Background: Arrhythmogenic right ventricular cardiomyopathy (ARVC), a rare inherited disease, causes ventricular tachycardia, sudden cardiac death, and heart failure (HF). We investigated ARVC clinical features, genetic findings, natural history, and the occurrence of life-threatening arrhythmic events (LTAEs), HF death, or heart transplantation (HF-death/HTx) to identify risk factors. Methods: The clinical course of 111 consecutive patients with definite ARVC, predictors of LTAE, HF-death/HTx, and combined events were analyzed in the entire cohort and in a subgroup of 40 patients without sustained ventricular arrhythmia before diagnosis. Results: The 5-year cumulative probability of LTAE was 30% and HF-death/HTx was 10%. Predictors of HF-death/HTx were reduced right ventricle ejection fraction (HR: 0.93; P =0.010), HF symptoms (HR: 4.37; P =0.010), epsilon wave (HR: 4.99; P =0.015), and number of leads with low QRS voltage (HR: 1.28; P =0.001). Each additional lead with low QRS voltage increased the risk of HF-death/HTx by 28%. Predictors of LTAE were prior syncope (HR: 1.81; P =0.040), number of leads with T wave inversion (HR: 1.17; P =0.039), low QRS voltage (HR: 1.12; P =0.021), younger age (HR: 0.97; P =0.006), and prior ventricular arrhythmia/ventricular fibrillation (HR: 2.45; P =0.012). Each additional lead with low QRS voltage increased the risk of LTAE by 17%. In patients without ventricular arrhythmia before clinical diagnosis of ARVC, the number of leads with low QRS voltage (HR: 1.68; P =0.023) was independently associated with HF-death/HTx. Conclusions: Our study demonstrated the characteristics of a specific cohort with a high prevalence of arrhythmic burden at presentation, male predominance, younger age and HF severe outcomes. Our main results suggest that the presence and extension of low QRS voltage can be a risk predictor for HF-death/HTx in ARVC patients, regardless of the arrhythmic risk. This study can contribute to the global ARVC risk stratification, adding new insights to the international current scientific knowledge.

Background Dental anesthetic management in implantable cardioverter defibrillator (ICD) recipients with cardiac channelopathies (CCh) can be challenging due to the potential risk of life-threatening arrhythmias and appropriate ICD therapies during procedural time. Objectives The present study assessed the hypothesis that the use of local dental anesthesia with 2% lidocaine with 1:100,000 epinephrine or without a vasoconstrictor can be safe in selected ICD and CCh patients, not resulting in life-threatening events (LTE). Methods Restorative dental treatment under local dental anesthesia was made in two sessions, with a wash-out period of 7 days (cross-over trial), conducting with a 28h - Holter monitoring, and 12-lead electrocardiography, digital sphygmomanometry, and anxiety scale assessments in 3 time periods. Statistical analysis carried out the paired Student’s t test and the Wilcoxon signed-rank test. In all cases, a significance level of 5% was adopted. All patients were in stable condition with no recent events before dental care. Results Twenty-four consecutive procedures were performed in 12 patients (9 women, 3 men) with CCh and ICD: 7 (58.3%) had long QT syndrome (LQTS), 4 (33.3%) Brugada syndrome (BrS), and 1 (8.3%) Catecholaminergic polymorphic ventricular tachycardia (CPVT). Holter analysis showed no increased heart rate (HR) or sustained arrhythmias. Blood pressure (BP), electrocardiographic changes and anxiety measurement showed no statistically significant differences. No LTE occurred during dental treatment, regardless of the type of anesthesia. Conclusion Lidocaine administration, with or without epinephrine, can be safely used in selected CCh-ICD patients without LTE. These preliminary findings need to be confirmed in a larger population with ICD and CCh.

Head up tilt testing (HTT) is now commonly used to investigate otherwise unexplained syncope and presyncope. This test has been used for over 20 years primarily to diagnose neurally mediated syncope, but HTT’s exact role in the diagnostic process remains uncertain. Recognized limitations include poor reproducibility, lack of prognostic role, and insufficient randomized studies to guide therapeutic choice. In this review, we describe the indications and methods recommended by present guidelines on utilizing HTT. In addition, present criticisms and limitations of this test, along with future perspectives, are outlined.

The routine use of local anesthetics associated to vasoconstrictors for the dental treatment of patients with cardiopathies is still controversial, due to the risk of adverse cardiovascular effects.To evaluate and compare the hemodynamic effects of the use of local anesthetics with a non-adrenergic vasoconstrictor in patients with ventricular arrhythmia, when compared to the use of anesthetics without vasoconstrictor.A prospective randomized study evaluated 33 patients with positive serology for Chagas' disease and 32 patients with coronary artery disease that presented complex ventricular arrhythmia at Holter monitoring (10 EV/h and NSVT), of which 21 were females, aged 54.73 + 7.94 years, submitted to routine dental treatment with pterygomandibular anesthesia. These patients were divided in two groups: group I received prilocaine 3% associated with felypressin 0.03 IU/ml and group II received lidocaine 2% without vasoconstrictor. The number and complexity of extrasystoles were analyzed, as well as the heart rate and systemic arterial pressure of the patients on the day before, one hour before, during the procedure and one hour after the dental procedure.No hemodynamic alterations or increase in the number and complexity of the ventricular arrhythmia related to the anesthetic used in the dental procedure were observed in either group.The results suggest that prilocaine 3% associated to a felypressin 0.03 IU/ml can be safely used in patients with Chagas' disease or coronariopathy with complex ventricular arrhythmia.

