RIVM rapport:Nederland streeft naar een circulaire economie in 2050. Een onderdeel daarvan is grondstoffen en materialen recyclen, zoals zonnepanelen. Op dit moment worden grondstoffen uit zonnepanelen na gebruik nog niet teruggewonnen. Naar verwachting zullen over ruim vijf jaar de eerste grote hoeveelheden zonnepanelen als afval vrijkomen. Het is belangrijk om hierop voorbereid te zijn en ze veilig en duurzaam te kunnen recyclen. Er zijn verschillende technologieën in ontwikkeling om zonnepanelen te recyclen. Het RIVM heeft vier mogelijkheden uitgewerkt om het glas, de zonnecellen en het achterblad ervan te recyclen. De teruggewonnen materialen uit zonnepanelen kunnen opnieuw worden gebruikt als grondstof voor verschillende toepassingen. In dit onderzoek is gekeken is welke mogelijkheden in de praktijk uitvoerbaar lijken en hoe milieuvriendelijk ze zijn. De vier varianten zijn vergeleken met de huidige situatie (de basisvariant). Daarin wordt vermalen glas van zonnepanelen als schuurmiddel in de metaalindustrie gebruikt en daarna verwerkt in bijvoorbeeld funderingsmateriaal voor wegen. Uit de analyse blijkt dat alle vier de varianten meer circulair en milieuvriendelijker zijn dan de basisvariant. Het energiegebruik verschilt iets per variant maar is veel lager dan dat van de basisvariant. Dat komt onder andere doordat het meer energie kost om nieuwe grondstoffen voor zonnepanelen te maken dan met gerecyclede grondstoffen te werken. Het meest circulair is de variant waarin van glas nieuw glas voor zonnepanelen wordt gemaakt. In deze variant kan ook de grondstof silicium worden herwonnen voor nieuwe zonnecellen. Dit is technologisch ingewikkeld maar wel mogelijk. Bij de recycling is aandacht nodig voor gevaarlijke stoffen in zonnepanelen: lood, antimoon en PFAS. Lood zit in het soldeermateriaal en antimoon zorgt voor de helderheid van het glas. PFAS zitten als fluorpolymeren in het achterblad van zonnepanelen, waardoor bij verbranding PFAS kunnen vrijkomen. Currently, no raw materials are recovered from End-of-Life solar panels. It is expected that the first generation of solar panels will start to be taken out of use in large numbers in five years' time. It is important to be prepared for this and to recycle the panels safely and sustainably. Various technologies to recycle solar panels are being developed. RIVM has detailed four options for recycling the glass, solar cells and back sheets of solar panels. The materials recovered from solar panels can be reused as raw materials for various applications. For this study, we looked into which recycling options appear to be feasible in practice and how environmentally friendly they are. We compared these four options to the current situation (the baseline), in which the solar panels are shredded, the glass is crushed and used as an abrasive medium in the metal industry, and the remainder is then processed for various applications, such as for road bases. Our analysis shows that all four options are more circular and environmentally friendly than the baseline. Energy consumption differs for each option, but is much lower than for the baseline. This is due to various factors, including the fact that it costs more energy to process new raw materials into solar panels than to work with recycled raw materials. The option whereby glass is recycled into new glass for solar panels is the most circular one. In this case, the raw material silicon can also be recycled for use in new solar panels. This is technologically complex, but feasible. In the recycling process, attention must be paid to hazardous substances in solar panels: lead, antimony and per- and polyfluoroalkyl substances (PFAS). 01 januari 2023, Item does not contain fulltext, The introduction of bowel cancer population screening programs has had a profound impact on gastrointestinal pathology. While the focus is mainly on quality assurance of diagnoses relevant for the outcome of these programs (colorectal cancer and its precursors), incidental findings are increasingly diagnosed. The incidence of such findings is largely unknown. Therefore, we investigated the incidence of incidental findings within the national screening program of the Netherlands. From the Dutch nationwide pathology databank (PALGA), we retrieved all histological diagnoses of patients participating in the national bowel cancer screening program from the start in 2014 until 1/1/2021. Descriptive statistics were used. During these 7 years, in total 9407 other polyps and malignancies (262 per 10,000 colonoscopies) were diagnosed. The majority (65%) were classified as inflammatory polyps. The most common malignancies were neuroendocrine tumours (n = 198, 6 per 10,000 colonoscopies); less common were lymphomas (n = 64) and metastases (n = 33). Mesenchymal polyps, such as leiomyomas and lipomas, were relatively common (27 and 16 per 10,000 colonoscopies, respectively), in comparison with neural polyps such as perineuriomas, ganglioneuromas, and neurofibromas (respectively 3, 2, and 1 per 10,000 colonoscopies). This is the largest study into the incidence of nonconventional colorectal polyps and malignancies in a homogeneous cohort of asymptomatic patients. Several of these diagnoses may have consequences for treatment and follow-up, in particular the malignancies and detection of patients with hereditary cancer syndromes.

