3. Influencia de un tratamiento activo en la calidad de vida, dolor y depresión en mujeres con fibromialgia: artículo de revisión.

- Abstract
Fibromyalgia is a syndrome that causes generalized and diffuse muscle pain accompanied by fatigue, although other symptoms also appear, such as depression, anxiety, sleep and rest disturbances, allodynia, and cognitive, psychological and social problems. These symptoms notably affect the patients' quality of life. The forms of presentation of the pathology increase the difficulty of diagnosis and treatment for professionals who treat these patients, with a significant prevalence ratio of 21:1 in favor of women over men. On the other hand, this disease entails a high economic cost, as 2/3 of the expenses are not directly invested to treat the pathology; therefore, they are considered losses. The objective of this review is to demonstrate the effectiveness of adding various active treatments, compared to the usual pharmacological treatment, in improving quality of life and reducing pain and depression in women with fibromyalgia. For this purpose, the WOS, Cochrane Library and PubMed databases were used. Seven articles were selected, aimed at analyzing the effectiveness and safety of therapeutic exercise, a highly cost-effective tool, as part of the treatment of women with fibromyalgia. Clinical trials were selected with at least one intervention group that performed some type of physical activity. The articles indicated that physical exercise as an additional therapy provided benefits in quality of life, pain relief and reduction of depression in women with fibromyalgia compared to those groups that simply followed the usual treatment. Therefore, we can conclude that the usual (drug-based) treatment, accompanied by an active treatment, offers greater benefits in the three abovementioned aspects when compared to treatment with drugs alone. Active treatment is considered safe to add to our toolbox for treating the symptoms of fibromyalgia. Depending on the patient's symptoms, tastes and preferences, we could recommend or prescribe different types of physical activity to promote adherence and tailor the treatment for each case. Introduction: pain is defined as «an unpleasant sensory and emotional experience associated with or resembling that associated with actual or potential tissue damage». It suggested that chronic pain in Latin America has regional differences. Characteristics of chronic pain in Mexican adults have been poorly documented. Material and methods: we use a researcher-administered in-person questionnaire that was applied to Mexican adults at urban areas from six major cities of six Mexican States (6 of 32, 19%). Results: researchers interviewed 1,755 subjects. 1,505 questionnaires contained complete data. 286 persons reported pain in one or more anatomical sites that persisted for over three months (19% of the responders). Conclusion: this is the first survey on chronic pain in Mexican adults from urban areas. We need a robust sample to estimate the impact of chronic pain in Mexico. [ABSTRACT FROM AUTHOR]

5. Fast track en cesáreas. Ensayo clínico para comparar el bloqueo TAP versus morfina epidural.

- Abstract
Introduction: there are few studies about fast track protocols in caesarean sections. They recommend peripheral nerve blocks for pain control. Cesarean section causes severe pain. There are analgesic techniques in the literature, but few studies compare them. Material and methods: 84 patients scheduled for cesarean section were randomized for a triple-blind clinical trial in three parallel groups (TAP, morphine and control). Hypothesis: TAP (transverse abdominal plane) block is not inferiority than epidural morphine. Primary outcome: pain after 24 hours. Results: pain mean at 24 hours and its standard deviation of the TAP group is 2.86 and 2.01 respectively, while in the morphine group it is 2.9 and 1.88. The mean difference between TAP and morphine is -0.04 (95%CI -1.07; 0.99). Considering that the upper limit of the interval is lower than the established non-inferiority limit, the non-inferiority hypothesis is established. TAP group presented less pain at 6 hours (p = 0.037/95%CI -2.52; -0.08) and walking at discharge (p = 0.001/95%CI -2.36; -0.65), starts walking earlier (p = 0.04/95%CI -2.52; -0.08), causes fewer side effects and improves overall satisfaction than the morphine group. Conclusions: the TAP block is not inferior to 1 mg epidural morphine for pain control in cesarean sections. [ABSTRACT FROM AUTHOR]

Introducción: La artritis reumatoide es una patología caracterizada por dolor en las articulaciones, fatiga, dificultad para realizar actividades cotidianas, inflamación persistente y daño progresivo en los tejidos. Según estudios previos, la artritis reumatoide es significativamente más frecuente en mujeres que en hombres. Objetivo: Analizar las experiencias vividas por mujeres con artritis reumatoide, pacientes de un hospital público en la ciudad de Asunción durante el año 2024. Metodología: Estudio cualitativo con un diseño fenomenológico. Se entrevistó a 15 mujeres con artritis reumatoide que se atienden en un hospital público de Asunción, Paraguay. Resultados: Se identificaron tres temas centrales: el primero, impacto en la vida diaria, donde las pacientes deben adaptar su rutina diaria debido a los síntomas de la enfermedad; el segundo, manejo emocional y social, en el cual el apoyo de la familia y amigos es esencial para facilitar el manejo de la enfermedad y proporcionar soporte emocional; y el tercero, tratamiento y manejo de la enfermedad, destacando que el uso de medicamentos es crucial para controlar los síntomas y reducir la progresión de la artritis reumatoide. Conclusiones: La artritis reumatoide impacta significativamente la vida diaria, el manejo emocional y el tratamiento de las mujeres, subrayando la necesidad de un enfoque integral que incluya adaptaciones en la rutina, apoyo social constante y un uso adecuado de medicamentos para mejorar su calidad de vida.

