To assess the clinical periodontal status of abutment teeth in regards of different design components of distal-extension removable partial dentures. Subjects (N=100) with acrylic or cobalt-chromium distal-extension removable partial dentures were enrolled and their periodontal parameters plaque and gingival indices [PI, GI], probing depths [PD], clinical attachment loss [CAL] and a mobility index [MI] evaluated. Denture base type, major connector, occlusal rests position, design of direct retainers, retention, stability and denture wearing habits were further evaluated. Acrylic RPDs were associated with higher mean±SE PI [1.70±0.74], GI [1.76±0.55], PD scores [2.47±1.02 mm] and CAL values [4.46±2.11 mm] compared to CO-CR RPDs [p⟨0.05]. For abutments, the PI [1.6±0.83], GI [1.72±0.57], PD [2.32±1.03] and CAL [4.26±2.08] were higher than their non-abutments counterparts [p⟨0.05]. CAL scores were found to be significantly higher for mandibular abutments compared to maxillary ones [P=0.002]. The highest PI [1.83±1.10] and GI [2.00±0.00] scores were associated with lingual bars and horse-shoe connectors, respectively. Full palatal coverage and lingual plates were associated with the highest PD [2.80±0.48] and CAL [4.70±0.37] scores. Acrylic RPDs, type of major connector, wrought wire clasps and distal occlusal rests may be considered as risk factors for periodontal disease progression in distal-extension removable partial denture wearers., (Copyright© 2023 Dennis Barber Ltd.)

Objective: To evaluate and compare the impact of two removable partial dentures (acrylic removable partial denture [ARPD] and metallic removable partial denture [MRPD]) on periodontal tissues of the remaining teeth in the first 12 months of denture use., Materials and Methods: This prospective clinical study included 40 patients, of which 20 received ARPDs, 20 received MRPDs, nine in the maxilla, and 11 in the mandible each. The patients were 45-65 years old; 24 were females, and 16 were males. Patients' demographic details, clinical indicators of periodontal complications, and biochemical measurement of Hs-C-reactive protein (CRP) and alkaline phosphatase (ALP) were considered. One-way analysis of covariance and Friedman were used to measure the differences in clinical periodontal parameters between the two types of dentures., Results: The significant findings were: Plaque index (PLAQ) scores for abutment teeth were higher in MRPD wearers (mean = 12.15) than ARPD wearers (mean = 10.45), whereas ARPD users had significantly higher mean bleeding on probing (BOP) values (mean = 1.5) than MRPD users (mean = 0.00); mobility of abutment teeth showed no significant differences; timeline comparisons showed a significant increase in the percentage of nonabutment teeth mobility in ARPD users (p = .028) compared with MRPD users over the same follow-up period (p = .102)., Conclusions: For a short-term period of 1 year, periodontal and mobility parameters have no significant impact on the abutment and nonabutment teeth of ARPD and MRPD users. BACKGROUND Prosthetic rehabilitation will have harmful effects on periodontal structure if the patient fails to maintain an oral hygiene protocol. This study aimed to evaluate oral hygiene in fixed and removable partial denture wearers in the Aseer Province of Saudi Arabia. MATERIAL AND METHODS This was a cross-sectional study done on 286 prosthesis wearers aged between 25 and 55 years; 142 were men and 144 women. Clinical examination was done using 3 periodontal parameters: plaque index, gingival index, and calculus surface index. RESULTS It was found that 72% and 25% of patients used fixed and removable partial prostheses, respectively. Most patients were in the 45-55 year age group (38.1%), were medically fit (78%), and used toothbrush and paste (70.6%). Most patients were instructed regarding the use of oral hygiene measures for their prostheses (71.3%). However, around half of the study group (52.8%) had odor from their prostheses. Most fixed prostheses were in the posterior teeth (73.2%) and had 3 units or more (58.7%). Removable partial dentures were mostly tooth-tissue supported (74%). A statistically significant difference was found for plaque index and gingival index between natural teeth and abutments in terms of various prosthetic parameters (P£0.001). The higher prevalence of gingival inflammation, plaque, and calculus accumulation in this study may be correlated to improper oral hygiene methods used by patients. CONCLUSIONS It can be concluded that there is a need to reinforce meticulous oral hygiene practices among patients using prosthodontic appliances.

Objective: The aim: Justify the choice of the construction material of dental prostheses, taking into account the clinical picture and concomitant diseases in military personnel and the study of the dynamics of military personnel seeking orthopedic dental care., Patients and Methods: Materials and methods: 185 military personnel were examined and fitted with prosthetics in the period from March 2022 to March 2023. Methods: general clinical, content analysis, bibliosemantic, medical and statistical., Results: Results: All patients who came to us for dental care had previously been replaced with fixed metal structures. Of them, 121 people indicated problems related to the use of metal prostheses. From these patients, a risk group of intolerance to prosthesis materials was formed and a search was made for base materials that would be biologically indifferent. For the manufacture of partial removable prostheses, we chose the basic thermoplastic material Acron, manufactured by Roko (Poland), which has high biocompatibility with the tissues of the prosthetic bed. When choosing the structures of partial removable prostheses in patients with various defects of the dentition, we were guided by a small number of clinical visits, which is relevant for military personnel who are on rotation and treated in the hospital., Conclusion: Conclusions: Our careful selection of structural material made it possible to prevent complications and produce high-quality removable prostheses in a short period of time. An analysis of the reasons for orthopedic care showed low awareness of the servicemen regarding the need for timely orthopedic treatment.

