Abstract

This article, published in the European Heart Journal, discusses the association between myocardial scar and long-term mortality in patients with severe aortic stenosis (AS). The study, conducted by a UK consortium, used cardiovascular magnetic resonance (CMR) imaging to detect myocardial scarring and found that it was independently associated with increased all-cause and cardiovascular mortality beyond 10 years of follow-up. The presence of scar reduced median survival by 3.5 years. The study also identified other factors, such as age, sex, type 2 diabetes mellitus, and atrial fibrillation, that were associated with mortality. The findings suggest that evaluating myocardial scar is important for assessing post-intervention risk in patients with severe AS. [Extracted from the article]

Abstract

Stroke is a leading cause of morbidity and mortality across the world and a significant portion of ischemic strokes have a cardiac source. We report a case of a 55‐year‐old male who presented with an ischemic stroke and bilateral pulmonary emboli secondary to an intra‐cardiac thrombus straddling a patent foramen ovale, which was clearly seen using transesophageal echocardiography. We discuss the management dilemma associated with this clinical picture given the risk of hemorrhagic transformation in the acute phase of an ischemic stroke. 44. Myocardial Calcium Handling in Type 2 Diabetes: A Novel Therapeutic Target.

Abstract

Type 2 diabetes (T2D) is a multisystem disease with rapidly increasing global prevalence. Heart failure has emerged as a major complication of T2D. Dysregulated myocardial calcium handling is evident in the failing heart and this may be a key driver of cardiomyopathy in T2D, but until recently this has only been demonstrated in animal models. In this review, we describe the physiological concepts behind calcium handling within the cardiomyocyte and the application of novel imaging techniques for the quantification of myocardial calcium uptake. We take an in-depth look at the evidence for the impairment of calcium handling in T2D using pre-clinical models as well as in vivo studies, following which we discuss potential novel therapeutic approaches targeting dysregulated myocardial calcium handling in T2D.
