This systematic review and meta‐analysis aimed to consolidate evidence on dietary interventions for atopic eczema/dermatitis (AD) skin symptoms in children without food allergies, following PRISMA 2020 guidelines. Systematic review updates were conducted in May 2022 and June 2023, focusing on randomized placebo‐controlled trials (RCTs) involving children with AD but without food allergies. Specific diets or supplements, such as vitamins, minerals, probiotics, prebiotics, symbiotics, or postbiotics, were explored in these trials. Exclusions comprised descriptive studies, systematic reviews, meta‐analyses, letters, case reports, studies involving elimination diets, and those reporting on food allergens in children and adolescents. Additionally, studies assessing exacerbation of AD due to food allergy/sensitization and those evaluating elimination diets' effects on AD were excluded. Nutritional supplementation studies were eligible regardless of sensitization profile. Evaluation of their impact on AD clinical expression was performed using SCORAD scores, and a meta‐analysis of SCORAD outcomes was conducted using random‐effect models (CRD42022328702). The review encompassed 27 RCTs examining prebiotics, Vitamin D, evening primrose oil, and substituting cow's milk formula with partially hydrolyzed whey milk formula. A meta‐analysis of 20 RCTs assessing probiotics, alone or combined with prebiotics, revealed a significant reduction in SCORAD scores, suggesting a consistent trend in alleviating AD symptoms in children without food allergies. Nonetheless, evidence for other dietary interventions remains limited, underscoring the necessity for well‐designed intervention studies targeting multiple factors to understand etiological interactions and propose reliable manipulation strategies. Background: Skin testing represents a commonly used first diagnostic method in clinical practice, but allergen extracts may vary in composition and often contain cross‐reactive allergens and therefore do not always allow the precise identification of the sensitizing allergen source. Our aim was to investigate the suitability of a single recombinant hybrid molecule, consisting of the four major timothy grass pollen allergens (Phl p 1, Phl p 2, Phl p 5, and Phl p 6) for in vivo diagnosis of genuine grass pollen allergy in children suffering from pollinosis. Methods: Sixty‐four children aged from 6 to 17 years with a positive skin reaction and/or specific IgE to grass pollen extract and respiratory symptoms of pollinosis as well as 9 control children with allergy to other allergen sources were studied. SPT was performed with the recombinant hybrid, the four recombinant timothy grass pollen allergens, and grass pollen extract. Specific IgE reactivity to 176 micro‐arrayed allergen molecules was determined using ImmunoCAP ISAC technology. IgE reactivity to the hybrid was detected by non‐denaturing RAST‐based dot blot assay. Results: Genuine grass pollen sensitization was confirmed in 94% of the children with positive SPT to grass pollen extract by SPT and IgE reactivity to the hybrid. The four hybrid‐negative children showed IgE reactivity to cross‐reactive allergens such as Phl p 4, Phl p 11, and Phl p 12 and had also sensitizations to pollen allergens from unrelated plants. The strong epidemiologic and pathophysiologic link between allergic rhinitis (AR) and asthma has led to the concept of ‘united airways disease’ or ‘respiratory allergy’, implying that allergy, in its widest sense, underlies this clinical syndrome. Progression from AR to asthma is frequent and part of the ‘atopic march’. Since pediatric immune responses are more adaptable and therefore may be more amenable to treatment, interventions at early childhood are characterized by a higher chance to affect the natural history of respiratory allergy. Although current treatments are quite effective in alleviating respiratory allergy symptoms, it has proven much more difficult to confirm any influence on the progression of the disease. Much more promising is the field of specific allergen immunotherapy, where current evidence, although not yet of ideal robustness, points towards a disease-modifying effect. Correct identification of the culprit allergen is an essential part of diagnosis and treatment in immunoglobulin E (IgE)-mediated allergic diseases. In recent years, molecular biology has made important advances facilitating such identification and overcoming some of the drawbacks of natural allergen extracts, which consist of mixtures of various proteins that may be allergenic or not, specific for the allergen source or widely distributed (panallergens). New technologies offer the opportunity for a more accurate component-resolved diagnosis, of benefit especially to polysensitized allergic patients. The basic elements of molecular diagnostics with potential relevance to immunotherapy prescription are reviewed here, with a focus on Southern European sensitization patterns to pollen allergens. We propose a basic algorithm regarding component-resolved diagnostic work-up for pollen allergen-specific immunotherapy candidates in Southern Europe; this and similar algorithms can form the basis of improved patient management, conceptually a 'Component-Resolved Allergy Management'. A letter to the editor is presented regarding the case of a 20-month-old boy with egg-induced anaphylaxis or anaphylactic reaction after having a second-degree skin burn.

