Objective Relating the helmet use with the severity of craniocerebral trauma in injured motorcyclists treated at a trauma hospital. Methods A cross-sectional, retrospective study. The study population consisted of 188 records of service to injured motorcyclists in a four-month period. The Glasgow Coma Scale was used to characterize the severity of trauma. Results The profile is 84.6% of males and 55.3% aged between 18 and 29 years. Regarding the use of helmet at the time of the accident, 51.6% used, 6.4% did not use, 17.6% used it inappropriately, and there were no records in 24.5%. Among the 51.6% of motorcyclists who used the protective gear, 86.6% had mild craniocerebral trauma, 12.4% had moderate, and 1% severe. The most serious injuries occurred in motorcyclists in which there were no records on helmet use. Conclusion The motorcyclists who used the helmet, had mild craniocerebral trauma in 44.7% of cases, moderate trauma in 6.4%, and severe trauma in 0.5%. Victims without records of the situation of helmet use had severe trauma (p≤0.000).

4. Randomized, Double-Blind, Placebo-Controlled Parallel Clinical Trial Assessing the Effect of Fructooligosaccharides in Infants with Constipation

Constipation often begins in the first year of life. The aim of this study was to assess the effect of fructooligosaccharides (FOS) in the treatment of infants with constipation. This randomized, double-blind, placebo-controlled clinical trial included infants with constipation who were randomly assigned to one of two parallel groups: FOS or placebo. Either the FOS supplement or the placebo was added to the infant formula. Thirty-six infants completed the 4-week intervention. Therapeutic success occurred in 83.3% of the FOS group infants and in 55.6% of the control group infants (p = 0.073; one-tailed test). Compared with the control group, the FOS group exhibited a higher frequency of softer stools (p = 0.035) and fewer episodes of straining and/or difficulty passing stools (p = 0.041). At the end of the intervention, the mouth-to-anus transit time was shorter (22.4 and 24.5 h, p = 0.035), and the Bifidobacterium sp. count was higher (p = 0.006) in the FOS group. In conclusion, the use of FOS in infants with constipation was associated with significant improvement in symptoms, but the results showed no statistical significance regarding the success of the therapy compared with the control group. FOS was associated with reduced bowel transit time and higher counts of the genus Bifidobacterium in the stool.

5. Atendimento em pronto-socorro: perfil dos pacientes com sequelas após alta hospitalar

Esta pesquisa teve como objetivo conhecer a prevalência dos pacientes que recebem alta hospitalar para a comunidade com sequelas ou necessidades especiais, necessitando de acompanhamento à saúde após alta. Trata-se de estudo retrospectivo de prevalência realizado em um hospital de pronto-socorro, direcionado para os registros de 1.794 altas hospitalares ocorridas entre janeiro e março de 2014. A amostra constitui-se de 19 registros de altas dos pacientes que ficaram com sequelas ou deficiências, representando 1,6% das 1.180 altas que ocorreram para a comunidade. Resultados evidenciaram 84,2% de pacientes do sexo masculino; média de faixa etária 37,6 anos, com sequelas diversificadas, tais como: motora 68,3% (n=13); motora e neurológica 5,3% 9 (n=1); neurológica 15,8% (n=8); pulmonar 5,3% (n=1); e metabólica 5,3% (n=1). Embora a prevalência de pacientes com sequelas ou necessidades especiais identificados na amostra tenha sido baixa, esse número é significativo quando se verifica que 73,9% dos pacientes tinham idade entre 10 anos e 49 anos, ou seja, jovens com todo o potencial produtivo.

A pesquisa teve como objetivo avaliar a autoestima de mulheres com transtornos alimentares. A amostra se constituiu de 51 participantes distribuídas em três grupos: anorexia nervosa (G1), bulimia nervosa (G2) e controle (G3). Os instrumentos utilizados foram: a Técnica de Rorschach e Escala de Rosenberg, para avaliar a autoestima e um questionário para levantamento de dados sociodemográficos e características da amostra. O EAT-26 e o MINI foram utilizados para triar o G3. A análise estatística foi por meio do Teste ANOVA e do Teste Qui-quadrado, com índice de significância de ≤0,05. Observou-se que os G1 e G2 apresentaram indicativos de baixa autoestima, autoimagem e imagem corporal negativas, prejudicadas por distorções no pensamento, comparados com o G3.

