12. The role of one-stop shops in energy renovation - A comparative analysis of OSSs cases in Europe

Abstract

With energy transition setting the ground for innovation and new ways of conducting business, one-stop shops (OSSs) have recently gained momentum in the renovation market. By transforming a complex set of multiple-actor decisions into a single entry and customer-centric service, OSSs have the potential of establishing a bridge between the fragmented demand and supply sides of the traditional renovation value chain. To assess the viability of the OSSs model as a vehicle of accelerating decarbonisation efforts in the European building stock, this paper collects and analyses 63 case studies of OSSs across Europe. The study offers insights into the dynamics of their business model, key benefits and ways forward, by explicitly exploring OSSs’ role in incentivising homeowners to decide to renovate. Our findings show that OSSs can be instrumental in addressing the multitude of barriers that prevent homeowners to renovate. With around 100,000 OSSs projects per year, their activity is expected to substantially contribute to the European renovation targets to rise, subject to favourable policy framework, availability of affordable financing solutions and experience sharing within and across countries. With some OSSs already supporting vulnerable households to renovate, OSSs might be well-placed in the future to contribute to tackle energy poverty by assisting in accessing financing and engaging property owners to renovate.

13. One-stop shops for residential building energy renovation in the EU

Abstract

This report is the second part of a Europe-wide survey and assessment of one-stop shops (OSS) for energy renovation of buildings. Based on the case studies of 63 OSS in the EU, we find that the approach has a potential to cover 5-6 of the renovation volume of 35 million buildings in 2030 set out by the Renovation Wave Strategy , at low social costs, integrating private investments with client-friendly methods. The current level of activity of the European OSS market is estimated to be around 100 000 projects per year. The OSS can bridge the gap between a fragmented residential building sector, with a large heterogeneous set of households, and the construction supply side. They can help increase the actual renovation rate by supporting potential clients through the various steps of the decision-making process. Their success lies in part with their locally embedded focus, engagement with interested but not yet committed energy users/asset owners and ability to form strong relationships with clients. They can support the renovation journey from start to finish and can facilitate access to financing, occasionally offering better rates. While our analysis identifies several structural, legislative, financial, and information measures with a positive impact on OSS-enabled renovation projects, it is possible to enhance their contribution by reviewing specific policies that support or hinder their success. Although not typical of OSS in general, but with proper incentives, they can improve the average renovation depth in terms of energy performance level by adopting a holistic approach and, at the same time, they can reach out to vulnerable populations, such as tenants of social houses, thus contributing to the alleviation of energy poverty. Finally, OSS can contribute to the enhancement of communities and neighbourhoods. They help current tenants to improve their living conditions and thus stay in the area.

Abstract

Background The aim of this study was to assess the efficacy and safety of infliximab biosimilar (IFX) IFX CT-P13 in inducing and maintaining remission in ulcerative colitis (UC) outpatients in Italian primary gastroenterology centers. Methods Patients were prospectively assessed at entry, after 8, 12, 24, 36, and therefore 52 weeks. Clinical activity was rated as per the Mayo Score. The primary endpoint was reaching of clinical remission (Mayo Score ≤2). Several secondary endpoints were clinical response to treatment, reaching of mucosal healing (MH), safety of the drug. Results Twenty-nine patients (16 males and 13 females, mean age 45 years, range 35-42 years) were enrolled. Eleven (37.9%) patients had previous exposure to other anti-TNF-α. Clinical remission was present in 78.5% at week 24, and in 100% at 12-month follow-up. Subgroup analysis did not reveal significant differences in clinical remission between IFX-naive patients and patients switching from originator to IFX biosimilar. A clinical response was observed in 92.3% at week 8, in 50.0% at week 16, in 100% at week 36 and in 100% at 12-month follow-up. MH occurred in 85.7% at week 24, and in 100% at 12-month follow-up Reduction of steroids was achieved in 92.3% at week 8, and in 100% during follow-up. One patient underwent proctocolectomy 3 weeks after starting IFX CT-P13. The median C-reactive protein and calprotectin levels during follow-up were significantly reduced during follow-up. No adverse events were observed during follow-up. Conclusions IFX CT-P13 seems to be very effective and safe in real-life experience at primary IBD centers.