Background: GA2LEN‐ADCARE is a branch of the largest multidisciplinary network of research centres and clinical care in allergy and asthma, GA2LEN, focussing on the field of atopic dermatitis (AD). AD is a chronic inflammatory skin disease with high burden and many comorbidities requiring different levels of treatment. The need for aligned information from all involved healthcare providers led to the discussion of an integrated care pathway (ICP) plan for AD patient care involving all stakeholders and considering the complexity and variability of the disease, with a particular focus placed on the large number of patients with milder forms of AD. Methods: The GA2LEN ADCARE network and all stakeholders, abbreviated the AD‐ICPs working group, were involved in the discussion and preparation of the AD‐ICPs during a series of subgroup workshops and meetings in years 2020 and 2021. Results: Here we discuss the unmet needs in AD, the methodology for devising an AD‐ICP and the ICP action plan. Conclusion: The GA2LEN ADCARE network has outlined the unmet needs in AD and provided an action plan for devising AD‐ICPs, considering the complexity and variability of the disease. Background: Ectoine is a widespread osmolyte enabling halophilic bacteria to withstand high osmotic stress that has many potential applications ranging from cosmetics to its use as a therapeutic agent. Objective: The aim of this study was to compare the efficacy and tolerability of ectoine 1% and hyaluronic acid 0.1% containing (EHA) cream with a vehicle cream in children with mild‐to‐moderate atopic dermatitis (AD). Methods: A randomized, controlled, observer‐blind, multicenter clinical trial was conducted in children aged 2–18 years, diagnosed with mild‐to‐moderate AD (SCORAD ≤20). Patients were randomized to either receiving EHA cream or vehicle cream twice daily for 4 weeks. The primary outcome measure was the mean change in objective SCORAD from baseline to the final visit. The secondary outcome measures included the mean change in Investigator's Global Assessment score, patient's judgment of efficacy and patient's assessment of pruritus. Safety of EHA cream was also assessed. Results: A total of 70 patients (35 in each group) were randomized and 57 were included in the final analysis set. Based on SCORAD measurements, patients using EHA cream achieved superior clinical improvement compared to the control group at 28 days (p <.001). EHA cream was also superior to the vehicle cream regarding all secondary outcome measures. Eight (23.5%) patients receiving EHA cream and two (5.7%) patients receiving vehicle cream experienced mild cutaneous adverse events (AEs). Background: Venom immunotherapy (VIT) has been shown to be an effective and safe treatment for preventing sting-induced anaphylaxis in patients with systemic reactions to hymenoptera stings. A remaining problem is the relative effectiveness and safety of different immunotherapy protocols used with respect to maintenance dose, injection interval, and duration. Objective: We aimed to describe a modified cluster VIT protocol with a maintenance dose of 50 μg lasting 5 yr and to evaluate retrospectively its safety and efficacy in children. Patients and Methods: Fifty four children 9.5 ± 3.2 yr old with a history of at least one anaphylactic reaction to hymenoptera stings underwent VIT between 1995 and 2006. The identification of the offending insect(s) was based on patient's report and documented with in-vivo (SPTs and IDs) and in-vitro (RAST/CAP) test results. A modified cluster outpatient protocol lasting 5 wks, reaching a maintenance dose of 50 μg was followed according to clinical history and test results. After the maintenance dose was achieved, the followed injection-intervals were 4 wks for the first year, 5 wks for the 2nd year and 3rd year, and 6 wks for the last 2 yr. Results: Of the 54 children, 52 tolerated the 50 μg VIT protocol without side effects. Twenty one of them reported at least one field sting from at least one of the culprit, for their allergy, insects, 6 ± 3.5 yr after they have started VIT treatment. In 11 of them, sting occurred 3.5 ± 2.9 yr after the VIT was completed, whereas the other 10 of them during immunotherapy, 3.2 ± 1.4 yr after they have started VIT. In the remaining two children, the maintenance dose was increased to 100 μg due to systemic reactions from the VIT. The data reflect outcomes 6-16 yr after the patients' initial allergic reaction. Conclusion: VIT with 50 μg maintenance dose lasting 5 yr appears to be safe and effective enough to induce tolerance in children with hymenoptera venom hypersensitivity. [ABSTRACT FROM AUTHOR]

