Abstract

Age-related macular degeneration (AMD) is the most common cause of blindness among the elderly in the developed world. While treatment is effective for the neovascular or "wet" form of AMD, no therapy is successful for the non-neovascular or "dry" form. Here we discuss the current knowledge on dry AMD pathobiology and propose future research directions that would expedite the development of new treatments. In our view, these should emphasize system biology approaches that integrate omic, pharmacological, and clinical data into mathematical models that can predict disease onset and progression, identify biomarkers, establish disease causing mechanisms, and monitor response to therapy.

Abstract

Purpose: To document the long-term visual outcomes in patients with Blau syndrome. Methods: A retrospective institutional cohort study was conducted, and 13 patients with genetically confirmed Blau syndrome were included. Demographic and clinical data were collected from standardised medical charts. Baseline was defined as the first detected uveitis and data were recorded onwards at intervals of 1, 3, 5, 10, 15 and 20 years. Results: Anterior uveitis was the most common classification at baseline (57.1%). Among patients with documented uveitis lasting 10 years or more, all of them developed panuveitis. Median logMAR visual acuity at baseline was 0 (range −0.5; 0.7), 0.19 (range 0; 1.5) at year 5, and 0.7 (range 0.1 – no perception of light) at year 20, as recorded in 13, 16, and 10 eyes, respectively. All patients received treatment with topical and oral steroids, and multiple systemic immunosuppressants including biologics. Disease control, defined as having cells <1+ in both eyes and using topical steroid eye drops less than twice daily, was achieved in 14.3% to 37.5% of patients at the different time points. Cataract surgery was performed in 12 eyes of 8 patients, 3 eyes of 3 patients necessitated glaucoma surgery, and 4 eyes of 4 patients required surgery for retinal detachment. Conclusion: Uveitis associated with Blau syndrome commonly leads to severe, chronic panuveitis, requiring long-term systemic immunosuppression. 18. Limited Utility of Keratic Precipitate Morphology as an Indicator of Underlying Diagnosis in Ocular Inflammation.

Abstract

Objectives: We aimed to establish the degree of consensus among clinicians on descriptors of KP morphology. Methods: A web-based exercise in which respondents associated KP descriptors, as identified through a scoping review of the published literature, to images from different disorders. Inter-observer agreement was assessed using the Krippendorff kappa alpha metric. Results: Of the 76 descriptive terms identified by the scoping review, the most used included "mutton-fat" (n = 93 articles, 36%), "fine/dust" (n = 76, 29%), "stellate" (n = 40, 15%), "large" (n = 33, 12%), and "medium" (n = 33, 12%). The survey of specialists (n = 26) identified inter-observer agreement for these descriptors to be poor ("stellate," kappa: 0.15, 95% confidence interval 0.13–0.17), limited ("medium": 0.27, 95% CI 0.25–0.29; "dust/fine": 0.36, 95% CI 0.34–0.37), or moderate ("mutton fat": 0.40, 95% CI 0.36–0.43; "large": 0.43, 95% CI 0.39–0.46). 25. The structural and functional characterisation of human Cdc7 kinase

Abstract

Cell division cycle 7-related Ser/Thr (Cdc7) kinase is conserved and essential across eukaryotes. When bound to its activator, dumbbell-forming factor 4 (Dbf4) it phosphorylates a number of target proteins involved in various aspects of the cell cycle, including replication initiation, meiosis, the intra S-phase checkpoint, the DNA damage response and mitotic exit. Cdc7 is overexpressed in a number of cancers and expression correlates with patient prognosis. Selective inhibition of Cdc7 leads to cell death through an aberrant S-phase in transformed cells, while healthy fibroblasts are able to survive such treatments. Therefore, Cdc7 is an attractive target for cancer therapeutics, and high-resolution structural information could be very informative for the development of small molecule inhibitors. Herein, I present crystal structures of a fully active Cdc7-Dbf4 heterodimeric construct bound to a potent Cdc7 inhibitor, a non-hydrolysable ATP analogue and an Mcm2-derived substrate peptide. These structures, refined to a high resolution, reveal a previously unseen Zn-binding domain that supports a fully open conformation of the active site. The structures also reveal features required for substrate binding and phosphorylation. In vitro assays have been employed to validate the functional significance of these features, and an in cellula system developed to investigate their importance in the cell. The new structural and functional information gained from this study will inform the design of small molecule inhibitors of Cdc7 while the cell-based system opens up new ways of addressing the functions of the unique kinase insert sequences of Cdc7.

Abstract

BACKGROUND: Posttraumatic stress injury (PTSI) is a term used to describe a range of psychiatric difficulties which arise following exposure to a psychologically traumatic event. The impact of being diagnosed with multiple psychiatric conditions on the return-to-work (RTW) outcomes of individuals with PTSI has not been adequately researched. OBJECTIVE: The current study examined whether the presence of two or more psychiatric conditions occurring simultaneously is predictive of RTW outcomes in workers with PTSI. METHOD: A population-based cohort design was conducted using archival data from injured workers admitted to a PTSI rehabilitation program. Differences in RTW outcomes and demographic, administrative, and clinical variables were compared between individuals with single and multiple psychiatric diagnoses. A range of variables were entered into a multivariable logistic regression model predicting RTW. RESULTS: The final logistic regression model indicated workers had higher odds of RTW if they had a single psychiatric diagnosis (Adjusted Odds Ratio (AOR) 2.20), non-elevated scores on a measure of traumatic stress (AOR 1.85), and reported higher self-perceived readiness to RTW (AOR 1.24). CONCLUSION: Being diagnosed with multiple psychiatric conditions appears to be associated with more negative RTW outcomes following PTSI rehabilitation. [ABSTRACT FROM AUTHOR]
