L’approche systematique et l’examen clinique des patients constituent des piliers centraux pour l’enseignement de la medecine. Cependant, tres peu d’etudiants en medecine apprennent a examiner un patient souffrant de troubles psychiatriques au cours de leur parcours. Nous proposons dans cet article la reflexion didactique qui a permis le developpement d’un guide pour un examen clinique psychiatrique standardise a destination des etudiants en medecine. Ce guide a ete construit a partir de la trame d’un examen medical classique (antecedents, histoire de la maladie, biographie, etc.). Il incorpore egalement les differents domaines et etapes du Mental Status Examination (MSE) ainsi que des questions issues de plusieurs outils standardises. Le document propose se veut simple et facile a utiliser (systeme de cases a cocher) afin de faciliter les premiers entretiens des etudiants en medecine et des jeunes internes. La maitrise de l’examen clinique est une etape essentielle dans le processus d’apprentissage de la medecine qui permettra ensuite d’integrer, avec l’experience, la souplesse et l’adaptabilite necessaire a tout entretien psychiatrique.[resume d’auteur]

Resume Les nouvelles technologies dediees a la sante connaissent actuellement un developpement important et constituent une opportunite interessante dans la prise en charge medicale des patients, a la fois pour le diagnostic et le suivi des pathologies, en complement aux outils diagnostiques habituels, mais egalement dans la prise en charge therapeutique, en proposant des alternatives non pharmacologiques. Cette revue presentera des solutions technologiques, comme les capteurs portatifs (actigraphie) et les capteurs environnementaux (analyse automatisee visuelle ou de la voix, smarthomes), ainsi que des solutions numeriques (serious games, exergames, realite virtuelle).

Background: Neuropsychiatric symptoms, also known as behavioral and psychological symptoms of dementia (BPSD), affect the majority of patients with dementia, and result in a greater cognitive and functional impairment. Objective: To investigate associations between BPSD and amyloid cerebral deposition as measured by 18F-Florbetapir-PET quantitative uptake in elderly subjects with and without cognitive impairment. Methods: Participants with cognitive impairment [mild cognitive impairment (MCI) or Alzheimer’s disease (AD)] and healthy controls (HC) from the ADNI cohort (Alzheimer Disease Neuroimaging Initiative) who underwent an 18F-florbetapir PET scan between May 2010 and March 2014 were included. Clinical assessments included the Clinical Dementia Rating, the Mini-Mental State Examination (MMSE), and the Neuropsychiatric Inventory. Freesurfer software was used to extract PET counts based on T1-based structural ROI (frontal, cingulate, parietal, and temporal). Spearman’s partial correlation scores between BPSD severity and regional amyloid uptake were calculated. Results: Data for 657 participants [age = 72.6 (7.19); MMSE = 27.4 (2.67)] were analyzed, including 230 HC [age = 73.1 (6.02); MMSE = 29 (1.21)], 308 MCI [age = 71.5 (7.44); MMSE = 28.0 (1.75)], and 119 AD subjects [age = 74.7 (8.05); MMSE = 23.1 (2.08)]. Considering all diagnostic groups together, positive significant correlations were found between anxiety and 18F-florbetapir uptake in the frontal (r = 0.102; p = 0.009), cingulate (r = 0.083; p = 0.034), and global cerebral uptake (r = 0.099; p = 0.011); between irritability and frontal (r = 0.089; p = 0.023), cingulate (r = 0.085; p = 0.030), parietal (r = 0.087; p = 0.025), and global cerebral uptake (r = 0.093; p = 0.017); in the MCI subgroup, between anxiety and frontal (r = 0.126; p = 0.03) and global uptake (r = 0.14; p = 0.013); in the AD subgroup, between irritability and parietal uptake (r = 0.201; p = 0.03). Conclusion: Anxiety and irritability are associated with greater amyloid deposition in the neurodegenerative process leading to AD.

Introduction Apathy and depression are the two most frequent neuropsychiatric symptoms in Alzheimer's disease (AD) and related disorders. Whereas neuroimaging studies have shown different neural pathways for these two symptoms, a clinical overlap between apathy and depression is frequently described. Objective To describe the prevalence of apathy and depression among elderly subjects with or without dementia criteria using specific diagnostic criteria for apathy and depression. Method Subjects from an ongoing cross-sectional study were recruited in 4 centers (France, Indonesia, Spain, Argentina). Apathy and depression were assessed using the diagnostic criteria for apathy and for depression. Additionally, the apathy and dysphoria domains of the NPI-C (Neuropsychiatric Inventory-Clinician), as well as the 12 domains of the original NPI (Neuropsychiatric Inventory) were assessed. Results 431 subjects (mean age=74.6 ± 10.4 ; gender=♂ 29%) were recruited (France=100, Spain=90, Indonesia=182, Argentina=79). Among them, 25% were healthy elderly subjects, 9% had a diagnosis of MCI (Mild Cognitive Impairment), 46% had a diagnosis of dementia (including AD, Lewy body, vascular, mixed and fronto-temporal dementia). In the overall population, the prevalence of apathy and depression were respectively 41% and 25%. More specifically, the prevalence of apathy and depression in the AD sample were 67% and 25%. Conclusion Diagnostic criteria for apathy and depression, as well as the NPI-C, are recent assessment methods in the field of dementia, developed to increase the accuracy of the clinical evaluation of BPSD. It is therefore critical to propose multicenter observational studies comparing these new tools with classical assessment methods.

