Abstract

BACKGROUND/OBJECTIVES: Cancer cachexia is a syndrome characterized by weight loss (WL) and sarcopenia. Aim of the study was to assess the impact of cachexia on head and neck changes during definitive cisplatin and image-guided volumetric- modulated arc radiation therapy in a series of locally advanced oropharyngeal cancer. SUBJECTS/METHODS: Volume variations of sternocleidomastoid muscle (SCM) were considered as surrogate of muscle changes related to sarcopenia. Two head and neck diameters, encompassing the cranial limits of II and III nodal levels (defined as 'head diameter' and 'neck diameter', respectively), were measured. All parameters were defined retrospectively by means of on-board cone beam computed tomography images at 1-8th to 15-22th and at last fraction (fx) of radiotherapy (RT). Cachexia was defined as WL >5% during treatment. Analysis was conducted correlating the parameter changes with three WL ranges: < 5, 5-9 and > 10%. RESULTS: Thirty patients were evaluated. One hundred and fifty contoured SCMs and three hundred diameters were collected. Median WL was 6.5% (range, 0-16%). The most significant SCM shrinkage was recorded at 15th fx (mean 1.6 cc) related to WL 5-9% and WL > 10% (P 0.001). For 'head diameter', the peak reduction was recorded at the 15th fx (mean 8 mm), statistically correlated to WL > 10% (P 0.001). The peak reduction in 'neck diameter' was registered at the 22th fx (mean 6 mm), with a gradual reduction until the end of treatment for WL >5%. CONCLUSIONS: In a homogeneous cohort of patients, present study quantified the impact of cachexia on head and neck changes. Present data could provide adaptive RT implications for further investigations. All participants were first treated with topical mometasone furoate (MF) 0.1%. When the symptoms disappeared, they were treated either with Zantogin®, a multicomponent topical formula containing anti-inflammatory and anti-itching natural actives, or a control cream for 60 days.Results: The study demonstrated that, in about 85% of the participants treated with Zantogin®, symptoms disappeared completely, and only 15% had to resort to MF as needed, with an average use of about three applications per subject (in total). In the placebo group, approximately 90% of participants had to resort to MF as needed, with an average use per person of more than 16 applications in 60 days.Conclusion: Our study demonstrates that, following use of a topical steroid, symptoms such as burning and itching can be validly controlled with subsequent and longer therapy with a herbal topical formula, Zantogin®, which is able to properly counteract itching and inflammation, prevent symptom relapse, and avoid the typical side effects associated with prolonged use of topical steroids.Keywords: vulvar dermatitis, lichen simplex chronicus, vulvar itching, Zantogin®, zanthalene

Abstract

The long term evolution of the Molise coast (South-East Italy) is analyzed using the Littoral Drift Rose (LDR) concept, coupled to the GENESIS numerical model, which is based on the one line contour equation. LDR has been used to define a single, climate-equivalent, time-invariant sea state, which has been supposed to entirely rule the shoreline changes. Particular attention has been drawn to a 5 km long reach of coast, just South to the mouth of the Trigno river. Results of the analysis indicate the adopted procedure, even if extremely simplified, can explain nearly 90% of the observed shoreline trend.

Abstract

BACKGROUND: The aim of the present work is to evaluate the effectiveness of the use of ultrasound scalpel during laparoscopic ovariectomy in the bitch. METHODS: Two groups of 10 subjects each, of different races and ages, were compared. In the first group, ovariectomy was performed laparoscopically, using harmonic scalpel to remove ovary. In the second group surgery was performed by means of classical laparotomy. RESULTS: Pre-operative time was similar in both groups. Total operative time, from incision to skin suture, showed significant difference between the two groups, being laparoscopy faster than laparotomy. Partial operative time for bilateral oophorectomy resulted lower using open technique, but, considering each ovary, there was no significant difference in both groups. CONCLUSIONS: The use of harmonic scalpel to perform ovariectomy during laparoscopy is an effective time-sparing surgical approach compared to the already great practicality of laparotomy.

Abstract

This study was conducted within the framework of the “Coastal Morphodynamics” Working Group (WG) of the Italian Association of physical Geography and Geomorphology (AIGeo), according to the Institute for the Protection and Environmental Research (ISPRA) for the updating of the legend for the “Geomorphological Map of Italy”. The WG deals with the legend for the coastal areas, focusingits work on marine, lagoon and aeolian landforms, processes and deposits. In particular, the legend aims to classify coastal landforms in order to contribute to hazard and risk assessment,for supportingland-use planning and management. The legend allows the mapping of each landform in function of its genesis as well as its evolution and present dynamics, providing information about morphological characteristics at small and large scales. The relict morphological features and the active ones are reported along with the quantitative parameters useful for the description of the present wave/climate conditions and morphodynamics. As a result of the activities and experiments carried out by the “Coastal Morphodynamics” AIGeo WG during the last years, some examples of coastal geomorphological mappings at different scales (1:5,000 and 1:25,000) have been developed and are presented in this paper. The maps focus both on littoral plains and rocky coast dynamics as well as on the interactions with anthropic modifications.

Abstract

In the last years, the activities of Working Groups (WGs), in the framework of Italian Association of Physical Geography and Geomorphology (AIGeo), were aimed at the new issue of the legend of the Italian Geomorphological Map. The activities were performed keeping in mind the necessity: 1) to add new symbols because the great progress made in geomorphology, in terms of new knowledge on geomorphological processes and landforms, after the first issue of the legend in 1994; 2) to have a legend usable with large scale maps, suitable for applied purposes; 3) to create a legend usable with softwares (GIS environment, etc). The WG on Coastal Morphodynamics contributed to this work, as well, promoting, through meetings and workshops, the debate among the Italian coastal geomorphologists on the approach needed to produce a modern geomorphological legend. The classic dichotomy forms-processes used in the geomorphological mapping is often based to static concepts, especially when applied to marine and coastal areas, affected by fast dynamic events, sometimes paroxysmal. We present the new geomorphological legend to represent both coastal and underwater contexts, aimed to the improvement of classic geomorphological legends. The main goal of the work was the production of a legend suitable to clarify the importance and the need of representing landforms in the context ofthe Quaternary evolution, as well as from more recent or modern processes.

Abstract

The amount of coastal subsidence on the Sele River coastal plain has been examined and measured with local vertical land movement data. The vertical displacements, derived by satellite radar differential interferometry processing (Ps‐InSAR), show that the analysed coastal sector is characterised by a south‐eastward decrease of vertical subsidence rates. These results have been coupled with sea‐level rise (SLR) scenarios, in order to identify the most critical areas. In general, the subsidence mostly affects areas where alluvial deposits are thicker, the back‐dune areas and the Sele River mouth, all late Holocene in age. Five local SLR scenarios allow identifying zones in the plain potentially prone to inundation and the shoreline retreat for the years 2065 and 2100. For these dates, 2.2% and 7.06% of the investigated area will have a topography lower than the estimated future sea level. Moreover, results show that the extent of the areas potentially exposed to inundation and erosion increases moving from south to north. [ABSTRACT FROM AUTHOR]