Objectives: The objective of this retrospective clinical study was to investigate the survival rates and complications of implant (I)-retained or tooth-implant (TI)-retained prostheses and abutments (teeth, implants) over a mean observation period of 11.26 years. The study also aimed to analyze the differences and complication rates between implant-retained double crown removable dental prostheses (I-DC-RDPs) versus tooth-implant-retained double crown removable dental prostheses (TI-DC-RDPs)., Material and Methods: We reviewed the clinical data of 110 nonsmokers (mean age = 53.9 years) who received DC-RDPs in maxillary or mandibular arches. 153 teeth and 508 implants were used to restore partially edentulous (PE; TI-DC-RDPs; n = 53) and completely edentulous (CE; TI-DC-RDPs; n = 57) arches. Two designs of the distal extension were used: cantilevers (CANs) and saddles (SADs). Restorations were examined for abutment survival, mechanical, or biological complications., Results: The 10-year survival rates were 99.3% (95% CI: 95.4-99.9%) for teeth and 99.3% (95% CI: 97.5-99.7%) for implants. The cumulative rates of TI- and I-RDPs free of technical complications were 77% and 86%, respectively. The risk of complications was not significantly different between the CAN and SAD subgroups of I-RDPs (p > 0.05). However, for TI-RDPs, technical complication risk was significantly higher in SAD type compared with CAN restorations (p = 0.02)., Conclusions: I- and TI-DC-RDPs seem to be recommendable for restoration of CE or PE arches. The technical and biological complication rates were lower for I-DC-RDPs in the CE arches than for TI-DC-RDPs in the PE arches. Purpose: To provide, in a clinical case-control study, 1-year data on edentulous residual ridge dimensional changes for patients wearing removable partial dentures (RPD) with Polyetheretherketone (PEEK) framework, fabricated with a digital workflow, and a control group of untreated patients., Materials and Methods: Ten patients were treated with PEEK RPD, and six controls were studied. Intraoral scans at baseline (T0) and after a median period of 1 year (T1) were superimposed, trimmed, and reoriented (T0r and T1r), moved to a metrology software, and realigned. A curve (C0) was then traced on T0r, along the residual ridge crest; its projection (C1) on T1r was obtained. The mean distance C0-C1 was the dependent variable of interest and represented the 1-year changes in the height of the edentulous ridge. In addition, mean 3D distance between T0 and T1 at each edentulous area was measured. Differences in these outcomes measured between RPD treated and control groups were statistically assessed., Results: Twenty-six and 14 edentulous areas were investigated in the RPD treated and control groups, respectively. No significant differences were observed for potentially confounding variables, such as median follow-up time (12.5 vs. 13 months, respectively), the alignment accuracy between T0r and T1r (0.01 mm vs. 0 mm, respectively), the median number of remaining teeth (6 vs. 8.5, respectively), and the median length of edentulous areas (25.5 mm vs. 22.8 mm, respectively). For the outcomes of interest, no statistically significant difference was seen in the mean distance between C0 and C1 (i.e., changes in residual ridge height: -0.39 ± 0.52 mm vs. -0.52 ± 0.54 mm, respectively) or in the mean 3D distance at corresponding points of the denture bearing areas (-0.3 ± 0.46 mm vs. -0.4 ± 0.35, respectively)., Conclusions: Although 1 year is a relatively short observation period, this clinical study shows that there are no short-term differences in edentulous residual ridge height and overall dimensions between patients wearing PEEK RPD, fabricated with a digital workflow, and controls without an RPD., (© 2021 by the American College of Prosthodontists.)

Objectives: The purpose of this systematic review and meta-analysis was to compare implant-supported removable partial dentures (ISRPDs) with distal extension removable partial dentures (DERPDs) in terms of patient-reported outcome measures (PROMs: patients' quality of life and satisfaction) and to determine mechanical and biological complications associated with ISRPDs., Material and Methods: An electronic search was performed on four databases to identify studies treating Kennedy class I or II edentulous patients and which compared ISRPDs with DERPDs in terms of PROMS and studies, which evaluated mechanical and biological complications associated ISRPDs. Two authors independently extracted data on quality of life, patient satisfaction, and biomechanical complications from these studies. The risk of bias was assessed for each study, and for PROMs, the authors performed a meta-analysis by using a random-effects model., Results: Thirteen articles were included based on the selection criteria. The difference in mean scores for quality of life (30.5 ± 1.8; 95% confidence interval [CI], 24.9-36.1) and patient satisfaction (-20.8 ± 0.2; 95% CI, -23.7 to -17.8) between treatments with conventional and implant-supported removable dentures was statistically significant (p < .05). Implant-supported removable dentures improved patients' overall quality of life and satisfaction. Some mechanical and biological complications, such as clasp adjustment, abutment or implant loosening, marginal bone resorption, and peri-implant mucositis, were noted in ISRPDs during patient follow-up. Studies assessing PROMs were very heterogeneous (I 2  = 65%, p = .85; I 2  = 75%, p = .88)., Conclusions: ISRPDs significantly improved quality of life and patient satisfaction. Some mechanical and biological complications have been associated with ISRPDs treatment, requiring regular monitoring of patients to avoid the occurrence of these complications., (© 2021 The Authors. The Aim: Of this study was the evaluation of the effect of the antioxidant gel on the oral mucosa and reduction of the time of adaptation to a removable denture in patients with partial adentia., Material and Methods: The study comprised 105 patients aged 30-65 years divided into three groups: the main group of 45 patients, the comparison group of 35 patients, the control group of 25 patients. The main group was given a prophylactic gel with natural astaxanthin after the delivery of a removable prosthesis. The patients of the control group and the comparison group do not received an antioxidant gel. Patients of all three groups underwent a study of the hygienic state of the oral mucosa and oral fluid to assess the state of oral microbiocenosis. The main group and the comparison group underwent visual examination and photoplanimetric control after the placement of a removable denture., Results: A positive preventive and anti-inflammatory effect of astaxanthin gel was found in patients who received an immediate and a partial removable dentures. The use of the gel for 7 days helps to reduce the values of hygiene indices, concentrations of bacterial plasmalogen and endotoxin in the oral fluid in the short-term perspective., Conclusion: Preventive dental gel with astaxanthin decreases the time of adaptation to removable dentures by 1.7-2 times and reduces the risk of prosthetic stomatitis due to its wound-healing, anti-inflammatory and antimicrobial actions.

Purpose: To determine the ideal position of a dental implant to assist a posterior extended partial removable dental prosthesis (PRDP), through stress values, displacement values, and deformation of periodontal ligament (PDL)., Materials and Methods: A finite element analysis of different implant positions was analyzed using a 3D mandible model from a human patient. Test models were created: model A (implant in second molar area), model B (implant in the first molar area), and model C (implant in premolar area). A control model without implant support was also created. Overall displacement values, von Mises stress distribution maps, and nonlinear deformations were evaluated., Results: Some differences could be observed between test models. The introduction of an implant in the edentulous area, unlike a conventional removable partial denture without implant support, decreases stress values in the biological structures such as: mandible, tooth, soft tissue, and PDL. Placing the implant in the first molar area resulted in improved displacement values, and reduced maximum stress values at the peri-implant bone area, metal structure, and implant were observed., Conclusions: Within the limitations of this study we can conclude that placing the implant in the position of the first molar improves biomechanical behavior of implant-assisted PRDPs., (© 2017 by the American College of Prosthodontists.)