Introduction Ambulatory actigraphy is a simple and objective method for the evaluation of motor activity that is used in several neuropsychiatric disoders such as depression, agitation, apathy, and sleep disorders. Objective To evaluate the sensitivity (Se) and specificity (Sp) of actigraphic postural detection versus postural data gathered by observer during multiple guided activities. Material and methods A voluntary healthy subject was equipped with different actigraphs (Actiwatch®; MicroMini- Motionlogger®; MotionLog®; Movilis®) then realized multiple guided activities during a 7-hour diurnal period : walking, standing, sitting, decubitus with sleep. Actigraphs were worn on different locations (wrists, thorax, ankles, belt) and two of them included postural recognition in addition to motor activity monitoring. Se and Sp for postural recognition compared to ground-truth were obtained from table of confusion based on raw activity data recordings; error of attribution due to devices is detailed in confusion matrix. Results Actigraphy postural recognition is efficient for lying position detection (MotionLog: Se 99% Sp 97%; Movilis: Se 80% Sp 72%). Standing position was mainly recognized as sitting position and therefore actigraphic detection lacks of accuracy for these positions. Walking is detected with 20% error. Conclusion Postural actigraphy is efficient for lying position detection and actigraphic postural recognition could improve detection and characterization of nocturnal awakenings and diurnal napping in clinical practice.

Objectif : Évaluer la prosodie en expression chez l’adulte masculin présentant un trouble du spectre autistique sans déficience intellectuelle (TSA SDI) par le biais de mesures acoustiques et quantitatives, en comparaison à un groupe témoin.Méthode : 10 sujets masculins adultes avec TSA SDI et 10 sujets masculins adultes témoins ont été enregistrés lors de tâches vocales et d’expression prosodique, incluant les versants linguistique et émotionnel de la prosodie, en lecture, répétition, production et récit d’événements personnels.Résultats : L’étude montre que plusieurs paramètres prosodiques sont différents entre les sujets avec TSA SDI et les sujets témoins, traduisant une désorganisation prosodique représentée par une fréquence fondamentale haute, des intonations et un volume réduit, un rythme ralenti et confus, ainsi qu’une accentuation atypique.Conclusions : Les résultats mettent en évidence la question d’un entraînement prosodique spécifique en intervention précoce chez les enfants présentant un TSA et la nécessité de la prise en soin de la population adulte présentant ce même trouble.

Introduction: Les corticoïdes présentent des effets indésirables somatiques et psychiatriques. Leur utilisation dans le traitement des poussées de Sclérose en Plaques nécessite de fortes doses à l'origine de potentiels effets psycho-comportementaux encore peu documentés.Objectif: il s'agit d'identifier les facteurs de risque à l'origine des effets psychiatriques dans le traitement par corticothérapie forte dose des poussées de Sclérose en Plaque.Matériel et méthode : les patients ont été inclus au service de l'Hôpital de Jour de Neurologie de Pasteur à Nice. Leur évaluation psychiatrique initiale était composée de la passation des échelles de la manie de YOUNG, d'anxiété d'Hamilton et de Dépression d'Hamilton ainsi que d'un questionnaire d'hétéro-évaluation. Une seconde évaluation était réalisée le huitième jour après l'inclusion à l'aide des mêmes échelles. Nous avons comparé les scores obtenus selon un test de Wilcoxon apparié.Résultats: sur les 43 patients initiaux, 3 ont été perdus de vue. 67,5% ont présenté des troubles cortico-induits. 22,5% présentaient des scores à J0 supérieurs à 7 à l'échelle de Manie de Young ( moy = 9,67, [ET]=1,94) et montraient à J8 une élévation significative de 4,89 points ( moy = 14,56; [ET]=1,01) à l'échelle de manie de Young (p=0,009). 6 patients présentant à J0 des scores élevés à l'échelle d'Anxiété Hamilton( moy= 12,17, [ET] =3,19) ont montré une régression de 8,2 points à J8 ( moy = 4,5; [ET]= 1,52) significative (p= 0,035) . Les scores de Dépression Hamilton n'ont pas été significativement modifiés après corticothérapie ( p = 0,593) mais 27,5% des sujets avec un score moyen de 11, 27 ( [ET]= 4,45 ) ont présenté des éléments de labilité thymique majeure.Conclusion: les effets psychiatriques secondaires aux corticoïdes restent encore peu documentés. Nous espérons que notre étude a pu apporter des éléments pour les appréhender et les comprendre.
