Systemic sclerosis (systemic scleroderma) is characterized by a heterogeneous range of clinical manifestations. Systemic sclerosis is classified into limited cutaneous systemic sclerosis and diffuse cutaneous systemic sclerosis subgroups based on the extent of skin involvement. Randomized controlled trials in scleroderma have mainly focused on diffuse cutaneous systemic sclerosis partly because the measurement of skin involvement, critical for evaluating a therapeutic intervention, is more dynamic in this subset. Nonetheless, limited cutaneous systemic sclerosis, the most common cutaneous subset (about two-third), is also associated with significant morbidity and detrimental impact on health-related quality of life. The lack of interventional studies in limited cutaneous systemic sclerosis is partly due to a lack of relevant outcome measures to evaluate this subgroup. Combining several clinically meaningful outcomes selected specifically for limited cutaneous systemic sclerosis may improve representativeness in clinical trials and responsiveness of outcomes measured in randomized controlled trials. A composite index dedicated to limited cutaneous systemic sclerosis combining such relevant outcomes could advance clinical trial development for limited cutaneous systemic sclerosis by providing the opportunity to test and select among candidate drugs that could act as disease-modifying treatments for this neglected subgroup of systemic sclerosis. This proposed index would include items selected by expert physicians and patients with limited cutaneous systemic sclerosis across domains grounded in the lived experience of limited cutaneous systemic sclerosis. This article reviews the reasons behind the relative neglect of limited cutaneous systemic sclerosis, discusses the current state of outcome measures for limited cutaneous systemic sclerosis, identifies challenges, and proposes a roadmap for a combined limited cutaneous systemic sclerosis-specific treatment response index.

Objectives: The objectives of this study were to describe the impact of systemic sclerosis associated interstitial lung disease, on quality of life, to estimate the correlation between quality of life and severity of lung disease and to assess the impact of interstitial lung disease on caregivers. Methods: Seven investigators included systemic sclerosis associated interstitial lung disease patients from December 2019 to April 2020. Sociodemographics and clinical data were collected. Patients reported outcomes and questionnaires were used with 1 generic patients reported outcome (EQ-5D-5L), 1 specific PRO (Brief Interstitial Lung Disease) and 2 self-reported questionnaires on impact of SSc complications and impact on caregivers. The correlation between forced vital capacity and EQ-5D-5L score was estimated with a multivariate linear regression model adjusted on several covariates. Results: In all, 89 patients were included. 26.4% were males, mean age was 58.2 ± 14.5 years. Mean EQ-5D-5L score = 0.79 ± 0.22 (median = 0.85). Mean EQ-5D-5L visual analog scale score = 60.8 ± 20.4 (median = 61.5). Mean King’s Brief Interstitial Lung Disease score = 58.4 ± 12.7 (median = 58.0). After adjustment on covariates, a significant correlation between forced vital capacity and EQ-5D-5L score was found with an increase of 0.003 of the EQ-5D-5L score for a 1% increase of FVC (p = 0.0096). No significant correlation between forced vital capacity and the EQ-VAS and King’s Brief Interstitial Lung Disease score were found. The impact of SSc on other organs was significantly correlated with EQ- 5D-5L score, respectively, for the impact scores on the lung system (p = 0.0003), heart system (p = 0.0182), Raynaud’s syndrome (p = 0.0015), digestive system (p = 0.0032), joints/muscles (p = 0.0003), skin (p < 0.0001), kidney (p = 0.0052) and gastro-oesophageal reflux (p = 0.0063). Significant correlations between King’s Brief Interstitial Lung Disease score and lung system (p < 0.0001), heart system (p < 0.0001), digital ulcers (p = 0.058), digestive system (p < 0.0001), kidney (p = 0.0004), skin (p = 0.0499) and gastro-oesophageal reflux (p = 0.0033) scores were found 68.5% of patients reported their need for a caregiver to help them in their daily life activities. Conclusion: Our study highlighted the strong burden of systemic sclerosis associated interstitial lung disease` for patients, especially with an impact on quality of life, on other organs manifestations and need for caregivers in their daily life.