Item does not contain fulltext, BACKGROUND AND AIMS: Screening for colorectal cancer (CRC) aims to decrease CRC incidence and mortality. Biennial fecal immunochemical test screening started in the Netherlands in 2014 for individuals 55-75 years of age. This study investigated the effect of screening on stage-specific incidence, with focus on stage III and IV CRC. METHODS: Inhabitants diagnosed with CRC in 2009-2018 were included. CRC incidence per stage, year, and detection method (ie, screen-detected vs clinically detected) was evaluated. Patient, tumor, and treatment characteristics, and survival of patients with stage III and IV CRC, were compared according to the detection method. RESULTS: Included were 140,649 CRCs in 136,882 patients. An initial peak of stage I-III CRC diagnoses after initiation of screening was followed by a continuous decrease within screening-eligible ages. Total CRC incidence remained higher than before screening, although stage II and IV CRC incidence decreased below prescreening levels. Screen-detected CRCs were significantly more frequently located in the left-sided colon (stage III; 43.7% vs 30.9%; stage IV: 45.1% vs 36.1%), and the primary tumor resection rate was higher (stage III colon: 99.8% vs 99.0%, rectum: 97.3% vs 89.7%; stage IV colon: 65.4% vs 56.6%, rectum: 47.3% vs 33.5%). Patients with screen-detected stage IV CRC had significantly more often single-organ metastases (74.5% vs 57.0%; P < .001) and more frequently received treatment with curative intent (colon: 41.3% vs 27.4%; rectum: 33.8% vs 24.6%). Overall survival significantly improved for patients with screen-detected CRCs (stage III: P < .001; stage IV: P < .001). CONCLUSIONS: Five years after the start of a nationwide CRC screening program, a decrease in stage II and IV CRC incidence was observed. Patients with screen-detected stage III and stage IV CRC had less extensive disease and improved survival compared with those with clinically detected CRC.

