Resumen Objetivo: Explorar los conocimientos y actitudes de los profesionales sanitarios con relación a las voluntades previas en dos regiones de España: Cantabria e Islas Baleares. Metodología: Estudio descriptivo transversal mediante cuestionario autoadministrado online. Se analizaron variables sociodemográficas, conocimientos y actitudes relacionados con las voluntades previas. El estudio se aprobó por el Comité Ético de Investigación Clínica de Cantabria. Resultados: Respondieron al cuestionario 274 profesionales sanitarios, de Cantabria 222 y de Islas Baleares 52. Un 75,6% respondió que otorgaría documento de voluntades previas y un 86,6% que lo recomendarían a sus pacientes crónicos. Un 15,5% expresó sentirse preparado para informar, un 95,2% que desearía recibir formación. Las enfermeras tuvieron más conocimientos concretos que otros profesionales con relación al procedimiento, validez y consulta del documento. Cantabria y Baleares presentaron diferencias estadísticamente significativas en intención de otorgar (p = 0,005) y en haber consultado o propuesto consultar el Registro de Voluntades Previas durante la práctica profesional (p = 0,000), en ambos casos con mayor porcentaje en Cantabria. Conclusiones: La percepción de los profesionales acerca de sus conocimientos relacionados con las voluntades previas es escasa, y su actitud es de respeto a estas, y las consideran útiles para todos los implicados. Su especial relevancia en la población de mayor edad constituye a las enfermeras especialistas en enfermería geriátrica-gerontológica principales beneficiarias e impulsoras de las voluntades previas.

Objetivos: Describir la forma en que se establecen normativamente el consentimiento informado (CI) y las directivas anticipadas (DA) en países latinoamericanos en relación a la asistencia en salud y la investigación, y su análisis desde la bioética. Métodos: Se realizó un análisis comparativo de las normativas vigentes de los 21 países latinoamericanos respecto al CI y las DA. Se confeccionaron reportes de extracción de datos que fueron diseñados antes de iniciar el proceso de investigación para cada país. Se analizaron cualitativamente similitudes y diferencias respecto a variables predefinidas. Se realizaron tablas para sintetizar la información donde fue posible. Resultados: Dieciocho países latinoamericanos prevén una norma específica para el uso del CI en el ámbito clínico. Seis países han incorporado normativas respecto al uso del CI en la virtualidad. Respecto al uso del CI en investigación, 20 países latinoamericanos prevén una norma específica para este ámbito. En el caso de la exposición académica, son 6 países los que nombran esta situación en la normativa. Siete países latinoamericanos cuentan con normativa para establecer DA. Conclusiones: América Latina ha presentado una notable evolución en torno a normar dos de los más importantes instrumentos del ejercicio de la autonomía, como los son el CI y las DA. La legislación sobre ambos instrumentos no garantiza mayor autonomía en salud, pero su existencia como reguladora de acciones para la toma de decisiones constituye el primer paso para su ejercicio.

Introducción: La ley número 18473 de voluntades anticipadas fue promulgada en Uruguay en el año 2009, reglamentándose cuatro años después. Esto evidencia una propensión hacia la autonomía y la planificación del futuro de los enfermos. Objetivo: Determinar el conocimiento por parte de los profesionales de la salud en cuanto a la ley, su empleo en la clínica y acceso al formulario de voluntades anticipadas. Método: Investigación descriptiva, mediante una encuesta anónima a médicos uruguayos generales y especialistas en actividad, enviada por redes sociales. Resultados: Se obtuvieron 363 respuestas, con un 97 % de profesionales que atendían enfermos, un 79,5 % no habían extendido el documento y más del 50 % no tenía conocimiento de la ley. Conclusión: El conocimiento sobre las voluntades anticipadas en el cuerpo médico uruguayo es escaso y es aún más su aplicación a los pacientes.

La Ley Nº 18.473 y el Decreto que la reglamenta (385/013) fueron concebidos con la finalidad de dar un marco normativo a la posibilidad que tiene una persona de establecer sus preferencias sobre ciertos actos médicos cuando todavía es capaz de consentir, ejerciendo su autonomía de manera anticipada ante la eventual pérdida de capacidad de decisión. Por lo tanto, el hecho de llevar a cabo o no un procedimiento médico no dependerá de la capacidad de consentir de la persona en ese momento, esto es lo que la diferencia del consentimiento informado habitual.

