2. Prevalence of dyslipidemia and its association with cardiometabolic factors and kidney function in the adult Romanian population: The PREDATORR study

The aims were to assess the prevalence and characteristics of dyslipidemia phenotypes in a Romanian population-based sample from the PREDATORR study. Methods PREDATORR was an epidemiological study with a cross-sectional, cluster random sampling design. Participants were classified into four dyslipidemia phenotypes based on the NCEP ATP III criteria: isolated hypertrigliceridemia, isolated hypoHDL-C, isolated hyperLDL-C and mixed dyslipidemia (≥2 standard lipid abnormalities). Overall, 2656 were included in the analysis by dyslipidemia phenotypes. Results An estimated 67.1% of Romanian adults have at least one lipid abnormality: 27.5% (95%CI26.0–28.9%) have elevated TG, 29.4% (95%CI27.9–30.8%) have low HDL-C and 47.8% (95%CI46.3–49.2%) have elevated LDL-C (26.2% had LDL-C levels ≥2.58 mmol/l associated with CHD or CHD risk equivalent). Also, 30% Romanian adults have mixed dyslipidemia with 7.6% (95%CI6.1–9.0%) having all three lipid abnormalities. The age and sex-adjusted prevalence of isolated dyslipidemia phenotypes in Romanian adult population was 23.7% (95%CI22.2–25.1%) for hyperLDL-Cholesterolemia, 9.3% (95%CI7.8–10.7%) for hypoHDL-Cholesterolemia and 4.1% (95%CI2.6–5.5%) for hypertriglyceridemia. Among participants with triglycerides ≥2.25 mmol/l, 15.2% (95%CI13.7–16.6%) of Romanian adults have non-HDL-C levels ≥3.36 mmol/l. Conclusions The PREDATORR survey indicated a high prevalence of dyslipidemia phenotypes in the Romanian population aged 20–79 years, providing data on its association with several cardiometabolic risk factors.

Background The PREDATORR (PREvalence of DiAbeTes mellitus, prediabetes, overweight, Obesity, dyslipidemia, hyperuricemia and chronic kidney disease in Romania) study is the first national study analyzing the prevalence of diabetes mellitus (DM) and prediabetes, and their association with cardiometabolic, sociodemographic, and lifestyle risk factors in the Romanian population aged 20–79 years. Methods This was an epidemiological study with a stratified, cross-sectional, cluster random sampling design. Sociodemographic, lifestyle, and anamnestic data were collected through self- and interviewer-administered questionnaires, and biochemical assays and oral glucose tolerance tests were performed. Results In all, 2728 participants from 101 clinics of general practitioners were randomly selected, with a probability proportional to population size according to the 2002 Romanian Census. The participation rate was 99.6%. Impaired glucose regulation (prediabetes, known and unknown DM) was found in 28.1% of the study population. The overall age- and sex-adjusted prevalence of DM was 11.6% (95%CI 9.6%–13.6%), of which 2.4% (95%CI 1.7%–3.1%) had unknown DM. The prevalence of DM increased with age and was higher in men than in women. The age- and sex-adjusted prevalence of prediabetes was 16.5% (95%CI 14.8%–18.2%), with the highest percentage in the 60–79 year age group and in women. Obesity, abdominal obesity, dyslipidemia, low education level, and a family history of diabetes were associated with glucose metabolism disorders. Conclusions The PREDATORR study shows a high prevalence of impaired glucose regulation in the adult Romanian population, providing data on the prevalence of DM and prediabetes and their association with several risk factors.

Objectives: The Metabolic Syndrome (MetS) and obesity are among the provencauses of vascular dysfunction. The cardio-ankle vascular index (CAVI) wasdeveloped as a method to assess arterial stiffness. The aim of the study was toestablish the influence of weight loss on arterial stiffness parameters. Material and Methods: 135 subjects completed a 6 months standardized life style interventionstudy. Average initial weight was 96.63kg with a BMI of 34.34kg/m2. CAVI and theankle-brachial index (ABI) were measured and body composition was assessed.Some biochemical markers were also determined. Results: The average weightdecreased to 84.61kg, with fat mass loss of 9.6 kg. Mean CAVI decreased from7.92±1.28 to 7.21±1.08. The decrease in CAVI correlates with the total weight andfat loss rather than the speed of weight loss. Conclusions: Our results show thatweight loss can influence arterial stiffness, mainly by decreasing fat tissue mass.