Background: Data on the prevalence and clinical course of food protein-induced enterocolitis syndrome (FPIES) vary between populations and according to the culprit food. Objective: To evaluate the incidence, clinical characteristics, and remission patterns of FPIES among children in a Greek pediatric allergy referral center. Methods: We retrospectively studied children with acute FPIES. Data on age, sex, type of reaction, the implicated food, and oral food challenge (OFC) outcomes at baseline and upon reevaluation were analyzed. Results: Between October 2010 and March 2017, 72 (38 males) out of 15,114 subjects who had been referred to our department due to any reported allergic symptoms were diagnosed with acute FPIES. The most commonly implicated foods were cow's milk (CM) (45.8%), fish (34.7%), rice (9.7%), egg (6.9%), and chicken (2.8%). The mean age at diagnosis was 7.1/19.3/9.1/18.7/8.7 months for those with CM/fish/rice/egg/chicken FPIES, respectively. Sixty-nine OFCs were performed, of which 8 were diagnostic and 61 for tolerance evaluation. The type of culprit food was significantly associated with the outcome of the tolerance OFCs. OFCs to fish resulted positive at a significantly higher rate (12/22; 54.5%) than OFCs to CM (4/29; 13.7%), rice (1/5; 20%), egg (0/3; 0%), and chicken (0/2; 0%) (p = 0.01). The time period between diagnosis and tolerance acquisition was prolonged in the fish FPIES cases (74.8 months; 95% CI: 57.9–91.6) compared to that with other foods such as CM (20.7 months; 95% CI: 17.3–24.1), rice (31.8 months; 95% CI: 21.9–41.7), and egg (24.3 months; 95% CI: 10.7–37.9), as shown in a Kaplan-Meier analysis (log-rank, p < 0.001). When the fish FPIES children were assessed for tolerance, OFCs were significantly more often positive than in CM FPIES children (52 vs. 18.1%; p = 0.03), despite the fact that the children were challenged at an older age (fish: 70.4 months, 95% CI: 58.3–82.5, vs. CM: 26.57 months, 95% CI: 21.1–32, p < 0.001). Conclusions: Acute FPIES had a low incidence in our population. CM and fish were the two most frequent elicitors. Background: There is little evidence on the incidence and characteristics of local allergic rhinitis (LAR) in children. Most studies have included subjects with perennial rhinitis only, and results are based on the investigation of no more than three allergens per study. Our aim was to determine the proportion of children with LAR amongst children with chronic, difficult‐to‐treat, perennial or seasonal, rhinitis but no evidence of sensitization to aeroallergens, or other alternative diagnosis. Methods: We performed multiple nasal provocation tests (M‐NPTs) with four locally relevant aeroallergens (Phleum pratense, Olea europea, Alternaria alternata, and Dermatophagoides pteronyssinus) in children with absence of aeroallergen sensitization, seen during a calendar year in a specialized rhinitis clinic. We additionally performed single NPT to children with allergic rhinitis (AR; positive control group). The result of the NPT was based on symptoms and acoustic rhinometry. Identification of nasal hyper‐reactivity (NHR) triggers was through a questionnaire. Results: Local allergic rhinitis was confirmed in 29.2% (7/24) of the negative SPT/blood testing population. All but one of the children reacted to one allergen and one to two. All AR children had positive single NPT with results similar to the LAR. There were no differences in age at examination and rhinitis onset, gender distribution, family atopy, and past or current environment of residency, while the prevalence of reported NHR triggers was comparable amongst the three groups. Conclusion: This is the first pediatric study where the seasonal or perennial rhinitis population was thoroughly tested for LAR against four aeroallergens. LAR is present in a considerable proportion of children with chronic, difficult‐to‐treat rhinitis and no sensitization to aeroallergens, and therefore, the performance of NPT should be strongly considered in these cases. The aim of the study is to determine the reliability and validity of the Greek version of the Food Allergy Quality of life Questionnaire-Child Form (FAQLQ-CF). After linguistic validation, the Greek FAQLQ-CF, Food Allergy Independent Measure (FAIM) and Pediatric Quality of Life Inventory (PedsQL™) were used by a physician to interview children diagnosed with food allergy and aged 8-12 via telephone. Cronbach's alpha was used to evaluate reliability, and factor analysis to assess construct validity. The correlation between FAQLQ-CF and FAIM was moderate (rho=0.509, P<0.001) and internal consistency was strong (Cronbach's alpha 0.905). FAQLQ-CF discriminated well each question's contribution to children's quality of life deterioration (32- 80%), each child's quality of life (17-89%), children differing in doing things with others (total score 3.55 vs 2.57, difference =0.98 > minimal clinical importance difference = 0.5; P<0.001), but not children differing in reporting anaphylaxis. The total FAQLQ-CF score correlated with the total PedsQL™ score and with the score of one of PedsQL™ subscales, demonstrating convergent validity. Factor analysis uncovered an underlying structure of four factors, explaining 50% of the variance. We can conclude that Greek FAQLQ-CF is a reliable, valid, discriminant tool for interviewing food allergic children aged 8- 12, detecting those in need for immediate care. [ABSTRACT FROM AUTHOR]

The article presents a study of five adolescents, mean aged 14.6 years, with refractory cold contact urticaria (CCU) where omalizumab was used an add-on treatment because of either severe adverse effects and non- responsiveness to antihistamines. It mentions that only side effects were noted in respect to omalizumab such as discomfort, localized swelling and minimal local bleeding due to injection procedure.

