Abstract: Background: The COVID-19 pandemic heightened risks for individuals with hematological malignancies due to compromised immune systems, leading to more severe outcomes and increased mortality. While interventions like vaccines, targeted antivirals, and monoclonal antibodies have been effective for the general population, their benefits for these patients may not be as pronounced. Methods: The EPICOVIDEHA registry (National Clinical Trials Identifier, NCT04733729) gathers COVID-19 data from hematological malignancy patients since the pandemic's start worldwide. It spans various global locations, allowing comprehensive analysis over the first three years (2020–2022). Findings: The EPICOVIDEHA registry collected data from January 2020 to December 2022, involving 8767 COVID-19 cases in hematological malignancy patients from 152 centers across 41 countries, with 42% being female. Over this period, there was a significant reduction in critical infections and an overall decrease in mortality from 29% to 4%. However, hospitalization, particularly in the ICU, remained associated with higher mortality rates. Factors contributing to increased mortality included age, multiple comorbidities, active malignancy at COVID-19 onset, pulmonary symptoms, and hospitalization. On the positive side, vaccination with one to two doses or three or more doses, as well as encountering COVID-19 in 2022, were associated with improved survival. Interpretation: Patients with hematological malignancies still face elevated risks, despite reductions in critical infections and overall mortality rates over time. Hospitalization, especially in ICUs, remains a significant concern. The study underscores the importance of vaccination and the timing of COVID-19 exposure in 2022 for enhanced survival in this patient group. Abstract: Patients affected by multiple myeloma (MM) have an increased risk of severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2) infection and subsequent coronavirus (20)19 disease (COVID-19)-related death. The changing epidemiological and therapeutic scenarios suggest that there has been an improvement in severity and survival of COVID-19 during the different waves of the pandemic in the general population, but this has not been investigated yet in MM patients. Here we analyzed a large cohort of 1221 patients with MM and confirmed SARS-CoV-2 infection observed between February 2020, and August 2022, in the EPICOVIDEHA registry from 132 centers around the world. Median follow-up was 52 days for the entire cohort and 83 days for survivors. Three-hundred and three patients died (24%) and COVID-19 was the primary reason for death of around 89% of them. Overall survival (OS) was significantly higher in vaccinated patients with both stable and active MM versus unvaccinated, while only a trend favoring vaccinated patients was observed in subjects with responsive MM. Vaccinated patients with at least 2 doses showed a better OS than those with one or no vaccine dose. Overall, according to pandemic waves, mortality rate decreased over time from 34% to 10%. In multivariable analysis, age, renal failure, active disease, hospital, and intensive care unit admission, were independently associated with a higher number of deaths, while a neutrophil count above 0.5 × 109/L was found to be protective. This data suggests that MM patients remain at risk of SARS-CoV-2 infection even in the vaccination era, but their clinical outcome, in terms of OS, has progressively improved throughout the different viral phases of the pandemic.

Abstract: Since the beginning of the COVID-19 pandemic, there has been an overall improvement in patient mortality. However, haematological malignancy patients continue to experience significant impacts from COVID-19, including high rates of hospitalization, intensive care unit (ICU) admissions, and mortality. In comparison to other haematological malignancy patients, individuals with chronic myeloid leukemia (CML) generally have better prognosis. This study, conducted using a large haematological malignancy patient database (EPICOVIDEHA), demonstrated that the majority of CML patients experienced mild infections. The decline in severe and critical infections over the years can largely be attributed to the widespread administration of vaccinations and the positive response they elicited. Notably, the mortality rate among CML patients was low and exhibited a downward trend in subsequent years. Importantly, our analysis provided confirmation of the effectiveness of vaccinations in CML patients.

Abstract: Objectives: Elderly patients with hematologic malignancies face the highest risk of severe COVID-19 outcomes. The infection's impact on different age groups remains unstudied in detail. Methods: We analyzed elderly patients (age groups: 65-70, 71-75, 76-80, and >80 years old) with hematologic malignancies included in the EPICOVIDEHA registry between January 2020 and July 2022. Univariable and multivariable Cox regression models were conducted to identify factors influencing death in COVID-19 patients with hematological malignancy. Results: The study included data from 3,603 elderly patients (aged 65 or older) with hematological malignancy, with a majority being male (58.1%) and a significant proportion having comorbidities. The patients were divided into four age groups, and the analysis assessed COVID-19 outcomes, vaccination status, and other variables in relation to age and pandemic waves. The 90-day survival rate for patients with COVID-19 was 71.2%, with significant differences between groups. The pandemic waves had varying impacts, with the first wave affecting patients over 80 years old, the second being more severe in 65-70, and the third being the least severe in all age groups. Factors contributing to 90-day mortality included age, comorbidities, lymphopenia, active malignancy, acute leukemia, less than three vaccine doses, severe COVID-19, and using only corticosteroids as treatment. Conclusion: These data underscore the heterogeneity of elderly hematological patients, highlight the different impacts of COVID-19 waves and the pivotal importance of vaccination, and may help in planning future healthcare efforts.

