INTRODUCTION: Frailty predicts functional decline and could be associated with adverse outcomes such as disability, multiple hospitalizations, falls, loss of mobility, and cardiovascular disease. In Colombia 12.5% of prevalence has been reported. In the present study, the different clinical variables associated with frailty were evaluated in a population of hospitalized patients older than 65 years in Bucaramanga, Colombia, in order to predict the behavior of these variables to generate measurement tools of greater applicability than that of currently existing tools. METHODS: An analytical observational cross-sectional study with nonprobabilistic sampling was conducted from January 2016 to June 2017 in patients older than 65 years of follow-up > 48 hours by the internal medicine service. Fried criteria were used to evaluate patients on their last day of hospitalization. RESULTS: A total of 155 patients were included, of whom 60.6% were frail. A combined analysis of the variables that showed association with frailty revealed that a calf circumference lower than or equal to 31 cm, a gait speed lower than or equal to 0.8 m/s, and age above 75 years were associated with frailty. It was also shown that being male and having a BMI > 27 kg/m2 are protective factors for frailty. CONCLUSIONS: The prevalence of frailty in hospitalized older adults was higher than that reported in local studies for the community population. According to multivariate analysis, the variables, when analyzed together, have a predictive ability of 92% to estimate frailty in hospitalized patients.

We report the feasibility of a combined approach of very low low tidal volume (VT) and mild therapeutic hypothermia (MTH) to decrease the ventilatory load in a severe COVID-19-related acute respiratory distress syndrome (ARDS) cohort. Inclusion criteria was patients ≥18-years-old, severe COVID-19-related ARDS, driving pressure ∆P >15 cmH2O despite low-VT strategy, and extracorporeal therapies not available. MTH was induced with a surface cooling device aiming at 34°C. MTH was maintained for 72 h, followed by rewarming of 1°C per day. Data were shown in median (interquartile range, 25%–75%). Mixed effects analysis and Dunnett’s test were used for comparisons. Seven patients were reported. Ventilatory load decreased during the first 24 h, minute ventilation (VE) decreased from 173 (170–192) to 152 (137–170) mL/kg/min (P = 0.007), and mechanical power (MP) decreased from 37 (31–40) to 29 (26–34) J/min (P = 0.03). At the end of the MTH period, the VT, P, and plateau pressure remained consistently close to 3.9 mL/kg predicted body weight, 12 and 26 cmH2O, respectively. A combined strategy of MTH and ultraprotective mechanical ventilation (MV) decreased VE and MP in severe COVID-19-related ARDS. The decreasing of ventilatory load may allow maintaining MV within safety thresholds.

Purpose: This study investigated the relationship between different pain levels in the affected arm and health status in long-term breast cancer survivors (LTBCSs) and identified predictors of pain at this stage of long-term survivorship. Methods: A cross-sectional study of 80 participants categorized LTBCSs by pain levels in the affected arm into three groups: no pain (0–0.99), mild pain (1–3.99), and moderate to severe pain (4–10). Variables assessed at least 5 years since diagnosis include pain in the non-affected arm, pain interference, cancer-related fatigue (CRF), physical activity (PA) level, fitness condition, mood state, and health-related quality of life (HRQoL). Results: A total of 36.25% of LTBCSs have no pain, 30% have mild pain, and 33.75% have moderate to severe pain. Furthermore, pain presence was associated with increased pain in the non-affected arm, pain interference, CRF, mood disturbances, and physical inactivity, as well as a decreased HRQoL (all p < 0.05). Regression analysis found “upset by hair loss”, CRF “affective domain”, “dyspnea”, and “alcohol consumption” as significant predictors of higher levels of pain in the affected arm (r2 adjusted = 0.646). Conclusions: A total of 63.75% of LTBCSs continue to experience mild to moderate to severe pain in the affected arm, negatively impacting their physical, mental, and emotional health status, with increased pain severity ≥5 years beyond cancer diagnosis. “Upset by hair loss”, CRF “affective domain”, “dyspnea”, and alcohol consumption collectively explain 64.6% of the affected-arm pain level in LTBCSs.

