PURPOSE: Two-dimensional radiotherapy is often used to treat cervical cancer in low- and middle-income countries, but treatment planning can be challenging and time-consuming. Neural networks offer the potential to greatly decrease planning time through automation, but the impact of the wide range of hyperparameters to be set during training on model accuracy has not been exhaustively investigated. In the current study, we evaluated the effect of several convolutional neural network architectures and hyperparameters on 2D radiotherapy treatment field delineation. METHODS: Six commonly used deep learning architectures were trained to delineate four-field box apertures on digitally reconstructed radiographs for cervical cancer radiotherapy. A comprehensive search of optimal hyperparameters for all models was conducted by varying the initial learning rate, image normalization methods, and (when appropriate) convolutional kernel size, the number of learnable parameters via network depth and the number of feature maps per convolution, and nonlinear activation functions. This yielded over 1700 unique models, which were all trained until performance converged and then tested on a separate dataset. RESULTS: Of all hyperparameters, the choice of initial learning rate was most consistently significant for improved performance on the test set, with all top-performing models using learning rates of 0.0001. In fluoroscopy-guided interventions (FGIs), obtaining large quantities of labelled data for deep learning (DL) can be difficult. Synthetic labelled data can serve as an alternative, generated via pseudo 2D projections of CT volumetric data. However, contrasted vessels have low visibility in simple 2D projections of contrasted CT data. To overcome this, we propose an alternative method to generate fluoroscopy-like radiographs from contrasted head CT Angiography (CTA) volumetric data. The technique involves segmentation of brain tissue, bone, and contrasted vessels from CTA volumetric data, followed by an algorithm to adjust HU values, and finally, a standard ray-based projection is applied to generate the 2D image. The resulting synthetic images were compared to clinical fluoroscopy images for perceptual similarity and subject contrast measurements. Good perceptual similarity was demonstrated on vessel-enhanced synthetic images as compared to the clinical fluoroscopic images. Statistical tests of equivalence show that enhanced synthetic and clinical images have statistically equivalent mean subject contrast within 25% bounds. Furthermore, validation experiments confirmed that the proposed method for generating synthetic images improved the performance of DL models in certain regression tasks, such as localizing anatomical landmarks in clinical fluoroscopy images. Through enhanced pseudo 2D projection of CTA volume data, synthetic images with similar features to real clinical fluoroscopic images can be generated. C-Arm positioning for interventional spine procedures is often associated with a steep learning curve. This task requires mentally reconstructing 3D surgical tools and patient anatomy from a 2D X-ray image, which is non-trivial and acquired through years of experience. Standard training via apprenticeship-based programs must be limited due to the unnecessary exposure to ionizing radiation. To this end, we propose a Virtual Reality C-Arm simulator for interventional spine procedure training. We implemented the simulator as an open-source module in Slicer, and evaluated its efficacy through a user study, recruiting medical residents and expert clinicians. Users showed an overall significant improvement in C-Arm placement with regards to angular accuracy (mean ~2 degree improvement), and total procedure time (mean 11 minutes less time). The face and content validity was evaluated positively through a Likert scale questionnaire, with a mean score of 4 (out of 5) or higher for each of the questions. The results show the simulator provides effective training for C-Arm positioning, while eliminating the exposure to ionizing radiation associated with the current training standard. Although this work is catered towards spinal procedures, the system is extendable to other fields, such as cardiac and orthopaedic, and will be explored in future works. [ABSTRACT FROM AUTHOR]

Background: Digitally reconstructed radiographs (DRRs) generated from weightbearing computed tomography (WBCT) may potentially substitute for weightbearing plain radiographs (XRs) but have not been clinically validated. This study aims to test the reliability of 6 radiographic parameters of progressive collapsing foot deformity (PCFD) as measured on DRR, to investigate whether DRR represents comparably to XR through the same measurements, and to compare agreement of DRR and XR measurements of a standardized arch height parameter with reference measurements made on WBCT. Methods: DRR generated from preoperative WBCT of 71 patients (72 feet) treated surgically for PCFD were retrospectively compared with preoperative weight-bearing XR after exclusion criteria were applied. Six radiographic measurements were performed, including Meary angle, calcaneal pitch (CPA), medial cuneiform height (MCH), AP talar–first metatarsal angle (T-1MT), talonavicular coverage (TNCA), and talar incongruency (TIA). Arch height was measured on XR, DRR, and WBCT using a validated, standardized, navicular-based index. Intraclass correlation coefficients assessed DRR intraobserver and interobserver reliability. Paired samples t tests tested differences between XR and DRR. Bland-Altman limits of agreement analysis compared DRR and XR agreement with WBCT measurements. Results: Measurements were within standard PCFD ranges on XR and DRR. All measurements demonstrated excellent intrarater reliability and good to excellent interrater agreement, consistent with previous literature on XR. No differences were found for Meary, CPA, or TNCA. Minor differences were observed for MCH, T-1MT, and TIA. DRR measurements demonstrated greater agreement with WBCT than XR measurements. Conclusion: DRR from WBCT may be a promising substitute for XR in the clinical evaluation of PCFD. Radiographic measurements made on DRR demonstrated good to excellent reliability. Although small differences were found between XR and DRR for certain measurements, DRR more accurately represented medial arch anatomy compared to gold standard WBCT data than XR. This study