The spread of antibiotic-resistant pathogenic bacteria is an increasing health issue worldwide. A possible origin of antibiotic resistance could be the persistence of antibiotics in the aquatic environment. To tackle this problem, restricted control of application of antibiotics in human and veterinary medicine has been proposed. However, these measures do not prevent the spreadof antibiotic resistance but may delay the process, since antibiotics are still continuously emitted into the environment and many persist there. Derived from ciprofloxacin (CIP), CIP-Hemi, a fluoroquinolone with improved environmental properties, was developed following the benign by design approach and using insilico and in vitro methods. CIP-Hemi was designed to maintain its required metabolic stability (human liver microsomes, intestinal microsomes, blood plasma) and antibiotic activity (MIC in the μg mL−1 range) against the target while transforming into an inactive fragment (CIP-d-CP) and a degradable linker present under acidic conditions, e.g., after excretion or when released into theenvironment. Moreover, CIP-Hemi and CIP-d-CP showed weaker cytotoxic and mutagenic or genotoxic effects compared to the parent compound CIP and therefore underline the feasibility of CIP-Hemi as a viable antibiotic drug candidate, demonstrating benign by design as a promising approach.

In this work, a computational fluid dynamics (CFD) analysis is conducted on the periodical domain of a CO2 centrifugal compressor impeller and vaneless diffuser, with a 2.85:1 pressure ratio, using ANSYS CFX. The fluid flow is assumed to be steady state, turbulent, and three dimensional. Since polar angles are not considered in the traditional one-dimensional analysis, eight polar angles on hub and shroud were considered for modeling, sensitivity analysis, and optimization. After numerical verification and validation, a sequential sensitivity analysis (SA) was performed to identify non-influential variables. From the same design of experiment (DoE) used by the Morris qualitative SA, a response surface (RS) was trained to perform a quantitative SA by the smoothing spline ANOVA (SS-ANOVA) method. The Morris method was found to be more conservative than SS-ANOVA, keeping more variables as influent for the analysis. Both methods agreed on influential variables ranking. Low computational effort was required to submit the RS to a constrained optimization procedure using the NSGA-II method. The polytropic efficiency of the optimal centrifugal compressor configuration increased 0.7%, keeping the pressure ratio above 2.85, and the required power and outlet temperature below the base compressor. The impact of the polar angles at trailing edge on output variables is higher than leading edge. The optimal centrifugal compressor found is submitted to different mass flow rates and the overall performance for the optimal angles of the trailing edge and leading edge of the impeller was higher than the base compressor. The strategy adopted herein related to qualitative and quantitative sensitivity analysis coupled with response surface and the constrained optimization was shown to be robust, which can be applied to high-dimensional CFD models to reduce the computational cost with suitable results regarding fluid flow phenomena. The present optimization study encompasses the variation of Reynolds number (5000 and 10,000) and of three geometric parameters for each row of rectangular winglets such as chord of the winglets ranging from 8 mm to 40 mm; height of winglets from 8 mm to 21 mm; and angles of attack from 15° to 45° (Flow-Up) and from −45° to −15° (Flow-Down). The Pareto frontiers show that periodic configurations are suboptimal solutions, i.e., the thermo-hydraulic performance of a solar air heater with rectangular winglet pairs not periodically arranged along the channel outperform the periodic arrangements. The aspect ratios and angles of attack for non-periodic and periodic optimized designs are completely different from each other. Moreover, the optimized solutions for Flow-Down arrangement present higher performance than Flow-Up, and this behavior is independent of the Reynolds number. Complex flow patterns and heat transfer mechanisms of the non-periodic optimized solutions are observed, especially the interactions of corner and main vortices along the duct. The first row of winglet pair play the most important role on pressure penalty and heat transfer enhancement when compared to the other rows of rectangular winglet pairs, mainly on Flow-Up arrangement at Re = 10,000. In general, Flow-Up arrangements create a vortex structure more intense than Flow-Down arrangement. In this paper, I develop an overlapping generations model to analyze the effects of property transfer taxes on homeownership, residential mobility, and welfare in the Netherlands. A revenue-neutral abolition of the 2% transfer tax increases the likelihood that homeowners sell their old house and buy a new one by about 40%. It also leads to a rise of the homeownership rate by 1-5 percentage points (depending on how revenue neutrality is achieved). Newborns prefer to live in an economy without property transfer taxes if the forgone tax revenues are replaced with higher annual property taxes, but not if revenue neutrality is achieved with higher income taxes. I also consider a partial reform that only exempts young first-time homebuyers from the transfer tax and is financed with higher annual property taxes. The resulting welfare gains are approximately one half of the welfare gains from the complete reform.

