1. Compromise or choose: shared movement decisions in wild vulturine guineafowl

Abstract

Abstract Shared-decision making is beneficial for the maintenance of group-living. However, little is known about whether consensus decision-making follows similar processes across different species. Addressing this question requires robust quantification of how individuals move relative to each other. Here we use high-resolution GPS-tracking of two vulturine guineafowl (Acryllium vulturinum) groups to test the predictions from a classic theoretical model of collective motion. We show that, in both groups, all individuals can successfully initiate directional movements, although males are more likely to be followed than females. When multiple group members initiate simultaneously, follower decisions depend on directional agreement, with followers compromising directions if the difference between them is small or choosing the majority direction if the difference is large. By aligning with model predictions and replicating the findings of a previous field study on olive baboons (Papio anubis), our results suggest that a common process governs collective decision-making in moving animal groups.

Abstract

Low acyl type gellan gum (GG) is a biocompatible gelling agent with lower degradation rates compared to its high acyl counterpart. This study investigated the hypothesis whether the incorporation of various low acyl GG concentrations into a constant concentration of poly(vinyl alcohol) (PVA) would facilitate the electrospinning towards the production of stable nanofibrous PVA-GG membranes, and how thermal treatment would affect the degradation rate of the membranes rendering them suitable as flexible scaffolds for bone tissue engineering and avoiding crosslinking agents. All compositions displayed fiber diameters ranging from 70 to 200 nm. Thermally treated membranes indicated a mass loss of 20–32 %, while non-treated ones 32–54 %, with the higher GG concentrations exhibiting lower degradation rates, and swelling capacity of at least 500 %. Rheological analysis illustrated differences in gelling capacity, ranging from fluid-like to critical gelation state with increasing GG content, with G′ values ranging between 3.5 ± 0.5 Pa for the lowest and 23.4 ± 1.5 for the highest GG concentration. Proliferation of strongly adhered pre-osteoblasts significantly increased over culture time. Osteogenic markers including alkaline phosphatase activity and calcium deposition were significantly upregulated compared to the control, and calcium secretion gradually increased up to three weeks, validating the capacity of the scaffolds to support osteogenesis.

Abstract

Abstract GPS‐based tracking is widely used for studying wild social animals. Much like traditional observational methods, using GPS devices requires making a number of decisions about sampling that can affect the robustness of a study's conclusions. For example, sampling fewer individuals per group across more distinct social groups may not be sufficient to infer group‐ or subgroup‐level behaviours, while sampling more individuals per group across fewer groups limits the ability to draw conclusions about populations. Here, we provide quantitative recommendations when designing GPS‐based tracking studies of animal societies. We focus on the trade‐offs between three fundamental axes of sampling effort: (1) sampling coverage—the number and allocation of GPS devices among individuals in one or more social groups; (2) sampling duration—the total amount of time over which devices collect data and (3) sampling frequency—the temporal resolution at which GPS devices record data. We first test GPS tags under field conditions to quantify how these aspects of sampling design can affect both GPS accuracy (error in absolute positional estimates) and GPS precision (error in the estimate relative position of two individuals), demonstrating that GPS error can have profound effects when inferring distances between individuals. We then use data from whole‐group tracked vulturine guineafowl Acryllium vulturinum to demonstrate how the trade‐off between sampling frequency and sampling duration can impact inferences of social interactions and to quantify how sampling coverage can affect common measures of social behaviour in animal groups, identifying which types of measures are more or less robust to lower coverage of individuals. Finally, we use data‐informed simulations to extend insights across groups of different sizes and cohesiveness. Based on our results, we are able to offer a range of recommendations on GPS sampling strategies to address research questions across social organizational scales and social systems—from group movement to social network structure and collective decision‐making. Our study provides practical advice for empiricists to navigate their decision‐making processes when designing GPS‐based field studies of animal social behaviours, and highlights the importance of identifying the optimal deployment decisions for drawing informative and robust conclusions.