Background To date, extensive research has been undertaken on a potential link of breastfeeding ( BF) to wheezing illnesses. Nevertheless, an association remains to be established, partly due to age-dependent discrepancies and different definitions of exposures/outcomes across studies. We thus investigated the relation of diverse infantile feeding patterns with wheeze/asthma prevalence in two cohorts of children of different ages (preschool and preadolescent). Methods Wheeze ever/in the last 12 months (current) and doctor-diagnosed asthma were retrospectively reported by parents of the participants of two cross-sectional studies: the Genesis study (1871 children aged 1-5) and the Healthy Growth study (1884 children aged 9-13). Information on feeding practices (exclusive breastfeeding vs. mixed vs. formula feeding) and their duration (2 vs. 4 vs. 6 months) was recorded. Perinatal and anthorpometric data were also collected. Results In pre-schoolers, regimes that did not entail exclusive BF were positively correlated to current/ever wheeze, both before and after adjustment for confounders. No differences between the associations of regimes with 2, 4 or 6 months of exclusive BF with current/ever wheeze were shown. Furthermore, there was no consistent correlation of feeding practices with physician-diagnosed asthma. In pre-adolescents, no association of infantile feeding patterns with the wheeze/asthma outcomes was observed. Conclusions Exclusive BF is associated with reduced prevalence of current/ever wheeze in pre-schoolers; however, this appears to wane in older children. The association of a period of exclusive BF as low as 2 months with pre-school wheeze prevalence, appeared to be comparable with that of 6 months of exclusivity. [ABSTRACT FROM AUTHOR]

Environmental tobacco smoke (ETS) is a significant risk factor for the presence and increased severity of asthma- and allergy-related symptoms in children. Smoking during pregnancy has detrimental effects on asthma-associated outcomes in childhood. Whether passive exposure of pregnant women to ETS may also lead to asthma in their offspring, is not known. The aim of this study was to investigate the association of passive exposure of pregnant women to ETS and asthma- and/or allergy-related symptoms in Preschool children. Cross-sectional data were collected with questionnaires from 2374 Preschool children, recruited from public and private nurseries and day-care centers. Parental smoking was significantly associated with wheezing symptoms in their children. Mother’s active smoking during pregnancy significantly increased the risk for occurrence of asthma symptoms and/or medically diagnosed asthma in Preschool children in a dose-dependent manner. Passive exposure to ETS, mainly during the third trimester of pregnancy, was significantly associated with asthma- and allergy-related symptoms after adjusting for several confounders in a multivariate analysis (current wheeze: OR = 1.42, 95% CI = 1.06–1.91, pruritic rash ever: OR= 1.45, 95% CI = 1.01–2.08). Passive exposure of pregnant women to ETS during the third trimester is positively associated with asthma- and allergy-related symptoms in their Preschool age children. Background: Although development of clinical tolerance is the rule in allergy to cow’s milk (CM), food challenges are required in order to reintroduce CM into the patient’s diet. Less ‘invasive’ procedures able to predict tolerance would be useful as clinical tools. The purpose of this study was to identify potential risk factors for clinical reactivity in CM-allergic children assessed for CM reintroduction. Methods: One hundred and sixteen open challenges performed in children 10–47 months old with IgE-mediated allergy to CM, in order to reintroduce CM into the diet, were retrospectively evaluated. Specific IgE (sIgE) levels assessed by the CAP System FEIA and skin prick tests (SPT) were obtained at diagnosis and prechallenge. Demographic parameters and measures of sIgE were evaluated as potential predictors of a positive challenge in univariate and multivariate logistic regression models. Results: Twenty-four out of 116 challenges were positive, 