Between 9000 and 14000 people have systemic sclerosis (ScS) in France. The work is often affected. Our study aims to assess the frequency of professional difficulties (DP) of scleroderma patients, identify these DP, the symptoms involved and the solutions used. A heteroquestionnaire was offered to scleroderma patients the Lille University Hospital. Data were cross-checked with the clinical database of the National Observatory of scleroderma patients. We used 104 questionnaires of which 84% are women. A total of 62.5%, (95% CI [52.5 to 71.8]) patients had experienced DP related to SSc, 55% reported symptoms worsened at work, 41% lower efficiency and a 19% job loss. Symptoms responsible for their DP were mainly asthenia, Raynaud syndrome, arthralgias and finger ulcerations. Only 40% of patients in difficulty were initiating steps with the county home for disabled people and 45% in informing their doctor at work (MT). The MT information absence was related to a misunderstanding of possible aid in 45% of cases, 57% of patients had not found a solution, 41% of them have lost their jobs and 31% of patients in the cohort experienced a drop in income. The impact of SSc the work is important. The aid job retention devices are under-used due to lack of knowledge of steps to take and the role of MT. Taking into account the symptoms responsible for PD must be improved to participate in this retention.

Introduction L’objectif principal de cette etude etait de decrire l’impact de la pneumopathie interstitielle diffuse associee a la sclerodermie systemique (PID-ScS) sur la qualite de vie (QdV). Les objectifs secondaires etaient d’etudier la relation entre la QdV et d’autres atteintes d’organes de la ScS et le recours a des aidants par les patients. Materiels et methodes Sept centres investigateurs ont inclus 89 patients atteints de PID-ScS entre decembre 2019 et avril 2020. Des donnees sociodemographiques et cliniques ont ete recueillies. Les patients ont renseigne un questionnaire de QdV generique (EQ-5D-5L), un questionnaire de QdV specifique (K-BILD) et un auto-questionnaire evaluant l’impact de la ScS sur differents organes selon un score variant entre 0 et 100, et le recours a des aidants. Des analyses de regressions lineaires multivariees ont ete conduites entre les scores de QdV, la capacite vitale forcee (CVF) et les scores des atteintes d’organes avec ajustement sur l’âge, le sexe, l’anciennete de la maladie, le type diffus ou non de la ScS et la presence d’auto-anticorps anti-topoisomerase. Resultats Parmi les 89 patients inclus, 73,6 % etaient des femmes. L’âge moyen etait de 58,2 ± 14,5 ans. La CVF moyenne etait de 86,8 ± 24,9 %. Les anticorps detectes etaient majoritairement des anti-topoisomerases (59,0 %). Le score moyen EQ-5D-5L etait de 0,8 ± 0,2 et le score moyen de l’echelle visuelle analogique (EVA) de l’ EQ-5D-5L etait de 60,8 ± 20,4. Le score moyen du K-BILD etait de 58,4 ± 12,7. La regression lineaire multivariee a retrouve une correlation significative entre la CVF et le score EQ-5D-5L avec une augmentation de 0,003 du score EQ-5D-5L pour une augmentation de 1 % de la CVF (p = 0,0096). Aucune correlation significative n’a ete identifiee entre la CVF d’une part et l’EVA de l’EQ-5D-5L ou le score du K-BILD d’autre part. Des correlations significatives ont ete retrouvees par les analyses de regression multivariee entre le score EQ-5D-5L et l’impact de la maladie sur le systeme pulmonaire (p = 0,0003), le systeme cardiaque (p = 0,0018), le syndrome de Raynaud (p = 0,0015), le systeme digestif (p = 0,0032), les articulations/muscles (p = 0,0003), la peau (p Discussion La population incluse presente des caracteristiques similaires aux sujets de la cohorte EUSTAR, ce qui est un gage de sa representativite et les resultats de cette etude sont coherents avec ceux d’autres travaux similaires. Conclusion Cette etude confirme l’impact de la PID-ScS sur la QdV avec la mise en evidence d’une correlation significative entre le score EQ-5D-5L et la CVF, mais egalement avec d’autres atteintes d’organes.

