Probiotic administration has become common practice in neonatal intensive care units (NICUs) to prevent necrotizing enterocolitis (NEC) and promote gut health in preterm infants. While probiotics are generally considered safe, rare cases of probiotic-related sepsis have been reported. We present a case of Lactobacillus rhamnosus sepsis in a preterm infant, highlighting the challenges involved in its diagnosis. The infant developed symptoms of sepsis on the 13th day of probiotic treatment. Laboratory analyses, including MALDI-TOF, BioFire BCID2 panel, and whole-genome sequencing (WGS), helped confirm the diagnosis and the presence of Lactobacillus rhamnosus . In this case, accurately identifying the Lactobacillus rhamnosus strain proved challenging, as initial analyses using the Vitek 2 system yielded incorrect identifications. This highlights the limitations of automated systems in distinguishing closely related species, reinforcing the need for advanced molecular techniques to achieve precise strain identification and confirm a probiotic-related infection. Given these diagnostic complexities, it is crucial for clinicians to maintain a high index of suspicion for probiotic-related infections in cases of unexplained sepsis, as this awareness can prompt further diagnostic investigations to ensure accurate pathogen identification. The infant responded to ampicillin therapy, showing clinical improvement within 10 days and was discharged in good health at 67 days of life. This case underscores the importance of advanced molecular diagnostic methods to confirm probiotic-related infections and highlights the need for caution in administering probiotics to vulnerable populations, such as preterm infants. Clinicians must maintain a high index of suspicion for probiotic-associated sepsis in unexplained cases of infection and tailor antibiotic therapy based on susceptibility profiles. These findings emphasize the need for rigorous monitoring, appropriate probiotic strain selection, and optimized safety protocols in NICUs to mitigate potential risks.

Life cycle thinking methods such as life cycle assessment (LCA) and costing (LCC) were originally developed to assess the performance of products and services (business-making decisions). However, they are increasingly deployed to support policy-making along the entire policy cycle, including via impact assessment (IA) of different policy options. These applications are associated with a number of challenges, mainly related to the dynamic and prospective nature of policy IA, typically forward-looking into 10-20 years ahead. This requires the application of prospective scenario analyses to develop the baseline scenario, reflecting the 'do nothing' into the future (business-as-usual), and the alternative scenarios, reflecting the consequences incurred by the implementation of the policy options under study. Such prospective and broadened boundary nature of policy IA challenges traditional LCA/LCC practices mostly based on retrospective, static scenarios and datasets. The present study provides an overview of recent IA studies supporting waste policy to show the state-of-the-art and the main challenges associated with the application of life cycle methods in IA, focusing on the most recent EU waste and circular economy policies. Moreover, since specific and transparent guidance on how to implement consistently these methods in policy IA is lacking, the study provides an initial guidance for application of life cycle thinking in IA drawing upon the knowledge obtained conducting waste policy IA studies. This study describes two cases of bacteraemia sustained by a new putative Pannonibacter species isolated at the U.O.C. of Microbiology and Virology of the Policlinico of Bari (Bari, Italy) from the blood cultures of two patients admitted to the Paediatric Oncohaematology Unit. Pannonibacter spp. is an environmental Gram-negative bacterium not commonly associated with nosocomial infections. Species identification was performed using Sanger sequencing of the 16S rRNA gene and Whole-Genome Sequencing (WGS) for both strains. Genomic analyses for the two isolates, BLAST similarity search, and phylogeny for the 16S rDNA sequences lead to an assignment to the species Pannonibacter phragmitetus . However, by performing ANIb, ANIm, tetranucleotide correlation, and DNA-DNA digital hybridization, analyses of the two draft genomes showed that they were very different from those of the species P. phragmitetus . MALDI-TOF analysis, assessment of antimicrobial susceptibility by E-test method, and Analytical Profile Index (API) tests were also performed. This result highlights how environmental bacterial species can easily adapt to the human host and, especially in nosocomial environments, also gain pathogenic potential through antimicrobial resistance.

This study reports a peculiar case of systemic candidiasis infection associated with pulmonary aspergillosis in an apparently immunocompetent alpaca. A captive 7-year-old female alpaca exhibited respiratory symptoms, underwent treatment with benzylpenicillin and dexamethasone, and succumbed to the infection 40 days later. During the post-mortem examination, subcutaneous emphysema, widespread pneumonia with multiple suppurative foci, scattered necro-suppurative lesions throughout the renal and hepatic parenchyma were evident. Histopathological analysis of the collected tissues revealed multifocal mild lymphoplasmacytic chronic interstitial nephritis, necro-suppurative pneumonia with the presence of fungal hyphae, multifocal foci of mineralization, and fibrosis in the liver. Fungal cultures confirmed the growth of Aspergillus fumigatus from the lungs, and Candida albicans from the liver, kidney, and heart. The only recognizable risk factor for candidiasis and pulmonary aspergillosis in this case was prior corticosteroid and antibiotic therapy. Nevertheless, it is crucial to consider systemic candidosis and pulmonary aspergillosis as potential differential diagnoses in respiratory infections among camelids. Prolonged treatment with glucocorticoids and antibiotics should be avoided as it could represent a risk factor for the onset of pathologies caused by opportunistic fungi such as Candida spp. and Aspergillus spp.

