Abstract

In a study conducted in Singapore, a country prone to dengue outbreaks due to its climate and urban population, researchers examined the effectiveness of releasing male Aedes aegypti mosquitoes infected with Wolbachia (wAlbB strain) to reduce dengue transmission. These infected males, when mating with wild-type females, produced non-viable eggs, leading to vector suppression. Extensive field trials involving over 600,000 residents in four townships were conducted from 2018 to 2022. The results showed a 57% decline in total dengue incidence and a 64% decline in clustered dengue incidence. This approach offers promise for large-scale dengue control in regions facing rising dengue cases, providing a critical solution in combating the disease.

Abstract

The success of any vector control strategy can be enhanced by onsite analysis and investigation. Combatting malaria, a global disease carried by the vector Anopheles gambiae, has led to the development of novel genetic technologies such as the use of HEG; homing endonuclease genes. This thesis explored the age and stage elements of the vector, building upon current biological understanding and using fitting algorithms with metapopulation matrices to create cohort orientated survival and transition. The environmental forces were analysed alongside this with emphasis on sub-model creation and tool design, employing an array of methods from RBF to satellite classification to couple the local environment and vector. When added, the four potential genetic strategies all demonstrated the ability to suppress a wild type population and even eradicate it, although reinvasion and hotspot population phenomena were reoccurring observations. The movement of the vector was an important factor in control efficiency, which was investigated as a series of different assumptions using wind driven movement and host attraction. Lastly, practical factors such as monitoring and resource distribution within a control project were assessed, which required routing solutions and landscape trapping assessments. This was explored within a framework of Mark-Release-Recapture experiment design that could provide critical information for efficient HEG release strategies.

Abstract

This article presents the findings of a study conducted in Singapore that examined the use of oral antivirals (OAVs) among older adults with COVID-19 in primary care during the Omicron waves. The study revealed that a small percentage of older adults received OAVs, with nirmatrelvir-ritonavir being the most commonly prescribed medication. The study also identified factors such as primary care characteristics and socioeconomic status (SES) indicators that influenced the likelihood of receiving OAVs. The authors emphasize the importance of addressing disparities in OAV access among older adults and socioeconomically disadvantaged groups. Abstract

Transmission potential of a pathogen, often quantified by the time-varying reproduction number Rt, provides the current pace of infection for a disease and indicates whether an emerging epidemic is under control. In this study, we proposed a novel method, EpiMix, for Rt estimation, wherein we incorporated the impacts of exogenous factors and random effects under a Bayesian regression framework. Using Integrated Nested Laplace Approximation, EpiMix is able to efficiently generate reliable, deterministic Rt estimates. In the simulations and case studies performed, we further demonstrated the method's robustness in low-incidence scenarios, together with other merits, including its flexibility in selecting variables and tolerance of varying reporting rates. All these make EpiMix a potentially useful tool for real-time Rt estimation provided that the serial interval distribution, time series of case counts and external influencing factors are available. Abstract

Background: A pertinent risk factor of upper respiratory tract infections (URTIs) and pneumonia is the exposure to major ambient air pollutants, with short term exposures to different air pollutants being shown to exacerbate several respiratory conditions. Methods: Here, using disease surveillance data comprising of reported disease case counts at the province level, high frequency ambient air pollutant and climate data in Thailand, we delineated the association between ambient air pollution and URTI/Pneumonia burden in Thailand from 2000 – 2022. We developed mixed-data sampling methods and estimation strategies to account for the high frequency nature of ambient air pollutant concentration data. This was used to evaluate the effects past concentrations of fine particulate matter (PM2.5), sulphur dioxide (SO2), and carbon monoxide (CO) and the number of disease case count, after controlling for the confounding meteorological and disease factors. Results: Across provinces, we found that past increases in CO, SO2, and PM2.5 concentration were associated to changes in URTI and pneumonia case counts, but the direction of their association mixed. The contributive burden of past ambient air pollutants on contemporaneous disease burden was also found to be larger than meteorological factors, and comparable to that of disease related factors. Conclusions: By developing a novel statistical methodology, we prevented subjective variable selection and discretization bias to detect associations, and provided a robust estimate on the effect of ambient air pollutants on URTI and pneumonia burden over a large spatial scale.