This article presents a rare incident of a bilateral maxillary removable partial denture as an airway foreign body. The removable partial denture and the surrounding soft tissue irritation caused partial obstruction of the airway. Impending airway collapse was avoided after successful removal of the foreign body. Removing a removable dental prostheses at night, not only to prevent soft tissue pathology but also to avoid ingestion or aspiration of the prosthesis, must be emphasized., (Copyright © 2018 Editorial Council for the Journal of Prosthetic Dentistry. The purpose of this study was to evaluate whether the use of removable partial dentures affects the periodontal status of abutment teeth. An observational cross-sectional study was done on a sample of patients rehabilitated with removable partial dentures (2010 to 2013). At a recall appointment, a clinical examination was done to collect data related to the rehabilitation and periodontal status of the abutment teeth. Of 145 invited patients, 54 attended the requested follow-up appointment (37.2%). Mean patient age was 59.1 years, and the study population was 42.6% male and 57.4% female. The mean follow-up time for the prosthesis was 26 months. Abutment teeth had higher values in all periodontal variables (P < .001). Occlusal clasps had the worst results in relation to clasps with a gingival approach (P < .005). Significant differences were also found in mandibular abutment teeth of Kennedy Class I and II compared to Class III (P < .048). The periodontal status of the abutment teeth of removable partial dentures is affected by these rehabilitations. A recall program for these patients involving removable prosthodontics and periodontology appointments is mandatory.

Objectives: The aim of the present study was a systematic review and subsequent meta-analysis on biological complications of removable prostheses in the moderately reduced dentition., Materials and Methods: A systematic literature search in established medical databases (MEDLINE, EMBASE, BIOSIS, SciSearch, Cochrane, FIZ Technik Web) and a hand search of relevant dental journals was conducted. The search terms were relevant MeSH terms, free search terms, and combinations of the two. The search included randomized controlled trials, prospective and retrospective studies with data on biological complications of removable dental prostheses in the moderately reduced dentition with at least 15 participants, an observation period of at least 2 years, and a drop-out rate of less than 25%. The selection of relevant publications was carried out at the title, abstract, and full-text level by at least two of the authors involved. The publications included were tabulated and analyzed., Results: Of the original 12,994 matches, 1923 were analyzed by title, 650 by abstract, and 111 according to the full text. A total of 42 publications were ultimately included. The following parameters were evaluated., Tooth Loss: Results varied, depending on the observation period, between 0 and 18.1% for clasp-retained removable dental prostheses (RDPs), between 5.5 and 29% for attachment-retained RDPs, and between 5.5 and 51.7% for double crown-retained RDPs., Caries: Results varied, depending on the observation period, between 0 and 32.7% for clasp-retained RDPs, between 1.8 and 29% for attachment-retained RDPs, and between 1.8 and 16.4% for double crown-retained RDPs., Endodontic Treatment: Results varied, depending on the observation period, between 3.5 and 19.2% for clasp-retained RDPs, between 6.9 and 16.4% for attachment-retained RDPs, and between 0.6 and 13.9% for double crown-retained RDPs., Tooth Fracture: Results varied, depending on the observation period, between 1.7 and 5.3% for clasp-retained RDPs, between 12.7 and 40% for attachment-retained RDPs, and between 0.4 and 4.4% for double crown-retained RDPs., Tooth Mobility: There were no changes or improvements for clasp-retained RDPs. The better the pre-treatment and supportive care is, the smaller the differences are. For double crown-retained RDPs, a slight increase was found in one study. The results for the parameters probing depth and radiological bone loss were inconclusive., Gingival Recession: Gingival recession seemed to be favored by a mandibular sublingual bar. Compared to fixed restorations, removable restorations seemed to be associated with a more pronounced need for dental treatment. Stringent pre-treatment and supportive care reduced the complication rates., Conclusions: Heterogeneous study designs and data analyses rendered a meta-analysis impossible, so that an evaluation at the highest level of evidence could not be performed., Clinical Relevance: Within the limitations of this study, it would be correct to state that removable dental prostheses require intensive maintenance. Suitable pre-treatment and supportive care can lower the complication rates, in the absence of which they constitute trigger factors for (additional) biological complications.

Purpose: To describe the restorative treatment profiles of long-span (> three units), implant-supported dental prostheses (LIDPs) prescribed in 27 private practices in the state of Victoria, Australia, during the period from January 1, 2005, to December 31, 2009. The restorative treatment profiles of these prostheses refer to the framework/veneering materials, number of prosthetic units/supporting implants, location in the oral cavity, retention methods, and cantilever designs. LIDPs refer to implant-supported prostheses of fixed or removable design; namely, fixed partial dentures (IFPDs), fixed complete dentures (IFCDs), removable partial dentures (IRPDs), and complete overdentures (IODs)., Materials and Methods: Private dental practitioners providing implant treatment in Victoria, Australia, were invited to enroll in this study, which was conducted through a dental practice-based research network (the eviDent Foundation). The enrolled practitioners' clinical records of the implant treatments provided during the specified period were accessed for data collection. Descriptive statistics, cross-tabulations, and linear mixed models were conducted for data analyses., Results: During the study period, 627 LIDPs were prescribed to 556 patients by 18 general dentists and 5 prosthodontists. LIDPs were more common in the maxilla than in the mandible except in the case of IODs. Of the fixed prostheses, 60% replaced missing teeth in more than one sextant, 33% in the anterior sextant, and 7% in the posterior sextant. The average number of prosthetic units per implant was higher in mandibular compared to maxillary IFPDs/IFCDs (2.5 vs 1.9, respectively, P < .001). Cantilever pontic(s) were present in 68% of IFCDs (more often in the mandible than in the maxilla) compared to 28% of IFPDs (use of cantilever pontics was about the same in the maxilla and mandible). Screw retention was the most commonly reported retention method in IFPDs/IFCDs (95%). Locator attachment was used for the majority of IRPDs/IODs (83%)., Conclusion: The clinicians enrolled in this study preferred fixed over removable LIDPs for partially dentate and maxillary edentulous cases. For mandibular edentulous patients, IODs were prescribed more often than IFCDs. A retrievable retention design was favored for IFPDs/IFCDs. IFPDs were predominantly metal-ceramic, and metal-acrylic designs were more popular for IFCDs.