Abstract: Patients with acute myeloid leukemia (AML) are at high risk of dying from coronavirus disease 2019 (COVID-19). The optimal management of AML patients with COVID-19 has not been established. Our multicenter study included 388 adult AML patients diagnosed with COVID-19 between February 2020 and October 2021. The vast majority were receiving or had received AML treatment in the preceding 3 months. COVID-19 was severe in 41.2% and critical in 21.1% of cases. The chemotherapeutic schedule was modified in 174 patients (44.8%), delayed in 68 and permanently discontinued in 106. After a median follow-up of 325 days, 180 patients (46.4%) had died ; death was attributed to COVID-19 (43.3%), AML (26.1%) or to a combination of both (26.7%), whereas in 3.9% of cases the reason was unknown. Active disease, older age, and treatment discontinuation were associated with death, whereas AML treatment delay was protective. Abstract: Limited data are available on breakthrough COVID-19 in patients with hematologic malignancy (HM) after anti–severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccination. Adult patients with HM, ≥1 dose of anti-SARS-CoV-2 vaccine, and breakthrough COVID-19 between January 2021 and March 2022 were analyzed. A total of 1548 cases were included, mainly lymphoid malignancies (1181 cases, 76%). After viral sequencing in 753 cases (49%), the Omicron variant was prevalent (517, 68.7%). Most of the patients received ≤2 vaccine doses before COVID-19 (1419, 91%), mostly mRNA-based (1377, 89%). Overall, 906 patients (59%) received COVID-19-specific treatment. After 30-day follow-up from COVID-19 diagnosis, 143 patients (9%) died. The mortality rate in patients with the Omicron variant was 7.9%, comparable to other variants, with a significantly lower 30-day mortality rate than in the prevaccine era (31%). In the univariable analysis, older age (P < .001), active HM (P < .001), and severe and critical COVID-19 (P = .007 and P < .001, respectively) were associated with mortality. Conversely, patients receiving monoclonal antibodies, even for severe or critical COVID-19, had a lower mortality rate (P < .001). In the multivariable model, older age, active disease, critical COVID-19, and 2-3 comorbidities were correlated with a higher mortality, whereas monoclonal antibody administration, alone (P < .001) or combined with antivirals (P = .009), was protective. Although mortality is significantly lower than in the prevaccination era, breakthrough COVID-19 in HM is still associated with considerable mortality. Results: Acute leukaemia and lymphoma were the most frequent HM (35.8% and 35.1%, respectively). Overall, 343 (76.2%) patients received treatment for HM, which was delayed for longer than one month since diagnosis in 57 (16.6%). An overall response rate was observed in 140 (40.8%) patients after the first line of treatment. After a median follow-up of 35 days, overall mortality was 177/450 (39.3%); 30-day mortality was significantly higher in patients not receiving HM treatment (42.1%) than in those receiving treatment (27.4%, p = 0.004), either before and/or after COVID-19, or compared to patients receiving HM treatment at least after COVID-19 (15.2%, p < 0.001). Age, severe/critical COVID-19, ≥2 comorbidities, and lack of HM treatment were independent risk factors for mortality, whereas a lymphocyte count >500/mcl at COVID-19 onset was protective. Conclusions: HM treatment should be delivered as soon as possible for patients with simultaneous diagnosis of COVID-19 and HM requiring immediate therapy.

Abstract: Limited data are available on breakthrough COVID- 19 in patients with hematologic malignancy (HM) after anti-severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccination. Adult patients with HM, ≥1 dose of anti-SARS-CoV-2 vaccine, and breakthrough COVID-19 between January 2021 and March 2022 were analyzed. A total of 1548 cases were included, mainly lymphoid malignancies (1181 cases, 76%). After viral sequencing in 753 cases (49%), the Omicron variant was prevalent (517, 68.7%). Most of the patients received ≤2 vaccine doses before COVID-19 (1419, 91%), mostly mRNA-based (1377, 89%). Overall, 906 patients (59%) received COVID-19-specific treatment. After 30-day follow-up from COVID-19 diagnosis, 143 patients (9%) died. The mortality rate in patients with the Omicron variant was 7.9%, comparable to other variants, with a significantly lower 30-day mortality rate than in the prevaccine era (31%). In the univariable analysis, older age (P < .001), active HM (P < .001), and severe and critical COVID-19 (P = .007 and P < .001, respectively) were associated with mortality. Conversely, patients receiving monoclonal antibodies, even for severe or critical COVID-19, had a lower mortality rate (P < .001). In the multivariable model, older age, active disease, critical COVID-19, and 2-3 comorbidities were correlated with a higher mortality, whereas monoclonal antibody administration, alone (P < .001) or combined with antivirals (P = .009), was protective. Although mortality is significantly lower than in the prevaccination era, breakthrough COVID-19 in HM is still associated with considerable mortality. Death rate was lower in patients who received monoclonal antibodies, alone or in combination with antivirals.

Abstract: We report follow-up results from the randomized, placebo-controlled, phase 3 HELIOS trial of ibrutinib+bendamustine and rituximab (BR) for previously treated chronic lymphocytic leukemia (CLL)/small lymphocytic lymphoma (SLL) without deletion 17p. Overall, 578 patients were randomized 1:1 to either ibrutinib (420 mg daily) or placebo, in combination with 6 cycles of BR, followed by ibrutinib or placebo alone. Median follow-up was 34.8 months (range: 0.1–45.8). Investigator-assessed median progression-free survival (PFS) was not reached for ibrutinib+BR, versus 14.3 months for placebo+BR (hazard ratio [HR] [95% CI], 0.206 [0.159–0.265]; P < 0.0001); 36-month PFS rates were 68.0% versus 13.9%, respectively. The results are consistent with the primary analysis findings (HR = 0.203, as assessed by independent review committee, with 17-month median follow-up). Median overall survival was not reached in either arm; HR (95% CI) for ibrutinib+BR versus placebo: 0.652 (0.454–0.935; P = 0.019). Minimal residual disease (MRD)-negative response rates were 26.3% for ibrutinib+BR and 6.2% for placebo+BR (P < 0.0001). Incidence of treatment-emergent adverse events (including grades 3–4) were generally consistent with the initial HELIOS report. These long-term data support improved survival outcomes and deepening responses with ibrutinib+BR compared with BR in relapsed CLL/SLL. © 2018, The Author(s).