Las Voluntades Anticipadas son una forma de defensa de la autonomía y dignidad de los usuarios en el final de la vida. En Chile, no existen leyes que la validen, por lo cual los enfermeros toman especial relevancia por ser quienes otorgan cuidados en todo el ciclo vital. Objetivo: Conocer la percepción de los profesionales de enfermería de una Unidad de Paciente Crítico sobre su rol en la voluntad anticipada en el final de la vida. Metodología: Estudio cualitativo, paradigma post positivista con enfoque de estudio de caso. Realizado entre Octubre y Noviembre del 2017. Se logró la saturación teórica mediante la realización de 6 entrevistas de tipo semiestructuradas. Se utilizó análisis de contenido. Resultados: Ningún profesional de enfermería manejaba el concepto, sin embargo, varios lograban identificar elementos vinculados a la temática. Además, se identificó un escaso abordaje del tema durante la formación de pre y postgrado y la necesidad sentida de ampliar los conocimientos. Algunos asociaron el rol con la educación y apoyo, otros no lograron identificar la vinculación. Se evidenciaron algunas prácticas que apuntan a respetar las voluntades de las personas, con diversas disyuntivas en el proceso. Conclusión: Falta un largo camino por recorrer en cuanto al respeto de las preferencias de las personas al final de la vida, y para lograrlo es esencial el aumentar el conocimiento que poseen los enfermos sobre la temática. Posteriormente, es importante el establecer claramente su forma de aplicación y estrategias para facilitar el proceso, además de incorporarlo en el lenguaje cotidiano.

La Convención Interamericana para la Protección de los Derechos Humanos de las Personas Mayores regula la obligación de los Estados parte de implementar un proceso para que la persona mayor pueda manifestar, de manera expresa, su voluntad anticipada y sus instrucciones respecto de las intervenciones en atención de la salud. Chile ha ratificado este instrumento. El presente trabajo analiza el estado de la institución desde una perspectiva comparada y considera la oportunidad de su regulación expresa en Chile.

Resumen Introducción: Los ingresantes a una carrera universitaria cuentan con representaciones acerca de la carrera elegida y de su futuro profesional. Objetivo: Identificar la representaciones de los aspirantes de la carrera de Medicina con respecto a la imagen del rol profesional. Metodología: Estudio observacional y transversal, a partir de una encuesta implementada a 1857 aspirantes. Análisis de los datos: ANOVA o Datos Categorizados, con un 95% de confianza. Resultados y Conclusiones: En la población se detectó una edad de 19,03±0,06 años, no inserta en el mercado laboral, una mayor frecuencia de padres con estudios universitarios. El área elegida para el desempeño del rol es la de “Especialidades Médicas”. Los estudiantes encuestados refirieron en cuanto a la imagen del rol profesional en primer lugar, las cualidades personales, la formación permanente y la responsabilidad. Predomina el deseo de ayudar y curar, a través de un rol profesional idóneo. En cuanto a los aspectos preventivos de la carrera como futuro profesional, el 48% enuncian el compromiso social y ayudar a la gente, reafirmando el aspecto social y solidario del Rol Profesional. El 50 % proyecta la idoneidad profesional y continuar con la formación permanente, enfatizando el aspecto cognitivo.