To evaluate the outcome of patients with NCS after interruption of pharmacological therapy and to investigate the possible clinical variables predicting recurrence.Thirty-seven patients (age 31+/-16 years) with refractory recurrent NCS being 19 females where prospectively studied. All patients became asymptomatic and had a negative tilt table test (TT) after pharmacological therapy. The treatment was interrupted and one month later, a new TT with no medication was carried out. The probability free of symptoms recurrence was analyzed according to sex, age, number of syncope episodes previously to the treatment, clinical history time, treatment time, drug free from treatment time and TT result.Twenty-two patients (59%) presented recurrence during a mean follow-up of 21+/-19.7 months. The variables related to greater recurrence were number of previous syncope (p=0.0248), positive TT after interruption of the therapy (p=0.0002) and female gender (p=0.0131).Most of the very symptomatic patients with NCS present recurrence after the suppression of a specific therapy. A TT carried out after treatment discontinuation can identify patients with higher risk of recurrence, specially in the first year of follow-up.

Study the prevalence of neurocardiogenic etiology in patients with unexplained syncope and ventricular premature beats, with right ventricle outflow tract morphology (RVOT) and no apparent structural cardiopathy.Ninety patients (66 women, mean age 40.2 +/- 16.95 years of age) with monomorphic premature beats originated at RVOT were evaluated prospectively. Fifty-four patients reported syncopes or near-syncopes associated to palpitations or not; twenty-seven presented palpitations with no pre-syncope or syncope, and nine were asymptomatic. All patients were submitted to echocardiogram, high resolution ECG and cardiac MRI to rule out structural cardiopathy, to exertion test to rule out adrenergic dependent ventricular tachycardia, and ECG prolonged outpatient monitoring (Holter and symptomatic events monitor) to correlate symptoms and ventricular arrhythmias. Investigation on the susceptibility to neurocardiogenic syncope was evaluated by Tilt Table Test (TTT). Groups were compared regarding gender, age, premature beats frequency and complexity, at exertion or not, TTT results and clinical course.In the syncope and pre-syncope groups, TTT was positive for 38% of cases, and in groups with palpitations and assymptomatics, it was positive for 11% (p = 0.0257). After recommendations and treatment of neurocardiogenic syncope, 85% of syncope and pre-syncope patients and positive TTT was asymptomatic along the 40-month follow-up. Two patients with syncope and negative TTT presented spontaneous, sustained ventricular tachycardia during clinic course.The prevalence of neurocardiogenic syncope in patients with idiopathic RVOT premature beats is high. Patients with recurrent, unexplained syncope and idiopathic VE must be kept under investigation.

The first description of severe pain in the distribution of the glossopharyngeal nerve is credited to Weisenberg, in 1910, in a patient with cerebellopontine angle tumor. However, it was Harris, in 1926, who coined the term glossopharyngeal neuralgia to describe this rare condition characterized by paroxysms of excruciating pain located laterally at the back of the tongue, soft palate, throat, and lateral and posterior pharynx, radiating to the ear. Swallowing, coughing, yawning or chewing may trigger pain, which usually lasts from seconds to minutes. The association between glossopharyngeal neuralgia and syncope is very rare, being identified by brief episodes of bradycardia, asystole, and hypotension. Such an association, with this same pathophysiology, was first described by Riley et al in 1942.

Fibrilação atrial é uma arritmia cardíaca mais comum e principal fator de risco para acidente vascular cerebral e eventos tromboembólicos. Os anticoagulantes orais de ação direta, introduzidos como alternativa aos antagonistas da vitamina K na prevenção de eventos tromboembólicos, disponíveis no Brasil, são: dabigatrana (inibidor direto do fator IIa), rivaroxabana, apixabana e edoxabana (inibidores diretos do fator Xa). Tais fatos têm resultado em um aumento de pacientes em uso dos anticoagulantes orais diretos, que ao necessitarem de intervenções cirúrgicas odontológicas, ficam expostos a sangramentos, sendo necessário conhecer riscos e benefícios da suspensão ou não dos anticoagulantes. O objetivo desse estudo foi avaliar a segurança de um protocolo que propõe a não suspensão do anticoagulante oral direto em extrações dentárias nos pacientes em uso contínuo de rivaroxabana 1x/dia (grupo R), dabigatrana ou apixabana 2x/dia (grupo DA), ou varfarina em dose terapêutica (grupo controle), por meio de um estudo prospectivo unicêntrico e cego. As exodontias foram realizadas no período de menor concentração sanguínea do medicamento (vale), considerado de menor risco hemorrágico: grupo R, após 14 + 1 hora da ingestão da rivaroxabana, prescrita 1xdia; grupo DA, 8 + 1 hora após ingestão da dabigatrana ou apixabana, prescritas 2xdia. Pacientes do grupo controle foram submetidos à exodontia quando valores de INR recente variaram entre 2,0 e 3,0. Anestésico local, medidas hemostáticas locais e cuidados pós operatórios, foram iguais nos três grupos. Risco de sangramento sistêmico foi mensurado pelo escore HASBLED e o risco de sangramento local foi avaliado por meio da anáise da condição periodontal, do trauma cirúrgico, do tempo operatório e do número de dentes extraídos simultaneamente. Nas primeiras quatro a oito horas após o procedimento, via telefone, os pacientes foram questionados quanto à presença de sangramento precoce. Retornavam presencialmente após 24 horas para avaliação de possível sangramento tardio, por um pesquisador cego para o anticoagulante. Foram incluídos 58 pacientes com mediana de idade de 63,5 a 66 