Abstract: Therapeutic plasma exchange (TPE) is used to treat more than 60 diseases worldwide and has drawn growing interest. Little is known about the current situation of TPE activity in Turkey, so we developed a survey to obtain information about this timely topic. We collected data on TPE from 28 apheresis units throughout Turkey. We performed a total of 24,912 TPE procedures with 3203 patients over the past decade. Twenty years ago, the majority of procedures were performed for neurological and hematological disorders, and today, most TPE procedures are done for the same reasons. The only historical change has been an increase in TPE procedures in renal conditions. Currently, renal conditions were more frequently an indication for TPE than rheumatic conditions. Fresh frozen plasma was the most frequently used replacement fluid, followed by 5% albumin, used in 57.9% and 34.6% of procedures, respectively. The most frequently used anticoagulants in TPE were ACD-A and heparin/ACD-A, used with 1671 (52.2%) and 1164 (36.4%) patients, respectively. The frequency of adverse events (AEs) was 12.6%. The most common AEs were hypocalcemia-related symptoms, hypotension, and urticaria. We encountered no severe AEs that led to severe morbidity and mortality. Overall, more than two thirds of the patients showed improvement in the underlying disease. Here, we report on a nationwide survey on TPE activity in Turkey. We conclude that there has been a great increase in apheresis science, and the number of TPE procedures conducted in Turkey has increased steadily over time. Finally, we would like to point out that our past experiences and published international guidelines were the most important tools in gaining expertise regarding TPE.

Abstract: Background: Isatuximab is a monoclonal antibody that binds a specific epitope on the human CD38 receptor and has antitumour activity via multiple mechanisms of action. In a previous phase 1b study, around 65% of patients with relapsed and refractory multiple myeloma achieved an overall response with a combination of isatuximab with pomalidomide and low-dose dexamethasone. The aim of this study was to determine the progression-free survival benefit of isatuximab plus pomalidomide and dexamethasone compared with pomalidomide and dexamethasone in patients with relapsed and refractory multiple myeloma. Methods: We did a randomised, multicentre, open-label, phase 3 study at 102 hospitals in 24 countries in Europe, North America, and the Asia-Pacific regions. Eligible participants were adult patients with relapsed and refractory multiple myeloma who had received at least two previous lines of treatment, including lenalidomide and a proteasome inhibitor. Patients were excluded if they were refractory to previous treatment with an anti-CD38 monoclonal antibody. Abstract: PubMed ID: 31131598, Objective: Significant developments occurred in the clinical management of acute lymphoblastic leukemia (ALL) in adults in recent decades. However, treatment results are still not satisfactory, especially in routine practice. The objective of this study was to evaluate the general clinical features, treatment details, and outcomes of a large group of patients followed in multiple centers in Turkey with a diagnosis of ALL. Materials and Methods: A retrospective analysis of the data of patients with ALL was made, the patients having been diagnosed and treated between January 2003 and June 2017 by different protocols in the hematology clinics of ten different centers. A total of 288 patients, aged between 17 and 76 years old, were included in the study. In this retrospective multicenter analysis of patients with ALL, classification of patients was performed based on treatment period, Philadelphia chromosome positivity, treatment regimen, and administration of allogeneic hematopoietic stem cell transplantation (allo-HSCT). Results: The majority of cases were B-cell in origin, while 224 patients had B-ALL and 64 of the patients had T-ALL. Median follow-up duration for all patients was 18.2 months (range: 0.03-161 months). Philadelphia chromosome positivity was determined in 49 patients (21.9%), and 54 patients (18.8%) were receiving allo-HSCT. After induction chemotherapy, 219 patients (76.0%) achieved complete remission, 32 patients (11.2%) were evaluated as treatment refractory, and 37 patients (12.8%) were deceased. Median overall survival was 47.7 months (95% confidence interval: 36.1-59.2) and median disease-free survival was 23.4 months (95% confidence interval: 6.7-40.0) for all patients. Conclusion: Multicenter studies are extremely important for defining the specific clinical features of a particular disease. The results of this study will make a significant contribution to the literature as they reflect real-life data providing valuable information about the Turkish ALL patient profile. © 2019 by Turkish Society of Hematology.