Abstract

Abstract Background A challenge faced by animals living in groups with stable long-term membership is to effectively coordinate their actions and maintain cohesion. However, as seasonal conditions alter the distribution of resources across a landscape, they can change the priority of group members and require groups to adapt and respond collectively across changing contexts. Little is known about how stable group-living animals collectively modify their movement behaviour in response to environment changes, such as those induced by seasonality. Further, it remains unclear how environment-induced changes in group-level movement behaviours might scale up to affect population-level properties, such as a population’s footprint. Methods Here we studied the collective movement of each distinct social group in a population of vulturine guineafowl (Acryllium vulturinum), a largely terrestrial and non-territorial bird. We used high-resolution GPS tracking of group members over 22 months, combined with continuous time movement models, to capture how and where groups moved under varying conditions, driven by seasonality and drought. Results Groups used larger areas, travelled longer distances, and moved to new places more often during drier seasons, causing a three-fold increase in the area used at the population level when conditions turned to drought. By contrast, groups used smaller areas with more regular movements during wetter seasons. Conclusions The consistent changes in collective outcomes we observed in response to different environments raise questions about the role of collective behaviour in facilitating, or impeding, the capacity for individuals to respond to novel environmental conditions. As droughts will be occurring more often under climate change, some group living animals may have to respond to them by expressing dramatic shifts in their regular movement patterns. These shifts can have consequences on their ranging behaviours that can scale up to alter the footprints of animal populations.

Abstract

Summary: Background: Dexamethasone was the first intervention proven to reduce mortality in patients with COVID-19 being treated in hospital. We aimed to evaluate the adoption of corticosteroids in the treatment of COVID-19 in the UK after the RECOVERY trial publication on June 16, 2020, and to identify discrepancies in care. Methods: We did an audit of clinical implementation of corticosteroids in a prospective, observational, cohort study in 237 UK acute care hospitals between March 16, 2020, and April 14, 2021, restricted to patients aged 18 years or older with proven or high likelihood of COVID-19, who received supplementary oxygen. The primary outcome was administration of dexamethasone, prednisolone, hydrocortisone, or methylprednisolone. This study is registered with ISRCTN, ISRCTN66726260. Findings: Between June 17, 2020, and April 14, 2021, 47 795 (75·2%) of 63 525 of patients on supplementary oxygen received corticosteroids, higher among patients requiring critical care than in those who received ward care (11 185 [86·6%] of 12 909 vs 36 415 [72·4%] of 50 278). Patients 50 years or older were significantly less likely to receive corticosteroids than those younger than 50 years (adjusted odds ratio 0·79 [95% CI 0·70–0·89], p=0·0001, for 70–79 years; 0·52 [0·46–0·58], p80 years), independent of patient demographics and illness severity. 84 (54·2%) of 155 pregnant women received corticosteroids. Rates of corticosteroid administration increased from 27·5% in the week before June 16, 2020, to 75–80% in January, 2021. Interpretation: Implementation of corticosteroids into clinical practice in the UK for patients with COVID-19 has been successful, but not universal. Patients older than 70 years, independent of illness severity, chronic neurological disease, and dementia, were less likely to receive corticosteroids than those who were younger, as were pregnant women. This could reflect appropriate clinical decision making, but the possibility of inequitable access to life-saving care should be considered. Abstract

Summary: We identify amino acid variants within dominant SARS-CoV-2 T cell epitopes by interrogating global sequence data. Several variants within nucleocapsid and ORF3a epitopes have arisen independently in multiple lineages and result in loss of recognition by epitope-specific T cells assessed by IFN-γ and cytotoxic killing assays. Complete loss of T cell responsiveness was seen due to Q213K in the A∗01:01-restricted CD8+ ORF3a epitope FTSDYYQLY207-215; due to P13L, P13S, and P13T in the B∗27:05-restricted CD8+ nucleocapsid epitope QRNAPRITF9-17; and due to T362I and P365S in the A∗03:01/A∗11:01-restricted CD8+ nucleocapsid epitope KTFPPTEPK361-369. CD8+ T cell lines unable to recognize variant epitopes have diverse T cell receptor repertoires. These data demonstrate the potential for T cell evasion and highlight the need for ongoing surveillance for variants capable of escaping T cell as well as humoral immunity.

