SUBCUTANEOUS emphysema , *HIDRADENITIS suppurativa , *LIQUID nitrogen , *KELOIDS , *COLD therapy - Abstract
Cryotherapy is a freeze-thaw treatment using liquid nitrogen on the skin, with side effects such as subcutaneous emphysema. The aim of this study was to report a case of subcutaneous emphysema associated with intralesional cryotherapy of a keloid on the thorax. Additionally, we performed a systematic search of cases in Embase, PubMed and BIREME using the keywords "cryotherapy" and "subcutaneous emphysema". In the literature, there are cases of post-cryotherapy subcutaneous emphysema for oncologic indications (6/11), keloids (3/11), and fistulous tracts in hidradenitis suppurativa (HS) (2/11); in the head (4/11), thorax (2/11), neck and thorax (2/11) and upper limb (2/11). At least one risk factor explaining post-cryotherapy subcutaneous emphysema is described, such as ulcers, curettage, previous infiltrations, atrophic skin due to sun damage or use of corticosteroids, and insufflation of fistulous tracts.

- Abstract
The use of optical imaging technology in recent years has improved diagnostics in the dermatology area. The techniques that can be named are optical coherence tomography, Raman spectroscopy, and optoacoustic imaging. Each technique has applications in general medicine and dermatology but with their respective limitations and focus. This work reviews the imaging techniques in general, prioritizing the theoretical background that allows the technique to operate, and then an image example obtained with the technique is shown. Then, an extensive review of the techniques of hyperspectral imaging and hyperspectral polarized dermatoscope is made. Finally, we talk about new trends in optical imaging future. - Abstract
BACKGROUND: Trichotillomania is defined by the DSM-5 as a disorder in which the patient pulls out their hair on a consistent basis, whether it be hair from the scalp, eyebrows, or any other region of their body. It causes significant functional impairment. This disorder is considered among the compulsive obsessive disorders. The areas most frequently affected are scalp, eyebrows, eye lashes and pubic hair. Lifetime prevalence is estimated between 0.6-1%. CLINICAL CASE: A 51-year-old female patient, who had presented symptoms of anxiety for the past 11 years. In the past 4 years symptoms had worsen progressively with compulsive night hair pulling which intensified after social stressors. At external inspection at the frontoparietal area of the scalp we found a localized alopecia with an irregular circumference of approximately 10 cm in diameter, so secondary neuromechanical alopecia was diagnosed. CONCLUSIONS: In several psychopathologies, hair pulling can be a symptom, so a psychiatric consult is very important to evaluate comorbidities and classify the symptom of hair pulling within the clinical picture. Like other dermatoses of psychic origin, the diagnosis of trichotillomania does not imply that these patients should be treated only by the psychiatrist. The Dermatology service plays an important role in hair pulling disorder. [ABSTRACT FROM AUTHOR]

This study aimed to report a case of atopic dermatitis in a dog with exclusive manifestation in the periocular region and its therapeutic management. A male dog, crossbreed, 3 years old, was evaluated with a history of pruritic dermatological problem with a time of evolution of two years. The animal presented ulceration, hyperpigmentation, lichenification, purulent discharge, and fissures in the periocular region, in 360°, bilaterally. Ectoparasites and food allergies had already been excluded. A skin biopsy followed by histopathological analysis was performed. The microscopic evaluation suggested allergic dermatopathy. Thus, the findings of physical and histopathologic exams conducted to the diagnosis of canine atopic dermatitis (CAD). Oclacitinib therapy was instituted with an initial dose of 0.4 mg/kg every 12 hours for 14 days followed by a maintenance dose of 0.4 mg/kg every 24 hours. In addition, a topical product based on fatty acids and ceramides was used weekly, continuously. After 30 days, regression of skin lesions and ocular signs was noted, as well as complete remission of pruritus and absence of side effects. After 240 days of treatment, the pruritus relapsed, requiring a change in the frequency of administration to 0.4 mg/kg, every 12 hours, with remission of clinical signs occurring. Therefore, it can be concluded that cases of chronic lesions and intense pruritus in the periocular region should be evaluated and is important to consider the possibility of atopic dermatitis. The use of drugs, such as oclacitinib, for the control of CAD has shown satisfactory results. [ABSTRACT FROM AUTHOR]