Land use is a major threat to terrestrial biodiversity. Life cycle assessment is a tool that can assess such threats and thereby support environmental decision-making. Within the Global Guidance for Life Cycle Impact Assessment (GLAM) project, the Life Cycle Initiative hosted by UN Environment aims to create a life cycle impact assessment method across multiple impact categories, including land use impacts on ecosystem quality represented by regional and global species richness. A working group of the GLAM project focused on such land use impacts and developed new characterization factors to combine the strengths of two separate recent advancements in the field: the consideration of land use intensities and land fragmentation. The data sets to parametrize the underlying model are also updated from previous models. The new characterization factors cover five species groups (plants, amphibians, birds, mammals, and reptiles) and five broad land use types (cropland, pasture, plantations, managed forests, and urban land) at three intensity levels (minimal, light, and intense). They are available at the level of terrestrial ecoregions and countries. This paper documents the development of the characterization factors, provides practical guidance for their use, and critically assesses the strengths and remaining shortcomings.

National studies on food waste quantification in EU countries present highly discrepant results due to the different quantification approaches adopted. The European Commission has published a delegated act establishing a common methodology and minimum quality requirements for the uniform measurement of food waste generated in Member States. Nevertheless, as EU countries are at different levels of development and implementation of national strategies for food waste quantification, there is a need to develop a harmonized modelling system that enables the estimation of food waste generated by Member States to assess the amounts reported by each country. The aim of this paper is to fulfil this need by presenting two modelling approaches to estimate food waste in EU countries. One approach is based on Material Flow Analysis (MFA) and combines statistical information on the production and trade of food products with food waste coefficients. The other approach is based on the estimation of food waste based on waste statistics. Three EU countries are used to illustrate the two approaches and compare the results obtained thereby. Food waste estimates from waste statistics are generally lower than those obtained using MFA, in particular at the early stages of the food chain. The MFA model presented in this article is the first of its kind developed to estimate food waste across Member States in a consistent way and through time. As a result of the growing awareness of the need to prevent food waste, several initiatives have been launched in the last few years to reduce food waste generated across the food supply chain. However, the evaluation of food waste prevention interventions is still at an early stage of development and appropriate methods to assess their effectiveness are missing, hampering the identification of best practices amongst existing initiatives and the prioritisation of those that are most promising. To address such needs and provide a common approach to consistently assess the performance of food waste prevention initiatives, the European Commission Joint Research Centre has developed an evaluation framework for food waste prevention actions. The framework supports the EU Platform on Food Losses and Food Waste, which has been established to identify best practices and share knowledge on food waste prevention initiatives. Additionally, a food waste prevention calculator, based on life cycle thinking, has been developed to support such an evaluation by a consistent assessment of the environmental and economic benefits of such initiatives, and the identification of potential trade-offs at early design stages. The main goal of this paper is to present the evaluation framework and the calculator developed, critically discussing how future initiatives should be designed, monitored and reported, to ensure sufficient and relevant data is made available to enable their proper assessment. The need to increase circularity of industrial systems to address limited resources availability and climate change has triggered the development of the food waste biorefinery concept. However, for the development of future sustainable industrial processes focused on the valorisation of food waste, critical aspects such as (i) the technical feasibility of the processes at industrial scale, (ii) the analysis of their techno-economic potential, including available quantities of waste, and (iii) a life cycle-based environmental assessment of benefits and burdens need to be considered. The goal of this review is to provide an overview of food waste valorisation pathways and to analyse to which extent these aspects have been considered in the literature. Although a plethora of food waste valorisation pathways exist, they are mainly developed at lab-scale. Introduction: Few studies have investigated factors possibly related to the source of Helicobacter pylori infection in humans., Materials and Methods: This was a retrospective study including a population of 201 H.pylori positive patients and 259 H. pylori negative subjects observed at a tertiary referral center in Apulia. The H. pylori status was assessed by urea breath test. Data on socio-demographic characteristics and the consumption of different foods and beverages in the last year were collected by a questionnaire., Results: No significant relationship was found between H. pylori infection and gender or age, type of employment, working in direct contact with the public, residence, level of education or exposure to pets. No association between H.pylori status and the consumption of fish, fruits, legumes, honey, spices, meats, milk and milk products including some typical product of our area was found. The same was true for the consumption of several kind of beverage including green tea and wine. Cigarette smoking and living in the same house with H.pylori positive relatives were significantly associated with H. pylori positivity. The intake of uncooked seafood (mussels and other molluscs) as well as some uncooked vegetables such as tomatos,pepper,and chicory,municipal water and the number of cups of coffee consumed per week correlated significantly with H. pylori status. The consumption of raw vegetables purchased from street vendors and the consumption of meals outside home were also associated with H.pylori infection., Conclusions: Foods may represent an important route in the transmission of H. pylori among humans., (Copyright © 2018. In 2015, the United Nations defined the Sustainable Development Goals (SDG), which include a target (12.3) on food waste. The target requires "by 2030, to halve per capita global food waste at the retail and consumer levels and to reduce food losses along production and supply chains, including post-harvest losses". The target has increased awareness about the food waste problem and boosted research in food waste quantification. Nevertheless, there is a lack of studies that adopt a systematic approach to account for food waste providing disaggregated values per food supply chain stage and per food groups. Such an approach could support policy makers in prioritizing interventions for food waste reduction. To fill this gap, this paper presents a high-level top-down approach to food waste accounting in the European Union. The study aims to support the understanding of the mass flows associated with food production, consumption, and waste, addressing different food groups along the food supply chain. The method for accountin is the mass flow analysis. According to the results, cereals, fruit, and vegetables as the food groups are responsible for the highest amount of food waste, and the consumption stage to be responsible for the largest share of food waste for most food groups. This work highlights the need for further primary research on food waste generation in the EU. Impacts associated with land use are increasingly recognized as important aspects to consider when conducting Life Cycle Assessment (LCA). Across the existing models accounting for land use activities in life cycle impact assessment, a balance is yet to be found between complexity and comprehensiveness on one hand, and applicability on the other hand. This work builds on the LANd use indicator value CAlculation (LANCA ® ) model, assessing the impacts of land use activities on five soil properties, and aims at developing an aggregated index to improve its applicability. First a statistical analysis is conducted, leading to the shortlisting of the four most significant soil quality indicators. Then two options for aggregating the selected indicators are presented: the soil quality index (SQI), based on linear aggregation, and the normalisation-based soil quality index (NSQI), where the aggregation process involves normalisation integrated into the characterisation step. Country-specific and global average characterisation factors (CFs) are calculated for 57 land use types considering both land occupation and land transformation interventions with the two suggested approaches. The two indices present similar ranking of land use types but the relative contribution of the separate indicators to the aggregated index varies according to the approach adopted. The differences between the aggregation approaches suggested are discussed, together with the limitations related to both the LANCA ® model and the aggregation approaches. This work represents a first step towards the widespread application of a comprehensive and robust land use model at midpoint level in LCA. Finally, a number of recommendations for the future development of the LANCA ® model and of the related soil quality models are provided.

