Purpose To evaluate in a large sample of patients from 10 different European centers the results of cataract surgery, characterizing the relationship between patient-reported outcomes (PROMs) and clinician-reported outcome measures (CROMs). Methods Prospective non-interventional multicenter observational descriptive study analyzing the clinical outcomes of a total of 3799 cases undergoing cataract surgery (mean age: 72.7 years). In all cases, the cataract surgery standard developed by the International Consortium for Health Outcomes Measurements (ICHOM) was used to register the clinical data. Three-month postoperative visual acuity and refraction data were considered CROMs, whereas Rasch-calibrated item 2 (RCCQ2) and total Catquest-9SF score (CQ) were considered PROMs. Results Postoperative corrected distance visual acuity (CDVA) was 0.3 logMAR or better in 88.7% (2505/2823) of eyes. Mean differences between preoperative and postoperative RCCQ2 and CQ scores were −3.09 and −2.39, respectively. Visual function improvement with surgery was reported by 91.5% (2163/2364) of patients. Statistically significant, although weak, correlations of postoperative CDVA with postoperative refraction, PROMs, and complications were found (0.133 ≤ r ≤0.289, p < 0.001). A predictive model (R2: 0.254) of postoperative CDVA considering 10 variables was obtained, including preoperative CDVA, different ocular comorbidities, age, gender and intraoperative complications. Likewise, another predictive model (R2: 0.148) of postoperative CQ considering a total of 14 variables was obtained, including additionally preoperative CQ, target refraction and previous surgeries. Conclusions Cataract surgery provides an improved functional vision in most of patients although this improvement can be limited by ocular comorbidities and complications. In dieser Studie wurde eine im Rahmen der Kataraktoperation implantierte bifokale Intraokularlinse (IOL) mit erweiterter Tiefenschärfe („extended-depth of focus“ – EDOF) anhand des Visus, einer Defokuskurve und der subjektiv empfundenen optischen Phänomene evaluiert. Die bifokale Intraokularlinse konnte 3 Monate nach binokularer Implantation, über einen segmentförmigen Nahteil von +1.5 dpt. Nahaddition, ein scharfes Sehen im Fern- und Intermediärbereich wiederherstellen und eine erweiterte Tiefenschärfe generieren. Somit wurde in der Arbeitsplatzentfernung eine Bril- lenlosigkeit und im Alltag eine Brillenunabhängigkeit erzielt. Nur wenige Patien- ten berichteten von gering ausgeprägten photopischen Phänomenen, wodurch bei fast allen Patienten eine hohe Zufriedenheit mit der optischen Qualität der Linse erzielt werden konnte. Intraoperativ konnte durch mikroinzisionale Kata- raktchirurgie und exakter Schnittführung auf dem steilen Meridian der Horn- hautastigmatismus im Mittel erfolgreich reduziert werden. Eine hohe Vorhersagbarkeit der postoperativen Refraktion konnte mit dem IOL- Master 500® und 700®, unter Verwendung der Haigis-Formel, (58) erzielt wer- den. Eine hohe Rotationsstabilität der Linse 3 Monate postoperativ, garantiert eine langfristige Basis für die exzellenten Visusergebnisse und verspricht auch eine hohe Planungssicherheit und Patientenzufriedenheit bei der Implantation einer torischen Version der EDOF IOL. Die LS-313 MF15 der Firma Oculentis ist als Alternative zur Monofokallinse mit einem Zusatznutzen im Intermediärbereich und als Alternative zur Multifokallin- se mit geringer Inzidenz von optischen Phänomenen einzuordnen. Um diese Ergebnisse postoperativ zu garantieren ist eine genaue Patientense- lektion obligatorisch. Zielgruppe sind Kataraktpatienten, die einen Zusatznutzen im Intermediärbereich wünschen, aber Sorgen wegen zusätzlicher optischer Phänomene haben. In dieser Studie konnte gezeigt werden, dass die Optik der IOL bei einem Durchmesser der Pupille von 3 mm seine volle Funktion entfalten kann. Ab einem Hornhautastigmatismus von über 1.0 dpt. sollte über die Im- plantation einer torischen Version der Oculentis Linse nachgedacht werden. The bifocal IOL with + 1.5 dpt. near addition generates high visual acuity at far distance and good visual acuity at intermediate distance and enhanced the depth-of-focus 3 month after binocular implantation. All patients are very satisfied and describe low optical phenomena but spectacle dependence for reading. Corneal astigmatism was successfully reduced using microincisional cataract surgery with a clear corneal incision on the steep meridian. A high postoperative predictability of the target refraction was guaranteed using both the IOL Master 500® and 700® with the Haigis formula. The IOL showed high postoperative rotation stability which is a important for good visual acuity. It also promises a high planing stability and patients satis- faction when toric version of the EDOF IOL is processed. On one hand the LS-313 MF15 is to classify as an alternative to a monofocal IOL with an additional benefit in intermediate distance. On the other hand it is an alternative to a multifocal IOL with less incidence of optical phenomena. To secure these results exact patient selection is necessary. Target group for this IOL are patients that wish an extra use in intermediate distance, but have concernes about optical phenomena. This study showed that the optic of the IOL can reveal it ́s function when pupil diameter is at 3 mm. When corneal astigmatism is above 1.0 dpt. an implantation of a toric version might be usefull. Beside cataract other visual acuity lowering diseases should be excluded.

Early recognition and management of patients at risk and more aggressive implementation of evidence-based resuscitation guidelines play a role to the reduction of patients' mortality. If, in paediatric emergency department, the proper adherence to the paediatric cardiac arrest guidelines is critical to increase the chance of survival, this adherence is unfortunately often suboptimal. Connected glasses, such as the Google Glass, offer an interesting support to provide guidelines at the point of care. However, existing guidelines format is not adapted to be used directly on the small screen of connected glasses. Their transformation to be displayed on the Google Glass is not a simple task. Problems such as the navigation and the formalization of the guidelines must be solved. In this article, we present the transformation process of the paediatric cardiac arrest algorithm from its paper version to its implementation on the Google Glass.

The administration of intravenous drugs is a significant source of medical errors. Protocol based care are have been demonstrated to be an efficient way to favor best practices and to avoid simple errors such as those related to expiration date, hygiene regulation among other. The recent availability of the Google Glass, a hands free wearable device offers new opportunities to access care protocols at patients' bedside. In this article, we present a prototype application for displaying care protocols developed through a user centered design. The software enables the navigation through the different steps of care protocols and their validation using barcodes. Three interactions paradigms, tactile, vocal and by eye blink are proposed in order to provide hands free manipulation when necessary. The realization of a concrete project revealed some limitations that should be taken in account in order to ensure the proper behavior of the tool. If no formal evaluation has been performed, the first feedbacks are very positive and encourage us to go forward and test the tool in real care situations.