The insulin -23Hph and IGF2 Apa polymorphisms were genotyped in Romanian patients with T1DM (n = 204), T2DM (n = 215) or obesity (n = 200) and normoponderal healthy subjects (n = 750). The genotypes of both polymorphisms were distributed in concordance with Hardy-Weinberg equilibrium in all groups. The -23Hph AA genotype increased the risk for T1DM (OR: 3.22, 95%CI: 2.09-4.98, p < 0,0001), especially in patients without macroalbuminuria (OR: 4.32, 95%CI: 2.54-7.45, p < 0,0001). No other significant association between the alleles or genotypes of insulin -23Hph and IGF2 Apa and diabetes or obesity was identified.

6. Improvements in life expectancy in type 1 diabetes patients in the last six decades

To investigate the survival with diabetes in patients treated with insulin from diagnosis.We analyzed 845 subjects, 55.9% males, registered at "I. Pavel" Bucharest Diabetes Centre, insulin-treated from diagnosis, aged40 years and deceased between 1946 and 2005. We divided the subjects in two groups by age at diagnosis: group A18 years and group B 18-39.99 years. We used 20 years time periods for year of death: 1946-1965, 1966-1985 and 1986-2005.The mean age at diabetes onset was 30.36+/-8.04 years, disease duration at death 20.98+/-11.62 years and age at death 51.34+/-14.37 years. The mean increase in survival with diabetes was 19.3 years for group A and 15.9 years for group B. There was a significant decrease in infections in both groups. The increase in coronary heart diseases and stroke is evident only in group B.We found no changes in age at onset, which combined with an increase in survival with diabetes lead to a significant increase in age at death over the six decades analyzed.

Glimepiride is an oral sulfonylurea drug; nicotinamide is an inhibitor of poly (ADP-ribose) synthetase and a precursor of NAD. Three studies were carried out to determine whether glimepiride and/or nicotinamide could prevent diabetes in BB rats. In Study I, we administered glimepiride treatment 200 mg/kg/day orally from the 35th to 143rd day of age. The incidence of diabetes in the glimepiride group was lower than in the control group (32% vs 55%, p < 0.02). In Study II, the treatment period was from the 35th to 147th day of age, and rats received glimepiride combined with nicotinamide (500 mg/kg/day IP). The treatment group showed a 22% incidence of diabetes compared to 53 in controls (p < 0.03). In Study III, nicotinamide treatment alone (1000 mg/kg/day orally) and combined with glimepiride were compared to untreated controls. The treatment period was from the 35th to 167th day of age. Nicotinamide-treated rats showed a 42% incidence of diabetes compared to 60% in controls (p = NS). In the nicotinamide combined with glimepiride treatment group, a lower incidence (28%) was observed when compared to the controls (60%, p < 0.05). These findings suggest that glimepiride can prevent diabetes in BB rats; however, nicotinamide when used in young animals shows only a trend to lower the incidence of diabetes, and when combined with glimepiride, no significant effect is observed.

The present study investigated the possible involvement of stress in the aetiopathogenesis of type 2 diabetes mellitus by using three different approaches: the role of chronic and subacute stress, assessed by a clinical interview in 300 newly discovered diabetic patients: the role of acute stress, by analysing the incidence of diabetes mellitus in connection with the earthquake of 4 March 1977; and investigating the autonomic reactivity of patients in whom stress was or was not involved in the onset of diabetes. The results showed: a chronic psychosocial stress was encountered in 44 per cent of 300 newly discovered type 2 diabetes mellitus patients: the number of newly registered diabetic cases was significantly (p < 0.001) higher in the month of May in 1977 (two months after the earthquake) as against the years preceding (1976) or following (1978) the earthquake, an autonomic hypereactivity was found in the patients in whom stress was involved in the onset of diabetes as against the controls or diabetics in whom this factor was not involved in the onset of the disease.