anos. Oitenta e quatro dentes foram extraídos. O principal diagnóstico para indicação da exodontia foi cárie dentária extensa (68,96%). A média de tempo de cirurgia foi semelhante nos grupos: 25, 27 e 17 minutos, respectivamente para os grupos R, DA e controle (p > 0,05%). Dois pacientes do grupo DA apresentaram sangramento menor precoce (11,1%) e evoluíram para sangramento menor tardio, enquanto no grupo R não houve caso de sangramento. No grupo controle, dois pacientes apresentaram sangramento menor precoce (10%), que evoluíram para sangramento menor tardio. Não houve diferença significativa entre os grupos (p=0,324). Os sangramentos foram assistidos e estancados com manobras hemostáticas locais, sem recidiva. Nenhum paciente apresentou sangramento maior. Os autores concluíram que o protocolo proposto é reprodutível na prática clínica, e considerado seguro quando conhecidos e respeitados o vale e pico de concentração plasmática dos anticoagulantes orais de ação direta estudados, além do acompanhamento do paciente e da aplicação de medidas hemostáticas locais Atrial fibrillation is the most common cardiac arrhythmia and a major risk factor for stroke and other embolic events. Direct oral anticoagulants were then introduced as an alternative to vitamin K antagonists in the prevention of thromboembolic events. Those available in Brazil are: dabigatran (direct factor IIa inhibitor), rivaroxaban, apixaban and edoxaban (direct factor Xa inhibitors). There is now an increase in patients using direct oral anticoagulants, who, when requiring dental surgery, are exposed to bleeding, and it is necessary to know the risks and benefits of suspending or not those medications. The objective of this study was to evaluate the safety of a protocol with the proposal of not suspending direct oral anticoagulants in dental extractions in patients on continuous use of rivaroxaban 1x/day (group R), dabigatran or apixaban 2x/day (DA group), or warfarin at a therapeutic dose (control group), through a single-center, blinded prospective study. The extractions were performed in the period of lower blood concentration of the drug (valley), considered a time of lower hemorrhagic risk: group R, after 14 + 1 hour of ingestion of rivaroxaban, prescribed once a day; DA group, 8 + 1 hour after ingestion of dabigatran or apixaban, prescribed twice a day. Local anesthetic, local hemostatic measurements, and postoperative care were the same in the three groups. The risk of systemic bleeding was measured by the HASBLED score. To assess the risk of local bleeding the periodontal condition of the extracted teeth, surgical trauma, operative time and number of extracted teeth were analyzed. In the first four to eight hours after the procedure, via telephone, the patients were asked about the presence of early bleeding. They returned in person after 24 hours for evaluation of possible late bleeding, by a researcher blinded to the anticoagulant. A total of 58 patients were included in this study. The average age of patients ranged from 63,5 to 66 years. Eighty- four teeth were extracted. The main reason for the indication of extraction was extensive dental caries (68,96%). The mean surgery time was similar in the groups: 25, 27 and 17 minutes, respectively for the R, DA and control groups (p > 0.05%). Two patients in the DA group presented early minor bleeding (11.1%) and progressed to late minor bleeding, while in the R group there was no case of bleeding. In the control group, two patients had early minor bleeding (10%), which progressed to late minor bleeding. There was no significant difference between groups (p=0,324). All bleeding was assisted and stopped with local hemostatic measures, without recurrence. No patient had major bleeding. The authors concluded that the proposed protocol is reproducible in clinical practice and considered safe when the valleys and peaks of plasma concentrations of the studied direct oral anticoagulants are respected, in addition to patient monitoring and application of local hemostatic measures

Introdução: A síncope vasovagal recorrente, embora apresente prognóstico benigno em termos de sobrevida, está associada a importante comprometimento da qualidade de vida, especialmente nos quadros refratários. Reações a antecedentes de perda podem influenciar na recorrência dos episódios sincopais e no grau de comprometimento social dos pacientes. Objetivos: 1) Comparar o número de síncopes e présíncopes entre o grupo no qual se acrescentou, ao tratamento médico habitual, sessões semanais de psicoterapia e o grupo que seguiu o cuidado médico convencional de forma isolada. 2) Avaliar o efeito da psicoterapia na economia psicossomática e na qualidade de vida dos pacientes com síncopes vasovagais recorrentes. 3) Analisar os processos psicossomáticos envolvidos em situações de adoecimento por síncope vasovagal e estabelecer possíveis relações entre a eclosão da doença e vivências anteriores de traumas, focando em prováveis somatizações. Métodos: Foi realizado um ensaio clínico controlado randomizado, envolvendo 10 pacientes com síncope vasovagal refratária. Doenças cardíacas e intervenções psiquiátricas e psicoterapêuticas em curso foram critérios de exclusão. Após a randomização, metade dos pacientes foi submetida a sessões semanais de psicoterapia durante um ano, embasada na linha teórica da Psicossomática Psicanalítica, e todos os pacientes (grupo controle e intervenção) continuaram com tratamento assistencial padrão. A recorrência, o número de síncopes/pré-síncopes e a qualidade de vida (SF36) foram avaliados no contato inicial e aos 6 e 12 meses após a inclusão. Resultados: Dentre os 10 pacientes incluídos, 80% eram do sexo feminino e a idade média de 48,8 (± 11,4) anos. A frequência média de eventos sincopais era de um episódio de pré-síncope por semana e quatro síncopes ao ano. Todos apresentavam tilt test positivo, sendo 80% com resposta vasovagal mista. A