According to national studies conducted in EU countries, fresh fruit and vegetables contribute to almost 50% of the food waste generated by households. This study presents an estimation of this waste flow, differentiating between unavoidable and avoidable waste. The calculation of these two flows serves different purposes. The first (21.1 kg per person per year) provides a measure of the amount of household waste intrinsically linked to the consumption of fresh fruit and vegetables, and which would still be generated even in a zero-avoidable waste future scenario. The second (14.2 kg per person per year) is a quantity that could be reduced/minimised by applying targeted prevention strategies. The unavoidable waste was assessed at product level, by considering the inedible fraction and the purchased amounts of the fifty-one most consumed fruits and vegetables in Europe. The avoidable waste was estimated at commodity group level, based on the results of national studies conducted in six EU member states. Significant differences in the amounts of avoidable and unavoidable waste generated were found across countries, due to different levels of wasteful behaviours (linked to cultural and economic factors) and different consumption patterns (influencing the amount of unavoidable waste generated). Schistosomiasis is the most prevalent tropical disease in the world after malaria. According to the World Health Organization, the disease afflicts more than 240 million people in about 80 countries. Recently, an epidemiological surveillance study performed between 1997 and 2010 by the European Network for Tropical Medicine and Health Travel regarding schistosomiasis between immigrants and travelers has been published. No data are available in the literature regarding the situation in South Italy. Herein, we report the prevalence of urinary schistosomiasis in a population of migrants in Apulia referring to our outpatient clinic for immigrant diseases in the period 2006-2016. Since all cases of schistosomiasis were related to the last three years of observation, the demographic and clinical characteristics of the study population were compared before and after 2014. Nearly 51% of all patients visited (1762) were from high/moderate endemic countries for schistosomiasis, and nine cases of urinary schistosomiasis were diagnosed. Prevalence was 1% among migrants from endemic areas and 10% in those from Mali and Senegal. Our findings confirm that schistosomiasis is a widespread infection among immigrants, even if it is often underdiagnosed because of the multifaceted clinical presentation. Changes in migratory dynamics can affect clinical observations very quickly.
