Abstract

Appropriate administration of intraoperative analgesia is an essential factor in care and reasonable recovery times. Inappropriate intraoperative analgesia puts the patient at risk of acute postoperative pain (APOP). The absence of an objective standard for intraoperative nociceptive monitoring complicates pain care. Heart rate (HR) and mean arterial blood pressure (MABP) have been suggested as useful parameters during general anesthesia for nociceptive monitoring. However, studies focusing on whether intraoperative heart rate variability (HRv) and mean arterial blood pressure variability (MABPv) during general anesthesia can accurately monitor nociception in patients have remained inconclusive. The current study aimed to (1) identify the association of intraoperative heart rate and blood pressure variability in patients undergoing low-risk surgery with the incidence of APOP in the immediate postoperative setting and (2) evaluate the associations of clinical demographic factors with the incidence of APOP.

Abstract

We study the problem of dynamically maintaining the connected components of an undirected graph subject to edge insertions and deletions. We give the first parallel algorithm for the problem which is work-efficient, supports batches of updates, runs in polylogarithmic depth, and uses only linear total space. The existing algorithms for the problem either use super-linear space, do not come with strong theoretical bounds, or are not parallel. On the empirical side, we provide the first implementation of the cluster forest algorithm, the first linear-space and poly-logarithmic update time algorithm for dynamic connectivity. Experimentally, we find that our algorithm uses up to 19.7x less space and is up to 6.2x faster than the level-set algorithm of HDT, arguably the most widely-implemented dynamic connectivity algorithm with strong theoretical guarantees., Comment: This is a preliminary version of a paper that will appear at VLDB'25

Abstract

Having worked mainly on the late Roman and Visigothic walls of the Spanish provinces, I came across the details of Dacian-Moesian frontier defences mostly by chance, through a book by Carl Schuchhardt and his contributions to linear earthworks. I would like to contrast these valla in the specific angle of agency, as the discussions on the Moldovan wall of Athanaric and its chronology are reminiscent of questions posed as well in southwestern imperial contexts. Although no precise equivalences for such extensive, often river-connected ramparts are known in Hispania, I wish to put a few points in perspective and to establish some not strictly archaeological, but rather interpretive correspondences. The purpose is framing a meta-analysis for early Gothic defensive concerns, with a focus on large built structures, from the standpoint of their perceived usefulness, as a trigger for agency.

Abstract

Accelerator-based techniques are considered among the leading methods to produce radioactive nuclei. The ISOLDE facility (Isotope mass Separator On-Line DEvice) at the European Organization for Nuclear Research (CERN) is a unique source of beams of radioactive nuclides that are used in a wide range of research domains, from nuclear astrophysics to life sciences. Over 400 ISOLDE targets are currently stored at CERN and an average of 30 targets are irradiated every year. All these targets are planned to be dismantled and disposed of as radioactive waste in a dedicated repository in Switzerland. This paper provides an overview of the challenges related to the radionuclide activity predictions, the number of activation scenarios, dismantling and conditioning of the Radioactive Waste (RW) using a hot cell (HC), the high dose rate, uncertainties related to unknown geometry parameters of the RW packages, and finally industrializing a complex RW elimination process as ITEP for an accelerator complex. The performed work addresses each of these challenges and offers technical solutions based on state-of-the-art computational codes, statistical techniques, and state of art Non-Destructive Assay (NDA) techniques using high energy-resolution gamma spectrometry. The methodology followed can be of guidance for the development of similar processes at other facilities.

