Abstract

Simple Summary: This study addresses the ongoing scientific debate regarding the existence, causes, characteristics and reversibility of cognitive sequelae associated with COVID-19 infection. The aim of the study is to describe the neuropsychological profile of individuals diagnosed with COVID-19, distinguishing between younger (<65 years) and older (≥65 years) adults and evaluating them at baseline and at 3 and 6 months after infection. Significant differences in cognitive performance between younger and older adults are observed, consistent with their different physiological conditions. However, distinct memory recall patterns not attributable to physiological differences are also observed. The results contribute to the understanding of COVID-19 cognitive sequelae, suggesting that cognitive deficits in COVID-19 survivors may primarily reflect difficulties in attention and concentration impacting retrieval processes, aligning with the concept of "brain fog" associated with post-COVID-19 syndrome. The literature on COVID-19 continues to increase daily. Cognitive sequelae associated with COVID-19 infection still draw the attention of the scientific community given the lack of consensus about their existence, etiology, characterization and reversibility. The aim of this study is to provide a neuropsychological identikit for younger (<65 years) and older (≥65 years) individuals diagnosed with COVID-19 infection, at baseline and after 3 and 6 months. In total, 226 individuals took part in a retrospective observational study and their cognitive performance was compared across groups (younger adults vs. older adults) and time (T0, T1, T2). The results highlighted differences between younger and older adults in the Montreal Cognitive Assessment (MoCA) global score, as expected in consideration of the different physiological conditions of the two populations. However, memory performance highlighted the two groups as characterized by a difference in patterns of recall that may move beyond a physiological explanation and provide information about COVID-19 cognitive sequelae. This study suggests that cognitive deficits observed in COVID-19 survivors may reflect a difficulty in attention and concentration that interferes mainly with retrieval processes. This result fits well with the concept of "brain fog" typical of post-COVID-19 syndrome and may also reflect the stress experienced while facing the pandemic. [ABSTRACT FROM AUTHOR]

Nevertheless, in older adults, it often remains undetected and untreated. This narrative review aims at giving an overview on the main definitions, clinical manifestations, risk and protective factors for depression in the elderly, and at discussing the main reasons for its under/misdiagnosis, such as cognitive decline and their overlapping symptomatology. Abstract

Little is known of the factors that transform fear of falling (FOF) from a normal adaptive to a maladaptive response that could alter its impact on fall risk. Focusing on judgment capacity, we investigated whether it is associated with FOF and FOF-related activity restriction (AR), and whether it modifies the influence of FOF on fall risk. Data came from 2625 community-dwelling older adults enrolled in the Progetto Veneto Anziani. Baseline FOF and AR were assessed through personal interviews, and judgment capacity-high, moderate, or poor-through situational tests. At follow-up after 4.4 years, self-reported falls during the previous year were recorded. The associations between judgment and FOF/AR, and between FOF and the risk of at least one fall or recurrent falls (two or more falls), stratified by judgment capacity, were evaluated using multinomial logistic regressions. Compared with high-judgment participants, lower judgment participants were 20% more likely to report FOF; moderate judgment participants were 54% more likely and poor judgment participants twice as likely to report AR. After adjusting for potential confounders, including physical activity and physical performance, FOF increased the reporting of at least one fall only in the poor judgment group. The association between FOF and recurrent falls was stronger in individuals with poor (odds ratio [OR] = 3.66, 95% confidence interval [CI]: 2.10-6.36) than with moderate (OR = 2.81, 95% CI: 2.22-3.55) or high (OR = 1.65, 95% CI: 1.48-1.83) judgment. Poor judgment capacity increases the probability of FOF and AR in older adults, and may exacerbate the effect of FOF in increasing fall risk.

