Recent studies suggest that the use of EN is successful and complications are rare. However, an underestimated mechanical complication of tube feedings seen in critically ill patients is the coagulation and solidification of the EN causing gastrointestinal obstruction. This report describes two clinical cases (1.23% of all cases seen at our clinic) of obstruction and perforation of the small bowel secondary to the solidification of EN. The understanding and early recognition of this potential complication are essential for the prevention and successful treatment of this condition. Abstract

The paper presents a study carried out on the island of Pantelleria regarding the interaction among distributed energy resource and energy storage systems in renewable energy communities. EU, in the Clean Energy Package, defines the 'Jointly acting renewables self-consumers' (JARSCs) and the 'Renewable Energy Communities' (RECs) as innovative models to share energy from renewables facilities. Nevertheless, the behaviour of JARSCs and RECs is designed to maximize the final users' self-consumption without considering the impacts on the power grid operation. A comparison of various operational strategies of BESS and an investigation of their effect on the distribution network are key elements for the evolution and development of JACSRs and RECs in small islands. In the following, a model is proposed to size a PV plant and a battery storage system in an energy community and to analyse the economic impact of the management of the battery, considering the Italyn regulatory framework for RECs and the peculiar conditions of the power system of Pantelleria. The Energy Community model consists of a set of electrical loads, some controllable and some not, a photovoltaic generator representing the set of generators installed at the Energy Community users and a storage system representing the set of Battery Energy Storage System of the Energy Community members.

Abstract

The paper presents a novel numerical approach for the parameters determination of crystalline silicon PV models four lumped parameters equivalent circuit. The novel approach basically deals with a new method, alternative to the iterative one known in literature, to calculate the series resistance. Particularly, it allows the evaluation of the lumped parameters on the basis of the manufacturer’s datasheet. The new approach has been validated both in the Maximum Power Point (MPP) and close to it by comparing values and curves obtained by means of the model implementation in Matlab/Simulink environment with those related to some five parameters models and manufacturer curves. Results show that when temperature and radiation are at their STC (Standard Technical Condition) values, the four lumped parameters model here developed shapes the manufacturer I-V characteristics curves better than the other models. Moreover, what above has been investigated to be true even if radiation is lower than 1000 W/m2 but not when temperature is higher than 298.15 K. Finally, the four lumped parameters model here developed is to be preferred to the five lumped parameters ones if temperature value is lower than 323.15 K

Abstract

The PLD, included in the group of malformative lesions of ductal plate, is characterized by progressive development of multiple parenchymal cysts. Different surgical treatments have been proposed for symptomatic patients with PLD: percutaneus aspiration, fenestration, hepatic resection and liver transplantation. The aim of this report is to outline the treatment of polycystic liver disease with laparoscopic fenestration.Of thirteen patients with PLD, in all cases associated with polycystic kidney disease, 7 patients underwent to laparoscopic treatment of cysts fenestration, and 6, asymptomatic and not amenable to surgical treatment, underwent to clinical follow-up. Of the 7 patients, 5 have symptoms related to "effect mass" 1-2-3 of hepatic cysts while in the 2 asymptomatics the fenestration of the cysts was performed during the procedure of laparoscopic cholecystectomy for cholelithiasis.The best indication for laparoscopic fenestration are those cases of PLD characterized by a relatively limited number of large cysts, preferably situated in the anterior segments of the liver and in the left lobe; in this patients, laparoscopic fenestration reduces significantly the volume of the liver and relieves symptoms. The laparoscopic fenestration of hepatic cyst, in carefully selected patients, is an effective technique in terms of morbidity, mortality, conversion rate and recurrence rates; while in patients with cyst diffuse in liver parenchyma is indicated the hepatic resection or liver transplantation. The optimal surgical approach is still evolving, the type of approach is related to extent and distribution of the cysts, and vascular anatomy of normal segment of the liver.Laparoscopic fenestration, Policystic liver.

Abstract

A new Diagnostic and Protective System (DPS) suitable for wheelchair electrical drive applications has been prepared. Such a system is able to perform both monitoring and long-distance transmission of key signals that are used as pre-fault and fault indicators. The system, moreover, may detect several anomalous working conditions by means of reliable diagnosis procedures and carries out actions to protect the electrical drive and to drive the disabled people in safe conditions. In particular the design, the realization and the performance analysis of the new dedicated diagnostic system, which exploits a programmable logic controller (PLC) as DRS hardware, are described and discussed. The effectiveness of the realization has been validated by experimental tests carried out in a laboratory test rig that has been purposely settled up. The experimental results show that the proposed DPS, thanks to the PLC performances, is able to detect the considered anomalous working conditions of the electric drive. Moreover the system is able to choose the correct protective action and to dialogue with a remote control center with trained people able to give first-aid support both in the medical and the technical fields. The information to the disabled people is supplied by means of telediagnostic facilities. The DPS is robust and friendly to use, flexible to program and easy to put on board in a wheelchair.