aimed to investigate the effect of two different image density adjustment parameters on the results of image matching at six degrees of freedom using radiographic images generated by the ExacTrac X‐ray system in brain stereotactic radiosurgery (SRS). This study comprised 32 patients who underwent brain SRS at our hospital from January 2020 to December 2020. In this study, (1) the default parameter (an image density parameter between "tissue" and "bone") was an image density parameter for digitally reconstructed radiograph (DRR) generation used at many facilities, and (2) the bone parameter was the steepest contrast parameter used at our hospital. Of the 32 patients, 24 (75%) had a couch angle of 0.5 mm or more in the translational direction or 0.5° or more in the rotational direction, and 10 (31%) had a couch angle of 1.0 mm or more in the translational direction or 1.0° or more in the rotational direction. Among the 131 cases of all couch angles, 46 (35%) cases had a translational direction of 0.5 mm or more or a rotational direction of 0.5° or more, and 15 (11%) had a translational direction of 1.0 mm or more or a rotational direction of 1.0° or more. The results of this study indicate the usefulness of using appropriate DRR parameters for each case, rather than using the default settings. The use of appropriate DRR parameters can lead to accurate position matching results, leading to fewer image‐guided radiation therapy shots and a lower imaging dose. [ABSTRACT FROM AUTHOR]

Purpose: Currently, calculations of proton range in proton therapy patients are based on a conversion of CT Hounsfield units of patient tissues into proton relative stopping power. Uncertainties in this conversion necessitate larger proximal and distal planned target volume margins. Proton CT can potentially reduce these uncertainties by directly measuring proton stopping power. We aim to demonstrate proton CT imaging with complex porcine samples, to analyze in detail three‐dimensional regions of interest, and to compare proton stopping powers directly measured by proton CT to those determined from x‐ray CT scans. Methods: We have used a prototype proton imaging system with single proton tracking to acquire proton radiography and proton CT images of a sample of porcine pectoral girdle and ribs, and a pig's head. We also acquired close in time x‐ray CT scans of the same samples and compared proton stopping power measurements from the two modalities. In the case of the pig's head, we obtained x‐ray CT scans from two different scanners and compared results from high‐dose and low‐dose settings. Results: Comparing our reconstructed proton CT images with images derived from x‐ray CT scans, we find agreement within 1% to 2% for soft tissues and discrepancies of up to 6% for compact bone. We also observed large discrepancies, up to 40%, for cavitated regions with mixed content of air, soft tissue, and bone, such as sinus cavities or tympanic bullae. Conclusions: Our images and findings from a clinically realistic proton CT scanner demonstrate the potential for proton CT to be used for low‐dose treatment planning with reduced margins. Purpose: Statistical shape and intensity models (SSIMs) and digitally reconstructed radiographs (DRRs) were introduced for non-rigid 2D-3D registration and skeletal geometry/density reconstruction studies. The computation of DRRs takes most of the time during registration or reconstruction. The goal of this study is to propose a particle-based method for composing an SSIM and a DRR image generation scheme and analyze the quality of the images compared with previous DRR generation methods., Approach: Particle-based SSIMs consist of densely scattered particles on the surface and inside of an object, with each particle having an intensity value. Generating the DRR resembles ray tracing, which counts the particles that are binned with each ray and calculates the radiation attenuation. The distance between adjacent particles was considered to be the radiologic path during attenuation integration, and the mean linear attenuation coefficient of the two particles was multiplied. The proposed method was compared with the DRR of CT projection. The mean squared error and peak signal-to-noise ratio (PSNR) were calculated between the DRR images from the proposed method and those of existing methods of projecting tetrahedral-based SSIMs or computed tomography (CT) images to verify the accuracy of the proposed scheme., Results: The suggested method was about 600 times faster than the tetrahedral-based SSIM without using the hardware acceleration technique. Abstract Background A large critical shoulder angle (CSA) >35° is associated with the development of rotator cuff tearing. Lateral acromioplasty (AP) has the theoretical potential to prevent rotator cuff tearing and/ or to reduce the risk of re-tears after repair. It is, however unclear which part of the lateral acromion has to be reduced to obtain the desired CSA. It was the purpose of this study to determine which part of the lateral acromion has to be resected to achieve a desired reduction of the CSA in a given individual. Methods First, the influence of the exact radiographic projection on the CSA was examined. Second, the influence of anterolateral versus strict lateral AP on the CSA was studied in eight scapulae with different anatomic characteristics. Differences in CSA reduction were investigated using paired t-test or Wilcoxon test. Results Scapular rotation in the sagittal and axial plane had a marked influence on the radiologically measured CSA ranging from -6 to +16°. Overall, lateral AP of 5/10mm reduced the CSA significantly greater than anterolateral AP of 5mm/10mm [5mm: 2.3° (range: 0.7°-3.6°) SD±0.8° vs. 1.2° (range: 0°-3.3°) SD±1.1°, p=0.0002]/[10mm: 4.8° (range: 2.1°-7°) SD±1.3° vs. 2.7° (range: 0°-5.3°) SD±1.7°, p=0.0001]. Depending on scapular anatomy anterolateral AP did not alter CSA at all. Conclusions For comparison of pre- and postoperative CSA, the exact orientation of the X-ray and the spatial orientation of the scapula must be as identical as possible. Anterolateral AP may not sufficiently correct CSA in scapulae with great acromial slopes and smaller relative external rotation of the acromion as the critical acromial point (CAP) may be located too posteriorly and thus is not addressed by anterolateral acromioplasty. Consistent reduction of the CSA could be achieved by lateral AP in all eight scapulae.

