Objetivo: Estimar el impacto presupuestario de un programa de deshabituación tabáquica en la empresa. Métodos: se desarrolló un modelo para simular una empresa de 1000 trabajadores, con un horizonte temporal de 3 años. Se consideró que la prevalencia media de fumadores era del 26% y que el 70% estaría dispuesto a dejar de fumar (datos nacionales publicados). Las tasas de cesación a un año se obtuvieron de ensayos clínicos (21% para vareniclina, 13% para bupropion, 12% para TSN y 7% para consejo/seguimiento) y se consideró que el 25% de los fumadores recibirían cada una de estas terapias. Se incluyeron los costes derivados del programa (tratamientos y puesta en marcha/seguimiento) y los ahorros producidos por la mejoría en el absentismo y la productividad laboral. Resultados: La inversión que tendría que realizar la empresa para poner en marcha este programa (54.794€) se recuperaría prácticamente en un año. Tras tres años, los ahorros totales para la empresa llegarían a los 85.445€; 4.092€ por cada trabajador que consigue dejar de fumar. Conclusiones: La implementación de un programa de deshabituación tabáquica en empresa reportaría no solo beneficios en salud sino importantes beneficios económicos a partir del primer año.Objective: To estimate the budgetary impact of a smoking cessation programme in a company. Methods: A model was developed to simulate a 1000-employee company, with a time horizon of 3 years. 26% of smoking prevalence was considered; 70% of smokers would be willing to quit (published national data). The smoking cessation rates where obtained from clinical trials (21% for varenicline, 13% for bupropion, 12% for NST and 7% for advice/counselling) and it was considered that the 25% of the smokers would take each of the therapies. The costs derived from the programme (treatment and initiation/counselling) and the savings due to the improvement in the work absenteeism and productivity were included. Results: The investment that the company would need for starting up the program (€54,794) would be almost recovered in one year. After the third year, the total savings for the company would rise to €85,445; €4,092 for each employee who achieves to quit smoking. Conclusions: The implementation of a smoking cessation programme would result in health benefits for the employees as well as savings for the company after the first year.

Introduction: The efficacy of galcanezumab has been demonstrated in randomized controlled trials, but evidence about its use under clinical practice conditions is still limited. This study aimed to describe the characteristics of the patients treated with galcanezumab in routine clinical practice in Spain as well as treatment patterns, persistence, and effectiveness., Methods: A retrospective chart review study was carried out in six hospitals. Information of adults with migraine, who started treatment with galcanezumab between November 2019 and September 2021, was analyzed until end or loss of follow-up. Continuous variables were described as mean (standard deviation, SD) and median (interquartile range, IQR), and categorical variables as frequency and percentages. Persistence to treatment was estimated using Kaplan-Meier analysis., Results: A total of 314 patients were analyzed over median follow-up period of 17.5 months (13.8-20.7), with a mean age of 46.3 (12.6), 85% women, 80.6% chronic migraine, and reporting a mean of monthly migraine days of 16.7 (7.8). Overall, 72.9% had comorbid conditions, with anxiety and depression disorders being the most frequent. More than 60% had received ≥ 6 previous preventive drugs, the most common being antiepileptics, antidepressants, and botulinum toxin (95.2%, 89.8% and 84.1%, respectively). Overall, 60.3% of the patients with other preventive treatments maintained them after galcanezumab initiation. The median time on galcanezumab was 14.6 months (9.4-22.8); 95.7%, 82.0%, 76.2% and 59.8% of patients were persistent to treatment at 3, 6, 9 and 12 months, respectively. Of the patients who discontinued (151: 48.1%), 57.6% were due to lack of effectiveness and 31.1% were due to improvement in migraine. The average reduction of monthly migraine days at 3, 6, 9 and 12 months was 7.9 (7.2), 9.1 (7.5), 8.8 (6.6) and 9.0 (6.9) days, respectively., Conclusions: In real clinical practice, galcanezumab is an effective treatment and has a high persistence in patients with migraine, mostly chronic and with multiple use of previous preventive treatments., (© 2024. Background: An estimated 17.2% of patients continue to smoke following diagnosis of cardiovascular disease (CVD). To reduce the risk of further morbidity or mortality in cardiovascular patients, smoking cessation has been shown to reduce the risk of mortality by 36% and myocardial infarction by 32%. The objective of this study was to evaluate the long-term health and economic consequences of smoking cessation in patients with CVD., Design and Methods: Results of a randomized clinical trial comparing varenicline plus counselling vs. placebo plus counselling were extrapolated using a Markov model to simulate the lifetime costs and health consequences of smoking cessation in patients with stable CVD. For the base case, we considered a payer's perspective including direct costs attributed to the healthcare provider, measuring cumulative life years (LY) and quality adjusted life (QALY) years as outcome measures. Secondary analyses were conducted from a societal perspective, evaluating lost productivity due to premature mortality. Sensitivity and subgroup analyses were also undertaken. Results were analysed for Belgium, Spain, Portugal, and Italy., Results: Varenicline plus counselling was associated with a gain in LY and QALY across all countries; relative to placebo plus counselling. From a payer's perspective, incremental cost effectiveness ratios were € 6120 (Belgium), € 5151 (Spain), € 5357 (Portugal), and € 5433 (Italy) per QALY gained. From a societal perspective, varenicline in addition to counselling was less costly than placebo and counselling in all cases. Sensitivity analyses showed little sensitivity in outcomes to model assumptions or uncertainty in model parameters., Conclusions: Varenicline in addition to counselling is cost-effective compared to placebo and counselling in smokers with CVD.