5. CT-based Anomaly Detection of Liver Tumors Using Generative Diffusion Prior

Abstract

CT is a main modality for imaging liver diseases, valuable in detecting and localizing liver tumors. Traditional anomaly detection methods analyze reconstructed images to identify pathological structures. However, these methods may produce suboptimal results, overlooking subtle differences among various tissue types. To address this challenge, here we employ generative diffusion prior to inpaint the liver as the reference facilitating anomaly detection. Specifically, we use an adaptive threshold to extract a mask of abnormal regions, which are then inpainted using a diffusion prior to calculating an anomaly score based on the discrepancy between the original CT image and the inpainted counterpart. Our methodology has been tested on two liver CT datasets, demonstrating a significant improvement in detection accuracy, with a 7.9% boost in the area under the curve (AUC) compared to the state-of-the-art. Abstract

Ultra-high resolution images are desirable in photon counting CT (PCCT), but resolution is physically limited by interactions such as charge sharing. Deep learning is a possible method for super-resolution (SR), but sourcing paired training data that adequately models the target task is difficult. Additionally, SR algorithms can distort noise texture, which is an important in many clinical diagnostic scenarios. Here, we train conditional denoising diffusion probabilistic models (DDPMs) for PCCT super-resolution, with the objective to retain textural characteristics of local noise. PCCT simulation methods are used to synthesize realistic resolution degradation. To preserve noise texture, we explore decoupling the noise and signal image inputs and outputs via deep denoisers, explicitly mapping to each during the SR process. Our experimental results indicate that our DDPM trained on simulated data can improve sharpness in real PCCT images. Additionally, the disentanglement of noise from the original image allows our model more faithfully preserve noise texture., Comment: 5 pages, 4 figures

Abstract

While drastic improvements in CT technology have occurred in the past 25 years, spatial resolution is one area where progress has been limited until recently. New photon counting CT systems, are capable of much better spatial resolution than their (energy integrating) predecessors. These improvements have the potential to improve the evaluation obstructive coronary artery disease by enabling more accurate delineation between calcified plaque and coronary vessel lumen. A new set of vessel phantoms has been designed and manufactured for quantifying this improvement. Comparisons are made between an existing clinical CT system, a prototype photon counting system, with images from a micro CT system being used as the gold standard. Scans were made of the same objects on all three systems. The resulting images were registered and the luminal cross section areas were compared. Luminal cross-sections near calcified plaques were reduced due to blooming, but this effect was much less pronounced in images from the prototype photon counting system as compared to the images from the clinical CT system.

Abstract

Recent development of photon-counting CT (PCCT) brings great opportunities for plaque characterization with much-improved spatial resolution and spectral imaging capability. While existing coronary plaque PCCT imaging results are based on detectors made of CZT or CdTe materials, deep-silicon photon-counting detectors have unique performance characteristics and promise distinct imaging capabilities. In this work, we report a systematic simulation study of a deep-silicon PCCT scanner with a new clinically-relevant digital plaque phantom with realistic geometrical parameters and chemical compositions. This work investigates the effects of spatial resolution, noise, motion artifacts, radiation dose, and spectral characterization. Our simulation results suggest that the deep-silicon PCCT design provides adequate spatial resolution for visualizing a necrotic core and quantitation of key plaque features. Advanced denoising techniques and aggressive bowtie filter designs can keep image noise to acceptable levels at this resolution while keeping radiation dose comparable to that of a conventional CT scan. The ultrahigh resolution of PCCT also means an elevated sensitivity to motion artifacts. It is found that a tolerance of less than 0.4 mm residual movement range requires the application of accurate motion correction methods for best plaque imaging quality with PCCT., Comment: 13 figures, 5 tables

Despite scientific evidence that better disease outcomes can be achieved by enhancing self-management, many COPD patients do not respond to self-management interventions. To move toward more effective self-management interventions, knowledge of characteristics associated with activation for self-management is needed. The purpose of this study was to identify key patient and disease characteristics of activation for self-management. Methods: An explorative cross-sectional study was conducted in primary and secondary care in patients with COPD. Data were collected through questionnaires and chart reviews. The main outcome was activation for self-management, measured with the 13-item Patient Activation Measure (PAM). Independent variables were sociodemographic variables, self-reported health status, depression, anxiety, illness perception, social support, disease severity, and comorbidities. Results: A total of 290 participants (age: 67.2±10.3; forced expiratory volume in 1 second predicted: 63.6±19.2) were eligible for analysis. While poor activation for self-management (PAM-1) was observed in 23% of the participants, only 15% was activated for self-management (PAM-4).