Introduction: The DRR has typically been employed for verifying the geometry in radiotherapy treatment, and there have been no documented instances of its use for dosimetric purposes. This research aims to introduce a new methodology, allowing us to generate a two - dimensional dose distribution within a treatment plan using the DRR for the first time. Methods: First, the study will investigate the relationship between the pixel values of the DRR and water - equivalent thickness (WET). Next, it aims to establish a link between depth and absorbed dose. By combining these relationships, a new equation will be developed to connect the pixel values of the DRR to the absorbed dose. This method will be utilized to calculate the dose distribution in the isocentric plane for both homogeneous and heterogeneous phantoms. To verify the accuracy of this approach, the results will be compared with the two - dimensional dose distribution from the treatment planning system (TPS). Results: At the isocenter, the point dose comparison indicates a variance of around 1% for the homogeneous phantom and 1.2% for the heterogeneous phantom. A gamma analysis (3% - 3mm criteria) was conducted to compare the region - based dose distribution. The pass rate achieved was 98.44% for the homogeneous phantom and 96.6% for the heterogeneous phantom. Conclusions: Confirming the dose distribution derived from this method with TPS introduces a novel use of DRR for assessing dose distribution in perpendicular planes to the treatment field's central axis. In liver stereotactic body radiotherapy (SBRT), precise image guidance is paramount, serving as the foundation of this treatment approach. The accuracy of SBRT in liver cancer treatment heavily relies on meticulous imaging techniques. The diaphragm, situated adjacent to the liver, is a crucial anatomical structure susceptible to positional and motion variations, which can potentially impact the accuracy of liver tumor targeting. This study explores the application of real-time kilovoltage projection streaming images (KVPSI) in comparison to digitally reconstructed radiography (DRR) for assessing diaphragm position deviations during breath-hold liver tumor SBRT. A 76-year-old male diagnosed with cholangiocarcinoma underwent breath-hold SBRT using split arc volumetric modulated arc therapy (VMAT), where a full arc was split into six sub-arcs, each spanning 60 degrees. The diaphragm dome positions were continuously monitored through KVPSI during treatment. The intrafractional position deviations of the diaphragm were calculated and analyzed for each split arc. The case report revealed a mean diaphragm dome deviation of 0.47 mm (standard deviation: 4.47 mm) in the entire arc. This pioneering study showcases the feasibility of intrafractional diaphragm position variation assessment using real-time KVPSI during the breath-hold liver tumor VMAT-SBRT. Integrating real-time imaging techniques enhances our comprehension of the intra-breath-hold variations, thereby guiding adaptive treatment strategies and potentially improving treatment outcomes. Objective To measure and analyze the set-up errors of postoperative intensity modulated radiation therapy of rectal cancer (PIMRTRC) with electronic portal imaging device (EPID), and to provide theoretical foundation for clinical realizing of accurate PIMRTRC. Methods Totally 30 patients after rectal carcinoma resection underwent sagittal and coronal photographing with EPID before the first time of therapy and one time per week during the treatment course. The obtained images were compared with DRR images in treatment planning system to get the setup errors at X (left-right), Y (head-foot) and Z (front-back) directions, and the extending margins of CTV and PTV in postoperative intensity modulated radiation therapy were calculated. EPID was used for setup correction, and SPSS 19.0 was involved in to execute statistical analysis. Results The linear displacement had the mean values plus/minus standard deviation at X, Y and Z directions before and correction being (-1.392 4±3.670 9)mm vs (-0.816 5±2.670 5)mm,(0.969 7±4.0761)mm vs(0.418 2±2.911 4)mm, and(0.704 4±1.805 6)mm vs(0.4717±1.6413)mm respectively;the extending margin had the values being 7mm vs 5mm, 6 mm vs 4 mm, and 4 mm vs 3 mm respectively. Conclusion EPID ensures the correctness and accuracy in postoperative intensity modulated radiotherapy of rectal cancer, which makes target area gifted with maximized dose, the surrounding tissue and organs at risk protected adequately, and provides theoretical support for extending CTV margin. Background Acromioplasty has been proposed as a means of altering elevated critical shoulder angles (CSAs). We aimed to localize the critical acromion point (CAP) responsible for the acromial contribution of the CSA and determine whether resection of the CAP can alter the CSA to a normal range. Methods The CAP and 3-dimensional (3D) CSAs were determined on 3D computed tomography reconstructions of 88 cadaveric shoulders and compared with corresponding CSAs on digitally reconstructed radiographs. The position of the CAP was fluoroscopically isolated in 20 of these specimens and the resulting fluoroscopic CSA compared with the corresponding 3D CAP and 3D CSA. We investigated the CSA before and after a virtual acromioplasty of 2.5 and 5 mm at the CAP in specimens with a CSA greater than 35°. Results The mean CAP was 21% ± 10% of the acromial anterior-posterior length from the anterolateral corner. There was no difference between the mean 3D CSA and the CSA on digitally reconstructed radiographs (32° vs 32°, P  = .096). No difference between the mean fluoroscopic CSA and 3D CSA was found (31° vs 31°, P  = .296). A 2.5-mm acromial resection failed to reduce the CSA to 35° or less in 7 of 13 shoulders, whereas a 5-mm resection reduced the CSA to 35° or less in 12 of 13. Conclusion The CAP was localized to the anterolateral acromial edge and was easily identified fluoroscopically. A 5-mm acromial resection was effective in reducing the CSA to 35° or less. These data can guide surgeons in where and how to alter the CSA if future studies demonstrate a clinical benefit to surgically modifying this radiographic parameter. [ABSTRACT FROM AUTHOR]

