To measure our opioid prescription rate, determine if our rate has decreased since 2019, and identify areas for future interventions to further decrease our opioid prescription rate. We retrospectively reviewed all pediatric urology patients (age..±18 years) who underwent a procedure between October 1, 2020 and October 22, 2021. We collected data on opioid prescribing, age, sex, surgeon, procedure, ethnicity, and race. We grouped procedures into 6 categories: circumcision, cystoscopy with the removal of foreign body/stone/stent, scrotal surgery, hypospadias repair/penile surgery, pyeloplasty/ureteral reimplant, and others. We analyzed 821 operative cases. Only 2.2% (18/821) of discharges included an opioid prescription. The prescription rate of 1 pediatric urologist was 4.6% (17/369), which was higher than the other 2 practitioners... (0.40%, 1/250%, and 0%, 0/202) (P.ß<.ß.001). The median age of patients who received an opioid prescription was older than patients without an opioid prescription (16.5 vs.ß5.0 years, P.ß<.ß.001). Surgery performed in an inpatient setting was more likely to result in an opioid prescription (9.7%, 3/31) than in the outpatient setting (1.9%, 15/790) (P.ß=.ß.03). No adverse effects of reduced opioid usage were noted. From October 2020 to October 2021, our institution had an opioid prescription rate of 2.2%. This represented a decrease from our previously reported rate of 8% in 2019. At the same time, we found no significant pain issues in our post-operative patients. Seventeen out of 18 prescriptions were written under 1 provider. Though heightened awareness has made a difference, targeted feedback is needed if we wish to reduce opioid usage further. The objective of this study was to assess the prostate cancer screening practices of Vermont primary care physicians and compare them with a prior study in 2001. An electronic survey was created and emailed to all currently practicing primary care physicians in Vermont. Data was stratified by practice length, practice location, university affiliation, and internal medicine versus family practice. Surveys were received from 123 (27.2%) primary care physicians. 27.7% of physicians in practice <10 years recommended prostate specific antigen (PSA) testing, compared with 55.9% of those practicing ≥10 years (p = 0.006). Of those who modified their recommendations in the past 5 years, 96.1% reported that the United States Preventive Services Task Force (USPSTF) 2012 statement influenced them. Respondents who continued to use PSA testing were less likely to stop screening after age 80 compared with those surveyed in 2001 (51% in 2014 vs. 74% in 2001; p <0.001). Primary care physicians in practice for 10 or more years were more likely to recommend PSA-based screening than those in practice for less time. The USPSTF statement discouraging PSA-based screening for prostate cancer has had significant penetrance among Vermont primary care physicians. [ABSTRACT FROM AUTHOR]

Objective: The aim of this report is to highlight the value of excision when treating and diagnosing endometriosis, as well as present an unusual case of concomitant endometriosis and pelvic malignancy in a young female. Case report: In a university hospital referral center, a 24-year-old nulligravid woman with suspected endometriosis had been treated unsuccessfully with empiric therapy for several years. Interventions were preoperative cyclic and continuous combined oral contraceptive pills and a NuvaRing for 2 years. Diagnosis was completed by laparoscopy and histologic evaluation. Surgical debulking by gynecology oncologist was followed by chemotherapy. The main outcome measure was definitive diagnosis via histology. Results: Histologic diagnosis of endometriosis and concomitant pelvic malignancy with successful surgical excision of both. Conclusions: When evaluating a patient with chronic pelvic pain, it is necessary to have a high degree of suspicion for endometriosis. Early surgical and histologic evaluation is imperative for definitive diagnosis. An added benefit of histologic diagnosis is the opportunity to assess for malignancy. [ABSTRACT FROM AUTHOR]

This paper examines how story proximity is related to news versus editorial format and how both proximity and format affect recall and awareness. Undergraduate student at Penn State University's main campus served as participants in this study. There were 53 females and 47 males in the sample. News stories use facts to present objective information, while editorials use arguments to present a particular position. The four story topics were regulation of cellular phones in cars, elective plastic surgery among college students, exercise preventing the common cold and drug use affecting financial aid awards. Results of the analysis of variance revealed a main effect for proximity, with participants displaying greater news awareness for local stories than non-local stories. News awareness, then, did not appear to be connected to the format of the news. Information that was local was recalled and understood significantly than information about more distant places. One possible reason that participants scored higher on the news awareness scale for local stories could simply be greater name recognition of the local place names. Although the news awareness scale included items that measured both recall and understanding, a low score on the scale could indicate only recall. In addition, social desirability may have played a role in participants' greater awareness of local news.

Objective: Psychological maltreatment (PM) is a widespread form of child maltreatment both in high-risk and maltreating families as well as in the general population of parents, yet there are no intervention programs that target it directly. The current study was designed as the first step in a larger program of research concerning educating parents about PM. In this study we evaluated the content of universal parenting programs to assess whether they include content on PM. Three questions were addressed: (1) Which types, if any, of PM were included in the content of these programs? (2) Which programs, if any, have content about each of the types of PM? (3) What are the implications for the development of PM curricula for parents? Method: Ten evidence-based, manualized, universal parenting programs identified from SAMHSA or a comparable model program registry were rated on how well their content covered 18 types of psychological maltreatment (PM), as defined by the American Professional Society on the Abuse of Children, APSAC (). Each type of PM was coded along several dimensions which resulted in two summary scores: (1) Does the program contain content designed to teach parents what not to do in regards to the 18 psychologically maltreating behaviors and (2) Does the program contain content designed to teach parents what to do instead? Results: Content related to most PM types were not included in the curricula, especially regarding “what not to do” and not one program was rated as having content related to teaching all 18 types of PM. Conclusions: Existing parenting programs do not currently cover content for teaching community parents about psychological maltreatment. [Copyright &y& Elsevier]