DESIGN: Prognostic model. SETTING: Dutch biennial FIT-based screening programme during 2014-2018. PARTICIPANTS: 265 881 participants completing three rounds of FIT, with negative test results (F-Hb <47 µg Hb/g faeces) in rounds 1 and 2. INTERVENTIONS: Colonoscopy follow-up in participants with a positive FIT (F-Hb ≥47 µg Hb/g faeces). MAIN OUTCOMES: We evaluated prognostic models for detecting advanced neoplasia (AN) and CRC in round 3, with as predictors, participant age, sex, F-Hb in rounds 1 and 2, and categories/combinations/non-linear transformations of F-Hb. Primary evaluation criteria included: risk prediction accuracy (calibration), discrimination of participants with versus without AN or CRC (optimism-adjusted C-statistics, range 0.5-1.0), the degree of risk stratification and C-statistics in external validation. RESULTS: Among study participants, 8806 (3.3%) had a positive FIT result, 3254 (1.2%) had AN detected and 557 (0.2%) had cancer. F-Hb concentrations in rounds 1 and 2 were the strongest outcome predictors, with adjusted ORs of up to 9.4 (95% CI 7.5 to 11.7) for the highest F-Hb category. Risk predictions matched the observed risk for most participants (calibration intercept -0.008 to -0.099; slope 0.982-0.998), and discriminated participants with versus without AN or CRC with C-statistics of 0.78 (95% CI 0.77 to 0.79) and 0.73 (95% CI 0.71 to 0.75), respectively. The predicted risk ranged from 0.4% to 36.7% for AN and from 0.0% to 5.5% for CRC across participants. In external validation, the model retained similar discrimination accuracy for AN (C-statistic 0.77, 95% CI 0.66 to 0.87) and CRC (C-statistic 0.78, 95% CI 0.66 to 0.91). 01 juni 2023, Item does not contain fulltext, OBJECTIVE: Assessment of the anatomical colorectal segment of polyps during colonoscopy is important for treatment and follow-up strategies, but is largely operator dependent. This feasibility study aimed to assess whether, using images of a magnetic endoscope imaging (MEI) positioning device, a deep learning approach can be useful to objectively divide the colorectum into anatomical segments. METHODS: Models based on the VGG-16 based convolutional neural network architecture were developed to classify the colorectum into anatomical segments. These models were pre-trained on ImageNet data and further trained using prospectively collected data of the POLAR study in which endoscopists were using MEI (3930 still images and 90,151 video frames). Five-fold cross validation with multiple runs was used to evaluate the overall diagnostic accuracies of the models for colorectal segment classification (divided into a 5-class and 2-class colorectal segment division). The colorectal segment assignment by endoscopists was used as the reference standard. RESULTS: For the 5-class colorectal segment division, the best performing model correctly classified the colorectal segment in 753 of the 1196 polyps, corresponding to an overall accuracy of 63%, sensitivity of 63%, specificity of 89% and kappa of 0.47. For the 2-class colorectal segment division, 1112 of the 1196 polyps were correctly classified, corresponding to an accuracy of 93%, sensitivity of 93%, specificity of 90% and kappa of 0.82. CONCLUSION: The diagnostic performance of a deep learning approach for colorectal segment classification based on images of a MEI device is yet suboptimal (clinicaltrials.gov: NCT03822390).

01 juni 2023, Item does not contain fulltext, BACKGROUND : Advanced serrated polyps (ASPs) have a comparable risk to advanced adenomas for progression to colorectal cancer (CRC). The yield of most CRC screening programs, however, is based on advanced adenomas and CRC only. We assessed the ASP detection rate, and positive predictive value (PPV) including ASPs in a fecal immunochemical test (FIT)-based screening program. METHODS : We analyzed the findings of follow-up colonoscopies of FIT-positive screenees in the Dutch CRC screening program from 2014 until 2020. Data were retrieved from the national screening and pathology database. An ASP was defined as any serrated polyp of ≥ 10 mm, sessile serrated lesion with dysplasia, or traditional serrated adenoma. The ASP detection rate was defined as the proportion of colonoscopies with ≥ 1 ASP. PPV was originally defined as the proportion of individuals with a CRC or advanced adenoma. The updated PPV definition included CRCs, advanced adenomas, and/or ASPs. RESULTS : 322 882 colonoscopies were included in the analyses. The overall detection rate of ASPs was 5.9 %. ASPs were detected more often in women than men (6.3 % vs. 5.6 %; P < 0.001). ASP detection rates in individuals aged 55-59, 60-64, 65-69, and 70 + were 5.2 %, 6.1 %, 6.1 %, and 5.9 %, respectively (P < 0.001). The PPV for CRCs and advanced adenomas was 41.1 % and increased to 43.8 % when including ASPs. The PPV increase was larger in women than in men (3.2 vs. 2.4 percentage points). CONCLUSIONS : 5.9 % of FIT-positive screenees had ASPs, but half of these were detected in combination with a CRC or advanced adenoma. Therefore, including ASPs results in a small increase in the yield of FIT-based screening.