The ability to form biofilms is a recognized trait of Stenotrophomonas maltophilia, but the extent of its clinical relevance is still unclear. The present multicenter prospective study (ANSELM) aims at investigating the association between biofilm formation and clinical outcomes of S. maltophilia infections. One hundred and nine isolates were collected from various geographical origins and stratified according to their clinical relevance. Biofilm formation was evaluated by the microtiter plate assay and correlated with microbiological and clinical data from the associated strains. Antibiotic susceptibility of the planktonic cells was tested by the disk diffusion technique, while antibiotic activity against mature biofilms was spectrophotometrically assessed. Most strains (91.7%) were able to form biofilm, although bloodborne strains produced biofilm amounts significantly higher than strains causing hospital- rather than community-acquired infections, and those recognized as &ldquo, definite&rdquo, pathogens. Biofilm formation efficiency was positively correlated with mechanical ventilation (p = 0.032), whereas a negative relationship was found with antibiotic resistance (r2 = 0.107, p &lt, 0.001), specifically in the case of the pathogenic strains. Mature S. maltophilia biofilms were markedly more resistant (up to 128 times) to cotrimoxazole and levofloxacin compared with their planktonic counterparts, especially in the case of bloodborne strains. Our findings indicate that biofilm formation by S. maltophilia is obviously a contributing factor in the pathogenesis of infections, especially in deep ones, thus warranting additional studies with larger cohort of patients and isolates.

Reservoir fluid composition and operating parameters of FPSOs are known to be important parameters for the total energy demand, and also for enhancing overall oil production. In the current research paper, the contribution of eight thermodynamic input parameters of a typical Brazilian FPSO operating on a pre-salt field is quantified by the Smoothing Spline ANOVA method for two responses: (i) energy consumption of the plant and; (ii) overall oil production. Three operation modes are considered in the analysis: Mode 1 (maximum oil/gas content), Mode 2 (50% BS&W) and Mode 3 (maximum water/CO2 content). The input parameters considered in the present research are: output pressure of the first control valve (P-1), output pressure of the second stage of separation train (P-2), input pressure of the third stage of separation train (P-3), output pressure of dilution water pump (P-4), output pressure of the main gas compressor (P-c), petroleum temperature of the first heat exchanger (T-1), petroleum temperature of the second heat exchanger (T-2), and dilution water temperature of heat exchanger (T-3) are considered as input parameters. In terms of energy consumption, parameters P-3 and P-c are always relevant for all operation modes. Furthermore, P-3, P-2, and T-2 are the most important parameters for overall oil production for all operation modes. Since productive performance in dairy cows is directly linked to the manner in which cows are raised, the breeding system can have a direct influence on the parameters of a farm. Unlike the main European and North American countries, in southern Brazil the use of confinement systems with access to pasture is widespread. Grazing cows are believed to be less stressed from the metabolic point of view. This study was conducted on nine commercial dairy farms located in the Serra Gaúcha, Rio Grande do Sul, Brazil, from September 2016 to November 2017. A total of 393 Holstein dairy cows were included immediately after birth. The dairy farms had an average of 45 lactating cows and were milked twice a day with an average milk production in 305 days of 9,500 kg / cow for the confinement system with access to pasture and 10,700 kg / cow for the confined system. The confinement system with access to pasture (CAP) was composed of five properties and was characterized by access to rotating pasture, while the confinement system without access to pasture (CNP) was composed of four properties. The study diagnosed uterine diseases, metabolic diseases and their correlations, and assessed the conception rate in the first service of the herds. Animals with access to pasture had generally lower production rates than animals reared in confinement, which suggests that the model with access to pasture used in the extreme south of the country can result in deleterious effects on animal production.