11. Prevalence of diagnosed and undiagnosed osteoarthrosis and associated factors in the adult general Spanish population

Objective: To determine the prevalence and related factors of diagnosed osteoarthrosis (DO) and undiagnosed osteoarthrosis (UO) in the general Spanish adult population. Setting: Cross-sectional study with data from the Spanish National Health Survey 2017. Participants: N = 23,089 adults. Three groups of people were defined: DO, UO, and no osteoarthrosis (NO). Main measurements: Sociodemographic information, lifestyle (tobacco, alcohol, physical activity, body mass index) and health factors (intensity of pain, pain drug consumption, mental health, self-perceived health status, pain involvement in daily living) were collected. Descriptive and bivariate analyses were performed, and a multinomial logistic regression model for the factors associated with each group. Results: The prevalence of DO was 22.4% (95%CI = 21.8;22.9) and 0.9% (95%CI = 0.8;1) of UO. With respect to NO, risk factors for DO and UO included higher pain levels and pain drug consumption. Better self-perceived health status was inversely related with both. More pain involvement in daily living was associated with increased risk of DO, but reduced risk of UO. Conclusions: The prevalence of DO and UO was similar to that reported in Europe, but slightly higher than in low/middle-income countries. It was more prevalent in females, older people, people with worse perceived health status and worse mental health. Higher pain levels and pain drug consumption were risk factors for DO and UO. Better self-perceived health status was protective. Pain involvement in daily living was a risk factor for DO, but protective for UO. Different public health strategies should be considered in view of this. Resumen: Objetivo: Determinar la prevalencia y factores asociados a la artrosis diagnosticada (DO) y no diagnosticada (UO) en la población general adulta española. Emplazamiento: Estudio transversal con datos de la Encuesta Nacional de Salud en España 2017. Participantes: n = 23.089 adultos. Tres grupos definidos: DO, UO y sin artrosis (NO). Mediciones principales: Datos sociodemográficos, estilo de vida (tabaco, alcohol, actividad física, índice de masa corporal [IMC]) y salud (intensidad del dolor, consumo de analgésicos, salud mental, estado de salud autopercibido, impacto del dolor en la vida diaria). Se realizaron análisis descriptivos, bivariados y un modelo de regresión logística multinomial para los factores asociados a cada grupo. Resultados: La prevalencia de DO fue 22,4% (intervalo de confianza [IC] 95% = 21,8;22,9) y UO, 0,9% (IC 95% = 0,8;1). Los factores de riesgo para DO y UO incluyeron mayores niveles de dolor y consumo de analgésicos. El mejor estado de salud autopercibido se relacionó inversamente con ambos. El mayor impacto del dolor en la vida diaria se asoció con un mayor riesgo de DO, pero menor riesgo de UO. Conclusiones: La prevalencia de DO y UO fue similar a la europea, pero ligeramente superior a países de bajo/medio nivel de ingresos, más prevalente en mujeres, personas mayores, con peor estado de salud autopercibido y peor salud mental. Los mayores niveles de dolor y consumo de analgésicos fueron factores de riesgo para DO y UO. El mejor estado de salud fue protector. El impacto del dolor en la vida diaria fue un factor de riesgo para DO, pero protector para UO. Se deben considerar diferentes estrategias de salud pública en vista de estos resultados.

Introduction: COVID-19 has caused many deaths on the planet. To date we do not know its long-term impact. Conversion of hospital centers and home reclusion impacted those with a disease. Currently, the presence of pain has been identified as a chronic complication of COVID-19. Objective: to document the impact of COVID-19 on patients with pain and its chronicity. Material and methods: literature search in the PubMed database for documents that contained the words «COVID», «SARS-CoV», «Pain» in the title. No metaanalyses were identified. We found systematized and documentary reviews on which we based our information. Conclusions: COVID-19 presents chronic pain as a long-term manifestation. Knee osteoarthritis is a prevalent and disabling musculoskeletal condition that affects a large number of individuals worldwide, characterized by its degenerative nature causing a significant impact on the quality of life of those who suffer from it. The study aimed to determine the effects of hydrotherapy in patients with knee osteoarthritis, carrying out a systematic review following the guidelines established by the PRISMA Statement, which included an exhaustive search of the scientific literature of the last 5 years in the following databases: PubMed, Scielo, PEDro and Google Scholar, clinical trials and pilot studies, in English and Spanish that evaluated hydrotherapy as a treatment for patients with knee osteoarthritis were included. The results obtained showed significant beneficial effects in the reduction of pain, joint stiffness, functional capacity and quality of life of patients. In this context, hydrotherapy is considered to represent an effective and safe therapeutic intervention for the management of knee osteoarthritis, complementing or potentially replacing conventional treatment approaches. 17. Tratamiento de la osteítis del pubis: artrodesis con nueva técnica a propósito de dos casos y revisión de la literatura.

