Objective: To find out the underlying causes of chronic diarrhea in soldiers and how it differs from general population. Study Design: Prospective comparative study. Place and Duration of Study: Combined Military Hospital (CMH) Multan and Pak Emeritus Military Hospital (PEMH) Rawalpindi, from Dec 2017 to Jan 2020. Methodology: All soldiers in active service, having diarrhea more than 4 weeks in study and non-soldiers of the same age were included in the control group. There were inquired in details about the diarrhea symptoms and investigations carried out accordingly to find out the underlying cause. The two groups were compared to find out any differences. The data were analyzed by SPSS version 22. Results: Seventy one in study and 50 patients in control had their mean ages of 34.61 ± 8.35 and 32.42 ± 10.28 years. Mean duration of symptoms were 18 and 15 months in study and control groups respectively. Irritable Bowel Syndrome (IBS) was more common in both groups i.e. 14 (20%) and 20 (40%) in study and control groups respectively but frequency was more in second one. Seropositive celiac disease 13 (18.5%), seronegative villous atrophy 10 (14%) in study and Inflammatory Bowel Disease IBD 6 (12%) in control group were second most common conditions. Conclusions: Irritable bowel syndrome is common in this age group. Celiac and seronegative villous atrophy is second common conditions in the study group.

In 2020, to trace the prevalence and evolution of bovine coronavirus (BCoV) in China, a total of 1383 samples (1016 fecal samples and 367 nasal swab samples) were collected from 1016 cattle exhibiting diarrhea symptoms on dairy farms and beef cattle farms in Heilongjiang Province, Northeast China. All samples were subjected to reverse transcription-polymerase chain reaction (RT-PCR) detection of the BCoV N gene, followed by an analysis of its epidemiology and genetic evolution. The results indicated that of the 1016 diarrhea-affected cattle, 15.45% (157/1016) were positive for BCoV, in which positive rates of the fecal and nasal swab samples were 12.20% (124/1016) and 21.53% (79/367), respectively. Of the 367 cattle whose nasal swab samples were collected, the BCoV positive rate of the corresponding fecal samples was 15.26% (56/367). BCoV infection was significantly associated with age, farming pattern, cattle type, farm latitude, sample type, and clinical symptom (p 0.05). Of the 203 BCoV-positive samples, 20 spike (S) genes were successfully sequenced. The 20 identified BCoV strains shared nucleotide homologies of 97.7-100.0%, and their N-terminal domain of S1 subunit (S1-NTD: residues 15-298) differed genetically from the reference strains of South Korea and Europe. The 20 identified BCoV strains were clustered in the Asia-North America group (GII group) in the global strain-based phylogenetic tree and formed three clades in the Chinese strain-based phylogenetic tree. Diarrhea is largely caused by dysbiosis accompanying the hyperproliferation of Escherichia coli (E. coli). While current treatments can resolve the symptoms, they cannot suppress the proliferation of pathogenic bacteria in the intestine. Probiotics have numerous beneficial effects on host health, including restoring the balance of the intestinal microbiota. This study investigated the effect of the probiotic Bifidobacterium bifidum G9-1 (BBG9-1), which is active in intestinal dysbiosis, in the incidence of diarrhea, in the composition of the intestinal microbiota, and in the intestinal tissue of a rat model of phytohemagglutinin (PHA)-induced diarrhea. The rats were treated with PHA, with and without BBG9-1, and the microbiota composition throughout the intestine and stool was examined using high-throughput 16S rRNA sequencing. In line with previous reports, PHA administration caused diarrhea as well as dysbiosis due to E. coli hyperproliferation. Histological findings indicated that the jejunal villus length was shortened. Rats that received BBG9-1 showed clear improvements in dysbiosis, diarrhea symptoms, and jejunal villus length. Principal coordinates analysis demonstrated the microbiota profile to be more similar between the BBG9-1 and normal groups than between the PHA and normal groups. These results indicated that BBG9-1 suppresses the hyperproliferation of E. coli and restores the jejunal villus length, thereby improving dysbiosis, and in turn, alleviating the symptoms of diarrhea.