Abstract

Background: aortic valve stenosis (AVS) is becoming more and more common in the elderly population, involving around 13.2% of subjects older than 75 years. In addition to worsening quality of life, untreated severe AVS has been associated with high short term mortality rate. However, these adverse outcomes could be modified by aortic valve replacement (AVR). In recent years, in particular, newer surgery procedures and anesthesiological techniques have allowed also older and frailer patients access to AVR procedures. Previous studies have evaluated the effectiveness of AVR in older subjects in terms of peri- and post-operative mortality, but its impact on frail patients’ global health has been scarcely investigated. Aim of the study: the aim of our study was to evaluate the impact of AVR procedure on older patients’ physical performance, cognitive status and quality of life at 45 days, three and six months after surgery. Subjects and methods: this prospective study included 46 patients over 70 years, enrolled in collaboration with the Department of Cardiac Surgery of the University of Padova. All subjects were affected by AVS, and were recommended to undergo AVR. Study participants were evaluated with a multidimensional geriatric assessment before AVR (T0) and 45 days (T1), three (T2) and six months (T3) after surgery. In particular, for each participant we collected data on clinical examination, self-sufficiency (using the Activites of Daily Living and Instrumental Activites of Daily Living scales), cognitive status (using the Mini Mental State Examination and the Montreal Cognitive Assessment), presence of depressive symptom (through the Geriatric Depression Scale), quality of life (using the Short-Form 36 items Health Survey) and physical performance (through the Short Physical Performance Battery, gait speed, 6-minute walking test, and measurements of upper and lower limbs strength). Results: of the initial sample of 46 patients, 22 reached the 6-month follow-up and were included in the study. Compared with the pre-operative evaluation, at T1 we observed a significant decline in nutritional status (BMI, arm circumference, MNA score) and in physical performance (SPPB, handgrip strength, lower limb flexion-extension and isometric strength). At T3, MMSE, MoCA, MNA and SPPB scores improved significantly from baseline. Upper and lower limbs strength at 6-month follow up was not significantly different from baseline. Quality of life remained nearly stable at T1, but improved significantly at T2 and T3. Considering the variation in gait speed from baseline to 6-month follow up, subjects that showed an improvement in this item compared to worsening subjects, at baseline had lower values of handgrip and limbs strength, gait speed and 6-MWT distance. Conclusions: our results show that in older AVS patients, AVR have a positive impact on nutrition, physical performance, cognitive functioning, mood and quality of life, whereas it does not have any significant impact on limbs muscle strength.

Abstract

Background: Reduced physical performance is predictive of deleterious outcomes in older adults. Data considering objective physical performance and incident depression is sparse.Objective: We investigated whether objective physical performance can predict incident depression among non-depressed older adults during a 4-year study.Design: longitudinal.Methods: From 3,099 older individuals initially enrolled in the Progetto Veneto Anziani study, 970 participants without depression at baseline were included (mean age 72.5 years, 54.6% females). Physical performance measures included the Short Physical Performance Battery (SPPB), 4m gait speed, five times sit-to-stand test, leg extension and flexion, handgrip strength, and 6-Minute Walking Test (6MWT), categorized in gender-specific tertiles. Depression was classified based on the Geriatric Depression Scale (GDS) and a diagnosis from a geriatric psychiatrist. Area under the curve (AUC) and logistic regression analyses were conducted.Results: At baseline, participants developing depression during follow-up (n = 207) scored significantly worse across all physical performance measures than those who did not develop depression. The AUC and predictive power for each physical performance test was similar for all the tests assessed. In logistic regression analysis, after adjusting for 14 potential confounders, worse physical performance across all tests increased the risk of depression. The lowest tertile of the SPPB were at notable odds of developing depression (OR = 1.79; 95%CI: 1.18-2.71). The association between poor physical performance and depression was typically stronger in women than in men, except for 4m gait speed.Limitations: no gold standard used for depression diagnosis; oxidative stress and inflammatory markers were not included; high rate of missing data at follow-up.Conclusion: Low physical performance appears to be an independent predictor of depression over a 4.4-year follow-up in our sample of elderly people.
