This study investigated the implication of monitoring hypertensive disorders in pregnancy (HDP) to prevent preeclampsia (PE) in pregnant women of advanced maternal age. Between January 2016 and April 2021, 262 consecutive pregnant women aged ≥40 years were recruited. Extensive monitoring of hypertensive disorders in pregnancy, including blood hypercoagulability screening and subsequent interventions, was performed in 129 pregnant women in our university hospital. The remaining 133 patients from other centres, who did not receive antenatal maternal pregnancy screening and preventive intervention during the same period, constituted the non-intervention group enabling comparison to mimic a trial. The incidences of hypertensive disorders, mild and severe PE, eclampsia, and chronic hypertension complicated by PE in the intervention group were significantly lower than in the non-intervention group (10.08 versus 20.30%, 8.52 versus 18.80%, 7.75 versus 21.05%, 0 versus 3.01%, and 3.86 versus 15.04%, respectively; P < 0.05). Premature birth, low birth weight, and foetal loss were significantly rarer in the intervention group than in the non-intervention group (6.98 versus 24.81%, 7.75 versus 21.80%, and 0.78 versus 14.29% respectively; P < 0.001). The comparison of MP with routine blood coagulation biochemical examination found that the MP detection system of Beijing Yes Medical Devices Co., Ltd., had similar sensitivity as thromboelastogram. Still, it was significantly better than the routine biochemical indicators (P < 0.01). Based on MP parameters, early anticoagulant treatment with low-molecular-weight heparin or low-dose aspirin in pregnant women with hypercoagulability can effectively prevent the occurrence of PE and significantly improve the prognosis of both mothers and infants.

This study examined the factor structure of the General Help‐seeking Questionnaire and how personal mental illness stigma was associated with different types of help‐seeking intentions among racial and ethnic minority immigrants in the United States. A sample of 202 immigrants aged 18−39 were recruited from a Qualtrics panel and completed the online survey. Confirmatory factor analysis and multiple linear regression were conducted. Results confirmed four dimensions of help‐seeking intentions: informal, formal, semi‐formal, and traditional sources. Immigrants with higher levels of mental illness stigma are more likely to seek help from traditional sources (e.g., religious leader or other community‐based nonfaith resources) and semi‐formal sources (e.g., web‐based resources). Lack of awareness of service options is a key barrier to formal help‐seeking. Results signal the importance of collaboration between mental health service providers. Adopting culturally sensitive care and web‐based delivery formats may help address immigrants' concerns about mental health stigma. [ABSTRACT FROM AUTHOR]

Fifty-one street dust samples were systematically collected from the urban core of Zhengzhou, China, and analyzed for potentially toxic metals. The concentrations of vanadium (V), manganese (Mn), copper (Cu), zinc (Zn), arsenic (As), lead (Pb), and nickel (Ni) in the samples surpassed the background values of the local soil, indicating a notable potential for contamination. Spatially, the traffic area was the most polluted with a total heavy metal concentration of Cu, Zn, As, Pb, and Ni, while the pollution levels were lower in the culture and education area and commercial area with total concentrations of V and Mn. Seasonal variations were discerned in the concentrations of heavy metals, with V, Cu, Zn, and As exhibiting heightened levels during the fall and winter, while Mn, Ni, and Pb reached peaks in the spring season. Zn exhibited the highest mean geo-accumulation index (Igeo) value at 2.247, followed by Cu at 2.019, Pb at 0.961, As at 0.590, Ni at 0.126, Mn at −0.178, and V at −0.359. The potential ecological risk index (RI) in the traffic-intensive area markedly exceeded other functional areas. Health risk assessments showed that children were more vulnerable to heavy metal exposure than adults, particularly through the ingestion pathway. Correlation analysis, principal component analysis (PCA), and cluster analysis (CA) were applied in conjunction with the spatial–temporal concentration patterns across various functional areas to ascertain the plausible sources of heavy metal pollutants. The results indicated that heavy metals in the urban street dust of Zhengzhou were multifaceted, stemming from natural processes and diverse anthropogenic activities such as coal burning, industrial emissions, traffic, and construction operations. In this study, a treatment method was assessed for the prevention and treatment of postpartum bleeding after combined surgery in patients having late pregnancy with the complication of acute Stanford type A aortic dissection. The clinical records of ten patients receiving treatment at the Second Xiangya Hospital of Central South University between March 2012 and March 2021 were retrospectively analysed. All patients were diagnosed with acute Stanford type A aortic dissection according to computed tomography angiography of the thoracic and abdominal aorta. Aortic valve function was assessed using two-dimensional echocardiography. All patients experienced uterine-incision delivery under systemic anaesthesia. During the operation, intrauterine Bakri balloon tamponade and cervical cerclage were performed. Postpartum bleeding was effectively controlled for all patients. The extracorporeal circulation time was 230–295 min, the postpartum 24 h bleeding volume was 500–870 mL, the volume of physiological saline injected into the balloon was 290–515 mL, and the intrauterine balloon compression time was 28–51 h. No postpartum bleeding occurred. A 42-days follow-up showed no late postpartum bleeding, poor uterine incision healing, or puerperal infection, and no uterine removal was performed. Intrauterine Bakri balloon tamponade plus cervical cerclage can effectively prevent intra- and postoperative postpartum bleeding in pregnant patients with aortic dissection.

