BACKGROUND Growth arrest-specific gene 6 (Gas6) is a pro-hemostatic protein with an unknown predictive ability for recurrent venous thromboembolism (VTE). In the elderly, VTE carries a higher mortality but not a higher rate of VTE recurrence than in younger patients. Consequently, anticoagulation management in the elderly is challenging. OBJECTIVE To prospectively investigate the performance of Gas6 to predict VTE recurrence, major bleeding and mortality in the elderly. METHODS Consecutive patients aged ≥65 with acute VTE were followed over 3 years. Primary outcomes were symptomatic VTE recurrence, major bleeding and mortality. Plasmatic Gas6 was measured by ELISA. RESULTS Gas6 was measured in 864 patients at the time of the index VTE (T1) and in 70% of them, also 12 months later (T2). Gas6 at T1 was discriminatory for VTE recurrence (C-statistics: 0.56, 95% confidence interval [CI]: 0.51 to 0.62), major bleeding (0.60, 95%CI: 0.55 to 0.65) and mortality (0.69, 95%CI: 0.65-0.73) up to 36 months. VTE recurrence up to 24 months after T2 was discriminated by Gas6 at T2 (0.62, 95%CI: 0.54-0.71). High Gas6 (>157%) and continuous Gas6 levels at T1 were associated with VTE recurrence up to 6 and 12 months, respectively. CONCLUSIONS In elderly patients, high Gas6 is associated with a higher risk of VTE recurrence, major bleeding and death. Background Recruitment into surgical RCTs can be threatened if new interventions available outside the trial compete with those being evaluated. Adapting the trial to include the new intervention may overcome this issue, yet this is not often done in surgery. This paper describes the challenges, rationale and methods for adapting an RCT to include a new intervention. Methods The By‐Band study was designed in the UK in 2009–2010 to compare the effectiveness of laparoscopic adjustable gastric band and Roux‐en‐Y gastric bypass for severe obesity. It contained a pilot phase to establish whether recruitment was possible, and the grant proposal specified that an adaptation to include sleeve gastrectomy would be considered if practice changed and recruitment was successful. Information on changing obesity surgery practice, updated evidence and expert opinion about trial design were used to inform the adaptation. Results The pilot phase recruited over 13 months in 2013–2014 and randomized 80 patients (79 anticipated). During this time, major changes in obesity practice in the UK were observed, with gastric band reducing from 32·6 to 15·8 per cent and sleeve gastrectomy increasing from 9·0 to 28·1 per cent. The evidence base had not changed markedly. The British Obesity and Metabolic Surgery Society and study oversight committees supported an adaptation to include sleeve gastrectomy, and a proposal to do so was approved by the funder. Conclusion Adaptation of a two‐group surgical RCT can allow evaluation of a third procedure and maintain relevance of the RCT to practice. A measurement is presented of W-boson production in PbPb collisions carried out at a nucleon-nucleon (NN) centre-of-mass energy root S-NN of 2.76 TeV at the LHC using the CMS detector. In data corresponding to an integrated luminosity of 7.3 mu b(-1), the number of W -> mu v(mu) decays is extracted in the region of muon pseudorapidity vertical bar eta mu vertical bar < 2.1 and transverse momentum p(T)(mu) > 25 GeV/c. Yields of muons found per unit of pseudorapidity correspond to (159 +/- 10(stat.) +/- 12(syst.)) x 10(-8) W and (154 +/- 10(stat.) +/- 12(syst.)) x 10(-8) W- bosons per minimum-bias PbPb collision. The dependence of W production on the centrality of PbPb collisions is consistent with a scaling of the yield by the number of incoherent NN collisions. The yield of W bosons is also studied in a sample of pp interactions at root S = 2.76 TeV corresponding to an integrated luminosity of 231 nb(-1). The individual W+ and W- yields in PbPb and pp collisions are found to agree, once the neutron and proton content in Pb nuclei is taken into account. Likewise, the difference observed in the dependence of the positive and negative muon production on pseudorapidity is consistent with next-to-leading-order perturbative QCD calculations.