Abstract

Factor XIIIa, a circulating form of transglutaminase, plays a key role in intestinal mucosal repair. We found that transglutaminase levels are decreased in serum of patients with inflammatory bowel diseases and demonstrated in a rat model of chronic colitis that serum transglutaminase is closely related to the severity of intestinal damage. We aimed, therefore, to correlate serum transglutaminase levels with standard endoscopic and histopathologic grading systems in patients affected by ulcerative colitis (UC). In 249 patients with UC, we assayed serum transglutaminase activity by a radioenzymatic method and measured clinical activity index (CAI) according to modified Rachmilewitz's criteria. In a subset of 82 patients undergoing colonoscopy, endoscopic and histologic indices were studied. Biopsy specimens were also taken from 28 patients to measure myeloperoxidase (MPO) as a marker of mucosa inflammation. Serum transglutaminase levels significantly correlated with the CAI scoring (r = -0.63; P0.01); likewise serum transglutaminase showed the best correlation with endoscopic (r = -0.71; P0.001) and histologic (r = -0.79; P0.001) scores. Myeloperoxidase activity was significantly higher in patients with active UC than those in remission (P0.01), showing a significant correlation with serum transglutaminase levels (r = -0.68; P0.01). Immunohistochemistry showed factor XIIIa localization in the extracellular matrix of damaged mucosa. In conclusion, these results suggest that transglutaminase assay can be useful in managing UC as a serological, noninvasive indicator of intestinal mucosal status.

Abstract

BACKGROUND AND STUDY AIM: The aim of this study was to assess the prevalence of nonpolypoid lesions (NPLs) in Italy and their risk of containing neoplasia or advanced histology. PATIENTS AND METHODS: This was a multicenter cross-sectional observational study on consecutive patients undergoing total colonoscopy over a 3-month period in 80 Italian centers. RESULTS: In all, 27,400 total colonoscopies were analyzed. Cancer was diagnosed in 801 patients (2.9 %). A total of 6553 precancerous lesions were detected in 5609 patients. Of these, 4154 patients (74.1 %) had polypoid lesions and 1455 patients (25.9 %) had NPLs. Therefore, the prevalence of NPLs was 5.3 % (95 %CI 5.0 - 5.6). NPLs larger than 10 mm were detected in 254 patients (17.5 %). NPLs were more predominant in the proximal colon (OR 2.92, 95 %CI 2.56 - 3.43; P < 0.0001 vs. polypoid lesions). Neoplastic tissue was diagnosed in 79.0 % and advanced histology (high-grade intraepithelial neoplasia or more) in 20.9 % of resected lesions. The risk of advanced histology was similar for polypoid and nonpolypoid lesions when adjusted for size. Depressed lesions had the highest risk of advanced histology (OR 10.56, 95 %CI 6.02 - 18.55; P < 0.0000 vs. flat-elevated). Age was an independent predictor of both neoplasia and advanced histology ( P = 0.0001). CONCLUSIONS: NPLs are relatively common in the Italian population, with a prevalence similar to that in other Western series. 32. Optimising Line 9's EPBM parameters.

Abstract

Ground and surface monitoring was carried out during a 1.3 km drive for Barcelona's metro Line 9 in order to optimise TBM parameters to minimise settlement for a future section of the line. The parameters investigated for the EPBM machine were face pressure, shield slurry injection and tail shield grouting. At the beginning of the drive large settlements were recorded but improved EPB management resulted in a systematic reduction of the volume loss during the second stage. Abstract

One-week triple therapy for Helicobacter pylori revealed, during these last few years, a decrease in the eradication rate, so that the prolongation of its duration has been proposed. A sequential scheme recently showed very satisfactory results. We performed a prospective randomised study with the aim of either evaluating whether the triple therapy prolongation may improve its effectiveness and comparing its outcome with that of sequential regimen.

Abstract

Contract 704 of the metro expansion involved excavating a long reach of tunnel which had a complex interface between fresh granite and overlying clayey sand. Two 6.5 m diameter earth-pressure-balanced TBMs were used to bore the tunnels. The machines were designed so that both hard and soft rock could be excavated: the cutter head was equipped with a combination of rippers and disc cutters, and soil conditioning was achieved by delivering foam through a multiple-port injection system.