- Abstract
Introduction: pain in the pubic symphysis, even if studied in athletes, still sets out diagnostic and therapeutic challenges in other patient groups. Within this context, refractory osteorthritis of the pubic symphysis presents itself as an issue lacking clear therapeutic consensus. Material and methods: two women over 65 years old and presenting osteoarthritis of the pubic symphysis were evaluated. Following unsuccessful conventional therapies, arthrodesis via subpubic plate, wire suture and autologous graft from the iliac crest was performed. Results: after a one-year of following, both patients experienced clinical and radiographic improvement. Bone arthrodesis was achieved without significant complications, proving to be a viable surgical option. Conclusion: this study supports the medium and long-term efficacy of arthrodesis of the pubic symphysis in refractory cases of osteoarthrisis. Therefore, the technique can be considered a surgical option in the management of said condition. [ABSTRACT FROM AUTHOR]

Introduction: Low back pain is presented in approximately 9.4% of the world population. Acupuncture is used by Chinese traditional medicine to stimulate certain points of the body with different types of needles. Among its beneficial effects the pain remission is described. Objective: To evaluate the effectiveness of the acupuntural treatment in patients with lumbar pain assisted in the emergency room. Methods: A quasi-experiment of therapeutic intervention study (without control group) was carried out in 35 patients with lumbar pain, who were assisted in the Natural and Traditional Medicine Emergency Room of Dr. Juan Bruno Zayas Alfonso Teaching General Hospital in Santiago de Cuba, from July to December, 2021. Results: In the investigation there was a prevalence the 40-49 age group (31.4%), female sex (57.1%), housewives, serious and moderate pain at the beginning of the treatment, as well as patients without pain one hour after the therapy. As for the final clinical course, the total of the sample classified in the relieved and improved categories. Conclusions: The effectiveness of acupuncture was demonstrated in patients with acute lumbar pain. [ABSTRACT FROM AUTHOR]

21. Bloqueo femoral vs. Bloqueo del canal de los aductores para analgesia postoperatoria en cirugía de rodilla

La cirugía de rodilla tiene como objetivoeliminar el dolor, restablecer la movilidad articular y mejorar la calidad de vida; está indicada en caso de dolor o incapacidad funcional cuya respuesta al tratamiento conservador no ha sido satisfactorio. Está asociada con dolor moderado a severo en el postoperatorio, motivo por lo que la finalidad en control del dolor es aumentar el confort y optimizar la recuperación, siendo el bloqueo femoral y del canal de los aductores opciones utilizadas actualmente. Objetivo: comparar el bloqueo femoral y del canal de los aductores para analgesia postoperatoria en cirugía de rodilla. Métodos: estudio experimental, comparativo, prospectivo, aleatorio. Muestra de 40 pacientes, ASA I-III, divididos en dos grupos (A o B). El grupo A recibió bloqueo femoral y el B bloqueo del canal de los aductores. La mezcla para ambos fue 10 ml de bupivacaína al 0,25% y 10 ml de lidocaína al 0,5%. Resultados: la intensidad del dolor varió entre leve-moderado, predominado en el grupo A. 9 pacientes ameritaron analgesia de rescate 67% en el A y 33% en el B. El bloqueo motor se reportó en el grupo A. La duración de la analgesia fue de 12 a 24 horas (A), y menor a 12 horas (B). El 80% manifestó satisfacción con la técnica analgésica. Conclusión: ambas técnicas aportaron beneficios similares y pueden ser consideradas buenas y seguras en el manejo multimodal del dolor, aunque, el bloqueo del canal de los aductores demostró acelerar la movilización y facilitar la rehabilitación del paciente.

En la rhêsis introductoria de Ifigenia entre los tauros, la protagonista detalla la historia de su sacrificio en Áulide, la razón de su presencia entre los tauros y su función de sacerdotisa. A continuación, relata el sueño que ha tenido y por el cual interpreta que su hermano Orestes ha muerto. Entra el coro integrado por mujeres griegas que asisten a Ifigenia en el templo y todas ellas entonan un thrênos por Orestes. Se trata de un lamento fúnebre a la distancia movilizado solo por un sueño y la convicción subsiguiente de ser la última representante de un linaje abatido. Es nuestro objetivo analizar el léxico del dolor y su gestualidad tal como este se despliega en el thrênos inicial, operando con las categorías de “guion emocional” y “flujo afectivo”.