Background: Foot morphology in children is a crucial factor influencing multiple aspects of their physical development. Between the ages of 5 and 10 years, the critical period of child development is when the movement and stability patterns are consolidated that can affect their long-term physical performance and quality of life. The aim of this study is to analyze how the type of foot influences different physical characteristics, laxity, strength, motor tests, and baropodometric variables in children aged 5 to 10 years. Methods: A cross-sectional study involving 196 children was conducted. Different physical characteristics, laxity, strength, motor tests, and baropodometric variables of the sample were analyzed for age and Foot Posture Index (FPI). Results: Differences in all variables were examined by age and FPI. Statistical analysis showed a moderate to high correlation (r > 0.6, p < 0.01) between FPI and the relaxed calcaneal stance position (RCSP) test. Some significant differences were also found in variables related to foot pronation and supination. These results provide valuable information for understanding differences in motor and functional development during childhood and pre-adolescence. Conclusions: The findings highlight the variability in physical and functional development between age and foot type groups, highlighting the importance of considering these differences in the assessment and management of foot-related conditions and biomechanics in childhood. Foot type significantly influences children’s growth and development.

La migración afecta tanto a los residentes del país receptor como a los migrantes que intentan mejorar su calidad de vida. Una de las consecuencias es la explotación laboral por la que se atraviesa bajo las leyes de un Estado o la informalidad de este. Los trabajadores explotados no suelen gozar de beneficios ni protecciones sociales. En el Perú, la explotación laboral es casi común en los inmigrantes venezolanos que desempeñan algún trabajo. Son mal remunerados y hasta discriminados. El objetivo de este artículo es analizar el discurso de la explotación laboral por la que pasan los ciudadanos venezolanos en Lima-Perú. Este es un estudio cualitativo. Recurre a la literatura empírica y las narraciones producto de una serie de entrevistas semiestructuradas realizadas a ciudadanos venezolanos. El trabajo concluye que en el discurso de los migrantes son cuatro las causas principales que sostienen el clima de explotación. Finalmente, se indica que, en contextos de migración, todo tipo de explotación debe evitarse y pensar en soluciones diversas y amplias para el migrante.

Background: Idiopathic central diabetes insipidus (DI) is a rare endocrine disorder that results from total or partial deficiency of vasopressin secretion. It is idiopathic when the cause is unknown, but in many cases, is associated with autoimmune disorders. Case presentation: We present the case of a 44-year-old male with vitiligo and a family history of diabetes mellitus and thyroid disease. The patient presented with polydipsia and polyuria greater than 8 L/day. After water deprivation test, the patient was diagnosed with partial central diabetes insipidus. Contrast-enhanced pituitary magnetic resonance imaging showed decreased brightness of the neurohypophysis and normal thickness of the pituitary stalk. Because desmopressin was not initially available, the patient was managed with chlorpropamide, carbamazepine, and hydrochlorothiazide, and afterwards substituted. During his outpatient checkups, he presented many episodes of polyuria, the last after 13 years, with polyuria of up to 15 L associated with weight loss, and abnormal blood glucose levels; anti-GAD 65 and IA-2 antibodies were negative. He was subsequently diagnosed with diabetes mellitus and received metformin and insulin; this latter was suspended in subsequent check-ups due to hypoglycemic episodes. Conclusion: We highlight the importance of treatment and adequate control of these pathologies, since they share similar clinical manifestations, can easily have electrolyte imbalance and represent a challenge for endocrinologists and internists.

Los páramos almacenan grandes reservas de carbono orgánico en el suelo (COS), influenciados por las condiciones climáticas y biogeoquímicas, propias del ecosistema; sin embargo, su progresiva conversión hacia cultivos, ganadería o minería incide directamente en las reservas de COS. Con el fin de determinar el efecto que ejerce el cambio de uso de suelo sobre la variabilidad de las reservas de COS, se realizó un monitoreo de COS entre 2013, 2018 y 2020, en el Parque Natural Regional Cortadera. Se estudiaron parcelas permanentes de muestreo (PPM), ubicadas bajo tres diferentes usos del suelo: conservado, en recuperación e intervenido. Se analizaron muestras de carbono orgánico y densidad aparente, a dos profundidades (0-15 cm y 15-30 cm). Se encontró que la PPM en escenario conservado presentó los contenidos más altos de COS, con valores de 290,37; 199,22 y 257,5 tC ha-1, para cada uno de los años en estudio; seguido por la PPM en recuperación, que evidenció valores de COS 215,3 tC ha-1, en el 2020, en contraste con la PPM intervenida, que presentó contenidos mínimos de 15,50; 34,01 y 88,06 tC ha-1. Se observó que los mayores contenidos de COS se encuentran a la profundidad de 15 a 30 cm. Dichos aspectos, resaltan la importancia de avanzar en acciones enfocadas a la protección de ecosistemas estratégicos, considerando las constantes amenazas relacionadas con la transformación del paisaje y, con ello, la posibilidad de proveer funciones y servicios asociados a la captura de carbono y la regulación climática.