[ES] Objetivo: Diseñar y validar un cuestionario dirigido a profesionales para la medición de la calidad del proceso de planificación anticipada de decisiones y determinar sus propiedades psicométricas. Método: Estudio instrumental de validación de cuestionario. Emplazamiento: centros de atención pri maria y consultas externas de hospitales de Ferrol, Ourense, Monforte-Lugo y el Área Sanitaria V del Servicio de Salud de Asturias. Participantes: médico/as y enfermera/os. Fases: 1) diseño de la primera versión del cuestionario mediante técnica Delphi; 2) construcción de la segunda versión de 21 ítems, tras acuerdo de jueces y prueba W de Kendal; 3) pilotaje, análisis de consistencia interna mediante alfa de Cronbach y coeficiente omega,test-retest mediante coeficiente de correlación de Pearson; y 4) validación, test de esfericidad de Barlett y medida de Kaiser-Meyer-Olkin, análisis factorial exploratorio con rotación varimax y estudio de las dimensiones del cuestionario (número, coeficiente de correlación intraclase y correlación), consistencia interna mediante alfa de Cronbach. Resultados: Estudio piloto con 28 profesionales. Fiabilidad ɷ = 0.917, α = 0, 841 y coeficiente de correlación test-retest 0,785 (intervalo de confianza del 95%: 0,587-0,894; p < 0,001). Validación del cuestionario (21 ítems) en 204 profesionales. El análisis de los ítems y el factorial exploratorio (test de Barlett [χ2 = 1298,789] y KMO = 0,808; p < 0,001) arrojaron un modelo de cinco factores que explican el 64,377% de la varianza total, con 18 ítems agrupados en 5 dimensiones (información, preferencias, intención de conducta, capacitación y comunicación). El alfa de Cronbach del cuestionario global fue de 0,841. Conclusiones: El cuestionario ProPAD-pro ha demostrado ser un instrumento válido y fiable para evaluar la calidad del proceso de planificación anticipada de decisiones. [EN] Objective: To develop a questionnaire aimed to measure the quality of the advance care planning process in healthcare professionals and to assess its psychometric quality. Method: Instrumental questionnaire validation study. Advance healthcare directives are intended for the individual to personally express their will and preferences about healthcare and treatment ahead of time. This literature review aims to describe the concept and structure of advance directives and the ethical aspects involved in patient care. Using the keywords "Advance Healthcare Directive" AND "Ethical Implication" and its Spanish equivalents (voluntades anticipadas, implicaciones éticas), five databases were accessed: ProQuest, Philosophy (JSTOR), PubMed, Web of Science and Scielo. Publications were narrowed down to the 2010-2018 period. Thirty-one articles were obtained and read critically. Results of this review were grouped into the following categories: concept, structure, clinical situations in which advance directives apply, strengths and limitations, as well as the ethical aspects involved. End of life-related experiences in which a person loses their ability to make decisions and cannot express their wishes, so it is impossible to know their will, are more common nowadays. Most health workers are not appropriately trained in the preparation and application of advance directives, which becomes an opportunity to research and delve deeper into the subject. Advance directives are a tool that gives health workers reliable information on a patient's values and wishes, so it is vital to train them to provide respectful quality care. [ABSTRACT FROM AUTHOR]

Resumo Este estudo teórico discorre sobre a bioética no sentido de promover e assegurar a dignidade do paciente de uma forma segura, que obedeça a sua tomada de decisão, seguindo normativas e protocolos que garantam o cumprimento dessa vontade, e trazendo também segurança para equipe assistencial. A ausência de consenso no meio jurídico, por falta de regulamentações que explicitem as regras para a elaboração de um modelo de diretivas de vontade, gera insegurança nos profissionais de saúde envolvidos, familiares e paciente. Para garantir esse direito, além da normatização jurídica, é necessário que os profissionais tenham pleno conhecimento do assunto, a fim de orientar e informar corretamente seus pacientes. Não basta assegurar ao indivíduo o direito de manifestar sua vontade, é preciso ainda a certeza de que esta será cumprida. Há a necessidade de ampliar discussões acerca da temática, com ênfase na realidade brasileira. Abstract This theoretical study discusses bioethics in the sense of safely promoting and ensuring the patients’ dignity, respectful of their decision making, following regulations and protocols that help fulfill this will and bring safety to the care team. The lack of clear and consensual legal regulations on how to elaborate an advance directive generates uncertainty among healthcare providers, patients and their families. Beyond legal standardization, professionals must also have full knowledge on the topic to correctly guide and inform their patients, thus guaranteeing this right. But ensuring the right to express one’s will is not enough—one must be assured that such whishes will be fulfilled. Further and broader discussions on this topic anchored on the Brazilian reality are needed. Resumen Este estudio teórico trata la bioética en el sentido de promover y garantizar la dignidad del paciente de forma segura, que acepte su toma de decisiones, siguiendo normas y protocolos que garanticen el cumplimiento de esa voluntad, además de brindar seguridad al equipo de atención. La falta de consenso en el ámbito legal respecto a normativa para la elaboración de un modelo de directivas de voluntad produce inseguridad en los profesionales de la salud involucrados, familiares y pacientes. La garantía de este derecho, además de la regulación legal, requiere que los profesionales conozcan el tema para orientar e informar correctamente a sus pacientes. No basta con garantizar al individuo el derecho a expresar su voluntad, sino que también es necesario la certeza de que esta se cumplirá. Son necesarios más estudios para ampliar las discusiones sobre el tema, con énfasis en el contexto brasileño.