Legionnaires disease is primarily a pneumonic illness with possible multisystem involvement. Major risk factors include immunodeficiency, smoking, alcoholism and chronic obstructive pulmonary disease among others. We report a peculiar case of Legionnaires disease presenting with diarrhea as the chief complaint and no respiratory symptoms throughout the course of disease. The patient had no risk factors for the disease and had no recent travel history or sick contacts. Acute diarrhea is not an uncommon manifestation of Legionnaires disease, although isolated diarrhea symptoms with the absence of concurrent respiratory symptoms and no risk factors for Legionella makes this case a diagnostic challenge, leading to possible delay in appropriate management. We are presenting this case to inform physicians of the possibility of Legionnaires disease presenting as an isolated gastrointestinal involvement with no clinical symptoms of pneumonia at presentation.

(1) Background: Poor nutritional status and diarrhea are common complications in cancer patients. (2) Methods: This multicenter, observational, prospective study evaluated the effectiveness of an oligomeric enteral nutrition (OEN) protocol in the improvement of nutritional status and reduction of diarrhea symptoms. Nutritional status was assessed with the Subjective Global Assessment (SGA), Body Mass Index (BMI) and albumin levels. Diarrhea was evaluated by the frequency and consistency of stools (Bristol Stool form scale). (3) Results: After 8 weeks of OEN protocol, the nutritional status improved in 48.3% of patients, with an increased proportion of patients at risk of malnourishment (+27.3%) at the expense of a decrease of moderately (&minus, 19.9%) and severely (&minus, 7.3%) malnourished patients (p &lt, 0.001). Serum albumin and BMI significantly increased after 8 weeks of OEN treatment (p &lt, 0.005). OEN showed a 71.1% effectiveness in the improvement of stool consistency. The mean number of stools per day significantly decreased from baseline (4.17 stools/day) to week 8 (1.42 stools/day, p = 0.0041). The nutritional status significantly improved even in those patients with persistent diarrhea. (4) Conclusion: The proposed OEN protocol seemed to be effective in improving the nutritional status, frequency and consistency of stools in patients with oncology treatment-related diarrhea even in persistent cases.

This study evaluated the effects of increasing doses of pre-fermented rapeseed meal (FRM) without or with inclusion of the brown macroalgae Ascophyllum nodosum (AN) on weaner piglets&rsquo, performance and gut development. Ten days pre-weaning, standardized litters were randomly assigned to one of nine isoenergetic and isoproteic diets comprising (on DM basis): no supplement (negative control, NC), 2500 ppm ZnO (positive control, PC), 8, 10, 12, 15 or 25% FRM, and 10% FRM plus 0.6 or 1.0% AN. Fifty piglets receiving the same pre-weaning diets were weaned at 28 days of age and transferred to one pen, where they continued on the pre-weaning diet until day 92. At 41 days, six piglets per treatment were sacrificed for blood and intestinal samplings. The average daily gain was at least sustained at any dose of FRM (increased at 8% FRM, 28 &ndash, 41 days) from 18 &ndash, 41 days similar to PC but unaffected by inclusion of AN. The percentage of piglets that completed the experiment was increased by FRM compared to NC, despite detection of diarrhea symptoms. FRM showed quadratic dose-response effects on colon and mid-jejunum crypts depth, and enterocyte and mid-jejunum villus heights with optimum development at 8% or 10% FRM, respectively, but this was abolished when AN was also added. In conclusion, FRM sustained piglet growth performance and intestinal development similar to ZnO with an optimum inclusion level of 8 &ndash, 10% of dietary DM.

Simple Summary The present study focuses on the metabolic changes in the diarrhea of calves, which are manifested with the following symptoms: a thin water-like stool, cold ears and nose, throbbing bowels, oliguria, a pale or yellowish complexion, a smooth mouth, and a slow pulse. The differential metabolic pathways in calves with diarrhea were screened by metabolomics. There were nine biomarkers in the serum of healthy calves and calves with diarrhea. On the basis of these biomarkers, their associated mineral absorption, protein digestion and absorption, and other metabolic pathways, the targeted regulation of the metabolic differences of calves with diarrhea may contribute to the diagnosis, treatment, and discussion of the mechanism of calf diarrhea. Abstract The complex etiology, higher morbidity and mortality, poor prognosis, and expensive cost of calf diarrhea have made it a catastrophic disease in the dairy industry. This study aims to assess the biomarkers in calves with diarrhea and to predict the biomarkers related to the pathway. As subjects, nine calves with diarrhea and nine healthy calves were enrolled, according to strict enrollment criteria. The serum metabolites were detected by a liquid chromatographic tandem mass spectrometry (LC-MS/MS), and then analyzed by online multivariate statistical analysis software to further screen the biomarkers. In addition, the biomarkers involved in the metabolic pathways of calves with diarrhea and healthy calves were analyzed. In the serum of calves with diarrhea, nine biomarkers were found to which several biomarkers exhibited a certain relation. Moreover, these biomarkers were involved in important metabolic pathways, including protein digestion and absorption, ABC transporters, aminoacyl-tRNA biosynthesis, mineral absorption, and fatty acid biosynthesis. All these findings suggested that the imbalance of these markers was closely related to the occurrence and development of calf diarrhea. The targeted regulation of metabolic pathways involved in these biomarkers may facilitate the diagnosis, treatment, and discussion of the mechanism of calf diarrhea.