In ancient Greek tragedy, the physical pain of male bodies is often depicted. Nonetheless, there are only a few examples of female bodies in pain. Tragic heroines rarely or never refer to their somatic anguish. In the Euripidean corpus, this pain is narrated by male and female Messengers. These Messengers in Alcestis, Medea, and Hecuba, describe the gestures and the miens of the heroines in pain with great detail. Special attention is paid to the skin of these women. This paper examines the gestures of these suffering heroines as responses to physical sources of pain, such as torment by the accumulation of excessive fluids under the skin, the eating of skin and flesh by poison, and the piercing of the skin by sharp objects.

Las artrodesis parciales tienen como objetivo conservar cierta movilidad al fusionar solo ciertas articulaciones dejando móviles solo las articulaciones no patológicas, mientras que las artrodesis totales involucran la fusión total de la muñeca, ofreciendo soluciones definitivas para el dolor intenso e inestabilidad.

Se describe la importancia del dolor crónico de estos pacientes, más adelante se desarrolla las diversas las terapias, así : entre las no farmacológicas se discute el rol de la educación del paciente, la familia, etc y de las diversas terapias farmacológicas: acetoaminofen, aines, opioides; luego se definen algunos principios generales. Finalmente se analiza con profundidad los factores de riesgo para el desarrollo de la gastropatía por AINE y el desarrollo de enfermedad coronaria por inhibidores de COX-2.

Background: Postoperative outcome measurement typically relies on postoperative radiological results and clinical–functional scales; however, there is a growing interest in considering patient satisfaction (including subjective aspects) as part of the success of forefoot surgery. Objective: To determine whether showing a preoperative photograph improves satisfaction in postoperative forefoot surgery patients. Material and methods: An observational, cross-sectional, analytical study was conducted in the foot and ankle unit of our centre. We included 120 participants between 18 and 90 years old who underwent forefoot surgery. The degree of satisfaction was compared using the PSQ-10 questionnaire between a group of patients who received a preoperative foot appearance image at their 3-month postoperative follow-up and those who did not receive it. Results: The overall satisfaction rate was 78.33% at 3 months after the intervention. The 93.6% of patients who received the photograph were satisfied at the postoperative follow-up, while in the control group, it was 86.2% with a p value of 0.218. Conclusion: The degree of satisfaction in patients undergoing forefoot surgery is not associated with the presentation of preoperative photographs. Resumen: Antecedentes: La medición del resultado posterior a la cirugía típicamente se basa en resultados radiológicos posoperatorios y escalas clínico/funcionales; sin embargo, en la actualidad hay un creciente interés por considerar al grado de satisfacción del paciente (que incluyen aspectos subjetivos) como parte del éxito de la cirugía de antepié. Objetivo: Determinar si el mostrar una fotografía de la apariencia preoperatoria mejora la satisfacción en los pacientes posoperados de antepié. Materiales y métodos: Se realizó un estudio observacional, transversal y analítico, en la unidad de pie y tobillo de nuestro centro. Se incluyeron 120 participantes entre 18 y 90 años, que fueron sometidos a cirugía de antepié. Se comparó el grado de satisfacción mediante el cuestionario PSQ-10 entre un grupo de pacientes que recibió, en su control posoperatorio a los tres meses, una imagen del aspecto preoperatorio del pie y aquellos que no la recibieron. Resultados: El grado de satisfacción total fue de 78,33% a los tres meses de la intervención. De los pacientes que recibieron la fotografía, 93,6% estaban satisfechos en el control posoperatorio, mientras que en el grupo control fue de 86,2% con una p = 0,218. Conclusión: El grado de satisfacción en los pacientes sometidos a cirugía de antepié no está asociado a la presentación de la fotografía preoperatoria.

Introduction: The total knee arthroplasty (TKA) has been shown to be a successful and cost-benefit procedure in terms of pain improvement in patient with symptomatic knee osteoarthritis. However, almost a 20% of the patients are not satisfied with the result of the surgery. Material and method: We have carried out a transversal unicentric cases controls study with clinical cases of the own hospital, obtained by a clinical records revision. A total of 160 patients with a TKA with at least 1 year of follow-up were selected. Demographic variables, functional scales (WOMAC and VAS) and rotation of the femoral component through the analysis of the images obtained by CT scan were collected. Results: The total was 133 patients that was divided in two groups. A control group and pain group. The Control group was made up of 70 patients with a mean age of 69.59 years (23 men and 47 women) and the pain group was made up of 63 patients with a mean age of 69.48 years (13 men and 50 women). We didn’t find difference regarding the analysis of the rotation of the femoral component. In addition, we were not found significant differences when applying a stratification by sex. And, the analysis of the malrotation of the femoral component, previously defining limits of value rotation considered as extreme, in any of the case did not show significant differences. Conclusion: The results of the study confirm that malrotation of the femoral component had no influence on the presence of pain at a minimum of one year of follow-up after TKA implantation. Resumen: Introducción: La artroplastia total de rodilla ha demostrado ser un procedimiento exitoso y coste-efectivo en cuanto a la mejoría del dolor en pacientes con artrosis de rodilla sintomática. Sin embargo, en torno a un 20% de pacientes no está satisfecho con el resultado de la intervención. Material y métodos: Realizamos un estudio transversal de casos y controles, unicéntrico con casos hospitalarios, obtenidos mediante la revisión de historias clínicas. Se seleccionaron 160 pacientes portadores de artroplastia total de rodilla con un seguimiento mínimo de un año. Se registraron variables demográficas, escalas funcionales (WOMAC y EVA) y la rotación del componente femoral mediante análisis de imágenes obtenidas por TC. Resultados: La muestra final estuvo compuesta por 133 pacientes divididos en grupo control formado por 70 pacientes, con una edad media de 69,59 años (23 hombres y 47 mujeres) y grupo dolor formado por 63 pacientes, con una edad media 69,48 años (13 hombres y 50 mujeres). No encontramos diferencias en cuanto al análisis de la rotación del componente femoral. Tampoco hubo diferencias significativas al aplicar una estratificación por sexo. El análisis de la malrotación del componente femoral, definiendo unos límites de valor de rotación considerados como extremos, no mostró diferencias significativas en ninguno de los casos. Conclusión: Los resultados del estudio confirman que la malrotación del componente femoral no tuvo influencia sobre la presencia de dolor a un mínimo de un año de seguimiento tras la implantación de una artroplastia total de rodilla.