The purpose of this study was to develop a novel technique to improve the accuracy of radiographic cup anteversion measurement by correcting the influence of pelvic tilt. Ninety virtual total hip arthroplasties were simulated from computed tomography data of 6 patients with 15 predetermined cup orientations. For each simulated implantation, anteroposterior (AP) virtual pelvic radiographs were generated for 11 predetermined pelvic tilts. A linear regression model was created to capture the relationship between radiographic cup anteversion angle error measured on AP pelvic radiographs and pelvic tilt. Overall, nine hundred and ninety virtual AP pelvic radiographs were measured, and 90 linear regression models were created. Pearson's correlation analyses confirmed a strong correlation between the errors of conventional radiographic cup anteversion angle measured on AP pelvic radiographs and the magnitude of pelvic tilt ( P  < 0.001). The mean of 90 slopes and y -intercepts of the regression lines were −0.8 and −2.5°, which were applied as the general correction parameters for the proposed tool to correct conventional cup anteversion angle from the influence of pelvic tilt. The current method proposes to measure the pelvic tilt on a lateral radiograph, and to use it as a correction for the radiographic cup anteversion measurement on an AP pelvic radiograph. Thus, both AP and lateral pelvic radiographs are required for the measurement of pelvic posture-integrated cup anteversion. Compared with conventional radiographic cup anteversion, the errors of pelvic posture-integrated radiographic cup anteversion were reduced from 10.03 (SD = 5.13) degrees to 2.53 (SD = 1.33) degrees. Pelvic posture-integrated cup anteversion measurement improves the accuracy of radiographic cup anteversion measurement, which shows the potential of further clarifying the etiology of postoperative instability based on planar radiographs. The current work introduces a system for fully automatic tracking of native glenohumeral kinematics in stereo-radiography sequences. The proposed method first applies convolutional neural networks to obtain segmentation and semantic key point predictions in biplanar radiograph frames. Preliminary bone pose estimates are computed by solving a non-convex optimization problem with semidefinite relaxations to register digitized bone landmarks to semantic key points. Initial poses are then refined by registering computed tomography-based digitally reconstructed radiographs to captured scenes, which are masked by segmentation maps to isolate the shoulder joint. A particular neural net architecture which exploits subject-specific geometry is also introduced to improve segmentation predictions and increase robustness of subsequent pose estimates. The method is evaluated by comparing predicted glenohumeral kinematics to manually tracked values from 17 trials capturing 4 dynamic activities. Median orientation differences between predicted and ground truth poses were 1.7 ∘ and 8.6 ∘ for the scapula and humerus, respectively. Joint-level kinematics differences were less than 2 ∘ in 65%, 13%, and 63% of frames for XYZ orientation DoFs based on Euler angle decompositions. Objective: The goal of this work was to methodically evaluate, optimize, and validate a self-supervised machine learning algorithm capable of real-time automatic registration and fluoroscopic localization of the spine using a single radiograph or fluoroscopic frame., Methods: The authors propose a two-dimensional to three-dimensional (2D-3D) registration algorithm that maximizes an image similarity metric between radiographic images to identify the position of a C-arm relative to a 3D volume. This work utilizes digitally reconstructed radiographs (DRRs), which are synthetic radiographic images generated by simulating the x-ray projections as they would pass through a CT volume. To evaluate the algorithm, the authors used cone-beam CT data for 127 patients obtained from an open-source de-identified registry of cervical, thoracic, and lumbar scans. They systematically evaluated and tuned the algorithm, then quantified the convergence rate of the model by simulating C-arm registrations with 80 randomly simulated DRRs for each CT volume. The endpoints of this study were time to convergence, accuracy of convergence for each of the C-arm's degrees of freedom, and overall registration accuracy based on a voxel-by-voxel measurement., Results: A total of 10,160 unique radiographic images were simulated from 127 CT scans. The algorithm successfully converged to the correct solution 82% of the time with an average of 1.96 seconds of computation. The radiographic images for which the algorithm converged to the solution demonstrated 99.9% registration accuracy despite utilizing only single-precision computation for speed. The algorithm was found to be optimized for convergence when the search space was limited to a ± 45° offset in the right anterior oblique/left anterior oblique, cranial/caudal, and receiver rotation angles with the radiographic isocenter contained within 8000 cm3 of the volumetric center of the CT volume., Conclusions: The investigated machine learning algorithm has the potential to aid surgeons in level localization, surgical planning, and intraoperative navigation through a completely automated 2D-3D registration process. Future work will focus on algorithmic optimizations to improve the convergence rate and speed profile.