Abstract Physical exercise can produce changes in the microbiota, conferring health benefits through mechanisms that are not fully understood. We sought to determine the changes driven by exercise on the gut microbiota and on the serum and fecal metabolome using 16S rRNA gene analysis and untargeted metabolomics. A total of 85 serum and 12 fecal metabolites and six bacterial taxa (Romboutsia, Escherichia coli TOP498, Ruminococcaceae UCG-005, Blautia, Ruminiclostridium 9 and Clostridium phoceensis) were modified following a controlled acute exercise session. Among the bacterial taxa, Ruminiclostridium 9 was the most influenced by fecal and serum metabolites, as revealed by linear multivariate regression analysis. Exercise significantly increased the fecal ammonia content. Functional analysis revealed that alanine, aspartate and glutamate metabolism and the arginine and aminoacyl-tRNA biosynthesis pathways were the most relevant modified pathways in serum, whereas the phenylalanine, tyrosine and tryptophan biosynthesis pathway was the most relevant pathway modified in feces. Correlation analysis between fecal and serum metabolites suggested an exchange of metabolites between both compartments. Thus, the performance of a single exercise bout in cross-country non-professional athletes produces significant changes in the microbiota and in the serum and fecal metabolome, which may have health implications.

To date, there is a lack of information about the optimal conditions of the warm-up to lead to a better performance in elite tennis players. The aim of this study was to compare the effects of two different warm-up protocols (dynamic vs. self-myofascial release with foam rolling) on neuromuscular variables associated with physical determinants of tennis performance. Using a crossover randomised experimental design, eleven professional men tennis players (20.6 ± 3.5 years) performed either a dynamic warm-up (DWU) or a selfmyofascial release with foam rolling (SMFR) protocol. DWU consisted of 8 min of dynamic exercises at increasing intensity and SMFR consisted of 8 min of rolling on each lower extremity unilaterally. Just before (baseline) and after completing warm-up protocols, players performed a countermovement jump (CMJ), the 5-0-5 agility test, a 10-m sprint test and the Straight Leg Raise and Thomas tests to assess range of motion. Compared to baseline, the DWU was more effective to reduce the time in the 5-0-5 test than SMFR (-2.23 vs. 0.44%, respectively, p = 0.042, ηp2 = 0.19). However, both warm-up protocols similarly affected CMJ (2.32 vs. 0.61%, p = 0.373, ηp2 = 0.04) and 10-m sprint time changes (-1.26 vs. 1.03%, p = 0.124, ηp2 = 0.11). Changes in range of motion tests were also similar with both protocols (p = 0.448–1.000, ηp2 = 0.00–0.02). Overall, both DWU and SMFR were effective to prepare well-trained tennis players for highly demanding neuromuscular actions. However, DWU offered a better preparation for performing change of direction and sprint actions, and hence, in high-performance tennis players, the warm-up should include dynamic exercises.

Does the analysis of social media reactions allow us to study interactions between territories? We tackle this issue by discussing the results of an investigation on tweets reacting to the attack of 14th July 2016 at Nice, aiming to show territorial scientific uses of Twitter data, their limits and benefits. This study falls within the current emerging domain of digital tracks in geography. It foreshadows future interesting works in a context of evolution of social media data, uses and users.