The current adverse event reporting systems are inadequate to prevent potential health damage caused by dietary supplement use, and a better case-collecting system is needed. The aim of this study was to examine whether online surveys would be a useful methodology for estimating ongoing adverse events, focusing on diarrhea as a model symptom. We used four online survey companies and requested that they administer the same questionnaire to their registrants regarding experiences of adverse events associated with supplement use and details of diarrhea symptoms, as well as the product and ingredient names of suspected supplements. Of 78,220 dietary supplement users, 9% to 25% reported experiencing adverse events in the previous year. A total of 723 participants who answered affirmatively about a putative causal relationship between supplement use and the onset of diarrhea and who clearly identified the product and/or ingredient were eligible for final analysis. Comparisons among the survey companies showed differences in the proportions of reported symptoms; however, the rank orders were consistent in the preliminary survey. Also, when analysis was limited to specific participants from a full-scale survey, even the numerical values of the proportions were consistent among the four companies. Herbal/botanical ingredients, specifically Coleus forskohlii, were the ingredients most frequently reported to be associated with diarrhea. These results indicate that online survey can be a useful method to complement the current system for estimating ongoing adverse events and identifying the responsible supplements.

Gran parte de las enfermedades diarreicas son prevenibles mediante el acceso al agua potable y a servicios adecuados de saneamiento e higiene. Objetivo: caracterizar los conocimientos, actitudes y practicas de cuidadores de ninos menores de cinco anos asociados a prevalencia de diarrea. Materiales y metodos: estudio observacional analitico de corte transversal, realizado durante el periodo 2014-2015. Se realizo un muestreo por conglomerados en la comuna norte de la ciudad de Bucaramanga y se aplico una encuesta dirigida. Resultados: solo uno de cada diez cuidadores conoce todas las caracteristicas de la diarrea aguda (cambios en la consistencia, numero y duracion de los sintomas) y ninguno reconoce totalmente los signos de deshidratacion. El uso de remedios caseros y la creencia en causas sobrenaturales y esotericas como causas de diarrea tuvo una alta prevalencia. La menor edad del cuidador, tener aseguramiento y mayor nivel conocimientos sobre la diarrea se asociaron a un mayor reporte de episodios de diarrea en los ultimos seis meses. Conclusiones: el 49% de todos los cuidadores encuestados manifestaron que sus ninos han tenido al menos un episodio de diarrea en los ultimos seis meses. El nivel de conocimientos de los cuidadores es bajo. Se encontro asociacion estadisticamente significativaentre la edad del cuidador, conocimiento de los sintomas diarreicos, regimen de aseguramiento y prevalencia de diarrea en menores decinco anos. Much of diarrheal diseases are preventable through access to safe water and adequate sanitation and hygiene. Objective: to characterize the knowledge, attitudes and practices of caregivers of children under five years associated with prevalence of diarrhea. Materials and methods: cross-sectional observational study, made during the period 2014-2015. Cluster sampling wasconducted in the northern district of the city of Bucaramanga. A targeted survey was conducted. Results: only one in ten caregivers know all the characteristics (changes in consistency, number and duration of symptoms) of acute diarrhea. None fully recognize the signs of dehydration. The use of home remedies and belief in supernatural and esoteric causes as causes of diarrhea had a high prevalence. Caregiver younger, having higher-level assurance and knowledge on diarrhea associated with increased reporting of episodes of diarrheain the last six months. Conclusions: 49% of all respondents report that their children caregivers have had at least one episode of diarrhea in the last six months. The level of knowledge of caregivers is low. Age of the caregiver, knowledge about diarrhea symptoms and healthcare affiliation was statistically associated with the onset of diarrhea in children up to five years. Background of the Study. As a widely used traditional medicine, Galla Chinensis is rich in tannins. However, there are few detailed studies about pharmaceutical preparations of Galla Chinensis tannin extract (GTE). In the present experiments, for better application and to investigate the possibility that Galla Chinensis tannin extract can be used as an antidiarrheal drug, we prepared Galla Chinensis oral solution (GOS). Materials and Methods. GOS was prepared with GTE, and its physicochemical and microbiological stability was evaluated. The oral acute toxicity of GOS was calculated by the 50% lethal dose (LD50). The antidiarrheal activity was determined in a castor oil-induced diarrhea model in mice through diarrhea symptoms, fluid accumulation ratio, and percentage of distance moved by charcoal meal. Results. The tannin content of GTE was 47.75%. GOS could endure a high temperature without a significant decrease of tannin content. After storage for six months, the tannin content of GOS was still more than 90%. GOS was determined to be nontoxic. Meanwhile, GOS showed significant antidiarrheal activity in a castor oil-induced diarrhea model in mice (P<0.01). Conclusion. The results suggested that GOS is an effective and stable antidiarrheal drug that can be used to complement other therapies.

