Microbial pathogens in the prostate cause inflammation that leads to prostatitis and proliferative inflammatory atrophy frequently associated with the development of prostate cancer. Bacterial lipopolysaccharides and DNA mediate immune responses by engaging Toll-like receptor (TLR) 4 and 9, respectively. Synthetic oligodeoxynucleotides containing CpG motifs (CpG-ODN) mimic bacterial DNA and signal through TLR9 to initiate innate immune responses. Here, we show that stimulation of DU145, PC3, or LnCap prostate cancer cells by the TLR9 agonists, CpG-ODN, induces mRNA expression of IL-6, IL-8, CXCL1, IP-10, CCL5, and TGFβ. In addition, activity of matrix metalloproteinase (MMP)-9 and -2 and cell migration increased on CpG-ODN treatment. Induction of cytokines and chemokines was mediated by NF-ΚB activation and translocation to the nucleus. Treatment with epigallocatechin-3-gallate (EGCG), the major constituent of green tea, prior to CpG-ODN stimulation, inhibits cytokine and chemokine gene induction, activity of MMP-9 and -2, and cell migration. EGCG treatment sequesters the p65 subunit of transcription factor NF-ΚB in the cytoplasm and inhibits transcriptional activity of the NF-ΚB-driven promoter in response to CpG-ODN. Our results suggest that the ability of the TLR9 agonists, CpG-ODN, to induce cytokines, chemokines, and MMP activity, as well as suppression by EGCG are independent of the androgen receptor and p53 status of the cells. EGCG may provide protective effects against inflammation in the prostate and benefit prostate cancer treatment. Keywords: CpG-ODN, EGCG, inflammation, NF-ΚB

7. Absence of long-range superconducting correlations in the frustrated 1/2-filled band Hubbard model

Abstract

We present many-body calculations of superconducting pair-pair correlations in the ground state of the half-filled band Hubbard model on large anisotropic triangular lattices. Our calculations cover nearly the complete range of anisotropies between the square and isotropic triangular lattice limits. We find that the superconducting pair-pair correlations decrease monotonically with increasing onsite Hubbard interaction U for inter-pair distances greater than nearest neighbor. For the large lattices of interest here the distance dependence of the correlations approaches that for noninteracting electrons. Both these results are consistent with the absence of superconductivity in this model in the thermodynamic limit. We conclude that the effective 1/2-filled band Hubbard model, suggested by many authors to be appropriate for the kappa-(BEDT-TTF)-based organic charge-transfer solids, does not explain the superconducting transition in these materials., Comment: 9 pages, 7 figures. Abstract

The occurrence of antiferromagnetism in kappa-(ET)_2X can be understood within an effective 1/2-filled band with dimers of ET molecules containing one hole each. We argue that while this effective model can describe the presence of antiferromagnetism, a complete description for these materials requires the correct carrier density of one-half per molecule. For dimerized and strongly frustrated 1/4-filled lattices we show that a singlet-paired state coexisting with charge ordering occurs that we have termed the Paired Electron Crystal (PEC). Here we investigate the 1/4-filled model on a dimerized lattice, showing regions where AFM, PEC, and the Wigner-crystal occur. We point out the need to go beyond quantum spin liquid concepts for highly frustrated materials such as kappa-(ET)_2Cu_2(CN)_3 and beta'-EtMe_3Sb[Pd(dmit)_2]_2 which we believe are PECs at low temperatures., Comment: 4 pages, 4 figures, proceedings of ISCOM 2011

Abstract

We consider the simplest example of lattice frustration in the 1/4-filled band, a one-dimensional chain with next-nearest neighbor interactions. For this zigzag ladder with electron-electron as well as electron-phonon interactions we present numerical results for ground state as well as thermodynamic properties. In this system the ground state bond distortion pattern is independent of electron-electron interaction strength. The spin gap in the ground state of the zigzag ladder increases with the degree of frustration. Unlike in one-dimension, where the spin-gap and charge ordering transitions can be distinct, we show that in the ladder they occur simultaneously. We discuss spin gap and charge ordering transitions in 1/4-filled materials with one, two, or three dimensional crystal structures. We show empirically that regardless of dimensionality the occurrence of simultaneous or distinct charge and magnetic transitions can be correlated with the ground state bond distortion pattern., Comment: 12 pages, 8 eps figures