Abstract

Objectives The steroid hormone vitamin D has roles in immunomodulation and bone health. Insufficiency is associated with susceptibility to respiratory infections. We report 25-hydroxy vitamin D (25(OH)D) measurements in hospitalised people with COVID-19 and influenza A and in survivors of critical illness to test the hypotheses that vitamin D insufficiency scales with illness severity and persists in survivors.Design Cross-sectional study.Setting and participants Plasma was obtained from 295 hospitalised people with COVID-19 (International Severe Acute Respiratory and emerging Infections Consortium (ISARIC)/WHO Clinical Characterization Protocol for Severe Emerging Infections UK study), 93 with influenza A (Mechanisms of Severe Acute Influenza Consortium (MOSAIC) study, during the 2009–2010 H1N1 pandemic) and 139 survivors of non-selected critical illness (prior to the COVID-19 pandemic). Abstract

A challenge of group-living is to maintain cohesion while navigating through heterogeneous landscapes. Larger groups benefit from information pooling, translating to greater ‘collective intelligence’, but face increased coordination challenges. If these facets interact, we should observe a non-linear relationship between group size and collective movement. We deployed high-resolution GPS tags to vulturine guineafowl from 21 distinct social groups and used continuous-time movement models to characterize group movements across five seasons. Our data revealed a quadratic relationship between group size and movement characteristics, with intermediate-sized groups exhibiting the largest home-range size and greater variation in space use. Intermediate-sized groups also had higher reproductive success, but having more young in the group reduced home-range size. Our study suggests the presence of an optimal group size, and composition, for collective movement.

11. Seasonal variation in the epidemiology of sepsis.

Abstract

Objective: We sought to investigate seasonal and regional variability in the epidemiology of sepsis and to identify underlying associations based on geography and seasonal viral infections. Understanding seasonal or regional variations may improve knowledge of sepsis epidemiology and pathophysiology and could affect healthcare planning and resource allocation.Design: Retrospective cohort study using the National Hospital Discharge Survey to identify cases of sepsis, severe sepsis, influenza, and viral pneumonia using ICD-9-CM codes. Incidence rates are reported as mean cases frequencies per season per 100,000 as calculated by normalization to the 2000 U.S. Census.Setting: Acute-care nonfederal U.S. hospitals.Patients: Patients hospitalized between 1979 and 2003 in acute-care nonfederal U.S. hospitals with a diagnosis of sepsis or viral respiratory infection.Interventions: None.Measurements and Main Results: The seasonal incidence rate of sepsis increased 16.5% from a low of 41.7 in the fall to a high of 48.6 cases per 100,000 in the winter (p<.05). Similarly, seasonal rates for severe sepsis statistically increased 17.7% from fall to winter at 13.0 and 15.3 cases per 100,000, respectively. The greatest change in sepsis incidence occurred with respiratory sources, increasing 40% during the winter compared with the fall (p<.05). Seasonal variations in viral respiratory infections paralleled changes in sepsis incidence but did not fully account for the changes. The greatest seasonal change in sepsis rates occurred in the Northeast (+30%). Sepsis case-fatality rates were 13% greater in the winter compared with the summer (p<.05) despite similar severity of illness.Conclusions: The incidence and mortality of sepsis and severe sepsis are seasonal and consistently highest during the winter, predominantly related to respiratory sepsis. Abstract