análise da recorrência de eventos e QV não mostrou diferença relevante no grupo controle, mas no grupo de intervenção houve redução significativa na frequência de pré-síncopes por mês (5,7 ± 1,4 x 1,7 ± 1,0; P < 0,01) e na frequência de síncopes em um ano (4,6 ± 0,9 x 1,0 ± 0,7; P < 0,01). Houve também uma melhora considerável na avaliação geral da QV (44,1 ± 10,0 vs. 70,3 ± 10,3; P < 0,01). Ao comparar ambos os grupos em um ano, houve significativamente menos síncopes e pré-síncopes e melhor qualidade de vida no grupo de intervenção. Em relação aos fatores psíquicos que precedem o diagnóstico de síncope vasovagal, foram identificadas experiências traumáticas. Conclusões: Pacientes com SVV xii refratária submetidos à intervenção psicoterápica apresentaram menor recorrência de episódios e melhora na qualidade de vida, com mudança favorável na maneira de lidar com as situações de perdas Introduction: Although recurrent vasovagal syncope has a benign prognosis in terms of survival, it is associated with a significant impairment of quality of life, especially in refractory conditions. Reactions to previous traumatic conditions can influence the recurrence of syncopal episodes and the social impairment of these patients. Objectives: 1) To compare the number of syncope and near-syncope events between the group submitted to weekly sessions of psychotherapeutic intervention in addition to conventional medical therapy and the control group, that followed the standard of care alone. 2) To evaluate the effect of psychotherapy on psychosomatic economy and quality of life in patients with recurrent vasovagal syncope. 3) To analyse the psychosomatic processes involved in situations of illness due to vasovagal syncope and establish possible relationships between the onset of the disease and previous experiences of trauma, focusing on probable somatization Methods: A randomized controlled clinical trial involving 10 patients with refractory vasovagal syncope was performed. Heart disease and ongoing psychiatric and psychotherapeutic interventions were exclusion criteria. After randomization, half of the patients underwent weekly sessions of psychotherapy based on the theoretical line of Psychoanalytic Psychosomatics for one year and all patients (control and intervention group) continued with standard care treatment. Recurrence, number of syncope/near-syncope and quality of life (SF-36) were assessed at baseline, 6 and 12 months after inclusion. Results: Among the 10 patients included, 70% were female, with a mean age of 47.4 ± 11.1 years. The average frequency of syncope events was approximately one presyncope per week and four episodes of syncope per year. All had a positive tilt test, 80% with mixed vasovagal response. The analysis of recurrence of events and QoL did not show any significant change in the control group, but in the intervention group there was a significant reduction in the frequency of presyncope per month (5.7 ± 1.4 x 1.7 ± 1.0; P < 0.01) and the frequency of syncope in one year (4.6 ± 0.9 x 1.0 ± 0.7; P < 0.01). There was also a considerable improvement in the overall assessment of QoL (44.1 ± 10.0 vs. 70.3 ± 10.3, P < 0.01). Comparing both groups after one year of follow-up, there were significantly less syncope and near syncope events and better QoL in the intervention group. Regarding the psychic factors that precede the diagnosis of vasovagal syncope, traumatic experiences were identified. Conclusion: Patients undergoing psychotherapeutic intervention had less recurrence of episodes of syncope and near-syncope and improved quality of life, with a favourable change in the way they deal with previous traumatic events

INTRODUÇÃO: A Hipersensibilidade do seio carotídeo (HSC) é a exacerbação do reflexo normal e foi definida como ocorrência de pausa ventricular 3 segundos ou redução da pressão arterial sistólica (PAS) 50 mmHg em resposta à massagem do seio carotídeo (MSC). Fenômeno relacionado à idade, raramente diagnosticado em pacientes com menos de 50 anos, tem recebido especial atenção como causa de síncope e quedas inexplicadas nos idosos, nas últimas décadas, com relatos de taxas de prevalências superiores a 45%. Entretanto, ainda não estão claras as implicações diagnósticas da HSC na síncope, com resultados controversos na literatura. OBJETIVOS: Determinar a prevalência da HSC em pacientes com sintomas de síncope e pré-síncope e comparar com indivíduos assintomáticos. Correlacionar a resposta à MSC com a rigidez aórtica e os parâmetros anatômicos e funcionais carotídeos. MÉTODOS: Foram avaliados em estudo prospectivo 99 pcts sintomáticos, com síncope ou pré-síncope a esclarecer (idade média de 69 anos, 41,4% homens), e 66 pcts assintomáticos para controle (idade média de 73 anos, 34,8% homens). Excluíram-se pacientes com cardiopatia estrutural ou com contraindicações para MSC. A MSC foi realizada no ponto com maior impulsão carotídea por 5 segundos, com o registro contínuo e não invasivo da pressão arterial (PA) e eletrocardiograma, com o paciente em postura ortostática a 70º. Foram consideradas respostas anormais: cardioinibitória (CI): assistolia 3 segundos e vasodepressora (VD): redução da PAS 50 mmHg. O índice da rigidez arterial foi obtido por meio de medida da velocidade de onda de pulso carotídeo-femoral (VOP). As características anatômicas e funcionais da carótida foram determinadas por medidas de diâmetro, espessura íntima-média carotídea (EIMC) e índice de distensibilidade. RESULTADOS: Não foram constatadas diferenças nas respostas obtidas na MSC entre os grupos, com 67,7% e 60,6% de respostas fisiológicas; 24,2% e 25,8% de respostas CI; 8,1% e 13,6% de respostas VD em grupo sintomáticos e assintomáticos, respectivamente (p=0,466). Não foram observadas correlações entre a resposta à MSC, tanto com a VOP como com a EMIC, a distensibilidade e o diâmetro carotídeo. CONCLUSÕES: 1- A prevalência de HSC e resposta hemodinâmica à MSC em pacientes com sintomas de síncope e pré-síncope foram semelhantes a pacientes assintomáticos provenientes da mesma instituição, com características clínicas semelhantes. 