Introducción. El HIV es el agente causal del SIDA. Entender las formas en las que evade la respuesta inmune del hospedador es crítico para el desarrollo racional de una vacuna efectiva. Aunque la amplia extensión del tratamiento antirretroviral es capaz de impactar significativamente en los procesos naturales de selección y escape, pocos trabajos han estudiado esta temática. El objetivo de esta tesis fue identificar aquellas regiones virales sometidas a presión de selección inmune y evaluar el impacto del tratamiento antirretroviral en su selección y persistencia. Metodología y resultados. Mediante análisis estadístico se identificaron 12 mutaciones virales asociadas a escape inmune, 6 de ellas en regiones no caracterizadas previamente como epítopes. El análisis de secuencias obtenidas en los últimos 20 años de epidemia sugirió que varias de ellas han incrementado su frecuencia hasta estar presentes en la mayoría de las cepas virales actuales. La evaluación de la respuesta inmune péptido específica mostró reconocimiento principalmente de dos péptidos con restricción para el alelo HLA A02 (A02P65 y A02P84), uno para el alelo HLA A03 (A03P28) y uno para el alelo HLA B07 (B07P357). Para los dos últimos y para A02P65 se demostró que la variante de escape disminuía el reconocimiento por la respuesta inmune mientras que, para todos los casos, se evidenció la capacidad de inducir una respuesta polifuncional secretora tanto de IFN como de IL 2 y MIP 1 . Se evaluó también el nivel de agotamiento inmune y, aunque la técnica de tetrámeros presentó varios inconvenientes, se detectó una mayoría de respuesta no exhausta en los casos exitosos. La secuenciación del gen viral gag mostró la presencia de escape viral en la mayoría de los epítopes analizados y sugirió que la positividad de la respuesta inmunológica estaba asociada a una mayor afinidad del epítope por el alelo HLA que lo presenta. Al mismo tiempo, el análisis de secuencia de dicho gen viral en muestras repetidas de 50 pacientes separadas por un promedio de 3 años, demostró la presencia de recambio activo de variantes virales hacia el estado de escape durante el período de muestreo. Se encontró también que la selección activa resulta menos frecuente en los pacientes bajo tratamiento y el análisis del virus en dichos pacientes sugirió que esto sería debido a una reducción significativa de la capacidad evolutiva del virus en el contexto del tratamiento altamente efectivo. Finalmente, un análisis filogenético más profundo de las secuencias estudiadas permitió datar el inicio de la epidemia del HIV 1 en Argentina en la primera mitad de la década de 1970. Conclusiones. El presente trabajo de tesis permitió caracterizar regiones de la proteína viral Gag de frecuente reconocimiento inmune sobre las cepas del HIV 1 circulantes en nuestra región y caracterizar su dinámica a nivel poblacional a lo largo de veinte años de epidemia. El conjunto de datos obtenido de los ensayos inmunológicos confirma la aproximación estadística para identificar sitios de reconocimiento inmune demostrando respuestas citotóxicas polifuncionales mientras que el estudio de muestras seriadas en pacientes, la mayoría de ellos en estados clínicos avanzados, sugiere que la selección del escape viral se extiende más allá de los primeros momentos de la infección. En este sentido, el inicio del tratamiento puede tener un beneficio adicional al prevenir dicho escape por reducción significativa de la evolución viral. Introduction. HIV is the etiological agent of AIDS. To understand the way the virus evade the immune response of the host is critical for the rational development of an effective vaccine. Although the expansion of treatment access is able to shape the natural process of selection and escape, few studies have analyzed this phenomenon. The objective of the present thesis was to identify viral genomic regions where the virus undergoes selective immune pressure and to evaluate the impact that the antiretroviral therapy may have in their selection and persistence. Methodology and results. A total of 12 viral mutations associated to immune escape were identified, six of them in regions not previously characterized as epítopes. The analysis of a set of viral sequences obtained from samples collected in the last twenty years suggested that many of them have increased their prevalence in time until become the most prevalent state of circulating viral strains. The evaluation of peptide specific immune response showed that the main targets of immune recognition were located in two HLA A02 restricted epítopes (A02P65 and A02P84), in a HLA A03 restricted one (A03P28) and in a HLA B07 restricted one (B07P357). For the last two and also for A02P65 we found that the escape mutations significantly impair recognition by the immune response. For all of them, the capacity of stimulating a polyfunctional response able to release IFN , IL 2 and MIP 1 was shown. Also, the evaluation of immune exhaustion through tetramer technique showed, in spite of many technical inconveniences, a low level of exhaustion in the majority of successfully analyzed samples. The sequencing of viral gene gag showed the presence of escape in the majority of the epítopes analyzed and suggested that detection of immune response was associated to a greater affinity of the epítope for the HLA allele. At the same time, the analysis of the same region in repeated samples collected from 50 patients within a time period of 3 years showed the presence of active replacement of viral sequences toward the escape state. That active replacement seems to be associated to the lack of antiretroviral therapy and the analysis of the virus quasispecies in under therapy patients suggested that the significant reduction of evolutive capacity of the virus in the context of the effective therapy is likely the reason for that observation. Finally, a deeper phylogenetic analysis of the sequences under study allowed us to locate the initiation of the HIV epidemic in Argentina at the first half of the 1970s decade. Conclusions. The present thesis allowed us to characterize the regions of the viral protein Gag under frequent immune recognition at the population level and to characterize their dynamics in the last twenty years of epidemic. The set of data obtained from the immunologic assays confirms the statistical approach of the identification of sites under immune recognition showing polyfunctional cytotoxic responses while the study of repeated samples from patients that were in their majority in advanced stages of infection show that selection of escape not only occurs in the first stages of infection. In this sense, the initiation of treatment could have an additional benefit in preventing the immune escape by significantly reducing the evolutive capacity of the virus. Fil: Dilernia, Darío A..