- Abstract
OBJECTIVE: To determine if values of serum ferritin level ≤ 70 ng/mL represents a factor associated to the risk for diffuse alopecia in female patients among 12 and 50 years old. MATERIALS AND METHODS: An observational, retrospective, analytical, case-control study was carried out in patients treated at the dermatology service of the Hospital Belen de Trujillo, Peru, from January 2015 to December 2021. Clinical and demographic data were collected, as well as levels of serum ferritin. Patients with other comorbidities that could be etiological factors of alopecia were excluded. RESULTS: Seventeen-eight female patients were included. They were grouped into 39 cases with a diagnosis of diffuse alopecia, while another 39 controls without diffuse alopecia. Low serum ferritin levels were found in 28% of the patients in the diffuse alopecia group, while in the control, 90% had serum ferritin levels within normal values. Patients with serum ferritin levels ≤ 70 ng/mL had a 3.43-fold increased risk of developing diffuse alopecia compared with patients with ferritin levels > 70 ng/mL (OR 3.4375; 95%CI: 0.98-11.97, p < 0.05). CONCLUSIONS: Low serum ferritin levels (≤ 70 ng/mL) in female patients are a risk factor of diffuse alopecia. [ABSTRACT FROM AUTHOR]

Foundation: basal cell carcinoma is a very common malignant tumor in humans. It is considered locally invasive, also aggressive and destructive. Objective: to show the behavior of patients with basal cell carcinoma diagnosed by dermoscopy in dermatology clinic of the Miguel Enríquez University Hospital, between June 2020 and May 2021. Method: a descriptive, cross-sectional study was carried out to show the behavior of patients with basal cell carcinoma who attended the dermatology consultation at the Miguel Enríquez University Hospital from June 2020 to May 2021. The universe of study was made up of patients older than 18 years who presented one or more presumptive lesions of basal cell carcinoma less than 3 cm in diameter. The variables used were: age, sex, skin phototype, clinical forms, dermoscopic patterns, topography of the lesion, occupation related or not to sun exposure. We worked with absolute frequencies, percentage, mean, standard deviation, maximum and minimum values, the ratio, chi-square and student's t tests, with significance p≤0.05. Results: it prevailed in the elderly (66.4±14.9 years), in men (54.1 %), with phototype 2 (57.8 %), and in the nose 33.9 %. The nodular form was more frequent (41.3 %), occupation exposed to the sun (66.9 %), average size of the lesion 1.4 cm. Dermoscopy showed a predominance of arborizing vessels (73.4 %). By histology, 90.8 % of the cases were confirmed. Conclusions: the elderly prevailed among the patients, with phototype 2. The nodular form was more frequent, the average size of the lesion was 1.4 cm. Dermoscopy showed a predominance of arborizing vessels. Dermoscopy turned out to be a necessary tool for systematic use in dermatology.

Oral roflumilast is a phosphodiesterase-4 inhibitor approved for the prevention of exacerbations of chronic obstructive pulmonary disease and chronic bronchitis. In dermatology, topical roflumilast is authorized by the US Food and Drug Administration for the treatment of plaque psoriasis and mild to moderate seborrheic dermatitis. Several studies have described the off-label use of roflumilast in dermatology, including a randomized controlled trial showing its usefulness in the treatment of psoriasis; case reports and small series have also reported successful outcomes in hidradenitis suppurativa, recurrent oral aphthosis, nummular eczema, lichen planus, and Behçet disease. Roflumilast has a favorable safety profile, similar to that of apremilast, and it is considerably cheaper than new generation drugs and even some conventional immunosuppressants. We review the pharmacokinetics and pharmacodynamics of topical and oral roflumilast and discuss potential adverse effects and both approved and off-label uses in dermatology. Roflumilast is a promising agent to consider.

