Of these, 333 (56%) validations concerned nine models (five versions of Framingham, SCORE, Pooled Cohort Equations and QRISK). The median and pooled C statistics were comparable for sex-predictor and female-specific models. In 260 articles the added value of new predictors to an existing model was described, however in only 3 of these female-specific predictors (reproductive risk factors) were added. Conclusions:There is an abundance of models for women in the general population. Female-specific and sex-predictor models have similar predictors and performance. Female-specific predictors are rarely included. Further research is needed to assess the added value of female-specific predictors to CVD models for women and provide physicians with a well-performing prediction model for women.

Abstract

Objective: Women with premature ovarian insufficiency (POI) enter menopause before age 40. Early menopause was associated with increased risk for coronary artery disease (CAD), death from cardiovascular disease and all‐cause mortality. We compared the prevalence of CAD between middle‐aged women on average 10 years following the initial POI diagnosis, with a population‐based cohort. Design: Cross‐sectional case‐control study. Participants: Women from two Dutch University Medical Centers above 45 years of age previously diagnosed with POI (n = 98) were selected and compared with age‐ and race‐matched controls from the Multi‐Ethnic Study of Atherosclerosis (MESA). Measurements: The primary outcome was detectable coronary artery calcium (CAC) determined by coronary computed tomography (CCT). Results: Women with POI had significantly higher blood pressure, cholesterol and glucose, despite lower BMI compared to controls. Similar proportions of detectable CAC (CAC score >0 Agatston Units) were observed in women with POI and controls (POI n = 16 (16%), controls n = 52 (18%), P = 0.40 and Padj = 0.93). In women with POI separately, we were not able to identify associations between CVD risk factors and CAC. The following CVD risk factors in controls were positively associated with CAC: age, diabetes mellitus, hypertension and LDL cholesterol. HRT use was negatively associated with CAC in controls. Conclusions: The presence of CAC did not differ significantly in women with POI around 50 years of age, compared to an age‐ and race‐matched control group. We observe no increased calcified coronary disease in POI patients, despite the presence of unfavourable cardiovascular risk factors in these women. [ABSTRACT FROM AUTHOR]

Abstract

Objectives: Compare the predictive performance of Framingham Risk Score (FRS), Pooled Cohort Equations (PCEs) and Systematic COronary Risk Evaluation (SCORE) model between women with and without a history of hypertensive disorders of pregnancy (hHDP) and determine the effects of recalibration and refitting on predictive performance.Methods: We included 29 751 women, 6302 with hHDP and 17 369 without. We assessed whether models accurately predicted observed 10-year cardiovascular disease (CVD) risk (calibration) and whether they accurately distinguished between women developing CVD during follow-up and not (discrimination), separately for women with and without hHDP. We also recalibrated (updating intercept and slope) and refitted (recalculating coefficients) the models.Results: Original FRS and PCEs overpredicted 10-year CVD risks, with expected:observed (E:O) ratios ranging from 1.51 (for FRS in women with hHDP) to 2.29 (for PCEs in women without hHDP), while E:O ratios were close to 1 for SCORE. Overprediction attenuated slightly after recalibration for FRS and PCEs in both hHDP groups. Discrimination was reasonable for all models, with C-statistics ranging from 0.70-0.81 (women with hHDP) and 0.72-0.74 (women without hHDP). C-statistics improved slightly after refitting 0.71-0.83 (with hHDP) and 0.73-0.80 (without hHDP). The E:O ratio of the original PCE model was statistically significantly better in women with hHDP compared with women without hHDP.Conclusions: SCORE performed best in terms of both calibration and discrimination, while FRS and PCEs overpredicted risk in women with and without hHDP, but improved after recalibrating and refitting the models. Abstract

Introduction:Accelerated reproductive aging, in women indicated by early natural menopause, is associated with an increased risk of coronary heart disease (CHD) in observational studies. Conversely, an adverse CHD risk factor profile has been suggested to accelerate menopause. We conducted a bidirectional Mendelian randomisation (MR) study to investigate the direction and evidence for a causal nature of the relationship between reproductive aging and (non-)fatal CHD and CHD risk factors. Genomic variants for age at natural menopause (ANM) have been implicated in genome stability, immune function and mitochondrial biogenesis, which are not sex-specific processes. We used ANM variants as a measure for genetically determined reproductive aging in women and also in men, since genome-wide association studies (GWAS) for reproductive aging traits in men are lacking.Hypothesis:Reproductive ageing is causally related to CHD in both women and men.Methods:We conducted a bidirectional two-sample Mendelian Randomization, using both cohort data as well as summary statistics, with four methods: simple median-based, weighted median-based, standard inverse-variance weighted (IVW) regression, and MR-Egger regression.Exposures:Genetically determined reproductive aging, CHD and CHD risk factors. Main outcomes:CHD, CHD risk factors and age at natural menopause.Results:Summary statistics were pooled from three studies with together 417,579 participants from European descent, including 49,150 CHD cases. Publicly available GWAS and EPIC-CVD were pooled for total cholesterol, high density lipoprotein cholesterol, triglycerides, HbA1c, and glucose. Across different MR methods genetically determined reproductive aging was not causally related to CHD risk in women (Relative Risk Estimate (RRE)IVW=0.99, 95% confidence interval (CI): 0.97;1.01), or any of the CHD risk factors. Also, no associations were found in men. Furthermore, CHD was not causally related to reproductive ageing.Conclusions:Reproductive aging is not causally related to CHD risk or CHD risk factors in women, nor in men. We found no evidence for a reverse association. The associations between early menopause and CHD risk in observational studies might be the result of residual confounding, reverse causation, or reflect a shared etiology that results in both earlier menopause and higher CHD risk.

41. The Pooled Cohort Equations Over-predict Risk in Women With Hypertensive Disorders of Pregnancy, Even After Refitting the Model.

Abstract

Introduction: The AHA guidelines for the Prevention of Cardiovascular Disease (CVD) in Women describe hypertensive disorders of pregnancy (HDP) as a failed stress test, which might unmask early CVD. An abundance of prediction models for CVD risk is available for the general population, but their validity in women with HDP is not established. Hypothesis: The prognostic performance of the Pooled Cohort Equations (PCE) is lower in women with HDP compared to women without HDP and recalibrating and refitting the model will improve the prognostic performance. Methods: Data were used from 27,339 women out of the MORGEN and PROSPECT cohorts; we excluded those who had never been pregnant. In total, 5,358 answered the question: 'Did you suffer from high blood pressure during pregnancy?' with 'Yes'; and 15,266 with 'No'. Outcome definition was equal to that in the original PCE model. MORGEN and PROSPECT were analyzed separately, because of differences in characteristics (e.g. MORGEN is younger and has more current smokers) and observed risks. First, we calculated the 10-year predicted risk and compared this with the observed risk. Subsequently, the model was updated in three steps: by recalibrating the mean linear predictor, by additionally updating the baseline hazard, and by refitting the full model. The performance of all models was quantified by calibration (calibration plot, expected:observed ratio) and discrimination (c-statistic). Results: The Table shows that the original model over-predicts risk in all women, but more in women without HDP. Calibration plots improved most after refitting, which is confirmed by the expected:observed ratio, although the model still over-predicts. Refitting only improved discrimination in women with HDP, but not in women without HDP. Conclusion: The PCE over-predicts risk in women with and without HDP, even after refitting the model. Discrimination is overall quite good, except for MORGEN women without HDP. Especially in women with HDP the model discrimination benefits from refitting. [ABSTRACT FROM AUTHOR]