Background: Employee job satisfaction and its relationship with health and quality of life has been an issue of major concern over the past decades. Nurses experience difficult working conditions that affect their job satisfaction, health, and quality of life. Methods: A cross-sectional study was undertaken in three general hospitals and their respective health centers. Stratified random sampling by level of education was used, and 508 nurses and nursing assistants were included. A self-administered anonymous questionnaire, which included the Measure of Job Satisfaction, the 36-item Short Form Health Survey, as well as demographic details, education, and work conditions data, was used. Results: Greek nurses were found to be dissatisfied with their job according to the total score of the job satisfaction scale, although personal satisfaction and satisfaction with support had had higher scores. Their general health was reported as average, because of physical and mental health problems, low vitality, low energy, and increased physical pain. Multivariate linear regression analysis revealed that males and those wishing to stay in the job had higher physical and mental health. Increased job satisfaction was related to increased physical and mental health. Conclusion: Although Greek nurses are not satisfied with their work, those with high levels of job satisfaction had better health-related quality of life. The findings suggest that improvement of the work environment would contribute to a healthier and more satisfied nursing workforce.

Abstract

The purpose of this study was to investigate the effectiveness of a resorbable alloplastic in situ hardening bone grafting material for alveolar ridge preservation in a swine model. Seven Landrace pigs were used. In each animal, the maxillary left and right deciduous second molars were extracted, and extraction sites were either grafted with a resorbable alloplastic in situ hardening bone substitute, composed of beta-tricalcium phosphate (β-TCP) granules coated with poly(lactic-co-glycolic) acid (PLGA), or left unfilled to heal spontaneously. Animals were euthanized after 12 weeks, and the bone tissue was analyzed histologically and histomorphometrically. Linear changes of ridge width were also clinically measured and analyzed. Pronounced bone regeneration was found in both experimental and control sites, with no statistically significant differences. At the experimental sites, most of the alloplastic grafting material was resorbed and remnants of the graft particles were severely decreased in size. Moreover, experimental sites showed, in a statistically nonsignificant way, less mean horizontal dimensional reduction of the alveolar ridge (7.69%) compared to the control sites (8.86%). In conclusion, the β-TCP/PLGA biomaterial performed well as a biocompatible resorbable in situ hardening bone substitute when placed in intact extraction sockets in this animal model.

Abstract

Background: Pulmonary edema is a cardinal feature of the life-threatening condition known as acute respiratory distress syndrome (ARDS). Patients with chronic alcohol abuse are known to be at increased risk of developing and dying from ARDS. Based upon preclinical data, we hypothesized that a history of chronic alcohol abuse in ARDS patients is associated with greater quantities and slower resolution of pulmonary edema compared with ARDS patients without a history of alcohol abuse., Methods: A PiCCO transpulmonary thermodilution catheter was inserted into 35 patients within 72 hours of meeting American European Consensus Criteria definition of ARDS. Pulmonary edema was quantified as extravascular lung water (EVLW) and measured for up to 7 days in 13 patients with a history of chronic alcohol abuse and 22 patients without a history of chronic alcohol abuse., Results: Mean EVLW was higher in patients with a history of chronic alcohol abuse (16.6 vs. 10.5 ml/kg, p < 0.0001). Patients with alcohol abuse had significantly greater EVLW over the duration of the study (RM-ANOVA p = 0.003). There was a trend towards slower resolution of EVLW in patients with a history of alcohol abuse (a decrease of 0.5 ml/kg vs. 2.4 ml/kg, p = 0.17) over the study period. A history of alcohol abuse conferred a greater than 3-fold increased risk of elevated EVLW [OR 3.16, (1.26 to 7.93)] using multivariate logistic regression analysis., Conclusions: In patients who develop ARDS, alcohol abuse is associated with greater levels EVLW and a trend towards slower resolution of EVLW. Combined with mechanistic and preclinical evidence linking chronic alcohol consumption and ARDS, targeted therapies should be developed for these patients.

