Abstract

Background Three variants of the human INHA gene have been reported to be associated with premature ovarian failure (POF) in case-control studies involving a small number of patients and controls. Since inhibin has a fundamental role in the control of ovarian function, it is important to establish the relevance of the reported variants for disease risk. Methods Three independent POF cohorts, recruited in Northern and Central Italy and in Germany consisting of a total of 611 patients and 1084 matched controls, were genotyped for the three variants: -16C > T, -124A > G and 769G > A. Results No significant difference was detected between allelic frequencies of the INHA promoter variants between POF patients and controls. The rare allele in the coding variant appeared to be more frequent among the control populations. Conclusions The association between the INHA promoter variants and POF could not be replicated, and our results suggest that this discrepancy is likely to be due to the small sample size of previous studies. The rare allele of the coding variant seems to exert a protective effect against loss of ovarian function, which should be confirmed in additional large and ethnically diverse cohorts.

Abstract

Introduction: Sexually transmitted infections (STIs) can be caused by a number of microorganisms that vary greatly in size, life cycle, clinical manifestations, and sensitivity to available treatments. Transmission of STIs can occur during unprotected (or condomless) sexual contact and through the exchange of body fluids during any type of activity. The prevalence of sexually transmitted diseases remains high in the world, despite diagnostic and therapeutic improvements for these infectious diseases that rapidly eliminate the contagiousness of patients. Our study determines the prevalence of STI pathogens in adolescents and young adults in the population of the Province of Macerata (Italy). We will analyze data in correspondence to age and gender, and we will compare our results to international studies., Materials and Method: We analyzed STI test results from the entire database of a Provincial Health Authority for the period 2021-2022. The samples came from the following age groups: 0-12, 13-18, 19-25, and 26-35 from 2021 to 2022. The results came from vaginal and cervical swabs (for females); urethral, rectal, and pharyngeal swabs (for males and females); and seminal fluid (for males) for the following infections: HPV, Chlamydia trachomatis , Mycoplasma genitalium , Ureaplasmas , Gardnerella , Trichomonas vaginalis , Neisseria gonorrhoeae , and Treponema pallidum . The results also came from blood tests for HIV, hepatitis C, hepatitis B, and Treponema pallidum (TPHA, VDRL). In addition, we examined results from urine tests for chlamydia, Neisseria gonorrhoeae , trichomonas, and Treponema pallidum ., Conclusions: The literature for other countries reports the need for comprehensive, culturally and developmentally sensitive care to address sexuality-related issues in adolescents and young adults, a need that also applies to Italy. These data will be of great importance in adopting evidence-based STI control programs in Marche Region. This study could, indeed, represent a landmark for public health officials and professionals, with the aim of promoting adolescents' access to sexual health services to receive useful information, strengthening preventive measures in younger age groups, and designing sexual education programs.

Abstract

This short communication describes the delayed onset of premature ovarian failure (POF) in some young patients, between 5 to 24 months after operative laparoscopy for ovarian endometriosis. This complication apparently does not have a link with traditional risk factors for POF. In view of this observation, further and consistent clinical investigations are necessary to seek an explanation for this possible iatrogenic effect, which is of particular concern to young patients who are referred for video laparoscopy as a treatment for ovarian endometriosis.

Abstract

The authors refer the results of their study in 15 healthy women in whom 24-hours serum melatonin (MT) and cortisol (F) were assayed by RIA method. Both hormones showed a clear circadian rhythm: Melatonin reached the highest values between 24.00 and 04.00 while cortisol were at the lowest values. The relationships between Corticotropin Releasing Factor (CRF) and pineal function are analysed. A strict correlation between the hypothalamus-pituitary-adrenal axis and melatonin is suggested.

Abstract

The correlation between the sex steroids and the immune system is already known but the relationships between testosterone, progesterone, estradiol, cortisol and the immune system are still controversial. The Authors have studied lymphocytes subset in ten women with polycystic ovarian disease (PCOD); it was observed: 1) a decrease of T8 in 64% and of natural killer activity (NK) in 60% of patients, and an increase of T4/T8 ratio in 55%; 2) a significant negative correlation between androstenedione and testosterone (P less than 0.05); 3) a significant positive correlation between the decreased T8 lymphocytes and NK cells and the increase of testosterone and androstenedione serum values. The data suggest that in PCOD a correlation between endocrine and immune system exists and the androgens can affect the lymphocyte subset.

Abstract

In 35 women, 16-32 years old (average age 24) affected by hirsutism associated to menstrual disorders as oligomenorrhea or secondary amenorrhea, the real utility of testosterone's free fraction assay (FT) in comparison with conventional hormonal assays has been valued. The free testosterone (FT) resulted the most frequently elevated hormone (69%). The analysis of the different groups shows a good sensitivity and specificity of FT assay in some conditions as PCOD, but it is not so sensitive and specific in other conditions as secondary amenorrhea. We conclude that FT assay is particularly useful for the hyperandrogenism study, especially in the PCOD; for its high diagnostic sensitivity it would be assayed in all the women with hirsutism, but it isn't sufficient for the screening of the different hyperandrogenic states.

Abstract

A compared study of neurovegetative modifications (following stress stimula) and of psychopathological aspects in patients affected by amenorrhea was carried out. Such an integrated approach is useful for a better definition of clinical characteristic and for a selection of a suitable therapeutic approach.