Abstract: Objectives: White blood cell scanning with Tc-99m-hexamethylpropylene amine oxime (HMPAO) has proven a sensitive and specific imaging method in the diagnosis of suspected prosthesis infection. The aim of this retrospective study was to evaluate the usefulness of SPECT/CT performed simultaneously using a hybrid imaging device of prosthesis infections. Materials and methods: Tc-99m-HMPAO scintigraphy was performed on 37 patients (11 men and 26 women; age range 38-84 years; mean age +/- SD, 65.7 +/- 5.6 years). Planar scans were acquired 2.4 and 24 hour after injection. SPECT/CT was obtained 4 h after injection, using a dual-head hybrid gama camera coupled with a low-power x-ray tube. In all patients, scintigraphic results were matched with the results of surgery, cultures and clinical follow-up. Results: Seventeen (45,9%) out of 37 patients had prosthesis infection and 20 (54,1%) out of 37 patients had non-infectious prosthesis pathologies with Tc-99m-HMPAO scintigraphy and SPECT/CT. The Tc-99m-HMPAO scintigraphy was true-positive for infection in 16 of 37 patients and true-negative in 20 of 37 patients. SPECT/CT provided an accurate anatomic localization of all positive foci. With regard to the final diagnosis, SPECT/CT added a significant clinical contribution in 22 of 37 patients (59,4%). Sensitivity, specificity, negative predictive value and positive predictive values were 100%, 59.1%, 100%, 62.5% in planar images with Tc-99m-HMPAO scintigraphy and 100%, 90.1%, 100%, 88.2% in the planar+SPECT/CT imaging, respectively. Discussion: Our results indicate that SPECT/CT performed using a hybrid device can improve imaging with Tc-99m-HMPAO scintigraphy in patients with suspected osteomyelitis by providing accurate anatomic localization and precise definition of the extent of infection. Abstract: We report follow-up results from the randomized, placebo-controlled, phase 3 HELIOS trial of ibrutinib+bendamustine and rituximab (BR) for previously treated chronic lymphocytic leukemia (CLL)/small lymphocytic lymphoma (SLL) without deletion 17p. Overall, 578 patients were randomized 1: 1 to either ibrutinib (420 mg daily) or placebo, in combination with 6 cycles of BR, followed by ibrutinib or placebo alone. Median follow-up was 34.8 months (range: 0.1-45.8). Investigator-assessed median progression-free survival (PFS) was not reached for ibrutinib+BR, versus 14.3 months for placebo+BR (hazard ratio [HR] [95% CI], 0.206 [0.159-0.265]; P < 0.0001); 36-month PFS rates were 68.0% versus 13.9%, respectively. The results are consistent with the primary analysis findings (HR = 0.203, as assessed by independent review committee, with 17-month median follow-up). Median overall survival was not reached in either arm; HR (95% CI) for ibrutinib+BR versus placebo: 0.652 (0.454-0.935; P = 0.019). Minimal residual disease (MRD)-negative response rates were 26.3% for ibrutinib+BR and 6.2% for placebo+BR (P < 0.0001). Incidence of treatment-emergent adverse events (including grades 34) were generally consistent with the initial HELIOS report. These long-term data support improved survival outcomes and deepening responses with ibrutinib+BR compared with BR in relapsed CLL/SLL.