- Abstract
Oxidative stress is thought to contribute to the initiation and progression of atherosclerosis. As glutathione peroxidase-1 (Gpx1) is an antioxidant enzyme that detoxifies lipid hydroperoxides, we tested the impact of Gpx1 deficiency on atherosclerotic processes and antioxidant enzyme expression in mice fed a high-fat diet (HFD). After 12 weeks of HFD, atherosclerotic lesions at the aortic sinus were of similar size in control and Gpx1-deficient mice. However, after 20 weeks of HFD, lesion size increased further in control but not in Gpx1-deficient mice, even though plasma and aortic wall markers of oxidative damage did not differ between groups. In control mice, the expression of Gpx1 increased and that of Gpx3 decreased at the aortic sinus after 20 weeks of HFD, with no change in the expression of Gpx2, Gpx4, catalase, peroxiredoxin-6, glutaredoxin-1 and -2, or thioredoxin-1 and -2. By comparison, in Gpx1-deficient mice, the expression of antioxidant genes was unaltered except for a decrease in glutaredoxin-1 and an increase in glutaredoxin-2. These changes were associated with increased expression of the proinflammatory marker monocyte chemoattractant protein-1 in control mice but not in Gpx1-deficient mice. In summary, a specific deficiency in Gpx1 was not accompanied by an increase in markers of oxidative damage or increased atherosclerosis in a murine model of HFD-induced atherogenesis. - Abstract
Critically ill patients with acute renal failure are traditionally treated with low-protein diets to help control uremia. This dietary approach may be deleterious to the patient's nutritional status and unnecessary, especially if continuous renal replacement therapies (CRRT) are used. However, the optimal amount of protein supplementation during CRRT is unknown. In patients receiving CRRT, a high protein intake may result in a positive nitrogen balance in the absence of uncontrolled uremia. Accordingly, we studied nitrogen metabolism in two consecutive cohorts of acute renal failure patients receiving equal amounts of calories but variable amounts of nitrogen. One group received protein according to the preferences of the attending clinician, the other a high and fixed amount of protein (2.5 g/kg/day). Patients treated according to attending clinician preferences received significantly less dietary protein (1.2 g/kg/day vs. 2.5 g/kg/day; p < 0.0001) and had a negative mean nitrogen 1 balance of -5.5 g/day. Patients receiving a high and fixed amount of protein had a less negative mean nitrogen balance (-1.92 g/day). Such patients were more likely to experience a positive nitrogen balance during any 24-h period (53.6% vs. 36.7%; p < 0.05). They also required more aggressive hemofiltration to maintain control of uremia (mean ultradiafiltrate volume: 2145 mL/h vs. 1658 mL/h; p < 0.0001) and had a significantly higher but still acceptable mean plasma urea level (26.6 mmol/L vs. 18 mmol/L; p < 0.0001). Survival was not significantly different in the two groups (37.5% vs. 31.3%). We conclude that a high- protein diet can be safely administered to critically ill patients with acute renal failure receiving continuous renal replacement therapy. Such a high protein intake improves nitrogen balance when compared to moderate protein intake. A low protein intake is unnecessary in patients treated with CRRT.

Bactericidal/permeability-increasing protein (BPI) and azurocidin (AZ) are recently described target antigens of antineutrophil cytoplasmic antibodies (ANCA). In this study, BPI-ANCA were demonstrated most often in patients with ulcerative colitis (36/92, 39%), Crohn's disease (17/66, 26%) and cystic fibrosis (11/14, 79%), but also in patients with rheumatoid arthritis (8/40, 20%), systemic lupus erythematosus (SLE) (111/65, 17%) and mixed connective tissue disease (4/18, 22%). BPI-ANCA were also common in sera containing antinuclear (ANA) (9/43, 21%) or antidouble-stranded (ds) DNA (7/28, 25%) antibodies. There was no increased frequency of abnormal alpha1-antitrypsin (alpha1-AT) phenotypes in patients with BPI-ANCA, and BPI-ANCA were not more common in individuals with an abnormal phenotype. The predominant IgG subclasses were IgG1 and IgG3; IgA but not IgM was present. Both IgG and IgA BPI-ANCA were high affinity antibodies, and the affinity of IgG antibodies did not change with time in the sera tested. Four of the five sera (80%) containing BPI-ANCA did not bind to denatured, reduced BPI, suggesting that most BPI-ANCA recognised conformational epitopes. AZ-ANCA were demonstrated in 2/11 patients (18%) with Wegener's granulomatosis, 3/12 (25%) with cystic fibrosis and 3/14 (21%) with chronic active hepatitis. AZ-ANCA were present in 5/25 sera (25%) with ANA, but the levels were only marginally elevated. AZ-ANCA were uncommon in patients with inflammatory bowel and rheumatological diseases, and in sera containing other autoantibodies. Again, there was no association with abnormal alpha1-AT phenotypes. BPI represents a major ANCA target antigen in patients with rheumatological as well as inflammatory bowel disease and cystic fibrosis, but AZ-ANCA are uncommon.