Entende-se que se deve buscar uma forma, ainda não encontrada e implantada no Brasil, de fazer que o indivíduo, mesmo incapaz de se comunicar, fique seguro do conhecimento de sua vontade por parte dos profissionais de saúde. Como método, o estudo busca aprofundar conceitos utilizados pela doutrina, resoluções e leis. Conclui-se que é preciso dar mais publicidade às diretivas antecipadas de vontade, informando a população sobre sua existência e permitindo que as pessoas expressem seus desejos relativos à saúde. Ademais, é necessário criar um banco de dados que permita o compartilhamento da manifestação de vontade do indivíduo com a rede hospitalar. Abstract This article analyzes how the autonomy and dignity of individuals in cases of terminality can be preserved using the advance directives of will. It is understood that a procedure, yet to be found or implemented in Brazil, should be sought to ensure that individuals, even when incapable of communicating, can be assured that their will by health professionals. As a method, this study seeks to deepen concepts used by doctrine, resolutions and laws. It is concluded more publicity should be assigned to the advance directives of will, informing the population about their existence and allowing people to express their desires regarding health. Furthermore, it is necessary to create a database that allows sharing an individual’s expression of will with the hospital network. Resumen El presente trabajo analiza cómo la autonomía y dignidad del individuo en casos de terminalidad puede ser preservada a través de las directivas anticipadas de la voluntad. Se entiende que se debe buscar un camino, aún no encontrado e implantado en Brasil, para que el individuo, incluso incapaz de comunicarse, esté seguro del conocimiento de su voluntad por parte de los profesionales de la salud. Como método, el estudio busca profundizar conceptos utilizados por doctrina, resoluciones y leyes. Se concluye que es necesario dar más publicidad a las directivas anticipadas de voluntad, informando a la población sobre su existencia y permitiendo que las personas expresen sus deseos con respecto a la salud. Objective: To understand how doctors and family caregivers understand the applicability of the Advance Directives to end-of-life care. Method: qualitative, descriptive and exploratory research, conducted with seven doctors of a university hospital in southern Brazil and seven family caregivers of patients at the end-of-life, from October to December 2014, from semi-structured interviews, submitted to discursive textual analysis. Results: two categories emerged: Advance Directives: the right to personal autonomy and conflicts and dilemmas in the applicability of Advance Directives. Conclusion: The Advance Directives are related to the respect of personal autonomy, professional support and reduction of the anguish and suffering of family care in view of the decision-making processes that involve the end of life. Aim: to know the perception of the health professionals about the applicability of the anticipated directives of will in the neonatal and pediatric context. Method: qualitative, descriptive and exploratory study, developed in pediatric and neonatal pediatric hospitalization units - of a university hospital in southern Brazil. Participated 16 professionals of the nursing and medical team. The collections took place through a semistructured interview, from September to October, 2015. A discursive textual analysis was performed. Results: Evidenced aspects that hinder the applicability of the Directives, such as: denial of death attitudes, weak communication, understanding of death as an event of a punctual nature, post hierarchy and professional instrumentalization. As facilitators, such as professional (multi) interaction, bonding and support in spirituality. Final considerations: There is a misunderstanding and unease about death and all its personal significance, reflecting the difficulty of applying the Directives in this context. Resumo A participação do paciente no planejamento de seus cuidados de saúde respeita seu direito à autodeterminação. Com isso, esta pesquisa teve como objetivo avaliar o posicionamento de médicos que atuam no Serviço de Emergência Hospitalar do Hospital de Clínicas de Porto Alegre em relação às diretivas antecipadas de vontade de pacientes. Trata-se de estudo transversal com 32 médicos. A maioria dos participantes (81,3%) afirmou conhecer as diretivas antecipadas de vontade, mas apenas 6,3% tinham conhecimento suficiente; 87,5% foram favoráveis à sua utilização e a vontade do paciente foi considerada determinante em três dos quatro cenários apresentados; e 84,4% consideraram necessária legislação específica além da Resolução 1.995/2012 do Conselho Federal de Medicina. Ao final do estudo foi concluído que a maioria dos médicos tinha