Abstract: Autologous hematopoietic cell transplantation (AHCT) is an established treatment option for adult patients presenting with multiple myeloma (MM), Hodgkin lymphoma (HL) and various subtypes of non-Hodgkin lymphoma (NHL) in upfront and/or relapsed/refractory disease settings. Although there are recently published consensus guidelines addressing critical issues regarding autologous hematopoietic progenitor cell mobilization (HPCM), mobilization strategies of transplant centers show high variability in terms of routine practice. In order to understand the current institutional policies regarding HPCM in Turkey and to obtain the required basic data for preparation of a national positional statement on this issue, Turkish Hematology Research and Education Group (ThREG) conducted a web-based HPCM survey. The survey was designed to include multiple-choice questions regarding institutional practice of HPCM in adults presenting MM, HL, and NHL. The representatives of 27 adult HCT centers participated to the study. Abstract: Thrombotic microangiopathies (TMAs) are rare, but life-threatening disorders characterized by microangiopathic hemolytic anemia and thrombocytopenia (MAHAT) associated with multiorgan dysfunction as a result of microvascular thrombosis and tissue ischemia. The differentiation of the etiology is of utmost importance as the pathophysiological basis will dictate the choice of appropriate treatment. We retrospectively evaluated 154 (99 females and 55 males) patients who received therapeutic plasma exchange (TPE) due to a presumptive diagnosis of TMA, who had serum ADAMTSI3 activity/antiADAMTS13 antibody analysis at the time of hospital admission. The median age of the study cohort was 36 (14-84). 67 (43.5%), 32 (20.8%), 27 (17.5%) and 28 (18.2%) patients were diagnosed as thrombotic thrombocytopenic purpura (TTP), infection/complement-associated hemolytic uremic syndrome (IA/CAHUS), secondary TMA and TMA-not otherwise specified (TMA-NOS), respectively. Patients received a median of 18 (1-75) plasma volume exchanges for 14 (153) days. 81 (52.6%) patients received concomitant steroid therapy with TPE. Treatment responses could be evaluated in 137 patients. 90 patients (65.7%) achieved clinical remission following TPE, while 47 (34.3%) patients had non-responsive disease. 25 (18.2%) non-responsive patients died during follow-up. Our study present real-life data on the distribution and follow-up of patients with TMAs who were referred to therapeutic apheresis centers for the application of TPE. (C) 2018 Elsevier Ltd. Abstract: AIMS: We ascertained effects of hand dominance in both right and left-handed volunteers on the thickness and cross sectional area (CSA) of pronator quadratus (PQ) muscle as measured by the axial and sagittal views of ultrasound imaging. Secondly, we also aimed to document the extent of the relationship between the PQ measurements and handgrip. MATERIALS AND METHODS: Healthy adults from the volunteer medical staff were included in the study. Handedness was assessed via The Flinders Handedness survey. The sonographic appearance of the PQ was evaluated on sagittal and axial images for both hands. CSA of PQ muscle was digitally drawn and calculated on the axial plane. Handgrip was measured by using adjustable-handle dynamometer. RESULTS: Eighty-nine healthy volunteers were included (54 right and 35 left-handed). Abstract: Most patients with chronic lymphocytic leukaemia or small lymphocytic lymphoma relapse after initial therapy. Bendamustine plus rituximab is often used in the relapsed or refractory setting. We assessed the efficacy and safety of adding ibrutinib, an oral covalent inhibitor of Bruton's tyrosine kinase (BTK), to bendamustine plus rituximab in patients with previously treated chronic lymphocytic leukaemia or small lymphocytic lymphoma.The HELIOS trial was an international, double-blind, placebo-controlled, phase 3 study in adult patients (≥18 years of age) who had active chronic lymphocytic leukaemia or small lymphocytic lymphoma with measurable lymph node disease (1·5 cm) by CT scan, and had relapsed or refractory disease following one or more previous lines of systemic therapy consisting of at least two cycles of a chemotherapy-containing regimen, an Eastern Cooperative Oncology Group (ECOG) performance status of 0-1, and adequate bone marrow, liver, and kidney function. Patients with del(17p) were excluded because of known poor response to bendamustine plus rituximab. Patients who had received previous treatment with ibrutinib or other BTK inhibitors, refractory disease or relapse within 24 months with a previous bendamustine-containing regimen, or haemopoietic stem-cell transplant were also excluded. Patients were randomly assigned (1:1) by a web-based system to receive bendamustine plus rituximab given in cycles of 4 weeks' duration (bendamustine: 70 mg/m(2) intravenously on days 2-3 in cycle 1, and days 1-2 in cycles 2-6; rituximab: 375 mg/m(2) on day 1 of cycle 1, and 500 mg/m(2) on day 1 of cycles 2-6 for a maximum of six cycles) with either ibrutinib (420 mg daily orally) or placebo until disease progression or unacceptable toxicity. Patients were stratified according to whether they were refractory to purine analogues and by number of previous lines of therapy. The primary endpoint was independent review committee (IRC)-assessed progression-free survival. Crossover to ibrutinib was permitted for patients in the placebo group with IRC-confirmed disease progression. Analysis was by intention-to-treat and is continuing for further long-term follow-up. The trial is registered with ClinicalTrials.gov, number NCT01611090.Between Sept 19, 2012, and Jan 21, 2014, 578 eligible patients were randomly assigned to ibrutinib or placebo in combination with bendamustine plus rituximab (289 in each group). The primary endpoint was met at the preplanned interim analysis (March 10, 2015). At a median follow-up of 17 months (IQR 13·7-20·7), progression-free survival was significantly improved in the ibrutinib group compared with the placebo group (not reached in the ibrutinib group (95% CI not evaluable) vs 13·3 months (11·3-13·9) in the placebo group (hazard ratio [HR] 0·203, 95% CI 0·150-0·276; p0·0001). IRC-assessed progression-free survival at 18 months was 79% (95% CI 73-83) in the ibrutinib group and 24% (18-31) in the placebo group (HR 0·203, 95% CI 0·150-0·276; p0·0001). The most frequent all-grade adverse events were neutropenia and nausea. 222 (77%) of 287 patients in the ibrutinib group and 212 (74%) of 287 patients in the placebo group reported grade 3-4 events; the most common grade 3-4 adverse events in both groups were neutropenia (154 [54%] in the ibrutinib group vs 145 [51%] in the placebo group) and thrombocytopenia (43 [15%] in each group). A safety profile similar to that previously reported with ibrutinib and bendamustine plus rituximab individually was noted.In patients eligible for bendamustine plus rituximab, the addition of ibrutinib to this regimen results in significant improvements in outcome with no new safety signals identified from the combination and a manageable safety profile.Janssen ResearchDevelopment.