30. Usefulness of virtual reality in the management of pain associated with venepuncture: a multicentre randomized clinical trial

Objective: To assess the efficacy of virtual reality (VR) in reducing pain and anxiety associated with scheduled blood draws. Material and methods: We conducted multicentre randomized clinical trial in both primary care and hospital settings. The study included children aged 7–12 years undergoing blood extraction procedures between March and October 2022. The intervention group used headsets, and the control group received usual care. Pain was assessed using the visual analogue scale and anxiety using the Groningen Distress Scale. The anxiety of the nursing staff and family satisfaction were assessed with numerical scales ranging from 1 to 10. Results: The study included 83 patients: 40 in the control group and 43 in the VR group. The median age was 10 years (range, 7−12 years). In the VR group, 83.7% of the children reported mild pain, compared to 57.5% in the control group (P = .012). Also, 93% of children in the VR group showed calm or mild anxiety (score, 1−2), compared to 72% of the control group, a difference that was not statistically significant (P = .08). Family satisfaction was higher in the RV group (score ≥ 9/10: 93% of RV group vs 72.5% of control group; P = .026). The nursing staff anxiety score was less than 5 in more than 90% of cases, with no differences between groups (P = .13). Conclusion: The use of VR during venepuncture decreases the pain perceived by children and increases the satisfaction of their families. Resumen: Objetivo: Comprobar la eficacia de la realidad virtual (RV) en la disminución del dolor y ansiedad asociados a extracciones sanguíneas programadas. Material y Métodos: Ensayo clínico aleatorizado multicéntrico realizado en atención primaria y hospitalaria. Se incluyeron los pacientes de 7 a 12 años que acudieron entre marzo y octubre de 2022 a realizarse una extracción sanguínea programada.En el grupo intervención se utilizaron gafas de RV y en el grupo control cuidados habituales. Se evaluó el dolor mediante la Escala Visual Analógica y la ansiedad mediante la Groningen Distress Scale. Se evaluó la ansiedad de enfermería y la satisfacción de los familiares, mediante escala numérica del 1 al 10. Resultados: Se incluyeron 83 pacientes: 40 en el grupo control y 43 en el grupo RV. La mediana de edad fue de 10 años (rango de 7 a 12). El 83,7% de los niños en el grupo RV refirió dolor leve, frente al 57,5% del grupo control (p = 0,012). El 93% del grupo RV mostró calma o ansiedad leve (puntuación 1−2), frente al 72% del grupo control, aunque sin diferencias estadísticamente significativas (p = 0,08).La satisfacción de los familiares fue mayor en el grupo RV (≥9/10: 93% de RV vs 72,5% de controles, p = 0,026). La ansiedad de enfermería fue < 5 en más del 90% de los casos, sin diferencias entre grupos (p = 0,13). Conclusiones: El uso de RV durante la venopunción disminuye el dolor percibido por los niños y aumenta la satisfacción de sus familiares.