10. The Paired Electron Crystal: order from frustration in the quarter-filled band

Abstract

We present a study of the effects of simultaneous charge- and spin-frustration on the two-dimensional strongly correlated quarter-filled band on an anisotropic triangular lattice. The broken-symmetry states that dominate in the weakly frustrated region near the rectangular lattice limit are the well known antiferromagnetic state with in-phase lattice dimerization along one direction, and the Wigner crystal state with the checkerboard charge order. For moderate to strong frustration, however, the dominant phase is a novel spin-singlet paired-electron crystal (PEC), consisting of pairs of charge-rich sites separated by pairs of charge-poor sites. The PEC, with coexisting charge-order and spin-gap in two dimension, is the quarter-filled band equivalent of the valence bond solid (VBS) that can appear in the frustrated half-filled band within antiferromagnetic spin Hamiltonians. We discuss the phase diagram as a function of on-site and intersite Coulomb interactions as well as electron-phonon coupling strength. We speculate that the spin-bonded pairs of the PEC can become mobile for even stronger frustration, giving rise to a paired-electron liquid. We discuss the implications of the PEC concept for understanding several classes of quarter-filled band materials that display unconventional superconductivity, focusing in particular on organic charge transfer solids. Our work points out the need to go beyond quantum spin liquid (QSL) concepts for highly frustrated organic charge-transfer solids such as kappa-(BEDT-TTF)_2Cu_2(CN)_3 and EtMe_3Sb[Pd(dmit)_2]_2, which we believe show frustration-induced charge disproportionation at low temperatures. Abstract

Indian organisations have started adopting sustainable supply chain management (SSCM) practices but their effectiveness and maturity depends on various factors such as external pressures and organisational internal environment. The research available, as on date, on the factors affecting the adoption of SSCM practices in Indian context is inadequate. This article presents a holistic view of the critical success factors (CSFs) impacting the SSCM process as well as the performance outcome. A conceptual model has been proposed which has been empirically tested by survey data collected from 145 industry practitioners through online and offline survey. The items for the survey were developed based on the literature and inputs received from industry experts. Using structural equation modelling (SEM) technique, the study finds that organisations’ internal environment is very positively associated with SSCM practices in the Indian steel sector. These findings can help the managers to create an awareness as well as favourable conditions within the organisation which facilitates the effective adoption of SSCM practices.

Abstract

Background Abdominal wall hernia is a common surgical condition. Patients may present in an emergency with bowel obstruction, incarceration or strangulation. Small bowel obstruction (SBO) is a serious surgical condition associated with significant morbidity. The aim of this study was to describe current management and outcomes of patients with obstructed hernia in the UK as identified in the National Audit of Small Bowel Obstruction (NASBO). Methods NASBO collated data on adults treated for SBO at 131 UK hospitals between January and March 2017. Those with obstruction due to abdominal wall hernia were included in this study. Demographics, co‐morbidity, imaging, operative treatment, and in‐hospital outcomes were recorded. Modelling for factors associated with mortality and complications was undertaken using Cox proportional hazards and multivariable regression modelling. Results NASBO included 2341 patients, of whom 415 (17·7 per cent) had SBO due to hernia. Surgery was performed in 312 (75·2 per cent) of the 415 patients; small bowel resection was required in 198 (63·5 per cent) of these operations. Non‐operative management was reported in 35 (54 per cent) of 65 patients with a parastomal hernia and in 34 (32·1 per cent) of 106 patients with an incisional hernia. The in‐hospital mortality rate was 9·4 per cent (39 of 415), and was highest in patients with a groin hernia (11·1 per cent, 17 of 153). Complications were common, including lower respiratory tract infection in 16·3 per cent of patients with a groin hernia. Increased age was associated with an increased risk of death (hazard ratio 1·05, 95 per cent c.i. 1·01 to 1·10; P = 0·009) and complications (odds ratio 1·05, 95 per cent c.i. 1·02 to 1·09; P = 0·001). Conclusion NASBO has highlighted poor outcomes for patients with SBO due to hernia, highlighting the need for quality improvement initiatives in this group., This study shows that small bowel obstruction due to abdominal wall hernia is common, typically involves a co‐morbid group of patients and has poor outcomes, including high mortality rates. Abstract