conhecimento prévio sobre diretivas antecipadas de vontade e se posicionara a favor da utilização deste tipo de documento em emergência hospitalar. Abstract Patient participation in planning their health care means respecting the patient’s right to self-determination. In this sense, this study aimed to examine the position of doctors working in the Hospital Emergency Service of the Hospital de Clínicas de Porto Alegre regarding patient advance directives. This is a cross-sectional study conducted with 32 physicians. Most participants (81.3%) declared to have knowledge about advance directives, but only 6.3% declared having sufficient knowledge; 87.5% were in favor of their use and the patient’s will was considered decisive in three of the four scenarios presented; and 84.4% considered that specific legislation on the matter is necessary in addition to Resolution 1995/2012 of the Federal Council of Medicine. Our results allowed us to conclude that most physicians had prior knowledge about advance directives and were in favor of using this type of document in hospital emergency care. Así, esta investigación tuvo como objetivo evaluar el posicionamiento de médicos que actúan en el servicio de emergencia hospitalaria del Hospital de Clínicas de Porto Alegre en relación a las directivas anticipadas de voluntad de los pacientes. Se trata de un estudio transversal con 32 médicos. Introduction: The prolongation of the survival of the elderly and the progress in the treatment of chronic diseases has increased the prevalence of functionally dependent persons. The advancement of the therapeutic and therapeutic resources of medicine in the last decades has brought dysthanasia as a side effect. A relatively recent phenomenon in society has led to discussions of a clinical, bioethical and juridical nature, in order to solve dilemmas and modernize laws, adapting to this new reality. Objectives: To analyze the role of advance directives in the context of palliative care. Methodology: A narrative review was carried out investigating articles published in the Journal of Bioethics of the Federal Medical Council of Brazil and its last resolutions in the past ten years. To this end, current data was collected in the legal context, analyzing the legal support of the decision-making process by the geriatrician. Result: There were identified nine articles and three main resolutions. Conclusions: In Brazil, there is no specific legislation on the subject. Although the new Code of Medical Ethics represented a breakthrough, it was not enough to introduce this practice into the physician-patient relationship. Objective: to understand how nurses understand the applicability of early intension directives in the care of the last phases of life. Methodology: qualitative, descriptive and exploratory research, carried out with eight nurses from a university hospital in the south of Brazil, in December 2014, based on semi-structured interviews, submitted to discursive textual analysis. Results: three categories emerged: conflicts in patient care in the final phase; Advantages and difficulties in the applicability of antecipated directives of intention; Benefits in the applicability of the antecipated intension directives by the nursing. En años recientes, las directivas anticipadas (DAs) han sido incorporadas de manera creciente en las sociedades latinoamericanas como un instrumento que permite a los pacientes especificar el modo en que desearían ser tratados en caso de que no sean competentes. El objetivo principal de estos instrumentos es, por lo tanto, incrementar el respeto por la autonomía de los pacientes. No obstante, es importante para ello conocer la experiencia que arrojaron las DAs en otros países, para poder diseñar así la mejor manera de implementarlas. Igualmente importante es identificar posibles facilitadores y barreras que pudiera ofrecer el contexto latinoamericano para llevar a cabo dicho proyecto. En este artículo se analiza la evolución de las DAs en los Estados Unidos, se concentra especialmente en las dificultades que surgieron a lo largo de este proceso, y que se deben fundamentalmente a un abordaje excesivamente individualista del proceso de toma de decisiones. En contraste con ello, se argumenta que el rol importante que desempeñan generalmente las familias, u otras personas cercanas al paciente, en las sociedades Latinoamericanas ofrece condiciones favorables para lograr un abordaje más relacional, capaz de superar los problemas del modelo individualista. Sin embargo, se argumenta también que se necesitan esfuerzos importantes para mejorar la calidad de la relación médico-paciente-familia. En particular, la superación de las acciones paternalistas todavía ejercidas por algunos médicos, frecuentemente en conjunto con los familiares, constituye una condición necesaria para la implementación de las DAs.

