Lymphoma is the second most common malignancy in the head and neck area, affecting both nodal and extranodal sites, including oral soft and hard tissues, usually in the form of non-Hodgkin’s lymphoma (NHL). However, lymphomas of the jaws, including diffuse large B-cell lymphoma (DLBCL), the most common type of NHL, are very rare and may cause significant diagnostic challenges resembling common jaw pathologies, such as periapical lesions, osteomyelitis and osteonecrosis. The aim of this paper is to present a rare case of DLBCL in an 84-years-old diabetic male patient on methylprednisolone treatment for autoimmune hemolytic anemia. The lesion appeared clinically as exposed necrotic bone of the maxilla with surrounding soft tissue ulceration and radiographically as an extensive osteolytic lesion with ill-defined borders. Despite the resemblance of the lesion with osteonecrosis or osteomyelitis that could be theoretically related to diabetes and/or systemic use of corticosteroids, histopathologic examination, necessitating a repeat biopsy in order to acquire sufficient tissue, revealed the final diagnosis of lymphoma. The need for increased clinical awareness and vigilance of this possible diagnostic conundrum is emphasized. © 2022, Journal of Clinical and Experimental Dentistry. All Rights Reserved.

Objectives: Salivary gland ultrasonography (SGUS) is increasingly applied for the management of primary Sjögren's syndrome (pSS). This study aims to: (i) compare the reliability between two SGUS scores; (ii) test the reliability among sonographers with different levels of experience. Methods: In the reliability exercise, two four-grade semi-quantitative SGUS scoring systems, namely De Vita et al. and OMERACT, were tested. The sonographers involved in work-package 7 of the HarmonicSS project from nine countries in Europe were invited to participate. Different levels of sonographers were identified on the basis of their SGUS experience and of the knowledge of the tested scores. A dedicated atlas was used as support for SGUS scoring. Results: Twenty sonographers participated in the two rounds of the reliability exercise. The intra-rater reliability for both scores was almost perfect, with a Light's kappa of 0.86 for the De Vita et al. score and 0.87 for the OMERACT score. The inter-rater reliability for the De Vita et al. and the OMERACT score was substantial with Light's Kappa of 0.75 and 0.77, respectively. Furthermore, no significant difference was noticed among sonographers with different levels of experience. We describe the case of a patient diagnosed with neurofibromatosis type 1 (NF1) where unusually extensive and multiple osteolytic lesions of both jaws consistent with central giant cell granulomas (CGCG) caused massive bone destruction and left her almost “jawless.” The patient was a 58-year-old woman who at the age of 15 years was diagnosed with NF1 and at the age of 53 years underwent radiation therapy for nasal obstruction due to CGCG. The most significant intraoral findings were brown tumors on the maxillary gingiva; bilateral expansion of the hard palate; and a yellow mass on the floor of the mouth. Head and neck examination revealed mandibular asymmetry and features consistent with NF1. Panoramic radiograph and cone beam computed tomography disclosed multiple radiolucent masses. Although the occurrence of multiple CGCG of the jaws in patients with NF1 is rare, early diagnosis, regular follow-up and proper therapeutic intervention may limit the extent of bone destruction. © 2018 The British Association of Oral Surgeons and John Wiley & Sons Ltd

A 49-year-old woman presented with a superficial, ulcerative laesion of approximately 7x3 mm of the marginal labial gingiva of the first upper incisor right. The laesion had returned twice after limited surgical excision. On the basis of a new, extensive excisional biopsy, additional investigation was carried out by the pathologist, which led to a diagnosis of localised (juvenile) spongiotic gingival hyperplasia (LJSGH). The aetiology of LJSGH is still unclear. A typical characteristic is the localised and solitary presence of the laesion. Localised (juvenile) spongiotic hyperplasia of the gingiva is most often seen on the marginal labial gingiva of the maxilla. The laesion can disappear spontaneously, but surgical excision is generally indicated due to the negative cosmetic effect and influence on oral hygiene. The risk of recurrence is high, particularly when the laesion is not radically removed.