Training with real patients is a critical aspect of the learning and growth of doctors in training. However, this essential step in the educational process for clinicians can potentially compromise patient safety, as they may not be adequately prepared to handle real-life situations independently. Clinical simulators help to solve this problem by providing real-world scenarios in which the physicians can train and gain confidence by safely and repeatedly practicing different techniques. In addition, obtaining objective feedback allows subsequent debriefing by analysing the situation experienced and learning from other people’s mistakes. This article presents SIMUNEO, a neonatal simulator in which professionals are able to learn by practicing the management of lung ultrasound and the resolution of pneumothorax and thoracic effusions. The article also discusses in detail the hardware and software, the main components that compose the system, and the communication and implementation of these. The system was validated through both usability questionnaires filled out by neonatology residents as well as through follow-up sessions, improvement, and control of the system with specialists of the department. Results suggest that the environment is easy to use and could be used in clinical practice to improve the learning and training of students as well as the safety of patients.

A partir de la biología teórica de Jakob Von Uexküll, el artículo expone cómo una protoetología constituye la condición de posibilidad del diseño de trampas y de todas las prácticas de cacería; en qué sentido la liberación del comportamiento garantizan la fidelidad de la investigación etológica aplicada a los hábitos de consumo; que el concepto de cibernética exhibe una lógica semiorgánica que permite aproximarse a ciertos objetos digitales como si fueran agentes más que herramientas; y la idea de una etología de la máquina como modo de acceso al comportamiento de los algoritmos.

The aim of this work was to study effects of ketotifen fumarate (KF) on prevention of tissue damage in testes of rats with experimental autoimmune orchitis (EAO) and on the contralateral testis in a model of prolonged testicular cord torsion (TCT). Rats with EAO or TCT were injected intraperitoneally once daily with KF or saline solution (vehicle group). Incidence and severity of testicular damage were evaluated by histopathology using an EAO score or a Johnsen score. Mast cells (MC) were identified by histochemistry and quantified. In EAO model, KF significantly reduced severity of histopathological testicular damage compared to rats in the vehicle group. KF also reduced the number of testicular MC compared to vehicle group. Similarly, in TCT model, multifocal damage of the contralateral testis was observed 30 days after testicular torsion characterized by sloughing of the germinal epithelium, seminiferous tubule atrophy, and interstitial edema. Focal signs of inflammation and fibrosis of seminiferous tubular walls were also observed. In contrast, sections of contralateral testis of rats injected with KF and killed 30 days after surgery showed normal histological features. A significant decrease in the number of MC was observed in rats treated with KF compared to untreated animals. In conclusion, we demonstrated that treatment with KF reduced testicular inflammatory process and MC infiltrates in both EAO and TCT models. The results suggest a promising treatment for infertile male patients with testicular pathologies associated with inflammation and germ cell loss.

ABSTRACT Hiccups are involuntary, spasmodic contractions of the diaphragm and intercostal muscles and can be classified as acute (< 48 h), persistent (48 h to a month) or intractable (> 1 month). A previously healthy 29-year-old man sought the Emergency Department with flu-like symptoms and a two-day history of persistent hiccups. His physical examination was otherwise unremarkable and vital signs were within normal limits. An unenhanced computed tomography scan of the chest showed small focal ground-glass opacities scattered throughout 25% of the lungs. A COVID-19 test was positive. Chlorpromazine was prescribed for the hiccups with improvement over 10 h. The patient was discharged home on the same day without hiccups and no other complications.

The potential role of cocoa supplementation in an exercise context remains unclear. We describe the effects of flavanol-rich cocoa supplementation during training on exercise performance and mitochondrial biogenesis. Forty-two male endurance athletes at the beginning of the training season received either 5 g of cocoa (425 mg of flavanols) or maltodextrin (control) daily for 10 weeks. Two different doses of cocoa (equivalent to 5 g and 15 g per day of cocoa for a 70 kg person) were tested in a mouse exercise training study. In the athletes, while both groups had improved exercise performance, the maximal aerobic speed increased only in the control group. A mitochondrial DNA analysis revealed that the control group responded to training by increasing the mitochondrial load whereas the cocoa group showed no increase. Oxidative stress was lower in the cocoa group than in the control group, together with lower interleukin-6 levels. In the muscle of mice receiving cocoa, we corroborated an inhibition of mitochondrial biogenesis, which might be mediated by the decrease in the expression of nuclear factor erythroid-2-related factor 2. Our study shows that supplementation with flavanol-rich cocoa during the training period inhibits mitochondrial biogenesis adaptation through the inhibition of reactive oxygen species generation without impacting exercise performance.