Resumen El artículo aborda la perspectiva de profesionales de oncología y cuidados paliativos de Argentina sobre la planificación anticipada de los cuidados en el final de la vida de pacientes con enfermedades oncológicas avanzadas. A partir de un diseño de investigación cualitativa, basado en entrevistas semi-estructuradas con profesionales de diferentes disciplinas, se analizaron las dificultades para la participación de los y las pacientes en el proceso de toma de decisiones y en los procesos de consentimiento informado en torno a los cuidados en el final de la vida. Pese a valorar la planificación anticipada del cuidado e instrumentos como las directivas anticipadas, se identifican barreras para la implementación de estas prácticas. La falta de herramientas comunicacionales para abordar este tipo de conversaciones con los pacientes, el ocultamiento de la información sobre el diagnóstico y/o el pronóstico de la enfermedad, la falta de articulación entre equipos que realizan el seguimiento de pacientes con enfermedad avanzada, y la resistencia de familiares y pacientes, limitan los márgenes de decisión y consentimiento por parte de los pacientes. Abstract The article addresses the perspective of oncologist and palliative care professionals of Argentina on the advance end of life care planning for patients with advanced cancer. Based on semi-structured interviews with professionals from different disciplines, the paper explores barriers for the patient participation in the decision-making process in end of life care. Despite the acceptance of advance care planning and instruments such as advance directives, barriers to the implementation of these practices are identified. The lack of communication skills to deal with this type of conversation with patients, the concealment of information on the diagnosis and prognosis of the disease, the lack of coordination between teams that keep track of patients with advanced disease, and the resistance from relatives and patients, limit the margins of decision and consent by patients. Resumo O artigo aborda a perspectiva dos profissionais de oncologia e cuidados paliativos na Argentina sobre o planejamento antecipado dos cuidados de fim de vida para pacientes com doenças oncológicas avançadas. Partindo de um desenho de pesquisa qualitativa, com base em entrevistas semiestruturadas com profissionais de diferentes disciplinas, o artigo analisa as dificuldades para a participação dos pacientes no processo de tomada de decisão e nos processos de consentimento informado em torno aos cuidados de fim de vida. Apesar de valorizar o planejamento do cuidado antecipado e instrumentos como as diretrizes antecipadas, identificam-se barreiras para a implantação dessas práticas. A falta de instrumentos de comunicação para atender esse tipo de conversa com os pacientes, o encobrimento do diagnóstico e / ou prognóstico da doença, a falta de articulação entre as equipes que acompanham os pacientes com doença avançada e a resistência dos familiares e pacientes, limitam as margens de decisão e consentimento por parte dos pacientes.