Neurological symptoms and gastrointestinal symptoms were rare at onset in COVID-19. Here we report a 37-year-old man with vertigo, fever, and diarrhea symptoms as the first manifestation. 18F-FDG PET/CT spotted multiple ground glass opacity (GGO) lesions in the lungs, with increased tracer uptake in both lung GGOs and the whole colon. Serial CT examinations showed the emersion and dissipation of lung GGOs. We illustrate the symptoms initiation, the laboratory test results, the imaging examination, and the treatment strategy in the duration of COVID-19 with a timeline chart.

Background : Cholera outbreaks are a continuing problem in Bangladesh, and the timely detection of an outbreak is important for reducing morbidity and mortality. In Matlab, the ongoing Health and Demographic Surveillance System (HDSS) data records symptoms of diarrhea in children under the age of 5 years at the community level. Cholera surveillance in Matlab currently uses hospital-based data. Objective : The objective of this study is to determine whether increases in cholera in Matlab can be detected earlier by using HDSS diarrhea symptom data in a syndromic surveillance analysis, when compared to hospital admissions for cholera. Methods : HDSS diarrhea symptom data and hospital admissions for cholera in children under 5 years of age over a 2-year period were analyzed with the syndromic surveillance statistical program EARS (Early Aberration Reporting System). Dates when significant increases in either symptoms or cholera cases occurred were compared to one another. Results : The analysis revealed that there were 43 days over 16 months when the cholera cases or diarrhea symptoms increased significantly. There were 8 months when both data sets detected days with significant increases. In 5 of the 8 months, increases in diarrheal symptoms occurred before increases of cholera cases. The increases in symptoms occurred between 1 and 15 days before the increases in cholera cases. Conclusions : The results suggest that the HDSS survey data may be able to detect an increase in cholera before an increase in hospital admissions is seen. However, there was no direct link between diarrheal symptom increases and cholera cases, and this, as well as other methodological weaknesses, should be taken into consideration. The aim of the present study was to investigate the genetic similarity amongst Campylobacter jejuni isolates from children with diarrhea symptoms and broilers in Chiang Mai, Thailand during 2002–2007. Sequence types of 12 isolates from children with symptoms of diarrhea and 9 isolates from broilers and other 42 Thai sequence types were determined by multilocus sequence typing. Cluster analysis was performed to determine the molecular relatedness of the isolates. Results showed diverse sequence types of C. jejuni isolated from both broilers and children. One isolate each from a broiler and a child shared a single sequence type (574). The majority of 13/16 sequence types belonged to previously reported sequence types in Thailand and two previously reported human sequence types were identified in broilers. The authors conclude that there is evidence of a genetic relationship between C. jejuni isolated from humans and broilers. MLST analysis of C. jejuni suggested that broilers are one of the sources of human infection in Chiang Mai, Thailand.