RESUMEN Introducción. El dolor es definido por la Asociación Internacional para el Estudio del Dolor (IASP, por su sigla en inglés) como una experiencia sensorial y emocional desagradable. Por tanto, la función que cumple la enfermería en esta quinta constante vital es fundamental. El objetivo de este estudio fue describir el nivel de conocimientos y actitudes de enfermeras profesionales y auxiliares respecto al dolor en una institución asistencial de tercer nivel de atención de Bucaramanga, Colombia. Metodología. Estudio descriptivo de corte transversal, con una muestra conformada por 162 participantes. Se utilizó el cuestionario Knowledge and Attitudes Survey Regarding Pain. Criterios de inclusión: enfermeras profesionales y auxiliares de enfermería con mínimo tres meses en el servicio; criterios de exclusión: estudiantes de enfermería y enfermeras con labores formativas y no clínicas. Resultados. El promedio de edad fue de 31 años, sexo femenino 86.4%, la media de años de experiencia en el servicio fue de 1.54 años. El porcentaje de respuestas correctas de conocimientos fue de 30.7% para enfermeras profesionales y 25.6% para auxiliares. Discusión. Aunque se han realizado a nivel internacional estudios que han evaluado el conocimiento y actitudes del dolor haciendo uso del mismo cuestionario, este estudio es el primero en Colombia que compara enfermeras y auxiliares, evidenciando mejores porcentajes de respuestas correctas y mayor número de preguntas con resultados clasificados como “satisfactorios”. Conclusiones. El nivel de conocimientos y actitudes respecto al dolor obtenidos de las enfermeras es mayor que en los auxiliares; sin embargo, se identificaron en ambas poblaciones debilidades específicas en estas áreas. The objective of this study was to describe the level of knowledge and attitudes of professional nurses and nursing assistants regarding pain in a tertiary health care institution in Bucaramanga, Colombia. Methodology. A descriptive cross-sectional study, with a sample of 162 participants. The Knowledge and Attitudes Survey Regarding Pain was used. Inclusion criteria: Professional nurses and assistants with at least three months in service. Exclusion criteria: Nursing students and nurses with formative and non-missional tasks. Results. The average age was 31 years, 86.4% female, and the average number of years of experience in service was 1.54 years. The percentage of correct answers in knowledge was 30.7% for professional nurses and 25.6% for assistants. Discussion. Although some international studies have evaluated knowledge and attitudes regarding pain using the same survey, this study is the first in Colombia that compares nurses and assistants, showing better percentages of correct answers and a greater number of questions with results classified as “satisfactory.” Conclusions. The level of knowledge and attitudes regarding pain obtained from the nurses is higher than that of assistants. However, specific weaknesses in these areas were identified in both populations. Resultados. A idade média foi de 31 anos, sexo feminino 86,4%, a média de anos de experiência no serviço foi de 1,54 anos. O percentual de respostas corretas de conhecimento foi de 30,7% para enfermeiras e 25,6% para auxiliares. Discussão. Embora tenham sido realizados estudos internacionais que avaliaram conhecimentos e atitudes sobre a dor usando o mesmo questionário, este estudo é o primeiro na Colômbia que compara enfermeiras e auxiliares, mostrando melhores percentuais de respostas corretas e maior número de perguntas com resultados classificados como “satisfatórios”. Conclusões. O nível de conhecimento e atitudes em relação à dor, obtido das enfermeiras é superior ao dos auxiliares. Objective: Parkinson's disease (PD) presents as a chronic condition with symptoms that worsen over time. Many PD patients experience pain at some point during their illness. This complaint is often overlooked because PD is primarily a motor disorder. The main objective is to assess the prevalence and the most frequent type of pain in this population, as well as its relation to common neuropsychiatric factors. Methods: A cross-sectional study was conducted including 196 patients diagnosed with PD. The variables analyzed included age, gender, smoking, alcohol consumption, anxiety, depression, antiparkinsonian treatment (levodopa, dopaminergic agonists, monoamine oxidase inhibitors, and amantadine), intake of antidepressants or antipsychotics, age of symptom onset, age of diagnosis, years of progression, total MDS-UPDRS 3.3 score, total MDS-UPDRS score, MDS-non-motor symptom scores, Hamilton depression and anxiety scales, and montreal Cognitive Assessment. Results: Our patient cohort consisted of 115 males (58.7%) and 81 females (41.3%), with a mean age of 63.56 ± 11.88. The mean disease duration was 7.18 ± 4.9 years. The most common type of pain was musculoskeletal pain, present in 66.7%, followed by radicular pain (24.2%), pain related to fluctuations (22.7%), chronic pain (20.7%), nocturnal pain (17.2%), discoloration, edema, or swollen pain (14.6%), and orofacial pain (5.6%). Conclusions: From the study carried out, it can be observed that the most common type of pain was musculoskeletal pain, followed by radicular pain. 33. Pain Patterns of the Temporomandibular Joint Dysfunction and Implications of Stress-Related Behaviors: A Pilot Study.

Temporomandibularjoint dysfunction interferes with the quality of life and activities of daily living among patients. The symptoms of temporomandibular dysfunction, including pain and clicking and popping sounds, are worsened during stressful events, and patients report increased pain around the temporomandibular joint. Stress-related behaviors, such as teeth clenching and teeth grinding, are commonly reported as increasing during stress. The prevalence of temporomandibular dysfunction and stress-related behaviors is reported differently in the literature. Stress in higher education is common. The purpose of this pilot study was to investigate the prevalence of temporomandibular joint dysfunction and stress-related behaviors among staff members at a local University. The study also sought to explore pain patterns described by people experiencing temporomandibular joint dysfunction and the relationship between stress-related behaviors and pain symptoms experienced. Further, the impact of stress on symptoms experienced by people with temporomandibular dysfunction was investigated in this pilot study. [ABSTRACT FROM AUTHOR]

