Objectives: We aimed to evaluate the efficacy and safety of granulocyte-colony stimulating factor (G-CSF) prophylaxis during chemoimmunotherapy with carboplatin plus etoposide and atezolizumab in extensive-stage small cell lung cancer (ES-SCLC). Methods: This retrospective, multicenter study enrolled ES-SCLC patients receiving carboplatin plus etoposide and atezolizumab, categorized into G-CSF and non-G-CSF groups. Demographic and disease-related data were collected. Response rates, progression-free survival (PFS), overall survival (OS), and toxicity were analyzed. Results: Of 119 patients (median age: 63 years), the overall response rate (ORR) and disease control rate (DCR) were 72.3% and 81.5%, respectively. In the G-CSF group, the ORR was 76.4% compared to 60.0% in the non-G-CSF group (p = 0.33), and the DCR was 85.4% versus 70.0%, respectively (p = 0.46). Median PFS was 8.3 months (95% CI, 6.8–9.8) in the G-CSF group and 6.8 months (95% CI, 6.2–7.5) in the non-G-CSF group (p = 0.24). Median OS was 13.8 months (95% CI, 9.6–18.1) for the G-CSF group and 10.6 months (95% CI, 7.9–13.3) for the non-G-CSF group (p = 0.47). Grade 3 ≥ adverse events were similar between groups (49.4% vs. 33.3%, respectively, p = 0.12). Conclusion: G-CSF prophylaxis can be safely used in ES-SCLC patients undergoing carboplatin plus etoposide and atezolizumab regimen without significantly altering efficacy or increasing toxicity. Background & aims: Prognostic factors of metastatic rectal cancer are not well known. We aim to determine prognostic factors affecting survival for metastatic rectal cancer patients and also to investigate the effect of tumor localization on overall survival. Methods: Metastatic rectal cancer patients who received treatment in 5 different centers between 2012 and 2022 were included. Prognostic factors for survival were evaluated using univariate and multivariate analysis. The statistical methods included Pearson's chi-square test, Fisher exact test, Log-rank test, and Cox regressionmodel. Results: A total of 283 patients with metastatic rectal cancer were included in the study. The median OS was not significantly different among the three groups (upper rectum 30.1 months, middle rectum 28.3 months, and low rectum cancer 24.8 months; log-rank p = 0.25). In univariate analysis, Grade 3, ECOGperformance status 2, the presence of multiple metastatic sites, the presence of KRAS mutation, the presence of liver metastases, the presence of nonregional lymph node metastases, and the presence of bone metastases were significant predictors of poor survival. In multivariate analysis, Grade 3, ECOG performance status 2, and the presence of multiple metastatic sites were determined as indicators of worse prognosis. Conclusion: Our findings, primary tumor location did not affect survival in metastatic rectal cancer. The most important factors affecting survival were multiple metastatic sites, tumor grade, and ECOG performance status. [ABSTRACT FROM AUTHOR]

Objectives: Pancreatic cancer (PC) is one of the cancers with the worst prognosis in the world. Despite protooncogenes such as BRCA and PALB2, effective, inexpensive, and simple methods for predicting the prognosis of patients with metastatic PC are still lacking. We aim to investigate whether mean platelet volume/platelet (MPV/PLT) and platelet indices such as MPV and plateletcrit (PCT) have a prognostic significance in patients with metastatic PC. Methods: Patients diagnosed with metastatic PC in 3 centers in Turkey between 2016 and 2022 were analyzed retrospectively. We recorded patient's demographic data such as age, gender, performance status and platelet, MPV and PCT. Overall survival (OS) and progression-free survival (PFS) were also recorded. There were 80 patients in our study. Results: Median PFS was found 6.2 months for MPV/PLT ratio <0.045 group and 6.3 months for MPV/PLT ratio >0.045 group retrospectively. The median PFS of the MPV/PLT ratio =0.045 groupwas shorter than MPV/PLT ratio >0.045 group, but there wasn't statistically meaningful difference between the groups (p:0,957). Median OS for the MPV/PLT ratio =0.045 groupwas 10.1 months and the MPV/PLT ratio for the >0.045 group was 9 months, but there wasn't statistically significant difference between the groups (p:0.506). There was nosurvival difference between the groups when comparing both MPV and PCT status. Conclusion: MPV/PLT ratio is a cheap, simple and useful marker and can be used in our daily practice to predict the prognosis of patients with advanced PC, if confirmed by prospective studies and larger patient numbers.