Abstract: Background: Most patients with chronic lymphocytic leukaemia or small lymphocytic lymphoma relapse after initial therapy. Bendamustine plus rituximab is often used in the relapsed or refractory setting. We assessed the efficacy and safety of adding ibrutinib, an oral covalent inhibitor of Bruton's tyrosine kinase (BTK), to bendamustine plus rituximab in patients with previously treated chronic lymphocytic leukaemia or small lymphocytic lymphoma. Methods: The HELIOS trial was an international, double-blind, placebo-controlled, phase 3 study in adult patients (≥18 years of age) who had active chronic lymphocytic leukaemia or small lymphocytic lymphoma with measurable lymph node disease (>1·5 cm) by CT scan, and had relapsed or refractory disease following one or more previous lines of systemic therapy consisting of at least two cycles of a chemotherapy-containing regimen, an Eastern Cooperative Oncology Group (ECOG) performance status of 0-1, and adequate bone marrow, liver, and kidney function. Patients with del(17p) were excluded because of known poor response to bendamustine plus rituximab. Patients who had received previous treatment with ibrutinib or other BTK inhibitors, refractory disease or relapse within 24 months with a previous bendamustine-containing regimen, or haemopoietic stem-cell transplant were also excluded. Patients were randomly assigned (1:1) by a web-based system to receive bendamustine plus rituximab given in cycles of 4 weeks' duration (bendamustine: 70 mg/m2 intravenously on days 2-3 in cycle 1, and days 1-2 in cycles 2-6; rituximab: 375 mg/m2 on day 1 of cycle 1, and 500 mg/m2 on day 1 of cycles 2-6 for a maximum of six cycles) with either ibrutinib (420 mg daily orally) or placebo until disease progression or unacceptable toxicity. Patients were stratified according to whether they were refractory to purine analogues and by number of previous lines of therapy. The primary endpoint was independent review committee (IRC)-assessed progression-free survival. Abstract: Aims: We ascertained effects of hand dominance in both right and left-handed volunteers on the thickness and cross sectional area (CSA) of pronator quadratus (PQ) muscle as measured by the axial and sagittal views of ultrasound imaging. Abstract: Amaç: Çalışmamızda 2000-2011 yılları arasında Dokuz Eylül Üniversitesi Tıp Fakültesi Hematoloj Bilim Dalı tarafından takip edilen 114 hastanın demografik verileri, tedavi endikasyonları, tedavi yanıtları ve total sağ kalım analizlerinin yapılması amaçlandı. Yöntemler: Kronik lenfoblastik lösemi tanısıyla takip edilen 114 hastanın verileri geriye dönük olarak değerlendirildi. Bulgular: Hastaların 60’ı erkek (%52,6), 54’ü kadındı (%47,4). Ortanca yaş 64 olarak saptandı. 61 hasta tedavisiz izlenirken 53 hasta tedavi aldı. Tanıdan ilk tedaviye kadar geçen süre ortalama 15,57 ay idi. İlk sıra kemoterapide en sık klorambusil kullanıldı (%62). Hastaların ilk sıra kemoterapi sonrası yapılan değerlendirmelerinde 40 hastada kısmi yanıt, 5 tam yanıt, 7 stabil hastalık ve 1 hastada progresyon izlendi ve progresyonsuz sağ kalım ortalaması 18 ay olarak bulundu. 25 hasta ikinci sıra kemoterapi aldı. İkinci sıra kemoterapinin progresyonsuz sağ kalım ortalaması 6,23 ay olarak hesaplandı. 6 hastaya üçüncü sıra kemoterapi verildi ve üçüncü sıra kemoterapinin progresyonsuz sağ kalım ortalaması 5,5 ay idi. Abstract: Primary skeletal muscle lymphoma is rare. It consists 0.5% of all extranodal lymphomas. The most common seen subtype is diffuse large cell lymphoma and has B cell immunophenotype. Extremity muscles; especially lower extremity muscles are the most effected sites. Most of the patients are older then 60 years old. Although the pathogenesis is not exactly known; one of the several hypothesis about the etiology is chronic inflamation secondary to infectious and autoimmune process. The basic treatment modalities for primary extranodal lymphoma with diffuse large cell histology are CHOP chemotherapy and radiotherapy. In our article we presented our case with primary muscle lyphoma at the temporal muscle and another case with primary muscle lyphoma at the back of the popliteal region of right limb, Primer olarak iskelet kası lenfoması nadirdir. Tüm ekstranodal tutulum gösteren lenfomaların %0,5’inde görülür. En sık görülen alt grubu diffüz büyük hücreli lenfomadır ve B hücreli immünfenotipe sahiptir. Ekstremite kasları; özellikle de alt ekstremite kasları en sık tutulan bölgelerdir. Çoğu hasta 60 yaş üzerindedir. Hastalığın patogenezi tam olarak bilinemese de; etyolojide sorumlu birkaç hipotezde infeksiyöz ve otoimmün sürece sekonder kronik enflamasyon yer almaktadır. 43. Tek Merkez Deneyimi: Relaps / Refrakter Akut Lösemi ve Lenfoblastik Lenfomalı Olgularda Ida-FLAG Kemoterapisi

Abstract: Amaç: Primer refrakter ve/veya relaps akut lösemi olgularının yönetimi son derece zor olup sınırlı sayıda tedavi seçenekleri vardır ve bu hastalarda tam remisyon oranı düşük ve remisyon süreleri çok kısadır. İda-FLAG rejimi erişkin relaps, refrakter ve sekonder Akut Myeloblastik Lösemi, yüksek riskli Myelodisplastik Sendrom ve relaps/refrakter Akut Lenfoblastik Lösemi olgularında yaygın olarak kullanılmakta ve bu hastalarda ikincil remisyon için iyi bir seçenek olarak kabul edilmektedir. Gereç ve yöntem: Bu yazıda, merkezimizde İda-FLAG kemoterapisi uygulanan, yaş ortalaması 43,5 olan 13 relaps ve/veya refrakter akut lösemi ve 1 B hücreli lenfoblastik lenfomalı 14 hastanın sonuçları retrospektif olarak değerlendirilmiştir. Bulgular: Hastalardan 11’i ex olmuş (%78,6) 3 hastada (%21,4) ise komplet remisyon sağlanmıştır. Hastaların mortalite nedenleri incelendiğinde; 7 hasta sepsis, 2 hasta hastalık progresyonu, 1 hasta kanama ve 1 hastada Tip I solunum yetmezliği nedeni ile ex olmuştur. Bizim çalışmamızda en sık yan etki enfeksiyon olup, 10 hastada (%71) Grade III-IV olmak üzere hastaların tümünde enfeksiyon gelişmiştir. Sonuç: Bizim sonuçlarımız literatür verileri ile karşılaştırıldığında hastalarımızda komplet remisyon oranı düşük, toksisite profili ise daha yüksek olarak bulunmuştur. Bu sonuçlarla Relaps/Refrakter Akut Lösemi olgularında ikincil remisyon indüksiyonu amacı ile kullanılan İda-FLAG rejiminin iyi bir seçenek olmadığı düşünülmüştür