Introduction:There is a need to provide analgesia strategies that encourage and promote women's participation in decision-making at the time of delivery and relaxation techniques could be a complementary and/or alternative non-pharmacological analgesic method to the widely used epidural anaesthesia in standard labour care. Objective: The objective of this study is to analyze the obstetric effects of relaxation techniques on pain management during labour. Method: A systematic review is performed with critical reading of included studies. The search for studies was carried out in the main databases MEDLINE, Cochrane Library, Cuiden, LILACS and SciELO. Studies published in English or Spanish between 2015 and February 2021 were included. Eleven studies were included, six of which are systematic reviews and five are randomised clinical trials. The interventions analysed were relaxation techniques such as hypnosis, intradermal injection of sterile water, warm water immersion, massage, acupuncture, music therapy, aromatherapy, continuous support and mind-body practices like relaxing breathing, yoga and meditation, among others. Conclusion: The main conclusion of this study is that relaxation techniques may decrease the level of pain during labour, although the current scientific evidence is limited and the methodological quality varies from low to moderate. Background: Levels of prolonged grief symptoms (PGS) and post-traumatic stress symptoms (PTSS) can be high, many years following bereavement after terror, but knowledge concerning somatic health is scarce. Terrorism is a serious public health challenge, and increased knowledge about long-term somatic symptoms and insomnia is essential for establishing follow-up interventions after terrorism bereavement. Objective: To study the prevalence of somatic symptoms and insomnia and their association with PGS, PTSS, and functional impairment among terrorism-bereaved parents and siblings. Methods: A cross-sectional quantitative study included 122 bereaved individuals from the Utøya terror attack in Norway in 2011. The sample comprised 88 parents and 34 siblings aged 19 years and above (Mage = 49.7 years, SDage = 13.8 years, 59.8% females). The participants completed questionnaires 8 years after the attack assessing somatic symptoms (Children’s Somatic Symptoms Inventory) and insomnia (Bergen Insomnia Scale) along with measures of PGS (Inventory of Complicated Grief), PTSS (Impact of Event Scale–Revised), and functional impairment (Work and Social Adjustment Scale). Results: Fatigue was the most frequently reported somatic symptom (88% of females and 65% of males). Females reported statistically significantly more somatic symptoms than males. In total, 68% of the bereaved individuals scored above the cut-off for insomnia. There were no statistically significant gender differences for insomnia. Female gender, intrusion, and arousal were associated with somatic symptoms. Intrusion and somatic symptoms were associated with insomnia. Somatic symptoms, avoidance, and hyperarousal were associated with functional impairment. Conclusion: Many bereaved parents and siblings report somatic symptoms and insomnia eight years after the terror attack. Somatic symptoms are associated with functional impairment. Long-term follow-up and support after traumatic bereavement should focus on somatic symptoms and insomnia. [ABSTRACT FROM AUTHOR]