9 of which required the use of adrenaline. In order of performance, prechallenge sIgE <3.94 kU/l, the combination of SPT and sIgE or an SPT wheal <4 mm could correctly predict a negative challenge outcome, whereas values of SPT >7.5 mm or sIgE >25.4 kU/l, or their combination, had a high positive predictive value. The presence of atopic dermatitis did not affect the predictive accuracy of these values. Conclusions: Milk sIgE level prechallenge is a useful predictor of challenge outcome in patients with milk allergy. OBJECTIVE: We aimed to evaluate the immune status and immune recovery after completion of chemotherapy and/or radiotherapy in children with lymphoma. METHODS: We prospectively evaluated humoral and cellular immunity in 22 children with lymphoma (11 with Hodgkin's disease [HD] and 11 with non-Hodgkin's lymphoma [NHL]) at the completion of therapy and every 6 months thereafter. RESULTS: Immunoglobulin (Ig) levels were normal before the onset of therapy in all but 1 child. At the end of therapy, Ig levels decreased: IgM in 18, IgG in 12, and IgA in 7 children. In addition, 17 of 22 had decreased CD19 levels. In HD after radiotherapy, IgG and CD19 levels increased significantly (P = .013 and .004, respectively). IgM levels remained abnormally low in 16 of 22 children up to 18 months after therapy completion. At the end of therapy, helper T lymphocyte (CD4) levels were low in 20 of 22 children, and suppressor (CD8) levels were elevated in 13 of 22 children. (For those with HD before radiotherapy, the CD8 level was high in 10 of 11 children, and the CD4 level was low in 6 of 11 children.} The suppressor CD8 level remained elevated in 12 of 20 children, and helper CD4 level remained abnormally low in 18 of 20 children for a period of 6 to 18 months after therapy. Some immunized children became nonimmune to polio (15 of 22), mumps (6 of 22), rubella (5 of 22), and measles (1 of 22). CONCLUSIONS: In children with lymphoma, 1gM levels remained low for long periods. Helper T lymphocyte levels were low and suppressor levels were high at the end of therapy. Suppressor cells normalized faster, whereas helper cell levels remained abnormally low for a long period. Most children became nonimmune to polio, whereas the majority had antibodies to measles, mumps, and rubella. Despite depressed immunity, serious infections were not documented. INTRODUCTION: Skin-prick test wheal size and serum-specific immunoglobulin E (IgE) levels are able to predict, to an extent, the presence of allergy to certain foods. Nevertheless, the predictive value of these markers is not enough to substitute for oral food challenges, which suggests the need for improvement. OBJECTIVE: The goal was to determine the prognostic value of specific IgE/total IgE (tIgE) ratio in patients with allergy to cow's milk by using receiver operating characteristic (ROC) analysis. METHODS: Thirty-four open challenges were performed in children with a mean age of 18.4 months (range: 10.3-69.2 months) who had a previously diagnosed IgE-mediated allergy to cow's milk to reintroduce milk into their diet. Specific IgE levels, assessed by ImmunoCAP fluorescence enzyme immunoassay (Pharmacia Corp, Bridgewater, NJ), were obtained, and skin-prick tests were performed just before the challenge. The specific IgE (f2)/tIgE ratio was evaluated as a potential predictor of a positive challenge and compared with f2 alone by using ROC analysis. RESULTS: Of 34 challenges, 6 (17.6%) results were positive. Prechallenge milk-specific IgE levels and the f2/tIgE ratio were significant predictors (Pf2 = .007; Pratio = .008) of a positive challenge outcome. After ROC analysis, f2 provided a discrimination (between positive and negative provocations) of 0.8601 (ROC area under the curve); however, the f2/tIgE ratio provided a significantly greater discrimination of 0.9464. Values of the f2/tIgE ratio that are >0.1121 provide a 100% diagnostic accuracy (probability of a positive provocation result). CONCLUSIONS: The f2/tIgE ratio may be a novel, promising predictor of positive oral food-challenge results and should be evaluated prospectively in a larger sample. [ABSTRACT FROM AUTHOR]