Purpose: To investigate the surface microstructural changes and the release of ions from metal alloys used in removable dental prostheses and the potential effects of acidic reflux found in patients suffering from gastroesophageal reflux disease (GERD)., Materials and Methods: Thirty-seven (37) patients were recruited. Data were gathered through a questionnaire and clinical examination. Samples of metal alloy from the dentures and patient's saliva were collected. GERD was confirmed using the GerdQ questionnaire. Denture samples were characterized using scanning electron microscopy (SEM) and energy dispersive spectroscopy (EDS), while salivary samples were tested for trace metal ions using inductively coupled plasma (ICP)., Results: Characterization of denture samples revealed the presence of nickel, cobalt, and chromium. Nickel-chromium exhibited an etched surface appearance, while cobalt-chromium exhibited no noticeable surface microstructural changes. Higher mean salivary levels of chromium and cobalt in patients wearing any metal alloy-based denture and of chromium and nickel in patients wearing Ni-Cr prostheses were found to be significant. No differences were found in salivary metal ion levels of patients suffering from GERD., Conclusions: Nickel-chromium alloy is prone to acid etching in the oral cavity, while cobalt-chromium alloy appears to be more resistant. Cobalt, chromium, and nickel are leached in saliva of patients using cast removable prostheses. The impact of gastric acid on metal ion release from dental metal alloys deserves further investigations., Clinical Significance: This preliminary study suggests that metal-based removable prostheses leach trace metal ions in saliva. Sjögren's syndrome (SS), an autoimmune disorder, affects the exocrine glands, including the lacrimal and salivary glands. It is characterized by symptoms of dry eye and dry mouth. As secretion of saliva decreases, patients with SS experience rampant caries, pain in the oral mucosa, inflammation and hardening of the salivary glands, abnormal taste, dysphagia, and loss of teeth earlier than healthy individuals. A removable partial denture is often used as a prosthesis after tooth loss. Compromised salivary lubrication, however, can produce traumatic ulceration of the mucosa, making use of a removable prosthesis in SS patients painful. In such cases, a dental implant is likely to be requested as an alternative. This report describes dental implant treatment in an SS patient, a 50-year-old woman who presented with the chief complaint of masticatory dysfunction and pain due to a removable partial denture. Eight implants were placed in the maxillary and mandibular first molar tooth and second molar tooth regions. Following a 4-month non-loading period, second-stage surgery and provisional restoration with a screwretained implant temporary crown were performed. Screw-retained superstructures were fitted by means of a customized titanium abutment and zirconia crown as the final restoration. No complications, including inflammation of peri-implant soft tissue or resorption of peri-implant bone, were observed at 3 years following placement of the superstructures.

Background/aims: Speech impairment during the initial phase of removable partial denture (RPD) treatment can prevent patient adaptation to RPDs. This study was undertaken to investigate the influence of oral morphology on speech production in subjects wearing RPDs with major connectors., Methods: Two types of connectors were fabricated for 17 subjects with normal dentitions: covering the middle palate (M-bar) and the anterior/posterior palate (AP-bar). Four target sounds ([∫i], [t∫i], [çi], and [ki]) were evaluated under 3 recording conditions: no connector, M-bar, and AP-bar. The mean appearance ratios of correct labels (MARCs) were calculated as parameters representing speech production accuracy with the speech evaluation system. Subgroup analysis was conducted based on palate height, dental arch width, and front space volume of the oral cavity., Results: Based on the multiple linear regression test, a significant association was found between the MARCs of [∫i] with M-bar and front space (p = 0.036). In the subgroup analysis, the AP-bar had a significant effect on the MARCs of [∫i] among subjects with high palate (p = 0.026), narrow arch (p = 0.004), and small front space (p = 0.014)., Conclusion: RPDs with major connectors could disturb speech production among patients with high palates, narrow arches, and small front spaces., (© 2018 S. Purpose: To evaluate the periodontal conditions and integrity of abutment and non-abutment teeth of patients evaluated 7 years after insertion of the removable partial denture (RPD)., Materials and Methods: Twenty-two patients (17 women, 5 men) were assessed at the moment of denture insertion and 7 years later. The following items were verified in each assessment: bleeding on probing (BP), probing depth (PD), gingival recession (GR), and mobility (M), comparing direct and indirect abutment teeth, and the teeth not involved in the denture design. Tooth integrity was also evaluated and classified as intact when no caries or fractures were observed. The Kruskal-Wallis test was used to reveal statistical significance between the groups (p = 0.05) as well as the Bonferrroni-corrected Mann-Whitney test for post hoc comparison. The Wilcoxon test was used for evaluation within the group over time. Fisher's exact test was applied to cross data about abutment integrity., Results: Statistically significant differences were found for GR (baseline, p < 0.001; 7 years, p < 0.001) and PD (baseline, p = 0.001; 7 years = 0.004) between the three groups at baseline and after 7 years of follow-up. Mean BP and M values increased from initial assessment to after 7 years of RPD use in every group, but no statistically significant difference was found between the groups. For abutment integrity, a statistically significant difference (p = 0.028) was observed, and the direct abutment exhibited more (33.3%) caries and fractures., Conclusion: RPDs generated more periodontal damage to direct abutments, since higher gingival recession probing depth indexes, and presence of caries and fractures were observed in comparison to indirect abutments and non-abutments., (© 2016 by the American College of Prosthodontists.)

Objectives: The purpose was to assess the early effects of removable partial denture (RPD) placement on abutment teeth in patients with periodontal disease., Methods: Fifteen distal-extension RPDs and 19 abutment teeth were evaluated in 13 patients with moderate-to-severe periodontal diseases. Clinical periodontal records and grey level (GL) score based on digital subtraction radiography were measured on the day of denture placement (baseline) and at 1day, 1 week, 1 month, 3 months, and 6 months after the placement. Occlusal force on total dentition (Ft) and abutment tooth (Fa) were recorded with (baseline) and without denture on the day of the placement, and with denture in the follow-up examinations., Results: No statistically significant difference between a follow-up period and the baseline was found in the clinical periodontal and occlusal force records, except for Ft with denture on the day of the placement, which was significantly lower than the baseline (p<0.05). The GL at distal crestal of the abutment was significantly higher at 1 week after the placement than at the baseline (p<0.05), while the scores of following examinations returned to the baseline level., Conclusions: The distal crestal bone of the abutment tooth transitorily reacts to mechanical stress due to the denture placement, while this does not suggest a detrimental effect within the limitation of the study design with a short-term observation period., Clinical Significance: The RPDs can be safely used for patients with periodontal diseases for at least 6 months if they are properly placed and maintained by professional care., (Copyright © 2017 Elsevier Ltd. Purpose: Colonization by Candida is related to wearing a denture. How denture type, status, and maintenance play a role in determining Candida colonization remains unknown. This work evaluated removable denture status in an elderly population and explored the association between denture-related factors and oral Candida colonization as determined by systemic and local factors., Materials and Methods: Socio-demographic, general health, and behavioral data and oral factors were registered by means of a questionnaire. Oral, dental, and denture-related factors were assessed by clinical examination, and yeasts were isolated from oral and denture specimens. Multiple logistic regression analysis was used to explore the relative relevance of risk factors., Results: Overall, 52.4% of subjects used complete dentures in both lower and upper arches, and 90% of the prostheses were in good condition. Denture plaque index was high in most cases, and 75% of subjects had yeasts in their oral cavities. Oral Candida colonization was significantly associated with low saliva pH, sugar consumption, and the fault of a denture component; however, only sugar consumption was found to be an independent factor related with oral Candida colonization, showing an odds ratio of 3.04 in a multiple logistic regression model., Conclusions: Elderly people in this study used a complete denture in good condition, with a median age of 15 years. Various pressure-indicating media are available to assess the adaptation of the intaglio surface of a removable dental prosthesis at the insertion and follow-up appointments. This clinical report describes the use of an elastomer that entered the maxillary sinus through an undetected oroantral communication at the 24-hour follow-up for an immediate maxillary complete removable dental prosthesis. Aim: The purpose of this intervention was to treat the patient, a 51-year-old woman, who was wearing a maxillary denture and a mandibular bilateral distal-extension partial denture (both unfit) and was suffering from temporomandibular disorder (TMD)., Methodology: The treatment suggested was to construct a superior repositioning splint (SRS), and after remission of the symptomatology, fabricate new dentures., Results: The occlusal splint was effective and the symptomatology ceased after about five months of treatment. The new dental prostheses were made and followed up for 10 years with no return of the pain., Conclusions: SRS and subsequent construction of new dentures after remission of the symptomatology was an effective treatment for TMD in the case described.

