18. Male involvement in prevention of mother-to-child transmission of HIV, and its association with knowledge and residence in Ethiopia: a systematic review and meta-analysis.

Abstract

Introduction: Male involvement is crucial in preventing vertical transmission of human immunodeficiency virus (HIV) among pregnant and lactating women. Different studies were conducted on male involvement in prevention of mother-to-child transmission (PMTCT) of HIV, but there is inconsistency among the findings. Thus, the aim of this systematic review and meta-analysis was to determine the pooled prevalence of male involvement in PMTCT of HIV, and its association with knowledge and residence in Ethiopia. Material and methods: Searching of articles was carried out from March 10 to April 20, 2020. Quality of each article was assessed by Newcastle-Ottawa scale for observational studies. Data were analyzed using STATA/SE version 14. Heterogeneity among the reported prevalence was assessed by computing values for x², I², and p-values. Random effects-model was applied to estimate the pooled effect. To minimize random variations among primary studies, sub-group analysis and meta-regression analysis were performed. For assessing publication bias, Egger and Begg tests were applied. Results: Sixteen research articles were included for this systematic review and meta-analysis, with 6,670 sample size. The overall pooled prevalence of male involvement in PMTCT of HIV was 37.93% (95% CI: 29.41-46.45%) in Ethiopia. Having good knowledge on HIV (OR = 4.77; 95% CI: 3.30-6.82%) and urban dwellers (OR = 2.69; 95% CI: 2.11-3.44%) were associated factors with male involvement in PMTCT. Conclusions: The level of male involvement in PMTCT of HIV in Ethiopia is low. Male involvement is influenced by knowledge on HIV and urban residence. The government of Ethiopia should give concern for rural male population to enhance their involvement in the prevention of mother-to-child HIV transmission. [ABSTRACT FROM AUTHOR]

Abstract

Background: Women who use hormonal contraception face delayed return of fertility upon discontinuation. There was limited evidence of fertility return after hormonal contraceptive discontinuation in the study area. Hence this study assessed fertility return after hormonal contraceptive discontinuation and associated factors among pregnant women attending Family Guidance Association Ethiopia (FGAE) Dessie model clinic, Northeast Ethiopia, 2019. Methods: A cross-sectional study was conducted on 423 samples selected by using systematic random sampling. Data were collected by face-to-face interview using a pretested and structured questionnaire and reviewing client records. Data were entered using Epi Data version 3.1 and analyzed using SPSS version 23. Both bi-variable and multivariable binary logistic regressions were used to identify predictors of delayed fertility return. Adjusted odds ratio (AOR) along with a 95% Confidence Interval (CI) was used to measure the strength and the direction of the association and statistical significance was declared at a P-value less than 0.05. Result: The proportion of fertility return among currently pregnant women after discontinuation of any hormonal contraceptive methods was 88.6% (95% CI; (85.6%-92%)). The proportion of fertility return among Depo-Provera, implant, Intrauterine Contraceptive Device (IUCD), and Oral Contraceptive Pill (OCP) users was 75%, 99.1%, 100%, and 97.8% respectively. Age, (AOR = 5.37, (95% CI; (1.48, 13.6)) and using Depo-Provera (AOR = 4.82, 95% CI; (1.89, 14.2)) had a significant association with delayed fertility return. Conclusions: The proportion of fertility return among women after discontinuation of any hormonal contraceptive methods was high. Age and using Depo-Provera had a positive association with delayed fertility return. This study recommends a contraceptive counseling approach that addresses concerns about delay in the return of fertility after hormonal contraceptive discontinuation to avoid confusion among family planning users. Teenage pregnancy is the main contributor to maternal and child morbidity and mortality, and the vicious cycle of ill-health and poverty. However limited evidence exists about individual- and community-level factors affecting teenage pregnancy in Ethiopia.Methods: This study used data from the 2016 Ethiopian Demographic and Health Survey (EDHS). A total of 3381 (weighted) teenagers aged 15– 19 years were included in the study. A two-stage stratified cluster was used. Data were analyzed using Stata version 14. Multilevel mixed effect logistic regression was used to identify factors affecting teenage pregnancy.Results: Being 17 (AOR=9.26, 95% CI=2.67– 32.04), 18 (AOR=9.53, 95% CI=2.97– 30.04) and 19 years old (AOR=20.01, 95% CI=5.94– 67.39), uneducated (AOR=3.83, 95% CI=1.05– 14.00), primary educated (AOR=3.34, 95% CI=1.01– 11.08), being married (AOR=70.12, 95% CI=27.55– 178.4), and communities with a higher proportion of poor (AOR=3.86, 95% CI=1.80– 8.26) were predictors of teenage pregnancy.Conclusion: Age, educational status, and marital status from individual-level factors, and community wealth status from community-level factors were predictors of teenage pregnancy. Evidence is still limited in this area and this study aimed to assess the magnitude of perceived stigma and associated factors among people living with HIV (PLWH) in public health facilities of Dessie city, Ethiopia.Methods: A health facility-based cross-sectional study was conducted among a total of 422 PLWH from September 1 to October 30, 2019 in Dessie city. A pretested structured interviewer-administered questionnaire was used to collect the data and it was analyzed using Stata/SE 14. Multivariable binary logistic regression analysis was used to identify factors associated with perceived stigma. The adjusted odds ratio (AOR) along with 95% confidence interval (CI) was estimated to measure the strength and direction of the association. Statistical significance was declared at P value less than 0.05.Results: The overall magnitude of perceived stigma among people living with HIV in public health facilities of Dessie city was 41.93% [95% CI: (37.25, 46.75)]. Perceived stigma was positively associated with being female [AOR=2.08, 95% CI: (1.26, 3.46)], living in rural areas [AOR=1.80, 95% CI: (1.10, 2.94)] and not disclosing HIV status [AOR=2.36, 95% CI: (1.19, 4.66)].Conclusion: In this study, the magnitude of perceived stigma was high as compared to UNAIDS standard, and sex, place of residence, and disclosure of HIV serostatus were significantly associated with perceived stigma. The findings suggested that special emphasis should be placed on rural residents in order to reduce perceived stigma. Moreover, counseling should focus on disclosing HIV serostatus to anyone else interested to disclose particularly for females.Keywords: stigma, sexually transmitted infections, HIV prevention, HIV, Dessie Ethiopia