01 juli 2023, Item does not contain fulltext, OBJECTIVE: To assess the impact of delayed invitation on screen-detected and interval colorectal cancers (CRC) within a faecal immunochemical testing (FIT)-based CRC screening programme. DESIGN: All individuals that participated in 2017 and 2018 with a negative FIT and were eligible for CRC screening in 2019 and 2020 were included using individual-level data. Multivariable logistic regression analyses were used to assess the association between either the different time periods (ie, 'before', 'during' and 'after' the first COVID-19 wave) or the invitation interval on screen-detected and interval CRCs. RESULTS: Positive predictive value for advanced neoplasia (AN) was slightly lower during (OR=0.91) and after (OR=0.95) the first COVID-19 wave, but no significant difference was observed for the different invitation intervals. Out of all individuals that previously tested negative, 84 (0.004%) had an interval CRC beyond the 24 months since their last invitation. The time period of invitation as well as the extended invitation interval was not associated with detection rates for AN and interval CRC rate. CONCLUSION: The impact of the first COVID-19 wave on screening yield was modest. A very small proportion of the FIT negatives had an interval CRC possibly due to an extended interval, which potentially could have been prevented if they had received the invitation earlier. Nonetheless, no increase in interval CRC rate was observed, indicating that an extended invitation interval up to 30 months had no negative impact on the performance of the CRC screening programme and a modest extension of the invitation interval seems an appropriate intervention.

Item does not contain fulltext, BACKGROUND AND AIMS: Endoscopic full-thickness resection (eFTR) is emerging as a minimally invasive alternative to surgery for complex colorectal lesions. Previous studies have demonstrated favorable safety results; however, large studies representing a generalizable estimation of adverse events (AEs) are lacking. Our aim was to provide further insight in AEs after eFTR. METHODS: Data from all registered eFTR procedures in the German and Dutch colorectal full-thickness resection device registries between July 2015 and March 2021 were collected. Safety outcomes included immediate and late AEs. RESULTS: Of 1892 procedures, the overall AE rate was 11.3% (213/1892). No AE-related mortality occurred. Perforations occurred in 2.5% (47/1892) of all AEs, 57.4% (27/47) of immediate AEs, and 42.6% (20/47) of delayed AEs. Successful endoscopic closure was achieved in 29.8% of cases (13 immediate and 1 delayed), and antibiotic treatment was sufficient in 4.3% (2 delayed). The appendicitis rate for appendiceal lesions was 9.9% (13/131), and 46.2% (6/13) could be treated conservatively. The severe AE rate requiring surgery was 2.2% (42/1892), including delayed perforations in .9% (17/1892) and immediate perforations in .7% (13/1892). Delayed perforations occurred between days 1 and 10 (median, 2) after eFTR, and 58.8% (10/17) were located on the left side. Other severe AEs were appendicitis (.4%, 7/1892), luminal stenosis (.1%, 2/1892), delayed bleeding (.1%, 1/1892), pain after eFTR close to the dentate line (.1%, 1/1892), and grasper entrapment in the clip (.1%, 1/1892). CONCLUSIONS: Colorectal eFTR is a safe procedure with a low risk for severe AEs in everyday practice and without AE-related mortality. These results further support the position of eFTR as an established minimally invasive technique for complex colorectal lesions.

We aimed to further elucidate the association between smoking, including possible dose-effects, and the development of colorectal neoplasia in patients with inflammatory bowel disease (IBD). METHODS: We performed a prospective multicenter cohort study including patients with colonic IBD enrolled in a surveillance program in four academic hospitals between 2011 and 2021. The effects of smoking status and pack-years at study entry on subsequent recurrent events of CRN (including indefinite, low- and high-grade dysplasia, and colorectal cancer [CRC]) were evaluated using uni- and multivariable Prentice, Williams, and Peterson total-time Cox proportional hazard models. Adjustment was performed for extensive disease, prior/index dysplasia, sex, age, first-degree relative with CRC, primary sclerosing cholangitis, and endoscopic inflammation. RESULTS: In 501 of the enrolled 576 patients, at least one follow-up surveillance was performed after the study index (median follow-up 5 years). CRN occurred at least once in 105 patients. Ever smoking was not associated with recurrent CRN risk (adjusted hazard ratio [aHR] 1.04, 95% confidence interval [CI] 0.75-1.44), but an increasing number of pack-years was associated with an increased risk of recurrent CRN (aHR per 10 pack-years 1.17, 95% CI 1.03-1.32; p < 0.05). Separate analyses per IBD type did not reveal differences. CONCLUSIONS: This study found that an increase in pack-years is associated with a higher risk of recurrent CRN in patients with IBD, independent of established CRN risk factors (NCT01464151).