Objective: To measure pain in postoperative knee arthroscopy patients after undergoing treatment with therapeutic laser vs. TENS currents. Materials and method: An experimental, longitudinal, prospective, and comparative study was carried out that included 14 patients who attended the Physical Medicine and Rehabilitation area of the Hospital Central Militar, identified with the diagnosis of postoperative knee arthroscopy, through randomization. simple, by lottery, to include patients in the work groups: treatment with therapeutic laser vs treatment with TENS currents. The visual analogue scale was applied in the evaluation, and measurement of both knees was carried out by circometry, pre- and post-treatment to measure edema. The analysis was carried out with the statistical program SPSS version 26 and the student's T test was used. Significance level p<0.05. Results: A total of 14 patients entered the study, 7 were treated with therapeutic laser and 7 with TENS currents. 50% (7 patients) were women and 50% (7 patients) were men. The average age was 44.85 years. When analyzing the visual analog scale, in the pre- and post-treatment evaluation, both groups decreased their pain intensity with statistically significant differences (with a standard deviation of 1.15 for TENS and a standard deviation of 1.35). When comparing both results, a difference was observed. statistically significant difference in favor of TENS Currents over therapeutic laser p<0.0001. Conclusions: In both treatments, both the therapeutic laser and the TENS currents, improvements were observed in the pain perceived by the patients. Having a greater reduction in pain with TENS currents. Therefore, in this study we conclude that treatment with TENS currents is more effective than therapeutic laser in pain management in post-knee arthroscopy patients. - Abstract
Introduction: Hemorrhoidectomy produces intense postoperative pain, recommending multimodal analgesia for its management, remaining as an unresolved problem. The aim of this study was to evaluate the analgesic effectiveness of three pharmacological combinations for post-hemorrhoidectomy pain. Material and Method: A prospective, randomized clinical study, conducted at the INDISA Clinic, between December 2019 and December 2021, including patients with an indication for elective hemorrhoidectomy. Hemorrhoidectomies associated with another surgical procedure, pregnant/lactating women, adverse drug reactions (ADRs) to the study drugs, liver and kidney diseases, or mental disorders/disabilities were excluded. Group I (control): Ketorolac, Tramadol, Paracetamol. Group II: Group I and Nifedipine 0.2% topical. Group III: Buprenorphine patch 10 mcg/hour, Paracetamol and Ketorolac. Associated with a diet rich in fiber, polyethylene glycol, sitz baths, and omeprazole. Descriptive and analytical statistics were used using Chi-square, ANOVA-Bonferroni, Kruskal Wallis, Wilcoxon and Fisher test. Software R, using an alpha of 5%. Of 117 patients, 39 = Group I, 41 = Group II and 37 = Group III were enrolled. Results: There were no differences in analgesic effectiveness (p = 0.45). For the ADRs, it was observed that the patients with Buprenorphine had more nausea (p = 0.08), vomiting (p = 0.04), dermatitis (p < 0.001) and itching (p = 0.006). Discussion and Conclusion: There were no significant differences for post-hemorrhoidectomy analgesic effectiveness when comparing the study groups. The use of topical nifedipine is recommended as a complement to multimodal therapy as it improves results without increasing adverse drugs reaction (ADR).The use of buprenorphine presented more ADR without better results as an analgesic.The main determinant for pain relief was the time elapsed since surgery. Introduction: Low back pain is presented in approximately 9.4% of the world population. Acupuncture is used by Chinese traditional medicine to stimulate certain points of the body with different types of needles. Among its beneficial effects the pain remission is described. Objective: To evaluate the effectiveness of the acupuntural treatment in patients with lumbar pain assisted in the emergency room. Methods: A quasi-experiment of therapeutic intervention study (without control group) was carried out in 35 patients with lumbar pain, who were assisted in the Natural and Traditional Medicine Emergency Room of Dr. Juan Bruno Zayas Alfonso Teaching General Hospital in Santiago de Cuba, from July to December, 2021. Results: In the investigation there was a prevalence the 40-49 age group (31.4%), female sex (57.1%), housewives, serious and moderate pain at the beginning of the treatment, as well as patients without pain one hour after the therapy. As for the final clinical course, the total of the sample classified in the relieved and improved categories. Conclusions: The effectiveness of acupuncture was demonstrated in patients with acute lumbar pain.

Objetivo: Relacionar el uso tópico de cannabis como alternativa terapéutica analgésica en pacientes con algunas enfermedades inflamatorias, de la IPS Salud Social, entre mayo y julio de 2023. Métodos: Se realizó un estudio analítico, retrospectivo. La población de donde se obtuvo la muestra, corresponde a pacientes diagnosticados con Artrosis no especificada, bocio multinodular no tóxico, Epilepsia tipo no especificado, insuficiencia venosa crónica y periférica, Lumbago no especificado, gonartrosis secundaria, síndrome de manguito rotador, síndrome del túnel carpiano, de la IPS Salud Social de Barranquilla, Atlántico. Se obtuvo una muestra de 23 pacientes diagnosticados con estas patologías mediante muestreo no probabilístico por conveniencia. Resultados: Todos los pacientes mostraron alivio del dolor, después de dos meses de seguimiento; dos experimentaron efectos adversos. Algunos estudios sugieren que los cannabinoides presentes en el cannabis, como el CBD y el THC, podrían tener propiedades analgésicas y antiinflamatorias que podrían aliviar el dolor y la inflamación asociados con estas afecciones, lo que es coherente con el presente estudio. Conclusión: El cannabis tópico se presenta como una alternativa terapéutica para enfermedades inflamatorias, sin embargo, es importante destacar que la investigación sobre el uso del cannabis en estas enfermedades es limitada y se necesitan más estudios para comprender completamente sus efectos y beneficios potenciales.

Introdución: Una adecuada evaluación y manejo del dolor repercute en el bienestar de los pacientes en hemodiálisis. Objetivo: Analizar el manejo del dolor por el profesional de enfermería durante la hemodiálisis a nivel nacional. Metodología: Estudio descriptivo transversal nacional, realizado en marzo-abril 2023, mediante un cuestionario online dirigido a enfermeras de hemodiálisis, previo consentimiento informado. Se recogen variables sociodemográficas, tipo de centro, experiencia laboral, procedimiento evaluación dolor y administración analgesia durante la hemodiálisis, autoevaluación conocimientos y demanda de acciones formativas. Se realizó un análisis descriptivo y bivariado (test Chi2, T-Student, U-Mann Whitney), nivel de significancia 0,05. Resultados: 241 participantes de 17 comunidades autónomas, 77,6% mujeres, edad media 43,70±9,99 años. Un 42,7% consideraba inadecuado el abordaje del dolor; el 80% no utilizaba protocolos específicos y el 95,9% opinaba que sería útil tenerlos. El 66% evaluaron sus conocimientos sobre el manejo del dolor como “Nada/Poco”. Un administraba el analgésico prefiltro. Un 96,7% consideraron útil recibir formación específica.