Abstract

Background HIV/AIDS has been a big public health problem in sub-Saharan African countries including Ethiopia. Comprehensive knowledge is a basis for the prevention, control and treatment of HIV/AIDS. Several studies were focused only on the individual-level characteristics. However, comprehensive knowledge of HIV/AIDS is a multi-factorial understanding on a different level. Thus, the aim of this study was to identify the individual- and community-level factors that determine comprehensive knowledge of HIV/AIDS in Ethiopia. Methods This study used data from the 2016 Ethiopian Demographic and Health Survey (EDHS). A total of 25,927 (weighted) people aged 15–49 years were included in the study. A two-stage stratified cluster was used. Data were analyzed using Stata version 14. Multilevel mixed effect logistic regression was used to identify predictors of comprehensive knowledge on HIV/AIDS. Results Various individual- and community-level factors were associated with comprehensive knowledge of HIV/AIDS. From individual-level factors such as sex (male), educational status (educated), media exposure, and ever been tested for HIV, and from community-level factors such as place of residence (urban) and region (developed region) were predictors of comprehensive knowledge of HIV/AIDS. Conclusion Both individual- and community-level factors were identified as predictors of comprehensive knowledge of HIV/AIDS. The government should design strategies to address the HIV/AIDS knowledge gaps among women and other underprivileged population sub-groups.

Thus, this study aimed to assess individual and community-level factors associated with discrimination towards people living with HIV/AIDS among 15– 49 age people in Ethiopia.Methods: A secondary data analysis was done on the 2016 Ethiopian Demographic and Health Survey dataset which was collected cross-sectional. A total of 25,927 weighted 15– 49 age people were included in the analysis. Multi-level mixed-effect logistic regression analysis was done by STATA version 14.0 to identify individual and community-level factors. Adjusted odds ratio with 95% confidence interval was used to show the strength and direction of the association and statistical significance was declared at P value less than 0.05.Results: From individual level factors, being female [AOR=1.47, 95% CI= (1.18, 1.83)], not attend education [AOR=5.88,95% CI= (4.50, 7.67)], attending primary education [AOR=3.03, 95% CI= (2.40, 3.81)] and attending secondary education [AOR=1.48, 95% CI= (1.19, 1.82)] have discrimination attitude towards people living with HIV/AIDS.