Background Patients with peritoneal malignancy often have multiple laparotomies before referral for cytoreductive surgery (CRS). Some have substantial abdominal wall herniation and tumour infiltration of abdominal incisions. CRS involves complete macroscopic tumour removal and hyperthermic intraperitoneal chemotherapy (HIPEC). Abdominal wall reconstruction is problematic in these patients. The aim of this study was to establish immediate and long‐term outcomes of abdominal wall reconstruction with biological mesh in a single centre. Methods A dedicated peritoneal malignancy database was searched for all patients who had biological mesh abdominal wall reconstruction between 2004 and 2015. Short‐ and long‐term outcomes were reviewed. All patients had annual abdominal CT as routine peritoneal malignancy follow‐up. Results Some 33 patients (22 women) with a mean age of 53·4 (range 19–82) years underwent abdominal wall reconstruction with biological mesh. The majority (23) had CRS for pseudomyxoma (19 low grade), six for colorectal peritoneal metastasis and four for appendiceal adenocarcinoma; 18 had undergone CRS and HIPEC previously. Twenty‐five of the 33 patients had abdominal wall tumour involvement and eight had concurrent hernias. The mean duration of surgery was 486 (range 120–795) min and the mean mesh size used was 345 (50–654) cm2. Ten patients developed wound infections and four had a seroma. Two developed early enterocutaneous fistulas. Mean follow‐up was 48 months. Five patients developed an incisional hernia. Four died from progressive malignancy. A further 15 patients had disease recurrence, but only one had isolated abdominal wall recurrence. Conclusion Biological mesh was safe and effective for abdominal wall reconstruction in peritoneal malignancy. Postoperative wound infections were frequent but nevertheless incisional hernia rates were low with no instances of mesh‐related bowel erosion or fistulation.

Abstract

Critical success factors (CSFs) are the enablers to address the successful implementation of sustainable supply chain management (SSCM) practices in organizations. This study identifies and consolidates various relevant factors to develop the SSCM constructs conducive to supply chains in the Indian steel sector. A comprehensive framework of sustainability measurement has been developed through successive stages of data collection, analysis and refinement. Data have been collected from various departments of Tata Steel, SAIL, Essar and Jindal in India. The outcome of this research is a set of reliable, valid and unidimensional first-order measurements that can be subsequently used in conceptualization and measurement of the sustainability of supply chains in steel industry. Using factor analysis, we identify four constructs, namely favourable organizational environment, sustainable procurement initiatives, compliances to sustainability standards and external environmental pressures, which the organizations need to focus on. Additionally, using relative importance index ranking based on the survey data, the top three CSFs are compliance to environmental standards (ISO 14001 certification), safety and health focus (OHSAS 18001 certification) and top leadership commitment and support-all of which are within the organization’s own control. This study contributes to the continuing research of supply chain sustainability and provides supply chain managers with a practical approach for measuring and implementing sustainability practices across the steel supply chains.

Abstract

Uncoupling proteins (UCP) are carrier proteins that release protons during respiration dissipitating energy in the form of heat. Expression of these genes plays an important role in feed efficiency through regulation of energy balance. Therefore present study was undertaken to study the differential expression of UCP2 and UCP3 gene in feed efficient cattle in comparison to low feed efficient animal. HFX Sahiwal male calves of same age group were selected randomly and divided in to two groups (feed efficient and less feed efficient) on the basis of residual feed intake (RFI) for the experiment. Differential expression analysis of UCP2 and UCP3 revealed significant upregulation of UCP2 gene in calves having less feed efficiency where as UCP3 gene was having non significant effect on residual feed intake.

Abstract

Background: \ud Small bowel obstruction is a common surgical emergency, and is associated with high levels of morbidity and mortality across the world. The literature provides little information on the conservatively managed group. The aim of this study was to describe the burden of small bowel obstruction in the UK.\ud \ud Methods: \ud This prospective cohort study was conducted in 131 acute hospitals in the UK between January and April 2017, delivered by trainee research collaboratives. Adult patients with a diagnosis of mechanical small bowel obstruction were included. The primary outcome was in‐hospital mortality. Secondary outcomes included complications, unplanned intensive care admission and readmission within 30 days of discharge. Practice measures, including use of radiological investigations, water soluble contrast, operative and nutritional interventions, were collected.\ud \ud Results: \ud Of 2341 patients identified, 693 (29·6 per cent) underwent immediate surgery (within 24 h of admission), 500 (21·4 per cent) had delayed surgery after initial conservative management, and 1148 (49·0 per cent) were managed non‐operatively. The mortality rate was 6·6 per cent (6·4 per cent for non‐operative management, 6·8 per cent for immediate surgery, 6·8 per cent for delayed surgery; P = 0·911). The major complication rate was 14·4 per cent overall, affecting 19·0 per cent in the immediate surgery, 23·6 per cent in the delayed surgery and 7·7 per cent in the non‐operative management groups (P < 0·001). Cox regression found hernia or malignant aetiology and malnutrition to be associated with higher rates of death. Malignant aetiology, operative intervention, acute kidney injury and malnutrition were associated with increased risk of major complication.\ud \ud Conclusion: \ud Small bowel obstruction represents a significant healthcare burden. Patient‐level factors such as timing of surgery, acute kidney injury and nutritional status are factors that might be modified to improve outcomes.