2- Não foi observada correlação significativa entre a rigidez arterial, medida por meio da VOP, EIMC, distensibilidade e diâmetro carotídeo e a resposta à MSC The carotid sinus hypersensitivity (CSH) is the exaggeration of the normal reflex and was defined by occurrence of asystole 3 seconds or fall in systolic BP 50 mmHg in response to carotid sinus massage (CSM). Phenomenon related to age, rarely diagnosed in patients younger than 50 years, has gained importance as a cause of syncope and unexplained falls in the elderly in recent decades with reported prevalence rates above 45%. However, the correlation between CSH and syncope etiology is still controversial. OBJECTIVE: To determine the prevalence of CSH in patients with syncope and near syncope of unknown origin and compare with asymptomatic individuals; to evaluate the correlation between CSM responses and arterial stiffness. METHODS: We studied prospectively 99 symptomatic pts with syncope or near syncope (mean age 69 years, 41.4% men) and 66 asymptomatic controls (mean age 73 years, 34.8% men). Patients with significant structural heart disease or with contraindications to CSM were excluded. The CSM was performed at the point with maximal carotid pulsation, for 5 seconds with continuous and noninvasive blood pressure and electrocardiogram recording at 70° in upright posture. Were considered abnormal responses: cardioinhibitory (CI): asystole 3 seconds and vasodepressor (VD): decrease in systolic BP 50 mmHg. The aortic stiffness was determined by aortic pulse wave velocity (PWV). The anatomical and functional characteristics of the carotid were determined by measurements of diameter, intima-media thickness (IMT) and distensibility index. RESULTS: There were no differences in the responses obtained in the CSM between the groups, being 67.7 % and 60.6% physiological responses, 24.2% and 25.8% CI responses and 8.1% and 13.6% VD responses in symptomatic and asymptomatic groups, respectively (p=0.466). There were no correlations between response to the CSM with VOP, IMT, carotid diameter and distensibility. CONCLUSIONS: The prevalence of CSH in patients with symptoms of syncope and near syncope was similar to asymptomatic patients from the same institution with similar clinical characteristics. There was no significant correlation between arterial stiffness, measured by PWV, IMT, carotid diameter and distensibility with the response to CSM

INTRODUÇÃO: Síncope inexplicada é considerada um fator de risco de morte súbita na cardiomiopatia hipertrófica (CMH). Em sua patogênese estão envolvidos meca-nismos diversos, incluindo a dificuldade de adaptação da resistência vascular sistêmica ao exercício e ao estresse ortostático, que pode ser influenciada por uma disfunção do sistema nervoso autônomo. Os objetivos deste estudo foram comparar a função nervosa autonômica em portadores de CMH com e sem síncope, bem como avaliar o valor diagnóstico do teste de inclinação (TI) na investigação de síncope nessa população. MÉTODOS: Foram incluídos 37 pacientes, 16 com síncope inexplicada à avaliação rotineira e 21 sem síncope. A função nervosa autonômica foi medida pela sensibilidade do barorreflexo (BR) espontâneo e do induzido por fenilefrina e pela variabilidade da freqüência cardíaca (VFC). As variáveis da VFC consideradas no domínio do tempo foram: desvio-padrão de todos os intervalos RR normais (SDNN); raiz quadrada da média do quadrado das diferenças entre intervalos RR normais adjacentes (RMSSD); e percentagem de intervalos RR adjacentes com diferença superior a 50 ms (pNN50), durante o eletrocardiograma de 24 horas. No domínio da freqüência, foram considerados os componentes de alta, baixa e muito baixa freqüência e a densidade total do espectro, tanto em valores absolutos como em unidades normalizadas, em repouso e aos 60 graus de inclinação. As medidas da pressão arterial sistólica e diastólica, batimento a batimento, e as medidas do índice sistólico, do índice cardíaco e da resistência vascular sistêmica, obtidas pela cardiografia por impedância, foram comparadas, entre os grupos, a 0, 30 e 60 graus de inclinação. O TI consistiu na exposição dos pacientes a 60º de inclinação por 40 minutos, ou até uma resposta positiva. RESULTADOS: A sensibilidade do BR, tanto espontâneo (16,46±12,99 vs 18,31±9,88 ms/mmHg, p=0,464) como induzido por fenilefrina (18,33±9,31 vs 15,83±15,48 ms/mmHg, p=0,521) foi semelhante nos grupos síncope e sem síncope. Não foram observadas diferenças nos valores de SDNN (137,69±36,62 vs 145,95±38,07 ms, p=0,389). O grupo síncope apresentou menores valores de RMSSD (24,88±10,03 vs 35,58±16,43 ms, p=0,042) e tendência a menor pNN50 (4,51±3,78 vs 8,83±7,98 %, p=0,085) e a menores valores do componente de alta freqüência da análise espectral, em repouso (637,59±1295,53 vs 782,65±1264,14 ms2, p=0,075). Não foram observadas diferenças significativas entre os grupos nos demais parâmetros analisados no domínio da freqüência. A adaptação das variáveis hemodinâmicas aos diferentes graus de inclinação foi semelhante entre os grupos nas várias posições estudadas. A positividade ao TI foi semelhante nos dois grupos (6% no grupo síncope e 33% no sem síncope, p=0,053). A sensibilidade, a especificidade e a acurácia do TI em identificar a causa da síncope na amostra foram, respectivamente, 6%, 66% e 40%. CONCLUSÃO: Uma menor atividade parassimpática, medida pela VFC, foi observada nos portadores de CMH com síncope. Não foram encontradas diferenças na reserva vagal e na adaptação hemodinâmica ao estresse ortostático entre os grupos. O TI revelou-se com baixa sensibilidade, especificidade e acurácia para o diagnóstico de síncope inexplicada nessa população. BACKGROUND: Unexplained syncope is considered a risk factor