Background and objective: Quality of professional life (QPL) is defined as a sense of well-being derived from a balance between the challenges of work and the resources available to deal with them. Impaired QPL can contribute to burnout. The aim of this study was to evaluate QPL in Spanish dermatologists using a validated questionnaire. Material and methods: We designed a cross-sectional study in which Spanish dermatologists were invited to complete an online questionnaire sent out by messaging applications. The dermatologists were asked to provide sociodemographic information, answer 3 open questions, and complete the 35-item Spanish QPL questionnaire (Spanish abbreviation, CVP-35). Results: We analyzed the information submitted by 106 dermatologists (62 women, 58%) with a mean age of 41 years (95% CI, 43.3-38.8 years). Women and department heads scored significantly higher in the workload domain of the questionnaire (P = .02 and P = .005, respectively). A heavy caseload was mentioned as the main factor contributing to impaired QPL and the main change in the wake of the COVID-19 pandemic. Conclusions: Female dermatologists reported heavier workloads. Heavy caseloads and more remote work were the main changes identified after the COVID-19 pandemic. Heavy caseloads have a significant impact on the QPL of dermatologists in Spain. Reducing caseloads would improve general job satisfaction and quality of care provision.

- Abstract
The skin is an important organ. Its function is influenced by the environment, as it is involved with the identity of the subject, self-esteem, and functions as the letter of identification of the person. The unbalance between mind and skin may cause certain diseases. Psycodermatology is a branch of Dermatology which works with the association between certain dermatological diseases and associated psychiatric illnesses. Many times, psychiatric diseases may trigger skin diseases, which means that a proper identification, diagnosis and treatment of these illnesses is a key piece, while a proper referral to mental health services is essential for an integral approach and treatment. We made a review of literature by means of the PubMed database from the years 2010 to 2021about psychodermatology, including the most relevant articles for our review. The keywords used for the search were dermatology, psychiatry, psychodermatology, somatic diseases, psychiatric skin diseases. We included articles in English or Spanish. We found 84 related articles but included only 30 relevant to our search and excluded 54 of them for lack of relevance. It is important to mention the finding of a lack of interdisciplinary work between Dermatology and Psychiatry for the proper diagnosis and approach of this group of diseases; emotional triggers may exacerbate the patient's skin illnesses. - Abstract
BACKGROUND: Histoplasmosis is the most common endemic systemic mycosis in Latin America. The causal agent is Histoplasma capsulatum, a dimorphic fungus, which after being inhaled as microconidia turns into yeast when entering the human body. It is found on land, in plant debris, and can be isolated from the guano of bats and some birds. Its prevalence has increased due to the use of immunosuppressive drugs and the increasing cases of HIV. In immunocompetent hosts, the infection may be asymptomatic and self-limited. The most common manifestation is acute pulmonary infection. However, immunosuppressed or extremely aged patients can have a wide variety of clinical presentations and extrapulmonary involvement. Its clinical manifestations are diverse, including acute, subacute, and chronic forms, with varying degrees of severity. CLINICAL CASES: We present two clinical cases, one in an immunocompromised patient with HIV who debuted with a nasal ulcer and another immunocompetent patient with oral lesions; we discuss the main points to consider in this disease. CONCLUSIONS: We emphasize the importance of having present this disease even in immunocompetent patients, in whom, despite being infrequent, mucocutaneous lesions may appear due to this infection.