International audience; The Electrical Submersible Pumping (ESP) system is one of the most commonly used artificial lift methods in the petroleum industry and one recent breakthrough in this system is enclosed in a capsule and positioned on the seabed in a skid. As it is a recent technology, with only a small amount of equipment currently in operation, there is a clear need for a greater understanding of the flow within this geometry with the objective of perfecting the design of this promising submarine boosting system. This paper presents a numerical investigation of single-phase flow within the scaled capsule-intake of an ESP system in the Skid considering a model with geometric and dynamic similarities in relation to a real equipment in operation in the Espadarte field, located in the Campos Basin, Brazil. The tridimensional and transient simulation for a case for one mass flow rate and inclination angles are investigated. While neglecting the effects of the diffuser and impeller in the system, the flow field features, axial and radial velocity profiles in the intake region were computed. The numerical results show that the flow in the intake region is dominated by the secondary flow, leading to the formation of bathtub vortex. It is expected that the vortices influence the flow pattern in the intake region, breaking the larger bubbles into smaller bubbles, making the transition from the slug flow pattern to the dispersed bubbles or bubbly pattern in which it would be more difficult to be dragged into the intake.

The compression systems used for this task work with CO2 in supercritical conditions, and the equipment used is energy intensive. The application of an optimization procedure designed to find the optimum operating conditions leads to reduced energy consumption, lower exergy destruction, and reduced CO2 emissions. First, this work presents two thermodynamic models to estimate the amount of power necessary for a multi-stage CO2 compression system in floating production storage and offloading (FPSO) using accurate polytropic relationships and equations of state. Second, a thermodynamic analysis using the first and second laws of thermodynamics is conducted to identify possible improvements in energy consumption and the sources of the compression unit’s irreversibilities. In the final step, optimization procedures, using two methods with different approaches, are implemented to minimize the total power consumption. As the number of stages and the pressure drop between them influence the total power required by the compressors, these are considered as the input parameters used to obtain the inlet pressure at each stage. Three different compositions with variations in CO2 content, i.e., pure CO2, pure CH 4, and 70% CO2 + 30% CH 4, are also investigated as three different operating scenarios. The optimal configurations and pressure ratios result in a reduction in power consumption of up to 9.65%, mitigation of CO2 emissions by up to 1.95 t/h, and savings in exergy loss of up to 23.9%, when compared with conventional operating conditions. Background Mobile health (mHealth) and web-based technological advances allow for new approaches to deliver behavioral interventions for chronic diseases such as obesity and diabetes. African American and Hispanic adults experience a disproportionate burden of major chronic diseases. Objective This paper reviews the evidence for mHealth and web-based interventions for diabetes and obesity in African American and Hispanic adults. Methods Literature searches of PubMed/Medline, The Cochrane Library, EMBASE, CINAHL Plus, Global Health, Scopus, and Library & Information Science Source were conducted for relevant English-language articles. Articles identified through searches were reviewed by 2 investigators and, if they met the inclusion criteria, were extracted and assessed for risk of bias. Findings were summarized in tabular and narrative format. The overall strength of the evidence was assessed as high, moderate, low, or insufficient on the basis of risk of bias, consistency of findings, directness, precision, and other limitations. Results Searches yielded 2358 electronic publications, 196 reports were found to be eligible for inclusion, and 7 studies met the eligibility criteria. All 7 included studies were randomized control trials. Five studies evaluated the effectiveness of an mHealth intervention for weight loss, including one that evaluated the effectiveness for diabetes and two studies focused on diabetes. Of all the studies that focused on weight loss, 3 reported significant differences in weight loss in participants in the intervention group compared with those in the usual care group. Although all studies on diabetes control showed greater improvement in glycemic control for the intervention group compared to that in the control group, only one study showed a significant difference between the 2 groups. Conclusions This analysis indicates that there are few published studies that assessed mHealth interventions among minority populations and focused on weight or diabetes. Although the overall strength of evidence was low for diabetes control, it was moderate for weight loss, and our findings suggest that mHealth and web-based interventions may provide a promising approach for interventions among African American and Hispanic adults who have obesity or diabetes.