for sudden death in hypertrophic cardiomyopathy (HCM). Several mechanisms are involved in its pathogenesis, including the difficulty in adaptation of the systemic vascular resistance to exertion and to orthostatic stress, which may be influenced by a dysfunction of the autonomic nervous system. The purposes of this study were to compare the autonomic nervous function in patients with HCM with and without syncope and to assess diagnostic value of the head-up tilting test (HUT) in this population. METHODS: Thirty seven patients were included: 16 with unexplained syncope at routine evaluation and 21 without syncope. The autonomic nervous function was assessed by spontaneous and phenylephrine-induced baroreflex sensitivity (BRS) and by heart rate variability (HRV). Considered HRV variables in time domain were: standard deviation of normal RR intervals (SDNN), root mean square of successive differences (RMSSD), and percentage of adjacent normal RR intervals which differ by at least 50 ms (pNN50), during 24 hours electrocardiogram recording. In frequency domain, high, low and very low frequency bands and the spectrum total power density were considered, both in absolute values and in normalized units, at rest and at 60-degree tilting. Measures of systolic and diastolic blood pressure, beat to beat, and measures of stroke index, cardiac index, and systemic vascular resistance, obtained by impedance cardiography, were compared between the groups, at 0-, 30- and 60-degree tilting. The HUT consisted in exposure to 60º for 40 minutes, or until a positive response. RESULTS: Spontaneous BRS measures were similar between the syncope and non-syncope groups (16.46±12.99 vs 18.31±9.88 ms/mmHg, p=0.464), as well as phenylephrine induced BRS (18.33±9.31 vs 15.83±15.48 ms/mmHg, p=0.521). No differences were found between SDNN values (137.69±36.62 vs 145.95±38.07 ms, p=0.389). The syncope group presented lower values of RMSSD (24.88±10.03 vs 35.58±16.43 ms, p=0.042) and a trend to lower pNN50 (4.51±3.78 vs 8.83±7.98 %, p=0.085) and to lower high frequency component of spectral analyses at rest (637.59±1295.53 vs 782.65±1264.14 ms2, p= 0.075). No differences were observed between the groups in the others parameters analyzed in the frequency domain. Adaptation of hemodynamic variables at different tilting degrees was similar between the groups at the various positions studied. Positive responses to HUT were similar in the two groups (6% in syncope group and 33% in no-syncope group; p=0.053). HUT sensitivity, specificity and accuracy in identifying the cause of syncope in this population were, respectively, 6%, 66% and 40%. CONCLUSION: A lower parasympathetic activity, measured by HRV, was observed in HCM patients with syncope. No differences were found in vagal reserve and in adaptation to orthostatic stress between the groups. HUT showed poor sensitivity, specificity and accuracy in diagnosing unexplained syncope in this population.

A utilização de anestésicos locais associados a substâncias vasoconstritoras para tratamento odontológico de rotina de pacientes cardiopatas ainda é controversa, devido ao risco de efeitos cardiovasculares adversos. Objetivo: avaliar possíveis alterações hemodinâmicas relacionadas ao uso de anestésico local com vasoconstritor não adrenérgico em pacientes portadores de arritmias cardíacas comparando ao uso de anestésico sem vasoconstritor. Métodos: Estudo prospectivo randomizado, avaliou 33 pacientes com sorologia positiva para Doença de Chagas e 32 pacientes com Doença Arterial Coronariana, portadores de arritmia ventricular complexa ao Holter (>10 EV/h e TVNS), 21 do sexo feminino; idade 54,73 ± 7,94 anos, submetidos a tratamento odontológico de rotina com anestesia pterigomandibular, divididos em dois grupos: Grupo I -utilizando-se prilocaína a 3% com felipressina 0,03 UI/ml e Grupo II -lidocaína 2% sem vasoconstritor. Foram avaliados o número e complexidade de extrassístoles, a freqüência cardíaca e a pressão arterial sistêmica dos pacientes no dia anterior, uma hora antes, durante e uma hora após o procedimento odontológico. Resultados: Não foram observadas alterações hemodinâmicas relacionadas ao anestésico utilizado, nem diferença estatisticamente significativa entre os grupos. Conclusão: Prilocaína a 3% com felipressina 0,03 UI/ml pode ser utilizada com segurança em pacientes chagásicos e coronarianos com arritmia ventricular complexa. The use of a local anesthetic solution with a vasoconstrictor agent for routine dental treatment in patients with cardiovascular disease is still controversial. Objetive: to evaluate the possible hemodynamics effects related to the use of a local anesthetic solution associated with a non-adrenergic vasoconstrictor agent, comparatively to another local anesthetic solution without a vasoconstrictor agent, in patients with cardiac arrhythmia. Methods: Prospective, randomized study, evaluated 33 patients with Chagas´s Cardiomyopathy and 32 with Coronary Artery Disease, 21 female; mean age 54,73 ± 7,94 of years old and complex ventricular arrhythmia detected by the 24 hours Holter monitoring. The patients underwent a routine dental treatment with pterygomandibular anesthesia, divided into two groups: Group I - received local anesthesia with 3% prilocaine associated with felypressin 0,03UI/ml and Group II - received 2% lidocaine without vasoconstrictor. Heart rate, number and complexity of premature ventricular beats and systemic blood pressure were monitored the day before, one hour before, during and one hour after dental treatment. Results: There were no changes in the homodynamic parameters related to the anesthetic solution and no significant differences were observed between the two groups. Conclusion: These findings suggest that 3% prilocaine with felypressin 0,03 UI/ml can be safely used in patients with complex ventricular arrhythmia associated with Chagas´s and coronary artery desease.