01 juli 2023, Item does not contain fulltext, BACKGROUND: Colonoscopy surveillance intervals are based on the predicted risk of metachronous colorectal cancer (CRC) after polyp removal. However, risk estimation per polyp subtype is difficult due to the fact that many patients have multiple polyps. To enable risk estimation per polyp subtypes we examined the metachronous CRC risk of subgroups based on presence or absence of co-occurring findings. METHODS: Using high-quality screening colonoscopies performed after a positive fecal immunochemical test between 2014 and 2020 within the Dutch CRC screening program, we applied Cox regression analysis to evaluate the association between findings at baseline colonoscopy and metachronous CRCs. For our primary outcome, we appointed each patient to unique subgroups based on removed polyp subtypes that were present or absent at baseline colonoscopy and used the groups without polyps as reference. High-risk subgroups were individuals with high-risk serrated polyps, defined as serrated polyp ≥10 mm, sessile serrated lesions with dysplasia, or traditional serrated adenomas, as well as high-risk adenomas, defined as adenoma ≥10 mm or containing high-grade dysplasia. FINDINGS: In total 253,833 colonoscopies were included. Over a median follow-up of 36 months (IQR, 21-57), we identified 504 metachronous CRCs. Hazard ratios for metachronous CRC was 1.70 (95% CI, 1.07-2.69) for individuals with high-risk serrated polyps without high-risk adenomas, 1.22 (0.96-1.55) for individuals with high-risk adenomas without high-risk serrated polyps, and 2.00 (1.19-3.39) for individuals with high-risk serrated polyps and high-risk adenomas, compared to patients without polyps. INTERPRETATION: Accounting for co-occurring findings, we observed an increased metachronous CRC risk for individuals that had high-risk serrated polyps with the presence of high-risk adenomas, or individuals with high-risk serrated polyps without high-risk adenomas. CONCLUSION: Risk of iPCCRC within a FIT-based CRC screening program was low during the first years after colonos-copy but, after 2.5 years, was the same as the risk in FIT-negative individuals at 2 years, when they are reinvited for screening. RIVM rapport:Een voorbeeld van een innovatieve manier van recyclen is autobanden verhitten zonder zuurstof (pyrolyse). Het RIVM heeft beoordeeld of deze manier van recyclen veilig is voor mens en milieu en duurzamer is vergeleken met de huidige verwerking. Zo is gekeken hoeveel materiaal behouden blijft om te worden hergebruikt. De conclusie is dat elke methode van recycling voor- en nadelen heeft, die goed tegen elkaar moeten worden afgewogen. Het RIVM beveelt daarvoor aan enkele onzekerheden weg te nemen, vooral over het recyclen van stalmatten. Recyclen van reststromen is belangrijk om in 2050 een circulaire economie te realiseren. Met pyrolyse wordt uit banden het materiaal ‘carbon black’ gewonnen, dat kan worden gebruikt om weer nieuwe banden te maken. Ook wordt er gas en olie uit verkregen, die als brandstof worden gebruikt. De pyrolyse is vergeleken met twee manieren waarmee autobanden nu worden verwerkt: gebruik als vervanger van kolen in cementovens en via mechanische recycling tot stalmatten voor vee . Deze casus is uitgewerkt met een methode die het RIVM eerder heeft ontwikkeld (Safe and Sustainable Material Loops). Bij een mechanische verwerking blijft er meer materiaal over dat geschikt is voor nieuwe producten, dan bij pyrolyse. De vraag is wel of die nieuwe producten na het tweede leven opnieuw te recyclen zijn. Het is bijvoorbeeld nog onzeker of stalmatten kunnen worden hergebruikt, omdat ze tijdens het gebruik vervuild raken. Over meerdere levenscycli valt dat voordeel dus mogelijk weg. Het RIVM beveelt aan om dit verder te onderzoeken. Het effect op het