Background: Clinical success of total ankle arthroplasty depends heavily on the available information on the morphology of the bones, often obtained from measurements on planar radiographs. The current study aimed to evaluate the intra-rater, inter-rater and inter-session reliability and the validity of radiograph-based measurements of ankle morphology, and to quantify the effects of examiner experience on these measurements. Methods: Twenty-four fresh frozen ankle specimens were CT scanned, data of which were used to reconstruct 3D volumetric bone models for synthesizing 2D radiographs. Two orthopaedic surgeons with different levels of clinical experience identified twenty landmarks five times on each of the synthesized sagittal and coronal radiographs and repeated the test on a subsequent day within 5 days. The landmarks were used to calculate fourteen morphological parameters. The two-way mixed-effects (ICC3,1), two-way random-effects (ICC2,k) and two-way random-effects (ICC3,k) models were used, respectively, to assess the intra-rater, inter-rater and inter-session reliability of measurements. The validity of the measurements for each examiner was assessed by comparing them with gold standard values obtained from the 2D radiographs projected from the 3D volumetric models using Pearson's correlation analysis and Bland and Altman plots, and the differences were defined as the measurement errors. Results: Most of the morphological parameters were of good to very good intra-rater, inter-session and inter-rater reliability for both examiners (ICC > 0.61). Experience appeared to affect the inter-rater and inter-session reliability, the senior examiner showing greater inter-session ICC values than the junior examiner. Most of the tibial parameters had moderate to excellent correlations with the corresponding gold standard values but were underestimated by both examiners, in contrast to most of the talar parameters that were overestimated and had only poor to fair correlations. Conclusions: Most of the morphological parameters of the ankle can be estimated from radiographs with good to very good intra-rater, inter-session and inter-rater reliability, for both clinically experienced and less experienced examiners. Clinical experience helped increase the reliability of repeated evaluations after a longer interval, such as in a follow-up assessment. Purpose: Real-time 6 degrees of freedom (6DoF) pose recovery and tracking from X-ray images is a key enabling technology for many interventional imaging applications. However, real-time 2D/3D registration is a very challenging problem because of the heavy computation in iterative digitally reconstructed radiograph (DRR) generation. In this paper, we propose a real-time 2D/3D registration framework using library-based DRRs to achieve high computational efficiency. Method: The proposed method pre-computes a library of canonical DRRs and reconstructs library-based DRRs (libDRRs) during registration without online rendering. The transformation parameters are decoupled to 2 geometry-relevant and 4 geometry-irrelevant ones so that canonical DRRs only need to cover the variation of 2 geometry-relevant parameters, making it practical to be pre-computed and stored. The 2D/3D registration using libDRRs is then solved as a hybrid optimization problem, i.e., continuous in geometry-irrelevant parameters while discrete in geometry-relevant parameters. Results: On 5 fluoroscopic sequences with 246 frames acquired during animal studies with a transesophageal echocardiography (TEE) probe in the field of view, 6DoF tracking of the TEE probe using the proposed method achieved a mean target registration error in the projection direction (mTREproj) of 0.81 mm, a success rate of 100 % (defined as mTREproj $$<$$ 2.5 mm), and a registration frame rate of 23.1 fps on a pure CPU-based implementation executed in a single thread. Conclusion: Using libDRRs with a hybrid optimization can significantly improve the computational efficiency (up to tenfold) for 6DoF pose recovery and tracking with little degradation in robustness and accuracy, compared to conventional intensity-based 2D/3D registration using ray casting DRRs with a continuous optimization. We present a framework that combines evolutionary optimisation, soft tissue modelling and ray tracing on GPU to simultaneously compute the respiratory motion and X-ray imaging in real-time. Our aim is to provide validated building blocks with high fidelity to closely match both the human physiology and the physics of X-rays. A CPU-based set of algorithms is presented to model organ behaviours during respiration. Soft tissue deformation is computed with an extension of the Chain Mail method. Rigid elements move according to kinematic laws. A GPU-based surface rendering method is proposed to compute the X-ray image using the Beer–Lambert law. It is provided as an open-source library. A quantitative validation study is provided to objectively assess the accuracy of both components: (i) the respiration against anatomical data, and (ii) the X-ray against the Beer–Lambert law and the results of Monte Carlo simulations. Our implementation can be used in various applications, such as interactive medical virtual environment to train percutaneous transhepatic cholangiography in interventional radiology, 2D/3D registration, computation of digitally reconstructed radiograph, simulation of 4D sinograms to test tomography reconstruction tools. Purpose: In orthopaedics, minimally invasive injection of bone cement is an established technique. We present HipRFX, a software tool for planning and guiding a cement injection procedure for stabilizing a loosening hip prosthesis. HipRFX works by analysing a pre-operative CT and intraoperative C-arm fluoroscopic images. Methods: HipRFX simulates the intraoperative fluoroscopic views that a surgeon would see on a display panel. Structures are rendered by modelling their X-ray attenuation. These are then compared to actual fluoroscopic images which allow cement volumes to be estimated. Five human cadaver legs were used to validate the software in conjunction with real percutaneous cement injection into artificially created periprothetic lesions. Results: Based on intraoperatively obtained fluoroscopic images, our software was able to estimate the cement volume that reached the pre-operatively planned targets. The actual median target lesion volume was 3.58 ml (range 3.17-4.64 ml). The median error in computed cement filling, as a percentage of target