44. Periferik Hematopoietik Kök Hücre Nakli Yapılan Lenfoma Hastalarımız. Tek Merkez Deneyimi

Abstract: Amaç: Birinci sıra tedaviye yeterli yanıt vermeyen yada relaps olan Hodgkin ve Non-Hodgkin lenfomalı hastalarda ikinci sıra tedavi sonrası otolog periferik kök hücre (OKIT) destekli yüksek doz kemoterapi yaygın olarak kullanılmaktadır. Biz bu çalışmada Dokuz Eylül Üniversitesi Tıp Fakültesi Hematoloji Bilim dalı tarafından 2005-2009 yılları arasında izlenen ve periferik kök hücre nakli yapılan 45 lenfoma hastasını sunmayı amaçladık. Gereç ve yöntemler: Hastaların yaşları 19-67 arasında ve yaş ortalamaları 38,87 idi. Tanılarına bakıldığında %24,4’ü Diffüz Büyük B hücreli NHL, %35,6’sı Nodüler Sklerozan Hodgkin Lenfoma, %17,8’i miks sellüler Hodgkin lenfoma, %8,9’u lenfoblastik lenfoma idi. Bulgular: Hastaların %90’ına OKIT destekli yüksek doz tedavi, %10’una ise allogeneik kök hücre nakli uygulandı. Nakil sonrası hastaların son yapılan değerlendirmelerinde 45 hastanın 13’ünün komplet remisyonda, 2’sinin progresif hastalık, 2’sinin parsiyel yanıtlı, 3’ünün stabil hastalık olduğu görüldü. 12 hasta hastalık ilişkili, 2’si hastalık dışı nedenlerle ve 11 hastada sepsis nedeni ile kaybedildi. Sonuç: Otolog periferik kök hücre destekli yüksek doz tedavi ile özellikle platin bazlı tedaviye yanıt veren lenfoma hasta gurubunda yüz güldürücü sonuçlar alınmaktadır.

Abstract: Objective: Classically, chronic intravascular hemolysis, bone marrow failure, thrombosis in Paroxysmal Nocturnal Hemoglobinuria manifested (PNH) is a clonal disorder of hematopoietic stem cells. Eculizimab is a monoclonal antibody for the treatment of PNH, proven to reduce thrombotic episodes and the hemolysis. Us our PNH patients with a rare disease and we wanted to share our experience with eculizumab treatment. Methods: Dokuz Eylul University Faculty of Medicine, hematology USA and the Katip Çelebi University of hematology clinic in PNH who had received a diagnosis of 18 clinical characteristics of the patient's demographics, history of thrombosis, treatment eculizumab fields and response, overall survival rates were analyzed retrospectively. Results: The median follow-up period was 41months of cases with 8 were female (44.4%) and 10 men (55.6%). Median age of 38.5. When the time of diagnosis laboratory values of the patients examined the time of diagnosis Hb mean 9g/dL, WBC mean 3.8910^3 /mL, plt mean 87.610^3 /mL, mean LDH 609U/L, mean reticulocyte 3.38 was observed. History of thrombosis was diagnosed in 2 patients immediately available. 5 of them have received eculizumab treatment of these cases, two patients were complet response, another patients were partial response. At the end of 41 months of median follow-up time is ex 5.6%, it was observed that 94.4% of the law. Conclusion: PNH is one of the rare hematological diseases. After understanding the pathogenesis popular than classical methods of treatment modalities are available eculizumab. 5 eculizumab treatment needs of our patients with hemolysis after treatment for our patients to have decreased the frequency of blood transfusion, the patient was observed to have increased our quality of life, Amaç: Klasik olarak kronik intravasküler hemoliz bulguları, kemik iliği yetersizliği ve trombozla kendini gösteren Paroksismal Noktürnal Hemoglobinüri (PNH), hematopoietik kök hücrenin klonal bir hastalığıdır. Eculizimab ise PNH tedavisinde hemolizi ve trombotik atakları azalttığı kanıtlanmış bir monoklonal antikordur. Biz de nadir bir hastalık olan PNH olgularımızı ve eculizumab tedavi deneyimimizi paylaşmak istedik. Yöntemler: Dokuz Eylül Üniversitesi Tıp Fakültesi Hematoloji AD ve Katip Çelebi Üniversitesi hematoloji kliniğinde PNH tanısı almış olan 18 hastanın demografik, klinik özellikleri, tromboz öyküsü, eculizumab tedavisi alanlar ve yanıtları, tüm sağkalım oranları retrospektif olarak incelenmiştir. Bulgular: Hastaların ortanca izlem süresi 41 ay (endüşük 3 ay, en yüksek 127 ay) olup olguların 8'i kadın (%44,4), 10'u erkek (%55,6)’tir. Ortanca yaş 38,5 idi. Hastaların tanı anındaki laboratuvar değerleri incelendiğinde tanı anında Hb ortalama 9g/dL, WBC ortalama 3,89 10^3/µL, plt ortalama 87,6 10^3/µL,LDH ortalama 609 U/L, Retikülosit ortalama 3,38 olduğu gözlendi. Olgulardan 5 tanesi eculizumab tedavisi almış, 2’si tam yanıtlı, diğerleri kısmi yanıtlıdır. Ortalama 41 aylık izlem süresi sonunda %5,6 olgunun ex olduğu, %94,4’ün yaşadığı izlendi. Sonuç: PNH hematolojik nadir hastalıklardan birisidir. Klasik tedavi yöntemleri dışında patogenezinin anlaşılmasından sonra bulunan popüler tedavi modalitesi eculizumabdır. Olgularımızdan eculizumab tedavi gereksinimi olan 5 hastamızda tedavi sonrasında hastaların hemolizinin, kan transfüzyon sıklığını azalmış olduğu, yaşam kalitesinin artırmış olduğu gözlenmiştir.