A devastating mass mortality event (MME) very likely caused by the protozoan Haplosporidium pinnae first detected in 2016 in the Western Mediterranean Sea, is pushing the endemic bivalve Pinna nobilis to near extinction. Populations recovery, if possible, will rely on larval dispersal from unaffected sites and potential recolonization through recruitment of resistant juveniles. To assess the impact of the MME on the species’ larval recruitment, an unprecedented network of larval collector stations was implemented over several thousands of kilometers along the Western Mediterranean coasts during the 3 years after the onset of the MME. The findings of this network showed a generalized disruption in recruitment with dramatic consequences for the recovery of the species. However, there were exceptions to this pattern and recruits were recorded in a few sites where the resident population had been decimated. This hints to the importance of unaffected populations as larval exporting sources and the role of oceanographic currents in larval transport in the area, representing a beacon of hope in the current extremely worrying scenario for this emblematic species.

BackgroundThere is an increasing interest in the use of eccentric muscle exercise to improve physical condition, especially with regards to its health-related benefits. However, it is known that unaccustomed eccentric exercise causes muscle damage and delayed pain, commonly defined as “delayed onset muscle soreness” (DOMS). The efficacy of herbal preparations in subjects suffering from DOMS has been reported in a few previous studies with small or moderate outcome measures related to muscle recovery. The present study aimed to evaluate the effects of a polyherbal mixture containing whole Zingiber officinale Roscoe and Bixa orellana L., powders called ReWin(d), in young male athletes suffering from DOMS induced by a 1 h session of plyometric exercises.MethodsThirty-three young male athletes participated in this randomized, Triple-blind, placebo-controlled trial: 17 of them assigned to the ReWin(d) group and 16 of them to the placebo group. Creatine kinase (CK) was measured as a muscle damage marker, pain was assessed using the Visual Analog Scale (VAS), muscle performance was measured through half-squat exercise (HS) monitored with an accelerometer (Encoder), and heart rate variability (HRV) was monitored for 5 min with the subjects in the supine position. All determinations were performed before and after the eccentric session and 24, 48, and 72 h after the session.ResultsThe eccentric exercise session caused an increase in CK at 24 and 48 h after exercise intervention in both groups (p < 0.001). There was no interaction between groups regarding muscle damage. The pain increased after the training session in both groups (p < 0.001), and a significant interaction was observed between groups at 48 h after exercise (p = 0.004). Lower limb muscular power showed a significant interaction between groups 24 h after exercise (p = 0.049); the placebo group showed a reduction in muscle power compared to the ReWin(d) group. The LF/HF ratio decreased significantly at 72 h after exercise in the herbal group but not in the placebo group.ConclusionThe herbal supplement maintained the maximum power of the lower limbs and attenuated muscle pain.Clinical Trial Registrationwww.ClinicalTrials.gov, identifier NCT03961022.

La región andina colombiana, en el departamento de Boyacá a 2560 msnm en el cordón lechero colombiano, las explotaciones ganaderas se caracterizan por tener la especie forrajera perenne predominante conocida como Kikuyo (Pennisetum clandestinum Hochst.ex Chiov), gramínea de crecimiento estolonífero. El objetivo de este estudio, fue obtener la ecuación de regresión lineal para la adecuada calibración del plato forrajero con el fin de predecir la disponibilidad de materia seca por hectárea. Esta investigación se realizó en el trascurso del año 2014, (desde marzo hasta diciembre); para esta investigación se realizaron mediciones con un Plato Forrajero en 21 predios ganaderos pertenecientes a la región del Alto Chicamocha. Se tomaron un total de cuatrocientas (400) muestras en época seca y de lluvias. Se procedió a desarrollar la formula por medio de un método matemático de regresión lineal simple, relacionando la altura comprimida del forraje y la disponibilidad real de materia seca obtenida mediante corte y posterior secado. %. De acuerdo a la formula se determinó que por cada unidad de medida del plato forrajero, la disponibilidad de fitomasa total se incrementó en 259.42 kg de Materia Seca (MS) y se obtuvo un intercepto de 2978.92. Estos resultados conllevan al uso de la ecuación como una alternativa de cálculo de la materia seca disponible durante todo el año en praderas de Kikuyo. La ecuación resultante para la calibración del Plato Forrajero en praderas de Kikuyo (Pennisetum clandestinum) fue Kg Ms/Ha = 259.42x + 2978.9 donde se reemplazó la altura comprimida por el plato forrajero y con un coeficiente de determinación (R2) de 77.67 %.