volume, was 5.3 % (range 2.2-14.8 %). Cement filling was between 17.6 and 55.4 % (median 51.8 %). Conclusions: As a proof of concept, HipRFX was capable of simulating intraoperative fluoroscopic C-arm images. Furthermore, it provided estimates of the fraction of injected cement deposited at its intended target location, as opposed to cement that leaked away. This level of knowledge is usually unavailable to the surgeon viewing a fluoroscopic image and may aid in evaluating the success of a percutaneous cement injection intervention. Objective: To determine the strength of common radiographic and radial CT views for measuring true femoral head asphericity. Patients and methods: In 15 patients with cam femoroacetabular impingement (FAI) and 15 controls, alpha angles were measured by two observers using radial CT (0°, 30°, 60°, 90°) and digitally reconstructed radiographs (DRRs) for the: anterior–posterior (AP), standing frog-leg lateral, 45° Dunn with neutral rotation, 45° Dunn with 40° external rotation, and cross-table lateral views. A DRR validation study was performed. Alpha angles were compared between groups. Maximum deviation from a sphere of each subject was obtained from a previous study. Alpha angles from each view were correlated with maximum deviation. Results: There were no significant differences between alpha angles measured on radiographs and the corresponding DRRs (p =0.72). Alpha angles were significantly greater in patients for all views (p≤0.002). Alpha angles from the 45° Dunn with 40° external rotation, cross-table lateral, and 60° radial views had the strongest correlations with maximum deviation (r =0.831; r =0.823; r =0.808, respectively). The AP view had the weakest correlation (r =0.358). Conclusion: DRRs were a validated means to simulate hip radiographs. The 45° Dunn with 40° external rotation, cross-table lateral, and 60° radial views best visualized femoral asphericity. Although commonly used, the AP view did not visualize cam deformities well. Overall, the magnitude of the alpha angle may not be indicative of the size of the deformity. Thus, 3D reconstructions and measurements of asphericity could improve the diagnosis of cam FAI. We analyzed the mean difference and correlation between 2D cup coverage measured from different projections and three-dimensional (3D) cup coverage to investigate their precise relationship in total hip arthroplasty (THA) among patients with developmental dysplasia of the hip (DDH). We created DDH-THA models on six foam pelvic models. 3D cup coverage was measured using a motion capture system and imaging software. Digitally reconstructed radiographs with predetermined pelvic rotations were simulated using image processing software at three different angles of rotation around the long body axis (0°, 25° and 45°). 2D cup coverage was then measured on these reconstructed radiographs. The 3D technique showed excellent intra-observer (K > 0.98) and inter-observer (K >0.99) reliability. The 2D technique tended to overestimate the real cup coverage by about 15%. The smallest difference between 2D and 3D cup coverage occurred when 2D measurement was performed on the radiographs with 45° of pelvic rotation toward the operated side (14.50%, P < 0.0001), meanwhile, the highest correlation coefficient between 2D and 3D cup coverage was also observed when the 2D measurement was performed on the radiographs at this same pelvic rotation (r=0.67, P=0.0003). Published recommenda-tions regarding the minimum cup coverage based on 2D measurement should be interpreted cautiously. The minimal cup coverage, as an intra-operative 3D parameter related to the long term fixation of the cup component, should be more accurately determined with intra-operative measurement. The use of biplanar videoradiography technology has become increasingly popular for evaluating joint function in vivo. Two fundamentally different methods are currently employed to reconstruct 3D bone motions captured using this technology. Marker-based tracking requires at least three radio-opaque markers to be implanted in the bone of interest. Markerless tracking makes use of algorithms designed to match 3D bone shapes to biplanar videoradiography data. In order to reliably quantify in vivo bone motion, the systematic error of these tracking techniques should be evaluated. Herein, we present new markerless tracking software that makes use of modern GPU technology, describe a versatile method for quantifying the systematic error of a biplanar videoradiography motion capture system using independent gold standard instrumentation, and evaluate the systematic error of the W.M. Keck XROMM Facility's biplanar videoradiography system using both marker-based and markerless tracking algorithms under static and dynamic motion conditions. A polycarbonate flag embedded with 12 radio-opaque markers was used to evaluate the systematic error of the marker-based tracking algorithm. Three human cadaveric bones (distal femur, distal radius, and distal ulna) were used to evaluate the systematic error of the markerless tracking algorithm. The systematic error was evaluated by comparing motions to independent gold standard instrumentation. Static motions were compared to high accuracy linear and rotary stages while dynamic motions were compared to a high accuracy angular displacement transducer. Marker-based tracking was shown to effectively track motion to within 0.1 mm and 0.1 deg under static and dynamic conditions. Furthermore, the presented results indicate that markerless tracking can be used to effectively track rapid bone motions to within 0.15 deg for the distal aspects of the femur, radius, and ulna. Both marker-based and markerless tracking techniques were in excellent agreement with the gold standard instrumentation for both static and dynamic testing protocols. Future research will employ these techniques to quantify in vivo joint motion for high-speed upper and lower extremity impacts such as jumping, landing, and hammering. [ABSTRACT FROM AUTHOR]