Physiological changes are responsible for the appearance of aesthetic dysfunctions in the maternal body, among which the following stand out: the appearance of stretch marks, geloid fibroedema, fluid retention, varicose veins, acne and melasma. The skin is mainly affected due to hormonal instability, resulting in the development of changes that are characteristic of the gestational period. The objective of this research was to review the main dermatofunctional physiotherapeutic interventions used in aesthetic dysfunctions acquired during pregnancy. This is an integrative review of the literature. The electronic databases used to carry out the work: PubMed and Google Scholar between the years 2018 and 2023. In the results, 28 articles were selected due to the relevance of which aesthetic dysfunctions occur with greater prevalence during pregnancy and which treatments could be used in physiotherapeutic care. It can be concluded from the study carried out that prevention is the main objective for aesthetic dysfunctions acquired during pregnancy. [ABSTRACT FROM AUTHOR]

Background: Teenagers’ Quality of Life (T-QoL) is an age-specific measure to assess QoL of teenagers suffering from different skin diseases. A validated Spanish language version is lacking. We present the translation, cultural adaptation and validation of the T-QoL into Spanish. Methods: A prospective study with 133 patients (between 12 and 19 years old), attended at the dermatology department of Toledo University Hospital, Spain (September 2019–May 2020), was carried out for the validation study. The International Society for Pharmacoeconomics and Outcomes Research (ISPOR) guidelines were used for the translation and cultural adaptation. We evaluated the convergent validity with the Dermatology Life Quality Index (DLQI), the Children's Dermatology Life Quality Index (CDLQI) and with a Global Question (GQ) on self-assessed disease severity. We also analysed internal consistency and reliability of the T-QoL tool and confirmed its structure with a factor analysis. Results: Global T-QoL scores significantly correlated with the DLQI and the CDLQI (r = 0.75) and with the GQ (r = 0.63). The confirmatory factor analysis showed optimal fit for the bi-factor model and an adequate fit for the correlated three-factor model. Reliability indicators were high (Cronbach's α = 0.89; Guttman's Lambda 6 index = 0.91; Omega ω = 0.91) and test–retest showed a high stability (ICC = 0.85). The results were consistent with those found by the authors of the original test. Conclusion: Our Spanish version of the T-QoL tool is valid and reliable to assess QoL of Spanish-speaking adolescents with skin diseases.

The generation of cell blocks (CB) obtained from ultrasound-guided fine needle aspiration biopsies (USFNAB) is a well-established technique in breast and thyroid pathology, but is rarely used in dermatology. We reviewed CBs obtained from USFNAB of skin lesions, which were categorized as malignant skin tumors, benign skin tumors, inflammatory skin tumors or deposit skin diseases. The diagnostic yield of each category was compared to histopathology. The USFNAB of 51 skin lesions was processed into CBs. There was overall agreement between histopathology and CBs in 84.31% of cases. Diagnostic group concordance for benign, malignant as well as inflammatory and deposit skin lesions were 69.2%, 93.7% and 86.3% respectively. Cell block generation from USFNAB aspirates of skin lesions should be considered as part of the dermatologic diagnostic armamentarium. Further experience is needed to better understand for which types of dermatologic lesions it would be clearly indicated.

Objective: This was a retrospective analysis of the epidemiological and clinical profile of ten main skin neglected tropical diseases (NTDs) and appraisal of the role of dermatologists in the control and elimination of those diseases. Methods: Analysis of routine data and special investigations carried out by the Ministry of Health between 2017 and 2021, and internet search of published information on skin NTDs in Angola. Results: Except for yaws, all skin NTDs under review remain endemic. Previously unknown foci of Buruli ulcer (BU), cutaneous leishmaniasis (CL), and guinea worm disease (GWD) were detected in recent years. Leprosy, onchocerciasis, lymphatic filariasis, scabies, and snake bites are endemic countrywide. Weak laboratory services are a major constraint to confirm clinically suspected cases. The growing collaboration between public health workers, family doctors, and dermatologists is strengthening integrated approaches in NTDs control and elimination programs. Conclusions: The elimination of leprosy, BU, CL, and GWD as public health problems does not warrant the total interruption of their transmission chain over time. Effective case diagnosis and epidemiological surveillance are key strategies to control and eliminate skin NTDs. In addition to patient centered disease diagnosis and management, dermatologists must actively support field investigations, innovative training programs, and advocacy on skin NTDs.

