Metastatic carcinomas to the thyroid are rare in daily clinical practice. However, when encountered they represent a diagnostic challenge, since it is difficult to distinguish them from primary thyroid lesions, especially when occurring in patients with occult malignant history. Nevertheless, it is critical to differentiate a metastatic tumor from primary thyroid lesions, as the clinical management and the prognosis are different for the two entities. More recently, elastosonography opened new possibilities to ultrasound in different fields, such as thyroid nodule differentiation. Herein, we report a case of metastatic signet ring cell carcinoma to the thyroid studied with quantitative elastography and acoustic radiation force impulse imaging.

In 1996 and 1997, a century after the excavations conducted by F. Halbherr, new soundings were conducted in the Greek agora at Gortyna as part of the activities of the Italian Archaeological School at Athens, and with the permission and cooperation of the 23th Ephoria of Antiquities of Heraklion. The excations started from the western edge of the agora, by the Mitropolianos torrent, where F. Halbherr and L. Pernier had discovered three parallel walls. A few targeted soundings, conducted stratigraphically, allowed three phases to be identified in the so-called double stoa, which delimitated the area to the north of the agora in the II-I centuries B.C., and was rebuilt in the Trajanic era, and lasted until late Antiquity. It represented a monumental wing of the square, which was perhaps also delimited on the west side by another wing of this imposing structure. The north wing, which we brought to light, was more than 70 meters (230 feet) long, had an interior coonnade of 14 Ionic columns, and probably 25 Doric columns on the front. The excavations also uncovered significant patches of the agora's Roman period pavement and, below it, that of the Hellenistic period. The ancient agora had been obliterated with a fill that was more than a meter (3,3, feet) thick, which provided us with abundant materials of the Byzantine period. The material included plain and imported pottery, lamps, amphorae, coins, and provides us with a vivid picture of Gortyna's life in its final stages around the VII-VIII centuries A.D.

Ectopic parathyroid adenomas represent a diagnostic challenge, since they are quite rare in clinical practice. We present a case of a 61 years old man with symptomatic hypercalcemia due to an ectopic parathyroid adenoma in the upper-anterior mediastinum that was not localized by the scintigraphy. Ultrasonography identified a nodule on the left upper-superior lobe of the thyroid gland, consistent with a parathyroid adenoma; scintigraphy showed two focuses of abnormal tracer uptake, one on the left upper-superior lobe of the gland and the other on the right inferior lobe of the gland. Patient underwent surgery to remove the adenoma on the left side, but postoperatively PTH and serum calcium level were still elevated. CT scan showed a mass of 27 x 22 mm in the upper anterior mediastinum, in front of the right emisoma of D2. The final surgery allowed us to remove the adenoma with a laterocervical approach. The histopathology was consistent with parathyroid adenoma.

To evaluate the convenience in terms of recurrence and fecal incontinence rates of fibrin glue versus surgical treatment in the management of fistula-in-ano.Randomized controlled trials (RCTs) and non-randomized studies (CCTs) comparing conventional surgical treatment versus fibrin glue treatment in patients with perianal fistulae were identified using a predefined search strategy. The post treatment anal incontinence rate and the fistula recurrence rates between the two operations were compared by using the methods provided by the Cochrane Handbook for Systematic Reviews of Interventions. The lack of homogeneity of results between the different studies did not allow to analyze other secondary outcomes. Patients with cryptoglandular and Crohn's anal fistula were enrolled in the analysis. The employed fibrin glue came from commercial kits: Beriplast (Aventis Behring, Sussex, United Kingdom) and Tisseal or Tissucol (Baxter, Inc, Mississauga, Ontario). Surgical conventional treatment consisted of fistulotomy, placement of a cutting or loose latex seton and advancement mucosal flap closure. All patients were followed up at 6 and 12 weeks, the longest follow up was 6 months.Two RCTs (106 patients) and 1 non randomized studies (232 patients) were identified. The recurrence rate is higher, although still not statistically significant, in those patients who underwent fibrin glue injection (44/81) versus conventional surgical treatment (108/230), (OR: 0.44; 95 %CI: 0.12-1.68; P = 0.23). Furthermore in the analysis of the subgroup of RCTs alone there were not significant differences with the previous results of RCTs with CCT analysis (OR: 0.33; 95 %CI: 0.03-3.66; P = 0.37). In the same way the analysis of the subgroup of RCTs with complex anal fistulae were not statistically significant and similar to the previous results regarding all type of fistulas (OR: 0.86; 95 %CI: 0.01-72.36; P = 0.95). The analysis of post-operative anal incontinence showed no difference between the group who underwent fibrin glue injection (9/230) and the conventional surgical treatment group (10/81), (OR: 1.00; 95 %CI: 0.43-2.34; P = 1.00). A very low heterogeneity in the analysis was detected (Chi-square = 0.04 - P = 0%).Our statistical analysis does not show any significant statistical difference between fibrin glue treatment versus conventional surgical treatment for all perianal fistulae in terms of recurrence (P = 0.23) and anal incontinence (P = 1.00).

The post-operative pain (POP) is accompanied by autonomic, psychological and behavioural responses which can result in a significant increase in morbidity and mortality. The purpose of this study was to compare the efficacy and safety of intravenous bolus administration of morphine versus subcutaneous administration.Fifty subjects scheduled for elective abdominal surgery were randomized into a group S (n = 26) and group E (n = 24). The first group was administered morphine (0.35 mg/kg) subcutaneously immediately after induction of anesthesia, while the second group the same drug at a dose of 0.25 mg/kg was administered intravenously 45' before the end of the intervention. All patients received intraoperative remifentanil (0.25-0.50 microg/kg/min), and at the end of surgery paracetameolo 1 g and ondansetron 4 mg. Upon awakening, the DPO has been reviewed by the NRS (numerical scale of pain) at time zero (TO), after 30' (t1), 1h (T2) and 24h (T3). Furthermore, it was annotated request for rescue doses of morphine and the collateral effects.The two study groups did not show significant differences related to the anthropometric characteristics, duration of surgery, type of surgery, ASA class. No difference between the two groups was highlighted during the evaluation of POP through NRS scale and quantity of "rescue" doses requested. In group E there was a single case of respiratory depression.Our study suggests that whether intravenous or subcutaneous administration of morphine, both simple and economical analgesic techniques, they are able to guarantee the same quality control of POP in patients undergoing abdominal surgery.