- Abstract
Introduction: Slide tracheoplasty has become the gold standard surgery for congenital tracheal stenosis (CTS). This condition is rare and the surgery can be challenging and is performed by experienced surgeons in tertiary centers. A few reports involving relatively small cohorts have been published. The aim of this review is to evaluate the post-operative mortality and morbidity of pediatric slide tracheoplasty for CTS., Methods: A systematic literature review was performed according to PRISMA guidelines. The Medline and EMBASE databases were screened using a search strategy defined in collaboration with a librarian. We included articles reporting the post-operative mortality rate of slide tracheoplasties for treatment of CTS in children, when at least 10 patients were included., Results: A total of 932 articles were reviewed, and 15 studies were eligible with a total of 845 patients. The overall post-operative mortality rate was 9.3 %, and most deaths were airway related. The open revision surgery rate after surgery was 2.8 % and the endoscopic revision rate was 27.6 %., Discussion: This study highlights key factors to consider before the surgery and helps anticipate post-operative follow-up considerations for children with CTS. Several factors were identified as predictors of mortality including young age, weight at the time of surgery and association with lung hypoplasia or aplasia., Conclusion: Although slide tracheoplasty has gained popularity in recent years due to better outcomes, it remains a major surgery with mortality risk and the need for multidisciplinary management., Competing Interests: Declaration of competing interest The authors declare that they have no known competing financial interests or personal relationships that could have appeared to influence the work reported in this paper., (Copyright © 2024. - Abstract
Background: There is significant interest in treatment de-escalation for human papillomavirus-associated (HPV
+ ) oropharyngeal squamous cell carcinoma (OPSCC) patients given the generally favourable prognosis. However, 15-30% of patients recur after primary treatment, reflecting a need for improved risk-stratification tools. We sought to develop a molecular test to risk stratify HPV+ OPSCC patients., Methods: We created an immune score (UWO3) associated with survival outcomes in six independent cohorts comprising 906 patients, including blinded retrospective and prospective external validations. Two aggressive radiation de-escalation cohorts were used to assess the ability of UWO3 to identify patients who recur. Multivariate Cox models were used to assess the associations between the UWO3 immune class and outcomes., Findings: A three-gene immune score classified patients into three immune classes (immune rich, mixed, or immune desert) and was strongly associated with disease-free survival in six datasets, including large retrospective and prospective datasets. Pooled analysis demonstrated that the immune rich group had superior disease-free survival compared to the immune desert (HR = 9.0, 95% CI: 3.2-25.5, P = 3.6 × 10-5 ) and mixed (HR = 6.4, 95% CI: 2.2-18.7, P = 0.006) groups after adjusting for age, sex, smoking status, and AJCC8 clinical stage. Finally, UWO3 was able to identify patients from two small treatment de-escalation cohorts who remain disease-free after aggressive de-escalation to 30 Gy radiation., Interpretation: With additional prospective validation, the UWO3 score could enable biomarker-driven clinical decision-making for patients with HPV+ OPSCC based on robust outcome prediction across six independent cohorts. Oral squamous cell carcinoma (OSCC) prognosis is related to clinical stage and histological grade. However, this stratification needs to be refined. We conducted a comparative proteome study in microdissected samples from normal oral mucosa and OSCC to identify biomarkers for malignancy. Fascin and plectin were identified as differently expressed and both are implicated in several malignancies, but the clinical impacts of aberrant fascin and plectin expression in OSCCs remains largely unknown. Immunohistochemistry and real-time quantitative PCR were carried out in ex vivo OSCC samples and cell lines. A loss-of-function strategy using shRNA targeting fascin was employed to investigate in vitro and in vivo the fascin role on oral tumorigenesis. Transfections of microRNA mimics were performed to determine whether the fascin overexpression is regulated by miR-138 and miR-145. We found that fascin and plectin are frequently upregulated in OSCC samples and cell lines, but only fascin overexpression is an independent unfavorable prognostic indicator of disease-specific survival. In combination with advanced T stage, high fascin level is also an independent factor of disease-free survival. Knockdown of fascin in OSCC cells promoted cell adhesion and inhibited migration, invasion and EMT, and forced expression of miR-138 in OSCC cells significantly decreased the expression of fascin. In addition, fascin downregulation leads to reduced filopodia formation and decrease on paxillin expression. The subcutaneous xenograft model showed that tumors formed in the presence of low levels of fascin were significantly smaller compared to those formed with high fascin levels. Collectively, our findings suggest that fascin expression correlates with disease progression and may serve as a prognostic marker and therapeutic target for patients with OSCC., Competing Interests: CONFLICTS OF INTEREST The authors declare that they have no competing interests.