ATOPIC dermatitis treatment , *DERMATOLOGY , *DERMATOLOGISTS , *PUBLIC health - Abstract
Introduction and Objective: The present study on atopic dermatitis (AD) in Portugal aims to characterize patient needs and discuss measures to improve health care in dermatology, particularly in cases of moderate to severe disease. Methods: The study was conducted in three phases--(1) data collection on the patient access to healthcare and subsequent analysis; (2) critical evaluation of the data in individual interviews with dermatologists and one pediatrician; and lastly, (3) data discussion in consensus meetings to validate the existing care capacity in dermatology, to identify gaps in care for patients with AD and to define mitigation strategies. Results: In Portugal, it is estimated that AD affects ~ 360,000 patients, 70,000 of whom have moderate to severe disease. Healthcare capacity analysis confirmed that the private system plays an important role in the management of AD. It is estimated that 30% of patients rely solely on the Portuguese public health service. Nevertheless, patients with moderate to severe disease can only access advanced targeted therapies from public healthcare providers. Analysis of public care capacity in dermatology shows relevant gaps in the referral system, the geographical coverage of specialized centers, the number of specialists and high waiting times for first appointments. Considering the negative impact of the disease on patients' quality of life, 86% of patients with AD use private settings to better manage their disease. Conclusion: In conclusion, private setting bridges the gaps in public health care capacity in dermatology, and therefore, it is crucial for patients with AD. However, a major limitation is the lack of reimbursement for advanced targeted therapies recommended for moderate to severe AD when they are prescribed in a private setting, thus compromising patient access to these therapies. A possible strategy could be to extend the prescription of these therapies to a private setting based on a reimbursement model similar to that outlined in Act 48/2016 of 22 March.

Introduction: Undergraduate dermatology courses vary in the nearly 50 Spanish medical faculties that teach the subject. This study aimed to describe the characteristics of these courses and to analyze whether the weight assigned to dermatology topics reflects the caseloads of primary care physicians and general dermatologists in the Spanish national health system. Material and methods: Cross-sectional study of syllabi used in Spanish medical faculties during the 2021–2022 academic year. We determined the number of teaching hours in public and private university curricula and compared the weight of dermatology topics covered to the dermatology caseloads of primary care physicians and general dermatologists as reported in published studies. Results: Most medical faculties taught dermatology for one semester. The median number of credits offered was 4.5. On average, lectures covered 24 theoretical topics, and seminars and workshops covered 9 topics. We identified a clear disparity between the percentage of time devoted to dermatology topics in course lectures and the skin conditions usually managed in primary care and general dermatology practices. Discussion: The skin diseases most commonly treated by primary care physicians and general dermatologists are underrepresented in the curricula of Spanish medical faculties. The topics that should be given more weight in syllabi, or recovered for inclusion in dermatology courses, should be re-examined. Our findings show that the topics that ideally should be emphasized more are types of dermatitis, infectious skin diseases, acne, psoriasis, rashes, and the differential diagnosis of benign and malignant neoplasms. There should be additional support for the theoretical teaching of these topics.