Estudo comparativo entre o tratamento farmacológico, o treinamento físico moderado e o treinamento postural passivo em pacientes portadores de síncope neurocardiogênica. A síncope neurocardiogênica é uma disfunção autonômica que leva a hipotensão e perda de consciência. Setenta pacientes foram randomizados em 4 grupos: controle, treinamento físico, treinamento postural e tratamento farmacológico. Avaliou-se recorrência clínica, índices de ansiedade e a sensibilidade barorreflexa para a freqüência cardíaca e atividade nervosa simpática muscular. Ocorreu diminuição da recorrência nos 4 grupos. Diminuição da ansiedade foi observada em 3 grupos, exceto no grupo controle. O treinamento físico melhorou a sensibilidade barorreflexa Neurocardiogenic syncope is an autonomic disfunction that leads to hypotension and loss of conciousness. Seventy patients were randomized in 4 groups: control, physical training, tilt training and pharmacological treatment. Clinical outcome, anxiety levels and baroreflex sensitivity for heart rate and muscle sympathetic nervous activity were evaluated. Lower recurrence rates were obtained in all groups. Anxiety levels decrease was achieved in 3 groups, except controls. Physical training improved baroreflex sensitivity

O sistema Holter é considerado padrão ouro para detecção de arritmias cardíacas, possibilitando o seu diagnóstico e sua relação com os sintomas do paciente. As novas gerações de marcapassos (MP) detectam e registram eletrogramas intracavitários e poderiam dispensar o Holter no seguimento dos pacientes. Os diferentes MP dispõem de algoritmos de detecção de arritmias ventriculares (AV) que variam de acordo com o modelo e sua correlação diagnóstica com Holter foi pouco estudada. O objetivo desse estudo foi avaliar 1. a correlação entre o monitor de eventos dos MP e do Holter na detecção de AV e 2. a concordância das AV detectadas nos dois sistemas. Foram estudados129 pacientes portadores de MP com função de detecção de arritmias, com idade média de 68,6+19.1 anos (entre 19 e 94), 54,8% do sexo feminino. Os pacientes foram submetidos à monitorização com Holter por 72 horas. Assim que o sistema foi ligado, os contadores de eventos dos marcapassos foram reiniciados e os relógios sincronizados, para que ambos os sistemas detectassem os eventos simultaneamente. Os MP foram programados para detecção de eventos com o menor valor de frequência ventricular (FV) e o menor número de batimentos sequenciais (BT) possíveis. Depois de 72 horas, os sistemas Holter foram retirados e os registros analisados, assim como os registros simultâneos dos gravadores dos MP. Foram qualificados como eventos arrítmicos no Holter e MP, respectivamente: EV isoladas e \"PVC\"; extra-sístoles em pares e \"couplets\"; TVNS (> 3BT) e \"triplets\"(3BT), \"runs\"(3 a 8 ou > 8 BT) e \"HVR\" (3 ou 4 BT). As correlações de Spearman foram utilizadas para avaliar se o marcapasso acompanha a detecção dos parâmetros do Holter. Os coeficientes de correlação intraclasse e os respectivos intervalos com 95% de confiança, calculados para avaliar a concordâncias entre os parâmetros equivalentes do marcapasso e do Holter. Foram calculados os coeficientes Kappa para avaliar a concordância na detecção de > 10 \"PVC\"/h com > 10 EV/h. Resultados: Os monitores dos marcapassos subestimaram o Holter. Os registros de \"PVC\", \"triplet\" e \"HVR\" por TVNS apresentaram correlações positivas em relação aos parâmetros do Holter, sendo a mais alta aquela entre \"PVC\" e EV (r=0,501). Porém, a concordância entre os tipos de arritmias detectadas foi baixa (CCI < 0,5), exceção feita à concordância de \"triplet\" com TVNS de três batimentos (CCI=0.984). A concordância na detecção de mais de 10 PVC/h e mais de 10 EV/h foi moderada (kappa= 0,483), embora para os MP da Medtronic, o coeficiente de concordância foi alto (kappa=0.877). Para os MP com algoritmo de detecção de sequências de três batimentos com FV menor que 140bpm ( < 140/3), a correlação entre HVR e TVNS foi expressiva (r = 1), sendo a concordância entre esses parâmetros também bastante alta (CCI = 0,800). Conclusões: A correlação e a concordância na detecção de AV registradas nos MP e no Holter foram inconsistentes. Padronização dos algoritmos de detecção de AV, semelhantes os do sistema Holter, é necessária para que os pacientes portadores de dispositivos implantáveis possam se beneficiar dessa função para seguimento clínico e estratificação de risco The Holter monitoring is considered the gold standard method for detection of cardiac arrhythmias, enabling its diagnosis and its correlation with the patient\'s symptoms. New generations of artificial pacemakers can detect and record intracavitary electrograms and, theoretically, could discard the Holter during the follow-up of the patients. Pacemakers have different ventricular arrhythmias (VA) detection algorithms, varying according to the model. Their diagnosis capacity comparing to Holter monitoring has been poorly studied. The aim of this study was to evaluate 1. the correlation between the event monitor of the pacemaker and the Holter in VA detection and 2. the agreement between the VA detected in both systems. We studied 129 patients with implanted pacemakers, which had arrhythmia detection function, mean age of 68.6 + 19.1 years (19 to 94), 54.8% female. The patients underwent Holter monitoring for 72 hours. Once the system was connected, event counters of pacemakers were reset and the clocks synchronized, so that both systems could detect the arrhythmic events simultaneously. The pacemakers were programmed to detect events with the lowest ventricular rate and lowest number of sequential beats allowed for each model. After 72 hours, Holter systems were removed and the records analyzed, as well as the simultaneous records of the pacemakers. Were considered as arrhythmic events: PVC isolated (in the pacemaker described as \"PVC\"); premature beats in pairs (pacemaker described as \"couplets\"); NSVT (pacemaker described as \"triplets\"- 3beats, \"runs\"- 4 to 8 or > 8 beats and \"HVR\"- 3 to 4 