Purpose: To evaluate the influence of time on masticatory efficiency and muscular activity during mastication of different types of test foods in participants fitted with a lower bilateral free-end saddle removable partial denture (RPD) with wide or narrow teeth., Methods: Thirty-six participants consented to undergo evaluation of their masticatory efficiency and recording of their muscular activity, while they wore an RPD with a unified bilateral design, during mastication of different types of test foods. The width of the artificial teeth was changed as follows. Initially (A), the length was adjusted so that the distal end of the mandibular second molars occluded with the distal end of the maxillary second molar (buccolingual width of 7-8 mm). Next (B), the width was decreased to half the maxillary second molars (buccolingual width of only 5 mm). A statistical analysis was performed using SAS software. One-way analysis of variance (ANOVA) was performed followed by Duncan's multiple range tests., Results: At the 2-month follow-up visit, wider teeth demonstrated greater masticatory efficiency than narrower teeth. However, after 4 months, no significant differences were observed between the two tooth widths. No significant differences in muscular activity were observed., Conclusion: A reduction of the occlusal table width in free-end saddle RPDs resulted in decreased masticatory efficiency 2 months after denture fabrication, but there was no significant difference in muscular activity between narrow and wide teeth. Statement of Problem: The loss of posterior teeth has a negative effect on the masticatory efficiency and activity of mastication muscles. How these muscles are affected by partial removable dental prostheses (PRDPs) with different cusp angles is unknown., Purpose: The purpose of this clinical study was to evaluate the influence of different cusp angles, with anatomic (33 degrees), semianatomic (20 degrees), and nonanatomic teeth (0 degree), on the masticatory efficiency and muscular activity of participants receiving mandibular bilateral distal PRDPs., Material and Methods: Fifteen participants consented to the evaluation of masticatory efficiency and electromyographic (EMG) activity with a PRDP with a unified bilateral design. The cusp angles of the artificial teeth were randomly changed from anatomic (33 degrees), semianatomic (20 degrees), and nonanatomic teeth (0 degree). Masticatory efficiency was evaluated by recording 5 parameters, including number of strokes, and time of mastication while the EMG activities of the masseter and temporalis muscles, measured using surface electromyography. The data were analyzed with 1-way ANOVA and the Tukey multiple post hoc test with statistical software (α=.05)., Results: With regard to masticatory efficiency, PRDPs with anatomic and semianatomic teeth exhibited higher masticatory efficiency than those with nonanatomic teeth, whereas no significant differences were found between PRDPs with anatomic and semianatomic teeth. With respect to EMG activity, the least EMG activity was observed in PRDPs with anatomic teeth, whereas the highest activity was observed with nonanatomic teeth. No significant differences were found between the effect of PRDPs with anatomic and semianatomic teeth on masseter activity., Conclusions: Changing the cusp angle from 33 to 20 degrees in PRDPs did not influence the masticatory efficiency or the EMG activity of the masseters. However, it did lead to increased EMG activities of the temporalis muscle. This study was performed to make comparative analysis of the clinical findings between the two different types of the implant-assisted removable partial dentures: removable partial dentures using implant surveyed bridge as an abutment (ISBRPD) and overdenture type of removable partial denture using implant attachment (IARPD). Implant cumulative survival rate, marginal bone resorption, probing depth, peri-implant inflammation, bleeding, plaque, calculus, and complications were evaluated on 24 patients who were treated with implants in conjunction with removable partial denture and have used them for at least 1 year (ISCRPD: n = 12; IARPD: n = 12). There was no failed implant and all implants were functioning without clinical mobility. Marginal bone loss of ISCRPD (1.44 ± 0.57 mm) was significantly lower than that of IARPD ( p < 0.05). There was no significant difference in probing depth, peri-implant inflammation, bleeding, and plaque between the two groups ( p > 0.05), while the calculus was significantly more observed in ISCRPD group than in IARPD group ( p < 0.05). The retention loss of IARPD was the most common complication. Within the limits of the present study, it was found that well-planned ISBRPD was clinically appropriate. Longitudinal and systematic clinical studies are necessary to confirm these results.