klimaat, een andere belangrijke indicator waarnaar is gekeken, is bij pyrolyse vrijwel even groot als bij de verbranding van autobanden in cementovens. Bij mechanische recycling tot stalmatten is het effect op het klimaat gunstiger. Vergeleken met het gebruik in cementovens blijft er bij pyrolyse wel meer materiaal over waarvan nieuwe producten kunnen worden gemaakt. Wat de samenstelling van het materiaal betref, One example of an innovative recycling method involves heating car tyres in the absence of oxygen (a process known as pyrolysis). RIVM has conducted an assessment to determine the safety of this recycling method for both humans and the environment, as well as its sustainability when compared to current processing techniques. This assessment included an examination of the extent to which materials are preserved for reuse. The conclusion is that each recycling method carries its own set of advantages and disadvantages, and these should be carefully weighed against each other. RIVM recommends addressing a few uncertainties, particularly concerning the recycling of stable mats. The recycling of residual flows plays a pivotal role in realising a circular economy by 2050. Through pyrolysis, a material called ‘carbon black’ is recovered from tyres, which can be used in the production of new tyres. Additionally, pyrolysis yields gas and oil that serve as valuable fuels. RIVM compared pyrolysis to two techniques currently employed for processing end-of-life car tyres: using them as a coal substitute in cement kilns and mechanical recycling into stable mats used for livestock. RIVM worked on this case study using a methodology developed previously (known as ‘Safe and Sustainable Material Loops’). Mechanical recycling results in recovering of more materials that are suitable for repurposing into new products than pyrolysis. However, the question is whether these new products can be efficiently recycled after this second use phase. For example, there is uncertainty surrounding the potential for stable mats to be reused, because of contamination during use. RIVM suggests further investigation into this matter. With respect to climate-impact , another crucial aspect under examination, pyrolysis was found to be nearly on par with the process of burning car tyres in cement kilns. BACKGROUND: Surgical resection followed by adjuvant mFOLFIRINOX (5-fluorouracil with leucovorin, irinotecan, and oxaliplatin) is currently the standard of care for patients with resectable pancreatic cancer. The main concern regarding adjuvant chemotherapy is that only half of patients actually receive adjuvant treatment. Neoadjuvant chemotherapy, on the other hand, guarantees early systemic treatment and may increase chemotherapy use and thereby improve overall survival. Furthermore, it may prevent futile surgery in patients with rapidly progressive disease. However, some argue that neoadjuvant therapy delays surgery, which could lead to progression towards unresectable disease and thus offset the potential benefits. Comparison of perioperative (i.e., neoadjuvant and adjuvant) with (only) adjuvant administration of mFOLFIRINOX in a randomized controlled trial (RCT) is needed to determine the optimal approach. METHODS: This multicenter, phase 3, RCT will include 378 patients with resectable pancreatic ductal adenocarcinoma with a WHO performance status of 0 or 1. Patients are recruited from 20 Dutch centers and three centers in Norway and Sweden. Resectable pancreatic cancer is defined as no arterial contact and ≤ 90 degrees venous contact. Patients in the intervention arm are scheduled for 8 cycles of neoadjuvant mFOLFIRINOX followed by surgery and 4 cycles of adjuvant mFOLFIRINOX (2-week cycle of oxaliplatin 85 mg/m2, leucovorin 400 mg/m2, irinotecan 150 mg/m2 at day 1, followed by 46 h