Abstract

BACKGROUND: Carbon dioxide embolus has been reported as a rare but clinically important risk associated with transanal total mesorectal excision surgery. To date, there exists limited data describing the incidence, risk factors, and management of carbon dioxide embolus in transanal total mesorectal excision. OBJECTIVE: This study aimed to obtain data from the transanal total mesorectal excision registries to identify trends and potential risk factors for carbon dioxide embolus specific to this surgical technique. DESIGN: Contributors to both the LOREC and OSTRiCh transanal total mesorectal excision registries were invited to report their incidence of carbon dioxide embolus. Case report forms were collected detailing the patient-specific and technical factors of each event. SETTINGS: The study was conducted at the collaborating centers from the international transanal total mesorectal excision registries. MAIN OUTCOME MEASURES: Characteristics and outcomes of patients with carbon dioxide embolus associated with transanal mesorectal excision were measured. RESULTS: Twenty-five cases were reported. The incidence of carbon dioxide embolus during transanal total mesorectal excision is estimated to be ≈0.4% (25/6375 cases). A fall in end tidal carbon dioxide was noted as the initial feature in 22 cases, with 13 (52%) developing signs of hemodynamic compromise. All of the events occurred in the transanal component of dissection, with mean (range) insufflation pressures of 15 mm Hg (12-20 mm Hg). Patients were predominantly (68%) in a Trendelenburg position, between 30° and 45°. Venous bleeding was reported in 20 cases at the time of carbon dioxide embolus, with periprostatic veins documented as the most common site (40%). After carbon dioxide embolus, 84% of cases were completed after hemodynamic stabilization. Two patients required cardiopulmonary resuscitation because of cardiovascular collapse. Abstract

Organizational sustainability performance is highly dependent on organization’s culture and work practices including sustainable supply chain management. External pressures influence organizational practices and this also reflects on the sustainability performance indicators. This study presents an analysis of their interrelationships in the Indian steel sector. 21 measures representing dimensions of external pressure, organizational internal practices and sustainability performance were developed with reference to literature review and experts’ opinion from industry. A conceptual model has been proposed which has been empirically tested by survey data collected from 145 industry practitioners through online and offline survey. Using structural equation modelling technique, the study establishes that organizational practices positively impact the sustainability performance in India’s steel sector. The external pressures, though, impact organizational practices, their impact on sustainability performance is insignificant. The findings should help the practitioners, academics as well as policy makers in focusing greater attention to creating favorable conditions in the organization. This research paves the way for further work in this area.

Background: Multicystic peritoneal mesothelioma (MCPM) is a rare neoplasm, generally considered a borderline malignancy, best treated by cytoreductive surgery (CRS) to remove macroscopic disease, combined with hyperthermic intraperitoneal chemotherapy (HIPEC). Owing to its rarity, little has been published on clinical presentation, clinical behaviour over time, or an optimal treatment approach. Methods: A prospectively developed peritoneal malignancy database was interrogated for the years 2001-2018. Details on all patients with MCPM as a definitive diagnosis after CRS and HIPEC were analysed, including previous interventions, mode of presentation, surgical treatment, postoperative outcomes, and late follow-up information from abdominal CT and tumour markers. Results: Some 40 patients with MCPM underwent CRS and HIPEC between 2001 and 2018. Of these, 32 presented with abdominal pain, distension or bloating, six patients presented with recurrence following previous surgery at the referring hospitals, and two had coincidental diagnoses during a surgical procedure. CRS involved peritonectomy in all 40 patients. Bowel resection was required in 18 patients, and seven had a temporary stoma. Thirty-eight patients were considered to have undergone a complete macroscopic tumour removal (completeness of cytoreduction CC0), and two had residual tumour nodules less than 2.5 mm in size, classified as CC1. Median duration of follow-up was 65 (range 48-79) months. There were no deaths during follow-up. The Kaplan-Meier-predicted recurrence-free interval was 115.4 months. Conclusion: MCPM is a rare peritoneal neoplasm with a heterogeneous pattern of presentation. CRS and HIPEC is an effective management option for this group of patients, with favourable long-term survival. [ABSTRACT FROM AUTHOR]