A foreign body impacted in the esophagus is not a rare incident among adults or children. In adults, a dental prosthesis is prone to become impacted in the esophagus. The diagnostic difficulty of this often causes a delay in its removal, which can lead to serious complications, including death. This report describes the autopsy case of a man who died of prolonged asphyxiation induced by the delayed removal of an impacted denture, which was misdiagnosed on his first visit notwithstanding that a part of the denture could be seen on X-rays. The aim of this study was to investigate factors associated with mucosal pain in patients with partial removable dental prostheses (PRDPs). In this hospital-based cross-sectional study, 333 patients wearing 500 PRDPs (mean age 71·4 years, men 33·3%) were consecutively recruited from prosthetic clinic of a dental hospital in Japan. Subjects rated pain intensity and frequency of denture-bearing mucosa. An examiner recorded age, gender and systemic diseases as well as dental, mucosa, denture, sensory, behavioural- and psychological-related characteristics that were possibly associated with the mucosal pain. Multivariate analyses were performed to analyse factors related to mucosal pain. Pain intensity was rated as more than score 0 (presence) in 34·2% (171/500) PRDPs, and pain was experienced after denture delivery in 42·8% (214/500) PRDPs. Logistic regression analyses showed that younger age, mucosal damage, poor mucosal condition, bone prominence, poor residual ridge, higher pain sensitivity, presence of awake bruxism, perception of oral dryness, interim denture wear and high number of missing teeth were significant independent predictors for the presence of the mucosal pain intensity and/or frequency (P < 0·05). Multiple factors are associated with mucosal pain in patients with PRDPs. Background: In studies comparing different prosthetic treatment concepts the repeated loss of teeth was chosen as the primary outcome. The resulting data appear to represent a data structure of recurrent events. However, the application of an existing method for recurrent events is far from straightforward. Often only the first event or the final state is analyzed using Kaplan-Meier survival statistics, thereby giving a great deal of information away., Methods: The paper presents a strategy for the analysis of recurrent data using a previously published study on the influence of different prosthetic treatment concepts for the shortened dental arch on tooth loss. A method based on cumulative sample history functions of recurrent events was adjusted for tooth loss. The shapes of these cumulative functions suggest a time dependency of the recurrence rate. To keep the model as simple as possible, a tripartite Poisson process (which assumes piecewise time-independent rates) was fitted to the cumulative mean functions stratified by treatment., Results: Within the middle interval of the three-phasic process, the treatment effects differ significantly, which is interpreted as a delay of tooth loss due to the use of one type of prosthesis (fixed) compared with the other (removable)., Conclusions: An analysis based on cumulative history functions is based on process, therefore, temporally changing characteristics are better captured than in methods for survival analyses. The presented approach offers useful new insight into the temporal behavior of ongoing tooth loss after prosthetic treatment., Trial Registration: The trial has been registered at controlled-trials.com under ISRCTN97265367 (registration date 4 April 2008).

18F-fluoride positron emission tomography (PET) can identify subtle functional variation prior to the major structural change detectable by X-ray. This study aims to investigate the mechanobiological bone reaction around the abutment tooth and in the residual ridge, induced by insertion of removable partial denture (RPD) within two different groups of patients: patients without denture experience (Group 1) and patients with denture experience before (Group 2), using 18F-fluoride PET imaging technique. 18F-fluoride PET/computerised tomography (CT) scan was performed to examine the bone metabolic change in mandible before and after the RPD treatment. Region of interests (ROIs) were placed in alveolar bone around abutment tooth and in residual bone beneath the RPD. Standardised uptake value (SUV), reflecting the accumulation of 18F-fluoride, was measured for each ROI. In all subjects of Group 1, SUVs after insertion were higher than before in both alveolar bone and residual bone, while there was less significant change in SUV in subjects of Group 2. This study demonstrated using longitudinal 18F-fluoride PET scans to effectively examine the bone metabolic change in mandible induced by occlusal loading after RPD insertion. Using this technique, within the six subjects in this study, it was shown that bone metabolism around abutment tooth and residual ridge increased after RPD insertion in case of first-time denture user, while there was no big change in the patient with experience of denture before. Oral mucosa reaction of prosthetic bed to the removable acrylic dentures was evaluated in 43 patients (12 male and 31 female) aged 56-69 years with partial and full teeth loss in one or both jaws. Patients of the first (control) group (17 patients) were not using additional tools improving fixation of the removable dentures during adaptation period, while patients of the second (main) group (26 patients) used Corega cream for dentures fixation for 30 days follow-up. Oral mucosa assessment was carried out on 3-4 and 28-30 day of dentures use by 3 end points: pain syndrome, moisture level, inflammation of a prosthetic bed. The results proved Corega cream to improve prosthetic bed mucosa condition reducing inflammatory response to polymeric materials of removable dentures basis.

Purpose: Many studies have identified risk factors for dental implant failure, although few have investigated the correlation among implant fixtures within single patients. A better analytical method may include repeated measures analysis including generalized estimating equations (GEE). This retrospective cohort study aimed to (1) identify the risk factors for failure of dental implantation and (2) evaluate an analytical method using GEE analysis., Methods: We analyzed data on early and late implant failures in 296 patients providing 721 rough surface dental implants (2.44 implants per patient). Potential predictors of implant failure included age, gender, smoking, location of implant, use of bone augmentation, number of remaining teeth, opposing tooth condition, fixture length, fixture diameter and type of suprastructure (fixed or removable partial denture). The likelihood of early and late implant failure was estimated by GEE., Results: The early failure rate was 1.5% (11/721 implants, 7/296 patients) and the 10-year cumulative survival rate was 94.0% (7/710 implants, 5/293 patients). The GEE analysis revealed that a significant risk factor for early implant failure was smoking (p<0.01), whereas significant risk factors for late failure were maxillary implant (p=0.02), posterior implant (p<0.01), number of remaining teeth (≥20) (p<0.01), opposing unit being a removable partial denture or nothing (p=0.04) and having a removable type suprastructure (p<0.01)., Conclusions: GEE analysis showed that smoking was a risk factor for early implant failure, and several risk factors were identified for late implant failure., (Copyright © 2015 Japan Prosthodontic Society. Ingested foreign bodies can prove a significant and potentially fatal threat. A case is reviewed where a patient swallowed his upper Kennedy Class I removable partial denture, highlighting the potential problems of this prosthesis design and the management of this scenario.