We read with great interest the Letter to the Editor by Pan et al commenting on our article “Does Orthodontic Extraction Treatment Improve the Angular Position of Third Molars? A Systematic Review.”1 They raised points about the assessment of risk of bias and the feasibility of conducting a meta-analysis. As we will show below, we used a state-of-the-art method2 to evaluate the quality of the included articles and, according to our protocol, we correctly chose not to perform a meta-analysis.

Xerostomia is the subjective feeling of oral dryness. The major causes are Sjögren's syndrome (SS), medication and radiotherapy to the head and neck. SS is a chronic systemic autoimmune disease characterized by infiltration of the exocrine glands, the salivary and lacrimal ones in particular. The pathogenesis involves systemic B cell hyperactivity and T cell lymphocytes targeting glandular epithelial cells. About 7.5% of patients with SS develop malignant B cell lymphoma, mostly mucosa-associated tissue lymphomas. Certain classes of drugs can induce hyposalivation and/or xerostomia by, e.g., targeting neurotransmitters and receptors. As a result, amongst others the production of fluid and electrolytes in salivary glands can be reduced and the salivary composition can change. During head and neck radiotherapy, the administration of high doses to the major salivary glands, which are located in the periphery of the head, leads to progressive loss of glandular function and a diminished salivary output. Reduction of the dose and the volume of irradiated salivary glands by advanced radiotherapy techniques can be highly beneficial for patients.

The three most frequently diagnosed salivary gland diseases are salivary gland infections, sialolithiasis and mucoceles. Salivary gland infections are usually of bacterial or viral etiology and can be divided into acute and chronic types. Occasionally they can result from obstruction of the salivary duct, an autoimmmune disease or cancer therapy. Infections can occur in all types of salivary glands and are observed at all ages. Sialolithiasis is characterized by the development of calcified structures in the salivary glands, especially in the submandibular gland. Sialoliths are generally attributed to retention of saliva and are usually accompanied by swelling and pain when a salivary stimulus is applied. Mucoceles can be differentiated into mucus extravasation phenomenon or mucus escape reaction, mucus retention cysts and ranulas. They result from extravasation of saliva into the surrounding soft tissues or from retention of saliva within the duct.

OBJECTIVE: To systematically review the current literature on the maxillary midline frenum and associated conditions and complications, as well as the recommended treatment options. METHOD AND MATERIALS: A detailed MEDLINE database search was carried out to provide evidence about the epidemiology, associated pathologies, and treatment options regarding the maxillary frenum. Of the 206 initially identified articles, 48 met the inclusion criteria. RESULTS: The maxillary frenum is highly associated with a number of syndromes and developmental abnormalities. A hypertrophic frenum may be involved in the etiology of the midline diastema. There is also a tendency by orthodontists to suggest posttreatment removal of the frenum (frenectomy). Studies on the cause of gingival recession due to the maxillary frenum are inconclusive. An injured frenum in combination with other traumas and doubtful history might point to child abuse. The involvement of hyperplastic frena in the pathogenesis of peri-implant diseases remains uncertain. There seems to be a clinical interest regarding lasers for surgery for treatment of maxillary frena. The superiority of laser treatment in relation to conventional surgical methods has not yet been demonstrated in the literature. CONCLUSION: A maxillary frenum is a clinical symptom in numerous syndromic conditions and plays a role in the development of the median midline diastema. Nevertheless, the contribution to gingival recession and peri-implant diseases in the region of the maxillary incisors is rather controversial. Laser techniques are reported as the method of choice for the surgical removal of frena; however, this needs to be substantiated by appropriate prospective controlled studies.

An 18-year old woman was referred to the department of oral and maxillofacial surgery with 1 to 2 week episodes of tongue ulcerations that had lasted for a year. Food intake was limited due to pain, resulting in a loss of 10 kg during the previous year. Following a clinical diagnosis of 'aphthous ulcers', she was treated with tetracycline and clobetasol mouthwash, which had little effect. An injection with triamcinolone acetonide did not prove effective either. Some time later, the patient reported at the emergency department with a deep, ulcerative wound to the tongue with characteristic features of bite trauma. She said she had consistently worn her protective mouthguard. The symptoms suggested self-mutilation. The patient was admitted to a psychiatric department, where a diagnosis of anxiety disorder and self-mutilation was reached.