- Abstract
Background: Pilomatricomas are neoplasms of hair follicles, located in the hair bulb, and the incidence is uncommon in the clinical-surgical clinical routine of dogs and cats. It commonly affects adult animals, with no predisposition to sex or race, and is mainly located in the neck, back, and tail region. The diagnosis is made by histopathological examination, where cells are observed in which their nucleus does not stain with hematoxylin and eosin - empty nucleus. The present work aimed to report a case of benign pilomatricoma since it is a rare condition in dogs and cats and, consequently, there is little information in the literature. Case: A 6-year-old male Shih-Tzu dog, not submitted to surgical contraception, weighing 6.9 kg, was treated at the Veterinary School Hospital (HVE) of the North Parana State University (UENP), Bandeirantes, PR, Brazil, with a history of nodules in the tail region, lasting 6 months. On physical examination, no changes were identified in the patient's physiological parameters. However, the presence of neoformations in the distal and medial region of the tail, similar to a nail, adherent, non-ulcerated and non-alopecic, and absence of pruritus or self-mutilation were identified. Vaccination and deworming were updated. Hematological examination, serum biochemicals (urea, creatinine, alanine aminotransferase, alkaline phosphatase, and albumin), and abdominal ultrasound showed no changes. According to the clinical and laboratory signs, it was decided to perform an excisional biopsy, using a caudectomy, for subsequent histopathological examination. The specimens were fixed in 10% formaldehyde and sent for histopathological examination. Histological examination was compatible with benign pilomatricoma. Postoperatively, cephalexin, dipyrone, tramadol hydrochloride, and meloxicam were prescribed, surgical wound cleaning, and the use of an Elizabethan collar until the suture was removed. After 10 days of the surgical procedure, the patient was asked to remove the sutures, and no changes were identified regarding the physiological parameters and blood count. Discussion: Pilomatricomas are commonly benign neoplasms arising from the germ cells of the follicular matrix. They present dermal or subdermal forms, with several cystic structures which are surrounded by keratinocytes, similar to the matrix cells of an anagen hair follicle, more keratinized and firmer areas, corroborating the results of the present report. In the ultrasound examination, the presence of intra-abdominal metastases was not identified, which is consistent with the literature, since in this type of neoplasm it is not common to identify foci of intra-abdominal and thoracic metastases. In the present report, immunohistochemistry was not used, even though it is used to differentiate follicular neoplasms from pilomatricomas. However, histopathological examination is considered the best method for the definitive diagnosis of pilomatricomas in dogs. It was concluded that the surgical treatment through excisional biopsy, with safety margins of 2 cm, was effective as a therapeutic method in the case of benign pilomatricoma, and the definitive diagnosis must be made through histopathological examination. - Abstract
OBJECTIVE: To recount the immunopathogenesis of SARS-CoV-2 infection and its skin manifestations. METHODOLOGY: A literature search was performed in the MEDLINE database through the PubMed search engine with the terms "SARS-CoV-2", "SARS-CoV", "Covid-19", "Skin", "Dermatology", "Cutaneous", "Immunology", "Immune". RESULTS: In COVID-19 disease, multiple pathophysiological mechanisms are described, among which the binding of the virus to the angiotensin-converting enzyme 2 (ACE2) receptors in endothelial cells that facilitate the involvement of adjuvant skin, the depletion of T lymphocytes (LT), the generation of a cytokine storm and the activation of macrophages, among others. CONCLUSIONS: An update is made on the knowledge about the immunopathogenesis of COVID-19 and its main cutaneous manifestations, highlighting the need to carry out more research studies that allow a better understanding of it and the creation of possible therapeutic targets. Objective: To determine the clinical and socio demographic features of patients attended at a public hospital in Trujillo, Peru. Methods: A cross-sectional study that included 385 skin biopsies obtained from the archives of the Pathology Department and from the clinical charts of these patients at Hospital Belen in Trujillo from January 2018 to December 2019 was carried-out. Results: The most common entity was basocellular carcinoma located on the face in patients above 60 years of age. The second most common entity was epidermoid carcinoma. Malignant lesions affected 66.7% of patients between 55 and 79 years of age with a mean of 67 years. Most of the patients were unemployed males (65.8%). The most common benign lesion was the epidemic inclusion cyst mostly in the trunk among males from 20 to 24 years of age. The second most common benign lesion was the pilomatrixoma mostly located in the upper limbs of patients below 10 years of age. Benign lesions affected the 59.6% of the sample between 30 to 54 years of age with a mean of 44 years, mostly among housewives. Most of the patients came from districts of Trujillo. The causal agent was not identified. Conclusions: In mestizo patients both benign and malignant lesions had a clinical presentation similar as reported in the literature regarding location, age and gender distribution. Occupation and precedence were not contributory. [ABSTRACT FROM AUTHOR]