Background: Friedman's standards, developed almost 50 years ago, may no longer align with the needs of today's obstetric population and current pregnancy management practices. This study aims to analyze contemporary labor patterns and estimate labor duration in China, focusing on first-stage labor data from Chinese parturients with a spontaneous onset of labor. Methods: This retrospective observational study utilized data from electronic medical records of a tertiary hospital in Changsha, Hunan. Out of a total of 2,689 parturients, exclusions were made for multiple gestations, preterm, post-term, or stillbirth, cesarean delivery, non-vertex presentation, and neonatal intensive care unit admission. Average labor curves were constructed by parity using repeated-measure analysis, and labor duration was estimated through interval-censored regression, stratified by cervical dilation at admission. We performed an analysis to assess the impact of oxytocin augmentation and amniotomy on labor progression and conducted a sensitivity analysis using women with complicated outcomes. Results: Nulliparous women take over 180 minutes for cervical dilation from 3 to 4 cm, and the duration from 5 to 6 cm exceeds 145 minutes. Multiparous women experience shorter labor durations than nulliparous. Labor acceleration is observed after 5 cm in nulliparous, but no distinct inflection point is evident in the average labor curve. In the second stage of labor, the 95th percentile for nulliparous, with and without epidural analgesia, is 142 minutes and 127 minutes, respectively. Conclusions: These findings provide valuable insights for the reassessment of labor and delivery processes in contemporary obstetric populations, including current Chinese obstetric practice. The Dalaku'an mafic–ultramafic intrusion, located in the western segment of the East Kunlun, presents conducive conditions for the magmatic Cu-Ni sulfide deposits. According to the detailed petrographic observation, the amphiboles within distinct rock types were analyzed by EPMA analysis. The crystallization conditions, such as temperature, pressure, oxygen fugacity, and water content of the magma, were calculated to explore the genesis of the intrusion. The amphiboles were divided into three types: Amp-I, characterized by low silicon content but enrichment of aluminum, titanium, and alkali, predominantly comprising Tschermakitic hornblende and Magnesio-hornblende with mantle-derived traits; Amp-II, exhibiting elevated silicon content but diminished levels of aluminum, titanium, and alkali, primarily constituted of Magnesio-hornblende; whereas Amp-III manifests as Actinolitic hornblende, indicative of crustal origins. The calculated temperatures of amphiboles ranged between Amp-I (955–880) °C, Amp-II (852–774) °C, and Amp-III (761–760) °C; the pressures ranged between Amp-I (454–274) MPa, Amp-II (194–93) MPa, and Amp-III (101–84) MPa; the oxygen fugacities (△NNO) ranged between Amp-I (0.93–2.17), Amp-II (1.55–2.52), and Amp-III (1.89); and the water contents (H2Omelt) ranged from (6.69–8.67) to (5.90–7.32). The magma experienced multiple stages of crystallization and underwent complex magma evolution at different depths. The high oxygen fugacity and water content could be attributed to the subduction of the oceanic crust. The magma source of the Dalaku'an intrusion was metasomatized by fluids from subducting plates, thereby originating within a post-collision extension. [ABSTRACT FROM AUTHOR]

Optogenetic control of retinal cells transduced with light‐sensitive channelrhodopsins can enable restoration of visual perception in patients with vision loss. However, a light intensity orders of magnitude higher than ambient light conditions is required to achieve robust cell activation. Relatively bulky wearable light amplifiers are currently used to deliver sufficient photon flux (>1016 photons/cm2/s in a ±10° emission cone) at a suitable wavelength (e.g., 600 nm for channelrhodopsin ChrimsonR). Here, ultrahigh brightness organic light‐emitting diodes (OLEDs) with highly directional emission are developed, with the ultimate aim of providing high‐resolution optogenetic control of thousands of retinal cells in parallel from a compact device. The orange‐emitting phosphorescent OLEDs use doped charge transport layers, generate narrowband emission peaking at 600 nm, and achieve a luminance of 684 000 cd m–2 at 15 V forward bias. In addition, tandem‐stack OLEDs with a luminance of 1 152 000 cd m–2 and doubled quantum efficiency are demonstrated, which greatly reduces electrical and thermal stress in these devices. At the photon flux required to trigger robust neuron firing in genetically modified retinal cells and when using heat sinking and realistic duty cycles (20% at 12.5 Hz), the tandem‐stack OLEDs therefore show a greatly improved half‐brightness lifetime of 800 h. Previous studies have shown the role of bacterial lipopolysaccharide (LPS) in promoting tumor progression. Our previous study found that the community richness of LPS‐producing bacteria was significantly increased in the fresh stool samples of esophageal cancer (EC) patients, but the relative LPS levels and underlying mechanism in EC progression remain unknown. In this study, an case‐control study found that the content of LPS was higher in serum of EC patients. Functional experiments of CCK8 assay and transwell assay showed that LPS contributed to the proliferation, migration, invasion of EC109 cells. Meanwhile, LPS induced EC109 to secrete IL‐6 and TGF‐β1. Western blot analysis revealed the level of TLR4 and NF‐κB increased significantly after LPS treatment. Epithelial marker E‐cadherin was significantly down‐regulated and interstitial marker N‐cadherin and Vimentin were up‐regulated after LPS treatment. However, TAK242 (TLR4 inhibitor) or PDTC (NF‐κB inhibitor) could eliminate the inflammatory and EMT—promoting effects of LPS. In total, our results suggested that LPS exacerbated to the migration, invasion, and epithelial‐mesenchymal transition of EC109 cells by TLR4/NF‐κB axis.