Implant-supported removable prostheses improve patients' satisfaction with treatment and quality of life. Improvements in the implant's surface and in attachment elements have made this treatment method very successful. However, some biological and mechanical complications remain. Mechanical complications associated with implant-supported overdentures and implant-supported removable partial dentures are loss of retention of attachment systems, the need to replace retention elements and to reline or repair the resin portion of the denture, and implant fracture. Objectives: To assess patient-perceived process-related quality of care in patients undergoing different prosthodontic treatments, and to investigate whether perceived burdens are related to specific procedures during prosthodontic treatments, i.e., to specific treatment components., Methods: In this non-randomised prospective intervention study, 104 dental patients (mean age: 52.2±13.9 years, 50% female) were included. Burdens during treatments with fixed dental prostheses (FDP, n=90), removable dental prostheses (RDP, n=10), and complete dentures (CD; n=4) were assessed with the Burdens in Prosthetic Dentistry Questionnaire (BiPD-Q). The BiPD-Q total score was calculated as mean of all 25 items, and subscale scores were derived from items corresponding to six specific treatment components (anesthesia, preparation, impression, provisional care/try-in, fixation, global perception), with values ranging from 0 (best) to 100 (worst). Subscale scores were compared by using repeated-measures ANOVA, paired t-tests, and effect sizes, to identify treatment components with highest burdens., Results: Mean BiPD-Q total score was 22.1, ranging from 8.6 for patients with CD to 24.0 for FDP patients. Perceived burdens differed significantly between treatment components, with highest burdens for anesthesia (score: 34.0), followed by preparation (score: 26.8) and impression (score: 24.7). When subscale score for anesthesia was compared to scores for other treatment components, effect sizes were medium to large., Conclusion: While prosthodontic treatment is overall perceived as only moderately burdening, specific treatment components differ significantly in perceived burdens., Clinical Significance: Anesthesia, preparation, and impression are treatment components with most unpleasant perceptions, offering the highest potential for improvements for patient-perceived process-related quality of care in prosthetic dentistry., (Copyright © 2014 Elsevier Ltd. Some dental patients use the orofacial region to play wind instruments; however, musical performance has not been objectively evaluated following prosthodontic treatment in such patients. The purpose of this report was to describe prosthodontic treatment for a clarinet player using sound analysis. The patient required a removable partial denture for his maxillary anterior teeth. Sound analysis was performed before and after denture adjustment, and the patient completed a questionnaire regarding his perceptions while playing his clarinet. In a multicentre randomised trial (German Research Association, grants DFG WA 831/2-1 to 2-6, WO 677/2-1.1 to 2-2.1.; controlled-trials.com ISRCTN97265367), patients with complete molar loss in one jaw received either a partial removable dental prosthesis (PRDP) with precision attachments or treatment according to the SDA concept aiming at pre-molar occlusion. The objective of this current analysis was to evaluate the influence of different treatments on periodontal health. Linear mixed regression models were fitted to quantify the differences between the treatment groups. The assessment at 5 years encompassed 59 patients (PRDP group) and 46 patients (SDA group). For the distal measuring sites of the posterior-most teeth of the study jaw, significant differences were found for the plaque index according to Silness and Löe, vertical clinical attachment loss (CAL-V), probing pocket depth (PPD) and bleeding on probing. These differences were small and showed a slightly more unfavourable course in the PRDP group. With CAL-V and PPD, significant differences were also found for the study jaw as a whole. For CAL-V, the estimated group differences over 5 years amounted to 0.27 mm (95% CI 0.05; 0.48; P = 0.016) for the study jaw and 0.25 mm (95% CI 0.05; 0.45; P = 0.014) for the distal sites of the posterior-most teeth. The respective values for PPD were 0.22 mm (95% CI 0.03; 0.41; P = 0.023) and 0.32 mm (95% CI 0.13; 0.5; P = 0.001). Background: The objective of this study was to assess the influence of lingual bar and lingual plate major connectors on plaque retention and gingival health among patients who presented in University College Hospital (UCH), Ibadan, Nigeria., Methods: This comparative intervention study was carried out among fifteen patients aged 28 to 60 years with Kennedy class III lower edentulous arch. Two metal dentures with different major connector designs (lingual plate and lingual bar) were fabricated for each patient. After professional scaling and polishing, a baseline score of the oral hygiene was done using the plaque index of Sillness and Loe and the gingival health using the gingival index of Loe and Sillness. Each patient was randomly assigned either a denture with the plate or bar design to use for three months and then recalled for reassessment of oral hygiene and gingival health. Professional scaling and polishing was done post removal and a new baseline assessment of oral hygiene and gingival health recorded. The dentures were now exchanged and the second major connector design inserted. Each patient was recalled for reassessment of the oral hygiene and gingival health after three months. The level of plaque accumulation and gingival inflammation with the use of lingual plate and bar dentures were determined and compared., Results: There was no statistically significant difference in the mean plaque index score for the lingual bar design at three months and the mean plaque index score at three months for the lingual plate (p > 0.05). However, a statistically significant lower mean gingival index score was noted three months post insertion for lingual bar designs when compared to plate designs., Conclusion: Within the limitation of this study, better gingival health was noted with the bar designs when compared with the plate designs.

Partial or complete edentulism impairs mastication. However, it is unclear how the chewing cycle is affected by prosthetics. We evaluated the chewing movements of patients fitted with complete (CD) or removable partial denture (RPD). A total of 29 subjects were kinesiographically evaluated during chewing of peanuts and Optocal portions in a random sequence. The subjects were divided into two groups according to prosthesis type. Group RPD was composed of 14 partially edentulous patients using a lower distal extension RPD (mean age 61 ± 8 years), and group CD contained 15 completely edentulous patients using CD (mean age 65·9 ± 7·9 years) in both jaws. Opening, closing, occlusal and masticatory cycle times, movement angle (opening and closing), maximum velocity (opening and closing), total area and chewing cycle amplitudes were evaluated. The results were subjected to anova and Tukey's HSD test at a significance level of 5%. The RPD group exhibited shorter opening and closing phases and masticatory cycle time (P < 0·05). Maximum velocities were also higher in the RPD group, irrespective of the test material (P < 0·05). The area and amplitude of the chewing envelope was smaller in the CD group (P < 0·0001). The test material did not influence chewing cycles in any of the parameters evaluated (P > 0·05). Background: Despite being functional and having aesthetic benefits, the acceptance of patients regarding the use of removable partial dentures (RPDs) has been low. In part, this is due to the deleterious effects that causes discomfort to the patient. Success depends not only on the care expended by the patient, including daily care and oral hygiene, but also on common goals set by their professional and clinical staff, aiming beyond aesthetics, to incorporate issues of functionality and the well-being of patients., Methods and Results: For rehabilitation treatment with RPDs to reach the desired level of success without damaging the support structure, all the steps (diagnose, cavity preparation, adaptation of the metal structures, functional of distal extension and posterior follow-up) in the rehabilitative treatment should be carefully developed. A literature review was carried out, searching through MEDLINE (PubMed) articles published between 1965 and December 2012 including clinical trials and reviews about the use of RPDs., Conclusions: This study describes factors that lead to failures and complications in oral rehabilitation through the use of RPDs and suggests possible solutions.