Objective Contagious disease outbreaks and related restrictions can lead to negative psychological outcomes, particularly in vulnerable populations at risk due to pre-existing medical conditions. No randomised controlled trials (RCTs) have tested interventions to reduce mental health consequences of contagious disease outbreaks. The primary objective of the Scleroderma Patient-centered Intervention Network COVID-19 Home-isolation Activities Together (SPIN-CHAT) Trial is to evaluate the effect of a videoconference-based program on symptoms of anxiety. Secondary objectives include evaluating effects on symptoms of depression, stress, loneliness, boredom, physical activity, and social interaction. Methods The SPIN-CHAT Trial is a pragmatic RCT that will be conducted using the SPIN-COVID-19 Cohort, a sub-cohort of the SPIN Cohort. Eligible participants will be SPIN-COVID-19 Cohort participants without a positive COVID-19 test, with at least mild anxiety (PROMIS Anxiety 4a v1.0 T-score ≥ 55), not working from home, and not receiving current counselling or psychotherapy. We will randomly assign 162 participants to intervention groups of 7 to 10 participants each or waitlist control. We will use a partially nested RCT design to reflect dependence between individuals in training groups but not in the waitlist control. The SPIN-CHAT Program includes activity engagement, education on strategies to support mental health, and mutual participant support. Intervention participants will receive the 4-week (3 sessions per week) SPIN-CHAT Program via videoconference. The primary outcome is PROMIS Anxiety 4a score immediately post-intervention. Ethics and dissemination The SPIN-CHAT Trial will test whether a brief videoconference-based intervention will improve mental health outcomes among at-risk individuals during contagious disease outbreak., Highlights • No RCTs have tested mental health interventions in contagious disease outbreaks. • People with scleroderma face fear and isolation during COVID-19. • SPIN-CHAT is designed to improve mental health in SSc patients during COVID-19. • SPIN-CHAT provides 12 group sessions over 4 weeks. • The SPIN-CHAT trial will randomize 162 participants to intervention or waitlist.

Introduction: Individuals with visible differences due to medical conditions, such as systemic sclerosis (SSc; scleroderma), have reported difficulty navigating social situations because of issues such as staring, invasive questions, and rude comments. Fears or anxiety linked to situations in which a person interacts with others is known as social interaction anxiety. However, there exists no validated measurement tool to examine social interaction anxiety in rheumatologic conditions. Methods: The present study examines the reliability (internal consistency) and validity (structural and convergent) of the Social Interaction Anxiety Scale-6 (SIAS-6) in a sample of 802 individuals with SSc, and compares these psychometric properties across limited and diffuse subtypes of the disease. Multi-group confirmatory factor analysis was used to examine the factor structure of the SIAS-6 in patients with both limited and diffuse SSc. Results: A one-factor structure was found to fit well for individuals with SSc with both limited and diffuse disease. The measure demonstrated strong internal consistency reliability and convergent validity with relevant measures in expected magnitudes and directions. Conclusions: The SIAS-6 is a psychometrically robust measure that can confidently be used in SSc populations to examine social interaction anxiety. Item does not contain fulltext Body concealment is an important component of appearance distress for individuals with disfiguring conditions, including scleroderma. The objective was to replicate the validation study of the Body Concealment Scale for Scleroderma (BCSS) among 897 scleroderma patients. The factor structure of the BCSS was evaluated using confirmatory factor analysis and the Multiple-Indicator Multiple-Cause model examined differential item functioning of SWAP items for sex and age. Internal consistency reliability was assessed via Cronbach's alpha. Construct validity was assessed by comparing the BCSS with a measure of body image distress and measures of mental health and pain intensity. Results replicated the original validation study, where a bifactor model provided the best fit. The BCSS demonstrated strong internal consistency reliability and construct validity. Findings further support the BCSS as a valid measure of body concealment in scleroderma and provide new evidence that scores can be compared and combined across sexes and ages. 8 p.

