25. Les cystadénocarcinomes pancréatiques sont-ils des cystadénomes mucineux dégénérés?

Abstract

Resume Objectif de l'etude Cette etude vise a definir les arguments concordants ou discordants avec le concept generalement admis de filiation entre les cystadenomes mucineux (CM) et les cystadenocarcinomes pancreatiques (CK). Materiel et methodes Cette etude a ete realisee a partir d'une revue de la litterature et d'une enquete retrospective multicentrique ayant reuni 150 CM, 79 CK et 55 tumeurs intracanalaires papillaires et mucineuses (TIPMP). Les elements comparatifs entre CM et CK etaient : l'âge moyen au moment du diagnostic, le sex ratio, l'association d'un diabete sucre, la coexistence d'une autre tumeur maligne, la localisation de la tumeur et sa taille, le caractere communiquant de la tumeur, l'existence d'une pancreatite d'amont, l'elevation du taux serique du CA 19-9. L'analyse a porte egalement sur les examens anatomopathologiques des 58 CK reseques. Resultats Deux arguments sont apparus en faveur de la filiation dans cette etude : l'âge plus eleve des CK (65 ans vs 50,5 ans) et la decouverte histologique de formes de passage entre CM et CK dans 55 % des cas. A l'oppose, deux elements contradictoires sont apparus: la proportion plus elevee des CK chez l'homme (40 % vs 13 %) et la frequence plus grande des localisations cephaliques des CK (49 % vs 27 %). Les autres differences entre CM et CK (diabete associe, elevation du CA19-9, alterations genetiques) n'etaient pas en contradiction avec la filiation CM-CK. Les TIPMP etaient caracterisees par une majorite de cas masculins (67 %) et de localisations cephaliques (67 %). Conclusion La filiation CM-CK ne peut pas etre remise en cause malgre la constatation de differences sur le sex ratio et la localisation tumorale. Celles-ci peuvent s'expliquer par l'integration de TIPMP malignes dans les CK. Le cadre nosologique des CK communiquant avec le canal de Wirsung est tres proche de celui des TIPMP.

Abstract

The classical localisation of chromaffin cell tumours is intra-adrenal. Ectopic or multiple tumours are not rare and are commonly observed in children. The authors report a case of ectopic pheochromocytoma with a double localisation in a 14 year old child (renal pedicle and right retropleural space), in which surgical ablation resulted in an immediate and sustained correction of the hypertension. Hypertension recurred 24 years later and a classical right adrenal pheochromocytoma was demonstrated by methyl-iodo-benzylguanidine (M.I.B.G.) scintigraphy and abdominal CT scan. Right adrenalectomy resulted in normalisation of the hypertension once again without antihypertensive therapy with a follow-up of three years. Regular follow-up is necessary after ablation of a pheochromocytoma, especially in children, even in the absence of a phacomatosis or multiple endocrine neoplastic syndromes.

Abstract

Objectives of the studyTo define arguments in agreement or disagreement with the generally accepted theory of transformation from benign mucinous cystadenoma (MC) to malignant cystadenocarcinoma (CC) of the pancreas.MethodsA review of the literature since 1978 was conducted together with a comparative analysis of a multicentre retrospective study of the different mucinous cystic tumours of the pancreas: 150 MC, 79 CC and 55 intraductal papillary mucinous tumours of the pancreas (TIPMP). Multiple epidemiological and clinicopathological data were compared between MC and CC: mean age at diagnosis, sex distribution, association with diabetes or concurrent cancer, tumour location and size, type of pancreatic main duct communication, pancreatic fibrosis and serum CA 19-9 levels. The analysis concerned also histopathological features of resected cystadenocarcinomas (n = 58).ResultsTwo features suggested malignant transformation of mucinous cystadenomas: older mean age at diagnosis of CC and histological characteristics revealing areas of benign-appearing epithelium associated with areas of invasive carcinoma in 55% of cases. Conversely, two statistically significant discordant features were observed: a higher proportion of men (40% for CC vs 13% for MC) and a more frequent location in the head of the pancreas (49% vs 27%). The other differences were not in contradiction with the malignant transformation of MC: associated diabetes mellitus, increasing serum CA 19-9 levels and chromosomal aberrations were more frequent in CC. TIPMP were predominant in men (67%), and more common in the head of the pancreas (67%).ConclusionThe risk of malignant transformation of MC should not be questioned, despite sex distribution and location differences between MC and CC. These differences may be due to inaccurate designation of intraductal mucinous carcinomas as communicating CC or of CC as pancreatic mucinous adenocarcinomas.