continuous infusion of 5-fluorouracil 2400 g/m2). Patients in the comparator arm start with surgery followed by 12 cycles of adjuvant mFOLFIRINOX. The primary outcome is overall survival by intention-to-treat. Secondary outcomes include progression-free survival, resection rate, quality of life, adverse events, and surgical complications. Background: Colonoscopy surveillance intervals are based on the predicted risk of metachronous colorectal cancer (CRC) after polyp removal. However, risk estimation per polyp subtype is difficult due to the fact that many patients have multiple polyps. To enable risk estimation per polyp subtypes we examined the metachronous CRC risk of subgroups based on presence or absence of co-occurring findings. Methods: Using high-quality screening colonoscopies performed after a positive fecal immunochemical test between 2014 and 2020 within the Dutch CRC screening program, we applied Cox regression analysis to evaluate the association between findings at baseline colonoscopy and metachronous CRCs. For our primary outcome, we appointed each patient to unique subgroups based on removed polyp subtypes that were present or absent at baseline colonoscopy and used the groups without polyps as reference. High-risk subgroups were individuals with high-risk serrated polyps, defined as serrated polyp ≥10 mm, sessile serrated lesions with dysplasia, or traditional serrated adenomas, as well as high-risk adenomas, defined as adenoma ≥10 mm or containing high-grade dysplasia. Findings: In total 253,833 colonoscopies were included. Over a median follow-up of 36 months (IQR, 21–57), we identified 504 metachronous CRCs. Hazard ratios for metachronous CRC was 1.70 (95% CI, 1.07–2.69) for individuals with high-risk serrated polyps without high-risk adenomas, 1.22 (0.96–1.55) for individuals with high-risk adenomas without high-risk serrated polyps, and 2.00 (1.19–3.39) for individuals with high-risk serrated polyps and high-risk adenomas, compared to patients without polyps. Interpretation: Accounting for co-occurring findings, we observed an increased metachronous CRC risk for individuals that had high-risk serrated polyps with the presence of high-risk adenomas, or individuals with high-risk serrated polyps without high-risk adenomas. This study reports a user evaluation of a DVD-based social simulator, developed for people who stutter to potentially gain confidence in using a learned fluency technique. The aim was to examine and evaluate the pilot of the DVD-based social simulator, Scenari-Aid, to inform the development of an online version of the program. Thirty-seven adults who were stuttering were recruited to the study from non-professional groups in Australia. The DVD comprised scenarios with actors in real-life settings that were designed to elicit verbal responses. Participants worked through the scenarios at their own rate and then completed an online survey. The survey comprised 29 statements requiring responses on a 5-point Likert scale and provided information about users' perceptions of participating in the social simulations. There was high positive agreement among the participants on all statements, the most important being that they perceived the scenarios represented in everyday speaking situations and that they felt immersed in them. Participants also agreed that both their fluency and confidence increased in everyday speaking situations as a result of working through the DVD scenarios. The developers were satisfied that, despite the subjective nature of the findings, the study provided sufficient support for constructing the online version, which is now available to the public free of charge. Further research is needed to provide empirical evidence of the contribution it can make to the efficacy of speech programs for adults who stutter.