46. Olgularımızın Retrospektif Tek Merkez Deneyimi: Folliküler Lenfomalı Değerlendirmesi

Abstract: Amaç: Folliküler lenfoma, ikinci en sık görülen lenfoma türü olup, bütün non hodgkin lenfomaların %20-25’ini, yavaş seyirli (indolen) lenfomaların ise %70’ini oluşturur. Ortalama yaşam beklentisi yaklaşık 10 yıldır. Fakat beklenen yaşam süresi monoklonal antikorların keşfi ile uzamıştır. Yöntemler: Bu çalışmada, folliküler lenfomalı 28 hastanın klinikopatolojik özellikleri, tedavi modaliteleri ve yanıt oranları retrospektif olarak değerlendirilmiştir. Bulgular: Hastaların %39,3’ünde B semptomları ve %35,7’sinde kemik iliği tutulumu mevcuttu. FLIPI skorlarına göre hastaların %37,5’i düşük, %21,4’ü orta ve %39,3’ü ise yüksek risk grubundaydı. Hastaların %32,1’i hastalığın erken evrelerinde idi. Hastaların %46,4’inde evre 1 ve %14,3’ünde evre 3 patolojik hastalık mevcuttu. %78,6 hasta kemoterapi almıştı ve en çok kullanılan rejim de CVP (%53,6) idi. %14,3 hastada radyoterapi uygulanmış ve hastaların %7,1’ i tedavisiz izlenmişti. 9 hastada rituksimab ilk sırada kullanılmış, 5 hastada ise ilk nükste kemoterapi ile birlikte uygulanmıştı. İlk sıra kemoterapi sonrası; toplam remisyon oranı %74 olmakla birlikte hastaların %46,4’ünde tam yanıt, %25’inde ise kısmi yanıt gözlendi. Ortanca izlem süresi 32,8 ay (5-101 ay) ve ortanca hastalıksız sağ kalım 22,5 aydı (1-67 ay). Sonuç: Çalışmamızın sonuçları doğrultusunda, folliküler lenfomada başlangıç tedavisinde rituksimablı kemoterapi rejimleri ve remisyon sağlanan olgularda idame rituksimab tedavisinin akılcı bir hasta yönetimi olduğunu düşünmekteyiz

Trombositopeninin derecesine bağlı olmak üzere, sıklıkla, purpurik deri lezyonları, mukozal kanamalar, nadiren iç organ kanamaları gibi hayatı tehdit eden kanamalar gelişir. Yöntemler: Bu çalışmada 1988 - 2011 yılları arasında tedavi ve takiplerine merkezimizce devam edilen 76 hastanın sonuçları retrospektif olarak değerlendirilmiştir. Bulgular: 76 hastanın 49’u kadın (%64,5), 27’si erkek (%35,5) olup, ortanca yaş 50,7 (19 – 83 yıl) idi. Olguların 20’si tedavisiz takip edilmiş, tedavi uygulanan 56 olgunun 53’ünde (%92) ise başlangıç tedavisi olarak 1 mg/kg metil prednizolon uygulanmıştı. Bu hastaların 24’ünde (%44,6) tam yanıt, 13’ünde (%23,2) yanıt elde edilmiş ve 17 (%31,5) olguda ise yanıt alınamamıştı. Steroide yanıt vermeyen ya da yanıt sonrası nüks eden hastalara splenektomi uygulanmıştı (37 hasta). Bu hastalardan 33’ünde tam yanıt, 2’sinde yanıt elde edilirken, 2 hastada yanıt alınamadı. Toplamda 8 hastaya Rituksimab verildi. 3 hastada tam yanıt, 2 hastada yanıt alındı. 3 hastada ise yanıt elde edilemedi. Sonuç: ITP hastalarında başlangıç prednizolon tedavisi ve splenektomi, olguların çoğunda etkili bir tedavi seçeneği olmakla birlikte, bu tedavilere cevap vermeyen refrakter ITP olgularında ise ritüksimab kullanımı etkili bir tedavi seçeneğidir

Abstract: Myeloid sarkom inmatür granülositlerden oluşan ve nadir görülen bir tümördür. Hematolojik malignitelere eşlik edebileceği gibi miyeloproliferatif hastalıkların başlangıç semptomu olarak izlenebilir. Biz bu yazıda bilinç bulanıklığı ve gözünde şişlik yakınması ile başvuran Kronik Miyelomonositer Lösemi hastasında eş zamanlı gelişen AML-M4 ile frontal lob ve orbitada gelişen granülositik sarkom birlikteliği olan hastayı sunuyoruz. AML tedavisi ile hastanın beyin parankimi ve gözündeki lezyonlar belirgin geriledi