Objective: to analyze the trends in the Brazilian production of theses and dissertations in the health area on Advance Directives of Will. Method: narrative review carried out in July 2021 in the Theses and Dissertations Bank of the Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (Coordination for the Improvement of Higher Education Personnel), and originated from the research question: "What is the trend in the productions of theses and dissertations on Advance Willingness?". The strategies: "advance directive", "living will" and "advance care planning" were used, and 86 productions were reported. Studies on the theme in the health area were included, without a time frame. After applying the inclusion criteria, the corpus of analysis was limited to 28 documents. Results: we identified a greater number of dissertations among the productions, mostly from the Southeast, in 2019, and in the area of Bioethics. The qualitative, descriptive and exploratory methodological design predominated. Two major themes were evident: the use of technologies as an auxiliary device to the applicability of Advance Directives of Will; and the perception of patients, families and health professionals about these directives. Conclusion: Advance Directives of Will are recognized by professionals, patients and families as instruments of autonomy, but there is ignorance, difficulties and limitations in their construction and application. Objetivo: analizar las tendencias de las producciones brasileñas de tesis y disertaciones del área de la salud sobre las Directivas Antecipadas de Voluntad. Método: revisión narrativa realizada en julio de 2021 en el Banco de Tesis y Disertaciones de la Coordenação de Aperfeiçoamento de Pessoal de Nível Superior, y originada a partir de la pregunta de investigación: "¿Cuál es la tendencia en las producciones de tesis y disertaciones sobre Directiva de Voluntad Anticipada? Se utilizaron las estrategias: "directiva anticipada", "testamento vital" y "planificación anticipada de cuidados", y se informó de 86 producciones. Se incluyeron estudios sobre el tema en el área de la salud, sin un marco temporal. Tras aplicar los criterios de inclusión, el corpus de análisis se limitó a 28 documentos. Resultados: Identificamos un mayor número de disertaciones entre las producciones, en su mayoría del sudeste, en 2019 y en el área de Bioética. Predominó el diseño metodológico cualitativo, descriptivo y exploratorio. Se evidenciaron dos grandes temas: el uso de las tecnologías como dispositivo auxiliar a la aplicabilidad de las Directivas Anticipadas de Conducta; y la percepción de los pacientes, familiares y profesionales de la salud sobre dichas directivas. Conclusión: Las voluntades anticipadas son reconocidas por los profesionales, los pacientes y las familias como instrumentos de autonomía, pero existe desconocimiento, dificultades y limitaciones en su construcción y aplicación. 47. Percepción y Expectativas de los Pacientes con Cáncer sobre las Directrices de Voluntad Anticipadas

Introduction: Anticipated Directives of Will is a document that allows patients to register previously and clearly expressed wills about care and treatments they want, or not, to receive in a situation of inability to express their will. Objective: Assess the perception and expectations of cancer patients about the concept and applicability of the Anticipated Directives of Will. Method: Descriptive, quantitative approach and cross-sectional study carried out with 346 patients from a reference hospital in oncology in the city of Recife-PE. The data were collected through the application of the game “Cards on the Table” (Go Wish®) and a questionnaire, according to the Likert-scale (degree of disagreement or agreement, from 0 to 10). Results: The mean of the knowledge of the term “Anticipated Directive of Will” was 0.64 points among patients. After explaining its meaning, the mean of the intent to prepare to was 8.58 points. The mean acceptance among patients was 9.42 points for the creation in the Brazilian legislation and 9.64 points for the implantation in the hospital where they were hospitalized. Conclusion: In the perception of these patients, although little known, the advance directives have shown to be an instrument capable of preserving their dignity and autonomy. Patients showed interest both in their elaboration and in their applicability, however, they expect to receive more information and guidance from health professionals. Introducción: La Directiva de Voluntad Anticipada es un documento que permite registrar deseos, expresados previa y expresamente por el paciente, sobre los cuidados y tratamientos que quiere, o no, recibir en una situación de incapacidad para expresar su voluntad. Objetivo: Evaluar la percepción y las expectativas de los pacientes con cáncer sobre el concepto y aplicabilidad de las Directrices Anticipadas de Voluntad. Método: Estudio descriptivo y transversal de carácter cuantitativo, realizado con 346 pacientes de un hospital de referencia en oncología de la