Following ocular surgery, dressings are commonly applied to the surgical wound. These dressings need to combine medical properties with ease of use while maintaining comfort for the patient. For the ocular area, this means that the dressings need to act as a microbial barrier, allow good conformability to the contours of the eye, and provide evaporative cooling to the inflamed area. Furthermore, the dressings should be transparent to allow for the inspection of the wound site by healthcare professionals without the need for removal. In this paper, we investigate a blend of native agarose (NA) and carboxylated agarose (CA) for producing elastic hydrogels with high water content that can be supplemented with antibiotics. It was found that in comparison to pure agarose hydrogels, the NA hydrogels blended with CA had a reduced Young’s modulus, reduced evaporation rate when exposed to air, and accelerated release rate of antimicrobial agents, whilst maintaining the same degree of transparency. By altering the formulation from 2 wt.% pure NA to 1 wt.% NA blended with 1 wt.% CA, we were able to observe an approximately 55% reduction in Young’s modulus, 25% reduction in evaporation rate, as well as a significant acceleration in the release rate of cefazolin and doxycycline, making this hydrogel blend a potential material for topical treatment applications.

BACKGROUND Per-oral endoscopic myotomy (POEM) is safe and effective for the treatment of achalasia. There is limited data on performance of POEM in patients with altered upper gastrointestinal anatomy, especially after bariatric surgery. Outcomes in patients with prior sleeve gastrectomy have not been reported. AIM To assess the efficacy and safety of POEM in patients with prior bariatric surgery. METHODS A prospective POEM database was reviewed from 3/2017-5/2020 to identify patients who underwent POEM after prior bariatric surgery. Efficacy was assessed by technical success (defined as the ability to successfully complete the procedure) and clinical success [decrease in Eckardt score (ES) to ≤ 3 post procedure]. Safety was evaluated by recording adverse events. RESULTS Six patients (50% male, mean age 48 years) with a history of prior bariatric surgery who underwent POEM were included. Three had prior sleeve gastrectomy (SG) and three prior Roux-en-Y gastric bypass (RYGB). Four patients had achalasia subtype II and 2 had type I. Most (4) patients had undergone previous achalasia therapy. Technical success was 100%. Clinical success was achieved in 4 (67%) patients at mean follow-up of 21 mo. In one of the clinical failures, EndoFLIP evaluation demonstrated adequate treatment and candida esophagitis was noted as the likely cause of dysphagia. There were no major adverse events. CONCLUSION POEM is technically feasible after both RYGB and SG and offers an effective treatment for this rare group of patients where surgical options for achalasia are limited.

The contributions of a set of design variables on heat transfer and pressure drop are evaluated for two geometry types, L1 and L2. The procedure to investigate the relative importance of the geometrical design variables on thermal-hydraulic performance is enabled by a Design of Simulations (DOS) method called Latin Hypercube Sampling (LHS) in association with a modern non-parametric statistical method (Smoothing Spline ANOVA method) and Computational Fluid Dynamics. Outcomes from the screening analyses turned out that the louver angle is the unique contributor to friction factor for both geometry types and this behaviour is independent of Reynolds number. With respect to heat transfer, the contributions of the input variables are different for both geometry types and Reynolds numbers. For Re-Dh = 120, no important two-factor interaction effects were observed for both L1 and L2, in the same way as occurred in terms of friction factor. Conversely, for Re-Dh = 240 one relevant interaction effect was observed. Limited therapy options due to antibiotic resistance underscore the need for optimization of current diagnostics. In some bacterial species, antimicrobial resistance can be unambiguously predicted based on their genome sequence. In this study, we sequenced the genomes and transcriptomes of 414 drug‐resistant clinical Pseudomonas aeruginosa isolates. By training machine learning classifiers on information about the presence or absence of genes, their sequence variation, and expression profiles, we generated predictive models and identified biomarkers of resistance to four commonly administered antimicrobial drugs. Using these data types alone or in combination resulted in high (0.8–0.9) or very high (> 0.9) sensitivity and predictive values. For all drugs except for ciprofloxacin, gene expression information improved diagnostic performance. Our results pave the way for the development of a molecular resistance profiling tool that reliably predicts antimicrobial susceptibility based on genomic and transcriptomic markers. The implementation of a molecular susceptibility test system in routine microbiology diagnostics holds promise to provide earlier and more detailed information on antibiotic resistance profiles of bacterial pathogens and thus could change how physicians treat bacterial infections., The spread of antibiotic resistance complicates infection treatment, requiring an optimization of current diagnostics.