Objective: To analyze the similarities and differences of the applications of “the lung and large intestine being interior-exteriorly related” between the ancient and modern clinical practices, and to reveal the theory meaning. Method: Based on the ancient and modern medical cases database, 245 ancient medical records and 373 modern medical records were studied with the general statistical description and analysis of association rules, and the results were compared to summarize the similarities and differences of the regular pattern of syndrome and treatment on lung-large intestine related diseases in ancient and modern medical records. Results: In modern medical cases, intestinal diseases appeared with the main symptom of constipation due to deficiency of lung qi and qi stagnation of large intestine were always treated by purgation together with replenishing and restoring lung qi. In ancient medical cases, large intestine heat was always caused by lung heat and a variety of diarrhea symptoms appeared. They were always treated by clearing heat and moistening lungs. In addition, the symptom of bound stool caused by qi stagnation of large intestine due to lung qi stagnation was always treated by lowering lung qi to regulate and smooth large intestine qi. And Armeniae Amarum Semen was used by both as a core medicinal herb. Conclusion: Ascending-descending of qi movement is the core of the lung and large intestine relationship. In other words, the lung and the large intestine communicate through qi.

Detectamos un resultado negativo de la medicacion por error en la pauta e indicacion del medicamento para la hipertrigliceridemia. Intervenimos explicandole que Tirodril® es el medicamento que le han prescrito para el hipertiroidismo y no Trialmin®, que esta indicado en hipertrigliceridemias. Se deriva al servicio de urgencias con un informe por escrito de la situacion dirigido al medico. Over the last month and a half, she has had to go to the emergency service several times due to major diarrhea symptoms, nausea and malaise. The doctors diagnosed gastroenteritis, administered intravenous fluid with antiemetics and prescribed paracetamol 1 g every 8 hours. During the consultation, her husband asked to be given Trialmin® 600 mg, which had been “prescribed for [his wife’s] thyroid” at a dose 6 pills per day. We detected a negative drug result stemming from a prescription error and the drug’s indicated use for hypertriglyceridemia. We acted by explaining that Tirodril® is the drug that had been prescribed for hyperthyroidism, rather than Trialmin®, which is indicated for hypertriglyceridemia. Highlights • We identified novel feline norovirus (FNoV) M49-1 strain. • Based on the analysis of VP1, FNoV M49-1 strain was classified into genogroup GVI. • FNoV M49-1 strain seems to be produced by recombination between GIV and GVI NoV. • Cats inoculated with FNoV gene-positive-fecal samples showed clinical symptoms., Norovirus (NoV) has been classified into 6 genogroups, GI-GVI. In the present study, we identified novel feline NoV (FNoV) M49-1 strain. The C-terminal of RNA-dependent RNA polymerase of the FNoV M49-1 strain was highly homologous with GIV FNoV and GIV lion norovirus, whereas VP1 was highly homologous with GVI canine NoV (CNoV). Based on the results of the Simplot analysis, the FNoV M49-1 strain may have been produced by recombination between GIV.2 FNoV and GVI.1 CNoV. In addition, specific pathogen-free cats inoculated with FNoV gene-positive-fecal samples developed diarrhea symptoms, and the viral gene was detected in their feces and blood.

Canine astrovirus (CAstV) is the causative agent of gastroenteritis in dogs. We collected rectal swabs from dogs with or without diarrhea symptoms in Japan and examined the feces for the presence of CAstV by RT-PCR with primers based on a conserved region of the ORF1b gene. The ORF1b gene of CAstV was not detected in the 42 dogs without clinical illness but was present in three pups out of the 31 dogs with diarrhea symptoms. Based on the full-length capsid protein, the CAstV KU-D4-12 strain that we detected in this study shared high homology with the novel virulent CAstV VM-2011 strain.

This pilot study explored predictors of adherence to exercise during and after neoadjuvant chemoradiotherapy (NACRT) in rectal cancer patients. Eighteen rectal cancer patients were prescribed three supervised aerobic exercise sessions/week during NACRT followed by ≥150 min/week of unsupervised aerobic exercise after NACRT. Although not statistically significant, adherence to supervised exercise during NACRT was meaningfully better for patients who were women (d = .82; P = .12), younger (d = -.62; P = .30), married (d = .62; P = .42), with better mental health (r = .32; P = .21), fewer diarrhea symptoms (r = .48; P = .052), and higher anticipated enjoyment (r = .31; P = .23), support (r = .32; P = .22), and motivation (r = .31; P = .23). After NACRT, adherence was significantly better for patients who reported worse mental health (r = -.56; P = .046) and meaningfully better for patients who were women (d = .54; P = .38), better educated (d = .77; P = .22), had no comorbidities (d = -.63; P = .17), and exercised at baseline (d = 1.05; P = .12). Demographics, tumor side effects, and motivational variables may predict adherence to exercise during and after NACRT.