The purpose of this study was to assess health-related quality of life (HRQoL) and disease perception in a large, international group of patients with systemic sclerosis (SSc).We placed a standardized questionnaire on a website for patient access. Socio-demographic information, disease characteristics, and self-assessment questionnaires-the Short Form 36 (SF-36) and the Revised Illness Perception Questionnaire (IPQ-R)-were collected.A total of 1902 patients from 60 countries were included. HRQoL appeared to be impaired in SSc, particularly for physical health (PCS, mean ± SD = 43.4 ± 23.4). SSc patients also had strong perceptions about the chronic nature and negative consequence of the disease, and experienced negative emotions due to SSc. Patients with diffuse cutaneous SSc had a poorer HRQoL than those with limited cutaneous SSc, for both physical (PCS, mean ± SD = 46.6 ± 23.7 vs. 39.8 ± 22.3; p0.0001) and mental components (MCS, mean ± SD = 53.8 ± 23.0 vs. 50.3 ± 23.2; p = 0.003). Late-stage SSc patients were more likely to perceive their disease chronic (p0.0001), less controllable (p = 0.03) and with more consequences (p = 0.008), but they had a better understanding of their disease and experienced fewer negative emotions. Raynaud's phenomenon and gastrointestinal complications were the organ involvements with the greatest impact on QoL, they were the two variables associated with the most negative perception of illness severity.This study, performed on the largest group ever set up for this purpose, confirms the major impact on QoL and the negative perceptions of their disease expressed by SSc patients. However, the perception of this illness tended to improve with disease duration, suggesting that patients find effective coping strategies.

When the diagnosis of systemic sclerosis (SSc) is made and you are told “ You have SSc ”, it is a strange feeling for the patient, because you don't yet know what it is exactly. It is a very intense shock to hear this, and it is also difficult to try to imagine what will follow. There is no cure for SSc; there are just treatments that are capable of reducing the symptoms. The main difficulties and pitfalls encountered by the patients occur: before the diagnosis; during treatment; at the hospital and with the physicians; if surgery is necessary; at home; with family and friends… How do you live with SSc? You need strong, active support to help you to live with this illness; without this, we cannot handle it. The way to communicate, to inform, to consider this disease is changing, because times are changing, patients are changing, and the opinions of the specialists are also changing. Of course it's a long way, but what we, the patients, hope for is to be better understood by you, the doctors. Please pay attention to what we say; we're the ones suffering from this disease. SSc is part of us; we are not merely medical cases! Please think about it. Together we can help you to fight, to find out more, to find a cure.

This paper describes the main characteristics of a new microprocessor-implemented 2400-bit/s data modem, the IBM 3863. In addition to the execution of signal processing tasks, the microprocessor provides a variety of other significant functions such as diagnostics and aids in network problem determination. The lack of hardware multiplication capability imposes certain constraints in the design of the signal processing algorithms. The analytical approach and computational techniques, based on the processing of signals in polar coordinates, which are used to circumvent these constraints are described. It is shown in particular that timing phase control, carrier recovery, and adaptive equalization can be achieved at the receiver by processing only the phase of the sampled signal. Additionally, experimental results are presented which demonstrate the superiority of this design over conventional coherent demodulators.

This paper shows how a Kalman filter may be applied to the problem of setting the tap gains of transversal equalizers to minimize mean-square distortion. In the presence of noise and without prior knowledge about the channel, the filter algorithm leads to faster convergence than other methods, its speed of convergence depending only on the number of taps. Theoretical results are given and computer simulation is used to corroborate the theory and to compare the algorithm with the classical steepest descent method.

This paper reviews some of the choices that will be available in the next few years, as the much-discussed move toward implementing voice and data integration within a single wide-area integrated private network proceeds. After the term :hp1de-area integrated private network:ehp1. has been defined, a discussion is given of requirements the network ought to satisfy for its users. Then two particularly promising approaches, fast packet switching (FPS) and hybrid switching (HS), are defined, and specimen design points for FPS and HS are postulated, so that the two can be compared. While a definitive comparison would require systematic cost and performance studies, much insight can be gained from the qualitative comparison that we present here. We assess some of the arguments that have been put forward in favor of FPS or HS and conclude that, although today both architectures have promise, and research on both should continue, FPS appears to be slightly simpler to implement and operate.