ciudad de Recife-PE. Los datos fueron recolectados mediante la aplicación del juego “Cartas sobre la Mesa” (Go Wish®) y un cuestionario, según la escala numérica Likert (grado de desacuerdo o acuerdo, de 0 a 10). Resultados: El conocimiento del término “Directiva de voluntad anticipada” alcanzó sólo un promedio de 0,64 puntos entre los pacientes. Tras explicar su significado, la intención de obtener una media de 8,58 puntos. La aceptación media entre los pacientes fue de 9,42 puntos para la implantación en la legislación brasileña y de 9,64 puntos para la implantación en el hospital donde fueron hospitalizados. Conclusión: En la percepción de estos pacientes, aunque poco, como directivas anticipadas representan un instrumento capaz de preservar su dignidad y autonomía. Los pacientes mostraron interés tanto en su evolución como en su aplicabilidad, sin embargo, agregando más información y orientación de los profesionales que atienden un tema. Método: Estudo descritivo e transversal, de natureza quantitativa, realizado com 346 pacientes de uma instituição hospitalar de referência em Oncologia no município de Recife-PE. Os dados foram coletados por meio da aplicação do jogo “Cartas na Mesa” (Go Wish®) e de um questionário, desenhado segundo a escala numérica de Likert (grau de discordância ou concordância, de 0 a 10). Resultados: O conhecimento do termo “Diretiva Antecipada de Vontade” alcançou apenas a média de 0,64 pontos entre os pacientes. Após a explicação de seu significado, a intenção de elaboração obteve média de 8,58 pontos. A média de aceitação entre os pacientes foi de 9,42 pontos para implantação na legislação brasileira e 9,64 pontos para implantação na instituição hospitalar onde estavam internados. Conclusão: Na percepção desses pacientes, embora pouco conhecidas, as diretivas antecipadas se mostraram um instrumento capaz de preservar sua dignidade e autonomia. Os pacientes demonstraram interesse tanto por sua elaboração quanto por sua aplicabilidade, no entanto, esperaram receber mais informações e orientações dos profissionais em relação a essa temática.

La presencia de pacientes con síntomas refractarios e intensas crisis emocionales en la fase final de la vida irrumpen en la actividad diaria de los profesionales sanitarios. Esta eventualidad puede convertirse en una experiencia desconcertante por la escasez de recursos y habilidades adquiridas durante el periodo de formación. La medicina y sus profesionales son inculpados de prolongar el proceso de agonía y no aliviar el sufrimiento y la angustia. La dificultad de atender al paciente moribundo reside en las demandas psicológicas que produce y los conflictos internos que genera. ¿Qué se necesitaríamos nosotros en esa situación?. Es imprescindible disponer de una guía de actuación que oriente nuestro proceso terapéutico defendiendo siempre el mejor interés de la persona. En el presente trabajo se hacen algunas reflexiones sobre la muerte, el énfasis en curar y cuidar, sobre las necesidades y carencias de las personas en la fase final de sus vidas, las directivas anticipadas, los síntomas refractarios y la sedación paliativa y en como saber actuar en situaciones donde las fronteras éticas y legales están poco definidas o en debate permanente.

Resumen Las maniobras de resucitación cardiopulmonar (RCP) introducen un importante cambio de paradigma acerca de la reversibilidad de la muerte, que produce grandes expectativas en la población general, incluso en los médicos. No obstante, a pesar de los avances médicos que han permitido obtener mejores desenlaces, la sobrevida a corto y largo plazo sigue siendo baja y, en muchas ocasiones, hay alteraciones significativas en la calidad de vida y funcionalidad de los sobrevivientes. Al conocer esta importante realidad, surgen como una alternativa las órdenes de no reanimación y diversas sociedades manifiestan la necesidad de instaurar indicaciones para el inicio y finalización de la RCP. Este artículo busca brindar herramientas que ayuden en la toma de decisiones sobre las indicaciones de la RCP, así como generar una reflexión que permita reconocer la limitación humana ante la muerte, entendiendo que en muchos casos lo más beneficioso será acompañar y no reanimar. Abstract Cardiopulmonary resuscitation (CPR) introduces an important paradigm shift regarding the revert of death, which generates great expectations in the general public and even so in physicians. However, despite tha fact that medical progress has led to improved outcomes, short- and long-term survival remains low, accompanied with significant alterations in quality of life and functionality of survivors. Recognizing this reality, “do not resuscitate orders” (DNR) emerge as an alternative and various societies suggest the need to establish indications, whether to start a CPR and when to stop CPR. This article provides tools for decision making on CPR, and to recognize our human limitation in the face of death, understanding that the most beneficial act in many cases will be to keep company and not resucitate.