Objective: to compare the health-related quality of life of cancer patients under chemotherapy treatment in Brazil and Spain. Methods: a cross-sectional study, whose data collection took place at the Hospital de Câncer and Hospital Universitário, with socio-demographic characterization instruments and European Organization for Research and Treatment of Cancer. Results: female, white, marital status, grade school students, catholic religion and the unskilled profession predominated in both countries. The scores of health-related quality of life in the health global status and the functions of role performance and cognitive functions were higher in the Brazilian sample, whereas fatigue, pain, dyspnea, insomnia and diarrhea symptoms were more present in the Spanish population, with statistically significant differences. Conclusion: health-related quality of life was better among Brazilians. Objetivo: comparar a qualidade de vida relacionada à saúde de pacientes com câncer em tratamento quimioterápico no Brasil e na Espanha. Métodos: estudo transversal, cuja coleta de dados ocorreu no Hospital de Câncer e Hospital Universitário, com os instrumentos de caracterização sociodemográfica European Organization for Research and Treatment of Cancer and Quality of Life Questionnaire-C30. Resultados: nos dois países predominaram sexo feminino, cor branca, estado civil casado, escolaridade fundamental, religião católica e a profissão não qualificados. Os escores de Qualidade de vida relacionada à saúde no Estado Geral de Saúde e as funções de Desempenho de papel e cognitiva foram maiores na amostra brasileira, enquanto que os sintomas de fadiga, dor, dispneia, insônia e diarreia foram mais presentes na população espanhola, com diferenças estatisticamente significantes. Conclusão: a qualidade de vida relacionada à saúde foi melhor entre os brasileiros.

Spleen Histology and Hematology of Mice Infected by Escherichia coli after Oral Administration of Humic Acid from Borneo PeatABSTRACTHumic acid compounds have an immunostimulatory effect. The aim of this research was to determine the effect of humic acid on the spleen of mice infected with Escherichia coli. The study used a completely randomized design with six treatments and five replicates. The treatments were normal control, negative control, positive control of isoprinosine, humic acid dose of 62.5; 125; and 250 mg/kg body weight (BW). The results showed that E. coli infection caused diarrhea symptoms and significant weight loss. There were significant differences (P0,05) pada berat limpa mencit antar perlakuan, melalui pengamatan histologi ditemukan perbedaan ukuran histologi limpa mencit. Perlakuan asam humat 250 mg/kg BB mempunyai ukuran pulpa putih (495,8 ± 58,2µm) dan perlakuan asam humat 125 mg/kg BB mempunyai nilai leukosit tertinggi (6725 ± 1018 sel/mL). Pada pulpa merah perlakuan kontrol negatif ditemukan banyak sel limfosit yang menggerombol.Kata Kunci: asam humat, Escherichia coli, limpa, pulpa putih, tanah gambut

Visceral hypersensitivity is enhanced in irritable bowel syndrome (IBS) patients. Treatment of IBS visceral pain by moxibustion methods has a long history and rich clinical experience. In the clinic, moxibustion on the Tianshu (ST25) and Shangjuxu (ST37) acupoints can effectively treat bowel disease with visceral pain and diarrhea symptoms. To investigate the regulatory function of moxibustion on the Tianshu (ST25) and Shangjuxu (ST37) acupoints on spinal cord NR1, NR2B, and PKCεprotein and mRNA expression in irritable bowel syndrome (IBS) visceral hypersensitivity rats, we did some research. In the study, we found that moxibustion effectively relieved the IBS visceral hyperalgesia status of rats. Analgesic effect of moxibustion was similar to intrathecal injection of Ro 25-6981. The expression of NR1, NR2B, and PKCεin the spinal dorsal horns of IBS visceral hyperalgesia rats increased. Moxibustion on the Tianshu and Shangjuxu acupoints might inhibit the visceral hypersensitivity, simultaneously decreasing the expression of NR1, NR2B, and PKCεin spinal cord of IBS visceral hyperalgesia rats. Based on the above experimental results, we hypothesized NR1, NR2B, and PKCεof spinal cord could play an important role in moxibustion inhibiting the process of central sensitization and visceral hyperalgesia state.