The aim of this study was to isolate, quantify, identify, and compare opportunistic microorganisms (Candida and Staphylococcus genera and Enterobacteriaceae/Pseudomonadaceae families) from prosthesis-fitting surfaces, the hard palate, and mouth rinses of individuals wearing removable maxillary prosthesis with (50) and without (50) lesions of denture stomatitis (DS). The strains were collected and identified using phenotypic, biochemical and molecular tests. The counts of microorganisms were significantly higher in the group of individuals with DS (P < 0.05). C. albicans was the most frequently isolated yeast species in both groups, following by C. tropicalis and C. glabrata. Six isolates were identified as C. dubliniensis. S. aureus and S. epidermidis were the most frequent Staphylococcus species in both groups. Klebsiella pneumoniae was the predominant species in both groups. The association between Candida spp. and bacteria isolated in this study with DS suggests that these microorganisms may play important roles in the establishment and persistence of this disease., (Copyright © 2013 Elsevier Inc. Purpose: This study aimed to compare the survival rates of remaining teeth between implant-supported fixed dentures (IFDs) and removable partial dentures (RPDs) in patients with large edentulous cases. The second goal was to assess the risk factors for remaining tooth loss., Materials and Methods: The study subjects were selected among those who received prosthodontic treatment at Okayama University Dental Hospital for their edentulous space exceeding at least four continuous missing teeth. Twenty-one patients were included in the IFD group and 82 patients were included in the RPD group. Survival rates of remaining teeth were calculated in three subcategories: (1) whole remaining teeth, (2) adjacent teeth to intended edentulous space, and (3) opposing teeth to intended edentulous space., Results: The ten-year cumulative survival rate of the whole remaining teeth was significantly higher in the IFD group (40.0%) than in the RPD group (24.4%). On the other hand, there was no significant difference between two groups in the survival rate of teeth adjacent or opposing to intended edentulous space. A Cox proportional hazard analysis revealed that RPD restoration and gender (male) were the significant risk factors for remaining tooth loss (whole remaining teeth)., Conclusions: These results suggest that IFD treatment can reduce the incidence of remaining tooth loss in large edentulous cases., (Copyright © 2013 Japan Prosthodontic Society. Purpose: To compare the frequency of denture stomatitis (DS) under maxillary complete dentures (CDs) in patients with opposing mandibular distal extension removable partial dentures (RPDs) and CDs., Materials and Methods: Participants included 365 maxillary CD wearers (241 women, mean age 70.5 ± 13.2 years; 124 men, mean age 71.5 ± 10.4 years) from 7 rest homes in Istanbul. A total of 268 had mandibular CDs; 97 had mandibular distal extension RPDs. Two independent, calibrated examiners performed oral examinations. Presence of maxillary denture-related stomatitis and the effect of risk factors on DS were evaluated and recorded., Results: The frequency of palatal DS (Newton I-III) was 45.1% (n = 121) in the mandibular CD group and 49.5% (n = 48) in the mandibular distal extension RPD group, a statistically insignificant difference (p= 0.4). Factors significantly associated with palatal DS were maxillary denture age (p= 0.02), reduced occlusal vertical dimension (p= 0.04), and nocturnal denture wear (p= 0.03)., Conclusion: In this study, DS beneath maxillary CDs did not differ between mandibular distal extension RPD and CD wearers. Purpose: The aim of this study was to compare vertical and horizontal mandibular alveolar bone resorption by measuring bone morphological variation in Kennedy Class II removable partial denture (RPD) wearers and non-wearers using cone-beam computed tomography (CBCT)., Materials and Methods: In total, 124 sites in the CBCT scans of 62 (29 RPD non-wearers, 33 RPD wearers) Kennedy Class II patients were analyzed retrospectively. Three-dimensional representations of the mandible with superimposed cross-sectional slices were developed with the CBCT scans to evaluate the mandibular alveolar height and width by measuring distances between the mandibular canal, mylohyoid ridge, alveolar crest, and lower border of the mandible in four regions (eight sites) of Kennedy Class II non-wearers and wearers of RPDs., Results: Mandibular alveolar bone height and width were significantly lower in edentulous sites when compared with dentate sites in both Kennedy Class II non-wearers and wearers of RPDs (p < 0.05). During the planning of oral rehabilitation, the vertical dimension of occlusion (VDO) is one of the first parameters to be measured since its improper restoration can lead to the failure of any prosthetic rehabilitation. A decreased VDO can lead to the appearance of lesions, such as angular cheilitis, facial disharmony, and temporomandibular disorders; meanwhile, an increased VDO may lead to the onset of joint and muscle pain, tension in functional speech, difficulty in swallowing, impaired chewing, tooth sensitivity due to traumatogenic forces, pathologic bone resorption, abnormal wearing of teeth, the appearance of an elongated face, and a facial expression of fatigue. Most scientific articles deal with methods and techniques for re-establishing VDO in edentulous patients or those who have lost their tooth reference due to prosthetic preparations. However, patients with increased VDO are also found in everyday practice. One treatment option for these patients is occlusal adjustment by selective tooth wear; it is still possible to perform orthodontic intrusion and/or orthognathic surgery in severe cases. The aim of this study was to discuss signs, symptoms, diagnosis, and treatment, and to report a clinical case of a patient with increased VDO.

The aim of this article is to investigate patient satisfaction, survival rate of implants, and prosthetic complications or maintenance for rehabilitation with removable partial dentures associated with implants in mandibular Kennedy class I and II cases. A systematic literature review was conducted by three independent reviewers including articles published from January 1981 through September 2011. Medline and Cochrane Library electronic databases were used in addition to hand searching to assess clinical outcomes for mandibular implant-supported removable partial denture with distal extension. This review yielded 1751 records that were narrowed down to 5. The studies revealed implant survival rates ranging from 95% to 100% with one failure reported of 98 implants. The removable partial dentures associated with implant in mandibular free-end arches showed some complications and need of repair for relining, pitting of the healing abutment, replacement of resilient component of the attachment, damage in framework, screw loosening and damage in acrylic denture base. Patient satisfaction was evaluated through a five-point questionnaire, and results ranged between 4.12 and 5.0, considering 1 as the least favourable situation. The literature review showed increase in patient satisfaction and high survival rates of implants associated with mandibular removable partial dentures with distal extensions. However, some complications and need of prosthetic repair were reported. Although this treatment approach could represent a low-cost and beneficial rehabilitation for free-end mandibular ridges, the lack of controlled and randomised well-designed clinical trials suggests further studies with more representative samples to validate the outcomes of this treatment modality., (© 2012 Blackwell Publishing Ltd.)

An 81-years-old man developed a severe osteoradionecrosis of the mandible following surgery and radiotherapy in the head and neck area as a result of a carcinoma at the front ofthe floor of the mouth. Presumably, the osteoradionecrosis had been developed by an injury of the oral mucosa due to a removable partial denture. There are no uniform guidelines for the treatment of osteoradionecrosis of the jaw bones. Treatment modalities vary from removal of sequesters, sometimes long-term use of antibiotics, treatment with hyperbaric oxygen and wide surgical resection with or without reconstruction of the jaw.