Abstract: Introduction: The periapical radiograph is used as an important tool in the assessment of periapical bone lesions in endodontic therapy. The purpose of this study was to develop a virtual simulation platform for radiographic research of periapical bone lesions based on a digitally reconstructed radiograph and to investigate the radiographic features of different simulated periapical bone lesions. Methods: A cadaver mandible was scanned by microcomputed tomography. The application framework for the creation of a digitally reconstructed radiograph with virtual periapical lesions was constructed. Subsequently, different size and shape periapical lesions were created virtually in an incisor, a premolar, and a molar, and the digitally reconstructed radiographs were produced. Result: The detection of periapical lesions based on digitally reconstructed radiographs was depended on lesion size, position, shape, and tooth position. Virtual periapical lesions could not be visualized with lesions smaller than 1 mm in the incisor, 2 mm in the premolar, and 3 mm in the molar, and these virtual lesions were confined within the cancellous bone. A 4-mm lesion in the molar was still not visualized even if it encroached on the cortical bone. If the lesions encroached on the junctional trabeculae and cortical bone or the lesion was created with the maximal buccal-lingual dimension in ellipsoid shape and confined within the cancellous bone giving it an abnormal shape, it could be seen, except for the “thinnest” 1-mm lesion in incisor region. Conclusions: The virtual simulation platform described here provides a reproducible assessment of periapical lesions and aids in a better understanding of the characteristics of periapical lesions. To provide faster calculation of digitally reconstructed radiographs (DRRs) in patient-positioning verification, we developed and evaluated a graphic processing unit (GPU)-based DRR software application and compared it with a central processing unit (CPU)-based application. The evaluation metrics were calculation speed and image quality for various slice thicknesses. The results showed that the GPU-based DRR computation was an average of 50 times faster than the CPU-based methodology, whereas the image quality was very similar. Purpose: In a recent study, large systematic setup errors were detected in patients with lung cancer when a conventional simulation procedure was used to define and mark the treatment isocenter. In the present study, we describe a procedure to omit the session at a conventional simulator to remove simulation errors entirely. Isocenter definition and verification was performed at a computed tomography (CT) simulator, and digitally reconstructed radiographs (DRRs) were used for setup verification and correction at the treatment unit.Methods and Materials: A CT simulation protocol was developed, in which radiopaque markers were used to verify the coincidence of the isocenter marked on the patients’ skin with the isocenter defined in the planning CT scan. This protocol was evaluated for 20 patients. Subsequently, electronic portal images were acquired at the treatment unit. The three-dimensional setup error was established from a template match of the appropriate anatomy visible in two orthogonal beams with the corresponding anatomy in DRRs. An offline setup correction protocol was applied to reduce systematic setup errors.Results: For all patients, the skin marks defined the planning CT scan isocenter to within ± 1.5 mm in each of the three main directions. Random setup errors at the treatment unit were 1.8, 2.0, and 1.9 mm (1 SD) for the lateral (x), the superior-inferior (y), and the anterior-posterior (z) directions, respectively. With the use of the correction protocol, the systematic errors for x, y, and z were 1.5, 1.5, and 1.3 mm (1 SD).Conclusions: Because the distributions of treatment setup errors measured against DRRs obtained in our CT simulation were equal to previously obtained distributions measured against simulator films, conventional simulation can be omitted and DRRs are well-suited for setup verification. By adopting our CT simulation procedure, the large systematic simulation setup errors, which remain hidden if a conventional simulation is performed, can be avoided. [Copyright &y& Elsevier]

SUMMARY The authors present 3 years of experience in using digitally reconstructed radiographs (DRR) for radiotherapy planning and verification. Comparison is made with simulation film (SF), to illustrate the advantages of DRR over SF. Emphasis is placed on using the appropriate equipment and applying the correct technique. A brief discourse on the principle of CT imaging is presented to illustrate the operation of CT software and optimization of image display for axial slices and DRR. Previous CT-based methods of measuring acetabular coverage of the femoral head have either been labor-intensive or have required extensive preprocessing of the data prior to visualization. We propose a method of measuring acetabular coverage using stereoscopic digitally reconstructed radiographs that required very little labor or image preprocessing time. Taking a craniocaudal view of the pelvis, we measured both preoperative and postoperative CTs of 10 patients treated with transtrochanteric periacetabular osteotomy. Measurements were then made in both monocular and stereoscopic rendering modes. Our method is fast, easy, and provides an intuitive means of visualizing an orthopedic parameter that is important in the progression of early hip arthritis. [ABSTRACT FROM AUTHOR]

Background: While transmitting the medical images in radiology information systems the adversary effect can break the CIA (Confidentiality, Integrity, and Availability) triads of information security. The objective of the study was to transmit the complete set of image objects in a dataset without data integrity violation., Methods: In this paper a hybrid cryptographic technique which combines the prime details from the patient dataset (stack of axial 2D images) and the Advanced Encryption Standard (AES) method has been proposed. The steps include a) Creating an artificial X-ray image (DRR) from the 3D volume, b) dividing the DRR image in x and y directions equally into four regions, c) applying the zig-zag pattern to each quadrant, and d) encryption of each quadrant with block cipher mode using the AES algorithm. After dataset transmission the DRR image was regenerated at the receiver and compared each of the deciphered blocks (transmitted ones) using the histogram technique., Results: The technique was tested on CT and MRI scans of sixty datasets. The image injection techniques, such as adding and deleting an image from the dataset and modifying the image pixels, were tested. The results were validated statistically using mean square error and histogram matching techniques., Conclusion: The combination of the DRR and the AES technique has ensured the secured transmission of the entire dataset and not an individual image.