This paper shows the detection of Diarrhetic Shellfish Poison (DSP) phycotoxins, using HPLC-FLD with pre-column derivatization procedure and HPLC-MS methods, in the analysis of shellfish extracts tested positive with the official DSP mouse bioassay. The shellfish samples were collected in Chiloe Island, Southern of Chile. The amount of Dinophysistoxin-3 (DTX-3) measured in the shellfish extracts were in average above the international safe limits for DSP content in the shellfish extracts analyzed. As internal control of detection and recovery, DTX-1 analytical standard was spiked into dichloromethane-clean shellfish extracts in order to calculate de extraction recovery of DTX-1. The average recovery was 97%. From all DSP toxins analyzed, the hydrolyzed extract samples appeared mainly DTX-3 in concentrations ranging from 99.40 +/- 1.22 to 257.73 +/- 12.46 ng/g digestive-glands. The acyl-Okadaic Acid (acyl-OA) was also detected in some samples, ranging from 1.02 +/- 1.4 to 3.07. +/- 1.6 ng of DSP toxin/g digestive-glands. This is the first report of acyl-OA ever found in Chilean shellfish samples. This data shows that shellfish samples were contaminated with a complex DSP toxins profile, in which DTX-3 is the major DSP toxin component, followed by DTX-1 and the acyl-OA as the minor one. The important findings showed in this study are the presence of both acyl-derivates (DTX-3 and Acyl-OA) which are the product of a main metabolic biotransformation that occurred inside the shellfish, in order to chelate DTX-1 and OA, transforming them into DTX-3 and the acyl-OA respectively. This metabolic biotransformation must be performed to avoid self-inhibition of their Protein Phosphatase 2A done by DTX-1 and OA, since both acyl-derivates (DTX-3 and acyl-OA) do not inhibit Protein Phosphatase 2A. This complex DSP toxins profile and the permanent presence of both acyl-derivates (DTX-3 and Acyl-OA) could explain the permanent diarrhea symptoms that experience patients who have ingested cooked shellfish in the southern of Chile. This diarrhea is not associated to Vibrio parahaemolyticus or other enteropathogens as had been suggested before. The massive shellfish consumption is an important Chilean cultural habit and now has become a major health issue in the southern of Chile.

A survey was carried out in a hospital of Nova Iguaçu, Rio de Janeiro, Brazil, in the period from July 1999 to March 2002, to determine the infection rate of Listeriamonocytogenes in HIV+ patients with diarrhea symptoms; 134 samples were processed by microbiological methods. The results demonstrated 12.68% of positive samples. However, no statistical differences were observed for age or sex in the studied group, suggesting that this microorganism should be regarded in the differential diagnosis of infectious processes in HIV+ patients in the area.

Purpose : Since children under two years with suspected urinary tract infections (UTIs) cannot control urination, urine cultures in such children are usually performed via urine bags. This method is noninvasive but has a high contamination rate. We studied the contamination rate of bag urine culture in diagnosing UTI in infants under two years and the factors responsible for contamination. Methods : We examined patients under 2 years in whom urine culture through the urine bag method yielded over 105 colonies of a single pathogen. We defined UTI by referring to the guidelines of The Korean Society of Pediatric Nephrology, 2005. We examined the factors responsible for contamination according to sex, duration of urine collection, and whether diarrhea took place with contamination rate. Results : We examined 717 patients (412 males and 305 females). The contamination rate of one bag urine culture was 37.9%. Gender was not related to the contamination rate (P>0.05). Duration of urine collection showed an association with the contamination rate. The longer the duration of collecting urine, the higher was the contamination rate. Duration of urine collection was divided into three groups: first group, 02 hours; second group, 24 hours; and third group, >4 hours. Contamination rates were 30.0%, 42.2%, and 43.7% for the first, second, and third groups, respectively, with statistical significance (P=0.001). Diarrhea at admission had no impact on the contamination rate (P>0.05). Conclusion : The contamination rate of urine culture in the examined patients was 37.9%. Gender and diarrhea symptoms were not responsible for contamination. In infants with a suspected UTI, urine should be collected within 2 hours through the urine bag method. If urine collection takes >2 hours, the urine bag should be resterilized and reattached to the patient.
