Davies Abstract: N-terminal pro-B-type natriuretic peptide (N-BNP) is elevated in left ventricular systolic dysfunction (LVSD) and may be cost-effective for screening in the community but is relatively nonspecific. We sought to improve specificity using inflammatory markers such as C-reactive protein (CRP) and myeloperoxidase (MPO), which have been implicated in cardiovascular disease. Abstract

From bureaucratic to programmatic institutions, hospitals across North America are experiencing restructuring. Using the results of a telephone survey and a semistructured questionnaire, this research study explores the effect of hospital restructuring on the organization and delivery of social work services and the strategies social workers select to manage change in a sample of Ontario hospitals. Respondents identified a number of issues related to control over the nature of work in the organization, social work roles, and the management of structural change. Abstract

Fluorimetric methods help evaluate the turnover number for sodium-hydrogen exchanger isoform 1 (NHE-1) in lymphocytes and MRC5 fibroblasts and determine their intracellular pH and Na+/H+ antiport function. Viral transformation of the cells enhances proliferation rate. The absence of NHE-2 and NHE-3 mRNA transcripts in fibroblasts and lymphocytes indicates that the Na+-dependent H+ efflux in these cells is due to NHE-1. NHe activity enhances pHi as seen by the increased pHi of transformed cells.

Abstract

The effect of changes in membrane cholesterol levels and pH on biological transport mechanisms was investigated using SV-40 virus transformed human MRC-5 fibroblasts. Results indicated an inverse relationship between cholesterol level and Na+-H+ antiport activity. Lower cholesterol led to an increase in antiport activity based on a higher maximal velocity of the transporter. The Na+-H+ antiport activation due to intracellular acidosis was not reversed by increased cholesterol levels but was influenced by osmotic stimuli. These suggested that activity of a transport process depended on the type of activating stimulus.

Abstract

In both essential hypertension and diabetic nephropathy (DN), the ubiquitous cellular [Na.sup.+]/[H.sup.+] exchanger (NHE) exhibits altered kinetics with increased transport activity. The mechanism for this phenotype and its dependence on the presence of serum are unknown, but increased lymphoblast NHE activity in DN has been attributed to a defect in post-translational processing of NHE-1 rather than an increased cellular exchanger nun-tber. Phosphorylation of NHE-1 has been proposed to play a role in its activation in a variety of cell models. We have examined, therefore, the role of NHE-1 phosphorylation and the effect of serum in determining the increased NHE-1 activity in lymphoblasts from patients with DN. Cells from these patients exhibited increased NHE activity in the presence and absence of fetal calf serum (range 42-59%, P < 0.006, analysis of variance) and an increased proliferation rate (P < 0.01) when compared with cells from both normoalbuminuric diabetic patients and nondiabetic control subjects. However, NHE-1 abundance was very similar among all groups in the presence and absence of serum, indicating that increased NHE activity in cells of nephropathy patients was due to an increased turnover number. This nephropathy phenotype was not accompanied by an increased net phosphorylation of NHE-1 in the presence or absence of serum. Our findings suggest that increased NHE-1 activity in cells of DN patients is independent of the presence of serum and is not attributable to altered NHE-1 phosphorylation. Additional post-translational mechanisms for activation of NHE-1, therefore, may be involved.