RIVM rapport:De Nederlandse overheid, zowel de rijksoverheid, als provincies en gemeenten, koopt elk jaar voor 85 miljard euro aan diensten, producten en werken in. Met dit bedrag wil de overheid niet alleen producten en diensten aanschaffen, maar ook het milieu minder belasten en sociale ontwikkelingen in relatie tot werk stimuleren. Dit heet Maatschappelijk Verantwoord Inkopen (MVI(maatschappelijk verantwoord inkopen)). Het RIVM volgt sinds 2015 elke twee jaar het effect van MVI op milieu en klimaat en sociale ontwikkelingen. In 2019 en 2020 zijn hierdoor minder broeikasgassen uitgestoten en zijn er minder grondstoffen gebruikt. Meer nog dan in de vorige monitoringsperioden. Dit blijkt uit een analyse van het effect van MVI in deze twee jaar. Deze stijgende lijn maakt duidelijk dat Maatschappelijk Verantwoord Inkopen een geschikt instrument is waarmee de overheid duurzame inkoop kan stimuleren. Aanbestedende diensten hebben er steeds meer oog voor. Wel zou meer effect mogelijk kunnen zijn als zij zich hier nog meer voor zouden inspannen. Door meer duurzame producten en diensten uit te vragen, wordt het ook voor de markt aantrekkelijker om erin te investeren. Door maatschappelijk verantwoord inkopen in 2019 en 2020 is naar schatting minstens 6,8 Mton(megaton) CO2(carbon dioxide)-equivalent minder broeikasgasemissies uitgestoten en is er 0,3 Mton CO2-equivalent gecompenseerd door bossen aan te planten. Dit is ongeveer hetzelfde als de hoeveelheid broeikasgassen die 345.000 Nederlandse huishoudens elk jaar uitstoten. Ook is er 1433 kilogram minder stikstofoxiden uitgestoten. Voor fijnstof is dat 91 kilo minder. Daarnaast is 292 ton aan grondstoffen bespaard. Verder is er 1203 fte aan werk gecreëerd mensen met een afstand tot de arbeidsmarkt en stagiairs. Het RIVM heeft zes clusters van producten en diensten bekeken met in totaal zestien productgroepen. This is known as socially responsible procurement (SRP). RIVM has been monitoring and reporting on the impact of SRP on the environment, climate and social developments every two years since 2015. In 2019 and 2020, it noted reductions in greenhouse gas emissions and the use of raw materials – more so than in earlier monitoring periods. This was the outcome of an analysis of the impact of SRP over these two years. This positive trend shows that SRP is a useful instrument for the Government to promote sustainable procurement. Although contracting authorities are increasingly aware of this, greater efforts on their part could achieve further impact. After all, more demand for more sustainable products and services would make investing in them more attractive to market parties. In 2019 and 2020, SRP is estimated to have led to a reduction in greenhouse gas emissions of at least 6.8 Mton CO2 equivalent. Another 0.3 Mton CO2 equivalent was offset by planting trees. This reduction was broadly equal to the total annual greenhouse gas emissions of roughly 345,000 Dutch households. In the same period, emissions of nitrogen dioxide decreased by 1,433 kilograms and of particulate matter by 91 kilograms. In addition, 292 tonnes of raw material were conserved. OBJECTIVE: To determine the nationwide use and outcome of tailored surgical treatment for symptomatic chronic pancreatitis (CP) as advised by recent guidelines.SUMMARY BACKGROUND DATA: Randomized trials have shown that surgery is superior to endoscopy in patients with symptomatic CP, although endoscopy remains popular Recent guidelines advice to "tailor surgery" based on pancreatic morphology meaning that the least extensive procedure should be selected based on pancreatic morphology. However, nationwide, and multicenter studies On tailored surgery for symptomatic CP are lacking.METHODS: Nationwide multicenter retrospective analysis of consecutive patients undergoing surgical treatment for symptomatic CP in all seven Dutch university medical centers (2010-2020). Outcomes included volume trend, major complications, 90-day mortality, postoperative opioid use and clinically relevant pain relief. Surgical treatment was tailored based on the size of the main pancreatic duct and pancreatic head (e.g. surgical drainage for a dilated pancreatic duct, and normal size pancreatic head).RESULTS: Overall, 381 patients underwent surgery for CP: 127 surgical drainage procedures ( 33%; mostly extended lateral pancreaticojejunostomy), 129 duodenum-preserving pancreatic head resections (DPPHR, 34%, mostly Frey), and 125 formal pancreatic resections (33%, mostly distal pancreatectomy). The annual surgical volume increased slightly (Pearson r=0.744). Mortality (90-day) occurred in 6 patients (2%), and was non-significantly lower after surgical drainage (0%, 3%, 2%; P =0.139). Major complications (12%, 24%, 26%; P =0.012), postoperative pancreatic fistula grade B/C (0%, 3%, 22%; P =0.038), surgical reintervention (4%, 16%, 12%; P =0.006), and endocrine insufficiency ( 14%, 21%, 43%; P <0.001) occurred less often after surgical drainage.