Abstract

Objectives: Measurements of extravascular lung water (EVLW) correlate to the degree of pulmonary edema and have substantial prognostic information in critically ill patients. Prior studies using single indicator thermodilution have reported that 21% to 35% of patients with clinical acute respiratory distress syndrome (ARDS) have normal EVLW (<10 mL/kg). Given that lung size is independent of actual body weight, we sought to determine whether indexing EVLW to predicted or adjusted body weight affects the frequency of increased EVLW in patients with ARDS., Design: Prospective, observational cohort study., Setting: Medical and surgical intensive care units at two academic hospitals., Patients: Thirty patients within 72 hrs of meeting American-European Consensus Conference definition of ARDS and 14 severe sepsis patients without ARDS., Interventions: None., Measurement and Main Results: EVLW was measured for 7 days by PiCCO transpulmonary thermodilution; 225 measurements of EVLW indexed to actual body weight (ActBW) were compared with EVLW indexed to predicted body weight (PBW) and adjusted body weight (AdjBW). Mean EVLW indexed to ActBW was 12.7 mg/kg for ARDS patients and 7.8 mg/kg for non-ARDS sepsis patients (p < .0001). In all patients, EVLW increased an average of 1.1 +/- 2.1 mL/kg when indexed to AdjBW and 2.0 +/- 4.1 mL/kg when indexed to PBW. Indexing EVLW to PBW or AdjBW increased the proportion of ARDS patients with elevated EVLW (each p < .05) without increasing the frequency of elevated EVLW in non-ARDS patients. EVLW indexed to PBW had a stronger correlation to Lung Injury Score (r2 = .39 vs. r2 = .17) and PaO2/FiO2 ratio (r2 = .25 vs. r2 = .10) than did EVLW indexed to ActBW., Conclusions: Indexing EVLW to PBW or AdjBW reduces the number of ARDS patients with normal EVLW and correlates better to Lung Injury Score and oxygenation than using ActBW. Studies are needed to confirm the presumed superiority of this method for diagnosing ARDS and to determine the clinical treatment implications.

Study Objectives: To evaluate the longitudinal epidemiology of sepsis in patients with a history of cancer and to specifically examine sepsis-related disparities in risk or outcome., Design: Sepsis cases from 1979 through 2001 using a nationally representative sample of nonfederal acute-care hospitalizations in the United States (the National Hospital Discharge Survey) integrated with cancer prevalence from the Surveillance, Epidemiology, and End Results database., Setting: Eight hundred fifty-four million acute-care hospitalizations and 8.9 million patients with cancer., Patients or Participants: Patients with a history of cancer hospitalized with a diagnosis of sepsis., Measurements and Results: From 1979 to 2001, there were a total of 1,781,445 cases of sepsis in patients with cancer, yielding a mean annual incidence rate of 1,465 cases per 100,000 cancer patients and a relative risk [RR] of 9.77 compared to noncancer patients (95% confidence interval [95% CI], 9.67 to 9.88). In contrast to the absolute number of cases, the incidence rate of sepsis decreased over time, from a peak of 1,959 cases per 100,000 cancer patients in 1987 to 995 cases per 100,000 in 2001. The distribution of infectious sources causing sepsis was associated with the type of malignancy. White cancer patients had a lower risk for sepsis compared to nonwhites (African-American RR, 1.28; 95% CI, 1.16 to 1.40) and other races (RR, 1.47; 95% CI, 1.22 to 1.72); and male cancer patients had a higher risk for sepsis compared to female cancer patients (male RR, 1.17; 95% CI, 1.10 to 1.23). Cancer was an independent predictor of death among sepsis patients by multivariable analysis (adjusted odds ratio for death, 1.98; 95% CI, 1.97 to 1.99)., Conclusions: Patients with a history of cancer are at increased risk for acquiring and subsequently dying from sepsis, compared to the general population, although incidence and fatality rates are decreasing over time. There are significant racial and gender disparities in the incidence and outcome of sepsis among cancer patients that require explanation.