beats). Spearman rank correlations were used to assess whether the pacemaker and Holter identified the same parameters. The intraclass correlation coefficients and the respective intervals with 95% confidence were calculated to evaluate the concordance between the equivalent parameters of the pacemaker and Holter. Kappa coefficients were calculated to assess the agreement in the detection of > 10 PVC/h by the pacemakers and by the Holter. Results: The pacemakers underestimated the arrhythmias detection of Holter. Records of \"PVC\", \"triplet\" and \"HVR\" by NSVT showed positive correlations with the Holter parameters, and the highest one was among \"PVC\" and EV (r = 0.501). The agreement between the types of arrhythmias detected was quite low (CCI < 0.5), except for \"triplet\" detected by pacemakers and three beats NSVT by Holter (ICC = 0.984). The correlation detection for more than 10 PVC /h was moderate (kappa = 0.483), except for Medtronic pacemakers (kappa=0.877). When the pacemaker was programmed to detect sequences of three beats with heart rate lower than 140bpm ( < 140/3), the correlation between HVR and NSVT was perfect (r = 1) and the agreement between these parameters was also quite high (ICC = 0.800). Conclusions: The correlation and agreement between pacemakers and Holter monitoring in the detection of VA were not consistent. A standardization of the pacemakers\' detection algorithms is necessary before using this function for clinical follow-up and risk stratification of the patients

Introdução: A morte súbita é a principal causa de morte na doença de Chagas, correspondendo de 55 a 65% dos casos. Observa-se que parte destas, ocorre em pacientes com função ventricular esquerda (FEVE) preservada, levando a acreditar que fatores desestabilizadores do substrato arritmogênico exercem um importante papel nestes eventos. Evidências já demonstraram a depressão parassimpática como fator contribuinte na gênese de arritmias diversas em presença de cardiopatia isquêmica. Assim, insiste-se na necessidade de se identificar precocemente quais os pacientes, no contexto da cardiopatia chagásica crônica, apresentam risco aumentado para o desenvolvimento de eventos arrítmicos complexos. Acredita-se que a avaliação autonômica identifique subgrupos distintos de risco. O presente estudo teve como objetivo determinar a sensibilidade do barorreflexo (SBR) em pacientes com doença de Chagas, nas formas indeterminada (GI) e arritmogênica com taquicardia ventricular não sustentada (GII) e com taquicardia ventricular sustentada (GIII) e, secundariamente, avaliar a associação entre a severidade da arritmia ventricular com o grau de comprometimento da SBR. Métodos: 42 pacientes foram submetidos à monitorização cardiovascular não invasiva pelo sistema Task Force ® onde foi determinada a SBR, utilizando o método da fenilefrina e analisada a variabilidade da frequência cardíaca (VFC) no domínio do tempo por meio da eletrocardiografia dinâmica de 24horas e a FEVE, por meio da ecocardiografia. Resultados: Observou-se diferença estatística significativa entre os grupos em relação à SBR em resposta à fenilefrina. O GIII apresentou o menor valor de SBR (6,09 ms/mmHg) quando comparado aos GII (11,84ms/mmHg) e GI (15,23ms/mmHg). Após comparação múltipla entre os grupos, verificou-se diferença significativa entre GI e GIII (p= 0,01). Quando se correlacionou SBR e densidade de extra-sístoles ventriculares (EV), observou-se que todos os pacientes portadores de baixa densidade de EV (< 10/hora) apresentavam SBR preservada (6,1ms/mmHg).Em contrapartida, entre aqueles com alta densidade de EV (>10/hora) somente 59% tinham SBR preservada (p=0,003). Nos pacientes com SBR deprimida (3,0-6,0 ms/mmHg) houve maior densidade de EV (p=0,01). Pacientes com SBR preservada apresentaram tanto função ventricular normal como moderadamente comprometida (66,7% com FEVE10/hour) had preserved BRS (p=0.003). Estudou-se o efeito do treinamento físico sobre a freqüência cardíaca (FC), a freqüência cardíaca intrínseca (FCI), o efeito vagal (EV), o tônus vagal (TV), o efeito simpático (ES) e o tônus simpático (TS), de ratos idosos em repouso volitivo, na esteira, e durante o exercício de intensidade progressiva (4 estágios de 5 min à 5; 7,5; 10 e 15 m.min-1). Verificaram-se, também, as respostas da FC à doses crescentes de agonistas ?-adrenérgico (isoproterenol) e muscarínico (metacolina). Utilizaram-se 20 ratos Wistar machos, aleatoriamente divididos em dois grupos: Treinado (T, 28+2 meses, 460+36 g), submetido a 10 semanas de treinamento físico de moderada intensidade; e Sedentário-controle (S, 28+2 meses, 461+43 g), apenas manipulado, três a cinco vezes por semana, durante nove semanas, e submetido a cinco minutos de exercício diário, na décima semana, para habituação ao pesquisador e ao ambiente experimental. Há escassos estudos sobre a monitorização eletrocardiográfica ambulatorial em casuísticas brasileiras de indivíduos assintomáticos submetidos ao exame clínico. Com a finalidade de avaliar a freqüência cardíaca, as arritmias cardíacas, e os distúrbios atrioventriculares de condução, foram estudados 625 indivíduos assintomáticos, com exame clínico normal. A idade variou de 15 a 83 (média 42, desvio padrão 11,9) anos; 276 (44,2%) eram homens e 349 (55,8%) mulheres. Os indivíduos foram submetidos ao exame clínico, que incluiu história, exame físico e avaliação laboratorial, além da monitorização eletrocardiográfica ambulatorial de 24 h. Foram estudadas as variáveis obtidas na monitorização eletrocardiográfica ambulatorial nas 24 h de gravação e nos períodos das 6h às 22h, e das 22h às 6h em relação às variáveis do exame clínico-laboratorial. Para a análise dos dados, depois da estatística descritiva, as variáveis contínuas foram examinadas com o emprego da correlação canônica, da regressão linear simples e da regressão linear múltipla. As variáveis categóricas foram analisadas por meio da regressão logística. A freqüência cardíaca mínima nas 24h e no período das 22h às 6h elevou-se com o aumento da idade apenas nos homens.