Performing large-scale three-dimensional radiation dose reconstruction for patients requires a large amount of manual work. We present an image processing-based pipeline to automatically reconstruct radiation dose. The pipeline was designed for childhood cancer survivors that received abdominal radiotherapy with anterior-to-posterior and posterior-to-anterior field set-up. First, anatomical landmarks are automatically identified on two-dimensional radiographs. Second, these landmarks are used to derive parameters to emulate the geometry of the plan on a surrogate computed tomography. Finally, the plan is emulated and used as input for dose calculation. For qualitative evaluation, 100 cases of automatic and manual plan emulations were assessed by two experienced radiation dosimetrists in a blinded comparison. The two radiation dosimetrists approved 100%/100% and 92%/91% of the automatic/manual plan emulations, respectively. Similar approval rates of 100% and 94% hold when the automatic pipeline is applied on another 50 cases. Further, quantitative comparisons resulted in on average < 5    mm difference in plan isocenter/borders, and < 0.9    Gy in organ mean dose (prescribed dose: 14.4 Gy) calculated from the automatic and manual plan emulations. No statistically significant difference in terms of dose reconstruction accuracy was found for most organs at risk. Background: A large critical shoulder angle (CSA) >35° is associated with the development of rotator cuff tearing. Lateral acromioplasty (AP) has the theoretical potential to prevent rotator cuff tearing and/ or to reduce the risk of re-tears after repair. It is, however unclear which part of the lateral acromion has to be reduced to obtain the desired CSA. It was the purpose of this study to determine which part of the lateral acromion has to be resected to achieve a desired reduction of the CSA in a given individual.Methods: First, the influence of the exact radiographic projection on the CSA was examined. Second, the influence of anterolateral versus strict lateral AP on the CSA was studied in eight scapulae with different anatomic characteristics. Differences in CSA reduction were investigated using paired t-test or Wilcoxon test.Results: Scapular rotation in the sagittal and axial plane had a marked influence on the radiologically measured CSA ranging from -6 to +16°. Overall, lateral AP of 5/10mm reduced the CSA significantly greater than anterolateral AP of 5mm/10mm [5mm: 2.3° (range: 0.7°-3.6°) SD±0.8° vs. 1.2° (range: 0°-3.3°) SD±1.1°, p=0.0002]/[10mm: 4.8° (range: 2.1°-7°) SD±1.3° vs. 2.7° (range: 0°-5.3°) SD±1.7°, p=0.0001]. Depending on scapular anatomy anterolateral AP did not alter CSA at all.Conclusions: For comparison of pre- and postoperative CSA, the exact orientation of the X-ray and the spatial orientation of the scapula must be as identical as possible. Anterolateral AP may not sufficiently correct CSA in scapulae with great acromial slopes and smaller relative external rotation of the acromion as the critical acromial point (CAP) may be located too posteriorly and thus is not addressed by anterolateral acromioplasty. In this study, computer simulations and experiments were used to verify the accuracy of a two-dimensional image registration program (program) for portal images that we previously developed. The program used a computed radiography cassette system and digitally reconstructed radiography images as planning images for external beam radiation therapy. Using this program, we also investigated the reason two-dimensional automatic image registration images experienced large misregistration in clinical practice using commercial image registration systems. Mutual information and normalized mutual information were used as the registration criteria. To investigate the influence of image background with or without a region of interest (ROI), results of image registrations were compared. Parameters of image registration were defined as translation in the horizontal and vertical directions (x and y, respectively) and rotation (θ) around the axis perpendicular to the x-y plane. There was no significant difference in image registration arising from the difference between mutual information and normalized mutual information. Image registration was improved with a ROI. Regardless of the registration criteria, errors in image registration with a ROI in the experimental study were ≤1.2 mm in directions x and y and ≤1.0 degree in rotation θ. We found that image registration required setting up as close to the planned position as possible.

The purpose of this work is to evaluate methods for producing a library of 2D-radiographic images to be correlated to clinical images obtained during a fluoroscopically-guided procedure for automated patient-model localization. The localization algorithm will be used to improve the accuracy of the skin-dose map superimposed on the 3D patient-model of the real-time Dose-Tracking-System (DTS). For the library, 2D images were generated from CT datasets of the SK-150 anthropomorphic phantom using two methods: Schmid's 3D-visualization tool and Plastimatch's digitally-reconstructed-radiograph (DRR) code. Those images, as well as a standard 2D-radiographic image, were correlated to a 2D-fluoroscopic image of a phantom, which represented the clinical-fluoroscopic image, using the Corr2 function in Matlab. The Corr2 function takes two images and outputs the relative correlation between them, which is fed into the localization algorithm. Higher correlation means better alignment of the 3D patient-model with the patient image. In this instance, it was determined that the localization algorithm will succeed when Corr2 returns a correlation of at least 50%. The 3D-visualization tool images returned 55-80% correlation relative to the fluoroscopic-image, which was comparable to the correlation for the radiograph. The DRR images returned 61-90% correlation, again comparable to the radiograph. Both methods prove to be sufficient for the localization algorithm and can be produced quickly; however, the DRR method produces more accurate grey-levels. Using the DRR code, a library at varying angles can be produced for the localization algorithm.

Background Accurate assessment of the critical shoulder angle (CSA) is important in clinical evaluation of degenerative rotator cuff tears. This study analyzed the influence of radiographic viewing perspective on the CSA, developed a classification system to identify malpositioned radiographs, and assessed the relationship between the CSA and demographic factors. Methods Glenoid height, width, and retroversion were measured on 3-dimensional computed tomography reconstructions of 68 cadaver scapulae. A digitally reconstructed radiograph was aligned perpendicular to the scapular plane, and retroversion was corrected to obtain a true anteroposterior (AP) view. In 10 scapulae, incremental anteversion/retroversion and flexion/extension views were generated. The CSA was measured, and a clinically applicable classification system was developed to detect views with >2° change in CSA vs. true AP view. Results The average CSA was 33° ± 4°. Intraobserver and interobserver reliability was high (intraclass correlation coefficient ≥ 0.81) but decreased with increasing viewing angle. Views beyond 5° anteversion, 8° retroversion, 15° flexion, and 26° extension resulted in >2° deviation of the CSA compared with the true AP view. The classification system was capable of detecting aberrant viewing perspectives with sensitivity of 95% and specificity of 53%. Correlations between glenoid size and CSA were small ( R ≤ 0.3), and CSA did not vary by gender ( P = .426) or side ( P = .821). Conclusions The CSA was most susceptible to malposition in anteversion/retroversion. Deviations as little as 5° in anteversion resulted in a CSA >2° from true AP view.