Objectives: This study investigates the sensitivity of various standard clinical techniques in the detection of albumin fragments. The significance of this work is in the detection of urinary proteins, such as albumin, which has recently been discovered to be excreted as mainly peptide fragments as a result of filtered albumin undergoing degradation during renal passage. All filtered proteins undergo a similar degradation process. Design and Methods: Albumin digested with trypsin was used as a model urine solution. The solution was assayed for albumin concentration by various methods including the biuret assay that is known to detect urinary albumin fragments. The digest solution was also analyzed by various clinically used chromagen assays, electrophoretic and chromatographic methods to determine whether they are able to detect the fragmented protein. Result(s): The benzethonium chloride, Coomassie blue, and pyrogallol red assays for urine protein, the immunoassay for human albumin and sodium dodecyl sulfate polyacrylamide gel electrophoresis with Coomassie blue staining were unable to detect the albumin fragments. Capillary electrophoresis was sensitive to the fragments but with low resolution. High-performance liquid chromatography gave the best results. Conclusion(s): Many techniques utilized to assay patient urine samples are unable to detect fragmented albumin and, hence, will severely underestimate albumin and protein excretion. PURPOSE. To investigate the morphologic and hypotensiveeffect of contact transscleral ciliary body PDT in pigmented rabbit eyes. METHODS. The right eyes of 33 pigmented rabbits were irradiated using chloraluminumsulfonated phthalocyanine as photosensitizer and a diode laser (670 nm) asthe light source. Twenty-five animals received a single treatment. Eight animalsreceived a second treatment 13 days after the first one. Photosensitizer wasadministered by means of continuous intravenous infusion. Ciliary body wasirradiated transsclerally by means of an optic fiber applied on the corneosclerallimbus. In all cases 14–16 laser applications were performed to cover360 degrees of the ciliary body. Animals were followed for a maximum of 30days by means of tonometry and biomicroscopy. Retreatments were performedusing the same irradiation protocol. At the end of the follow up time animalswere sacrificed and their were eyes prepared for light and electron microscopy. RESULTS. Transscleral ciliary body PDT resulted in significant but temporaryreduction of IOP in all cases. The effect lasted about two weeks. Retreatmentled to a new significant drop of the IOP, which lasted about two weeks again.In histological examination the initial effect was vascular thrombosis, followedby edema and disintegration of the ciliary epithelial layers. In all casesthe appearance of the ciliary body had returned to normal 15 days after irradiation. CONCLUSION. Contact transscleral PDT with the treatment parameters usedin this study results in significant but temporary functional and morphologicalalteration in pigmented rabbits ciliary body. 1 -antitrypsin (α1 -AT) is a naturally occurring inhibitor of proteinase 3 (PR3) and elastase, two of the target antigens of anti-neutrophil cytoplasmic antibodies (ANCA). An increased incidence of α1 -AT phenotypes associated with dysfunctional α1 -AT or low serum levels has been reported in patients with anti-PR3 antibodies. We have studied the relationship between ANCA, and phenotypes and serum levels of α1 -AT. Phenotypes usually associated with a moderate or severe reduction in α1 -AT serum levels or in dysfunctional activity were found more often in individuals with anti-PR3 antibodies than in the general population: four of the 31 patients (13%) with anti-PR3 antibodies had phenotypes MZ (n = 2), S (n = 1) or Z (n = 1) (P < 0.05). However, the corresponding α1 -AT serum levels were normal (n = 3) or elevated (n = 1). None of the 31 sera with anti-PR3 antibodies had low levels of α1 -AT. No abnormal α1 -AT phenotype was demonstrated in seven patients with anti-elastase antibodies, despite a low level of α1 -AT in one serum. Anti-myeloperoxidase antibodies are common in patients with ANCA, but no abnormal phenotype or low serum α1 -AT level was demonstrated in any of 29 sera containing these antibodies. Finally anti-glomerular basement membrane (GBM) antibodies occur occasionally in patients with ANCA-associated diseases, but again none of 10 sera had an abnormal α1 -AT phenotype or low serum level. ANCA were not demonstrated by indirect immunofluorescence in any serum from 73 patients with abnormal α1 -AT phenotypes. These results confirm that patients with anti-PR3 antibodies often have α1 -AT phenotypes that are usually associated with low serum levels of α1 -AT or with dysfunctional protein. Nevertheless, the incidence of anti-PR3 antibodies in patients with abnormal α1 -AT phenotypes is very low. This probably reflects the rarity of Wegener's granulomatosis, the major disease associated with anti-PR3 antibodies, and the relative frequency of abnormal α1 -AT phenotypes. The mechanism for the development of anti-PR3 antibodies in patients with abnormal α1 -AT phenotypes is not clear, but may relate to the increased propensity of unbound and uninhibited PR3 to stimulate autoantibody production.