Benzo(a)pyrene (BaP), the prototype of polycyclic aromatic hydrocarbons, is known to exhibits genotoxic and carcinogenic effects promoting molecular impacts. The dataset presented here is associated with the research article paper entitled “Transcriptome Analysis Reveals Novel Insights Into the Response of Low-dose Benzo(a)pyrene Exposure in Male Tilapia”. In this article, we presented a transcriptomic characterization of male tilapia exposure to BaP in the short term. This data provides an extended analysis of changes in the gene expression and identification of pathways in the liver and testis of male tilapia exposure to BaP. We used gene set enrichment analysis (GSEA) and sub-network enrichment analysis (SNEA) to identify gene networks and pathways associated with molecular adverse effects of BaP exposure. The data indicates that target pathways related to promoting carcinogenesis such as DNA repair and DNA replication were affected as well as other crucial biological processes. Moreover, to determine whether some of the key reported genes of DNA damage are affected by BaP exposure, Quantitative PCR (qPCR) was performed. Gene set categories and sub-networks are provided and the corresponding signature differences from BaP exposure are listed. The information in these datasets may contribute to understanding the potential carcinogenesis mechanism of action from low BaP exposure.

In Boyacá, the Alto Chicamocha irrigation and drainage district (DRACH, by its initials in Spanish) is the department’s main agricultural production unit, covering an area of 8016.78 ha and due to the natural conditions and the management that has been given to the high basin of the Chicamocha River, salinization has been recognized as a limiting factor. Therefore, we sought to determine the predominant soluble salts in the soils that comprise the DRACH. Based on the chemical soil analysis information of 301 samples, obtained from studies conducted by GISSAT-UPTC and Corpoica, E.C, pH, anions and cations present in the soil were determined. For the spatial analysis of the variables studied, the ArcGis 10.3 software was used. Thirty-one water samples were collected in wells of the phreatimetric network to carry out the chemical characterization of the water. It was found that 48.01% of the soils of the district were non-saline, 22.93% slightly saline, 14.74% moderately saline and saline 14.33%. The main soluble salts in the soil were Na2SO4, Ca2SO4, NaCl2 and CaCl2, which are related to the lacustrine origin and the presence of thermal springs in the region. The areas with greater problem of salinization occur in the municipalities of Tibasosa, Patrocinio, Ucaca, Las Vueltas; in Santa Rosa de Viterbo in the village of Salitre; in Duitama in the villages Cebadero and Higueras with E.C. greater than 2 dS m-1.

Derivation of human induced pluripotent stem cells (hiPSCs) produces primed hiPSCs that can in turn be converted to naive hiPSCs. Here, the authors directly reprogram somatic cells to form both naive and primed isogenic hiPSCs and confirm the similarity of naive hiPSCs to their in vivo counterparts.

La objeción de Harel y Teichman en contra de que los gobiernos sigan las “recomendaciones de política pública provenientes de los teóricos conductistas”, para llevar a la gente a obedecer las reglas penales y, de este modo, para prevenir la ocurrencia de crímenes, puede ser entendida como cuestiones acerca de la permisibilidad ética y la utilidad práctica de aplicar lo que llamaré “política criminal del nudge”. Mi propósito en este escrito no es ofrecer una definición exhaustiva de este tipo de política pública ni tampoco justificar su uso desde una perspectiva ética, sino tan solo enunciar algunos retos a las teorías tradicionales del derecho penal en las que Harel y Teichman asientan su objeción.

In this article, we study women’s tattoos from a lived religion perspective. We describe how women’s tattoos express their inner lives, the religious dynamics associated with tattooing, and how they negotiate them with others. The sample used came from surveys and interviews targeting tattooed women at a confessional college on the East Coast of the United States. Women appropriate a prevalent cultural practice like body art to express their religious and spiritual experiences and ideas. It can be a Catholic motto, a Hindu or Buddhist sign, or a reformulated goddess, but the point is that women use tattoos to express their inner lives. We found that women perceive workplace culture as a hostile space for them to express their inner lives through tattoos, while they are comfortable negotiating their tattoos with their religious traditions. And they do so in a Catholic university.
