Abstract

The Marginal Ice Zone (MIZ) forms a critical transition region between the ocean and sea ice cover as it protects the close ice further in from the effect of the steepest and most energetic open ocean waves. As waves propagate through the MIZ, they get exponentially attenuated. Unfortunately, the associated attenuation coefficient is difficult to accurately estimate and model, and there are still large uncertainties around which attenuation mechanisms dominate depending on the conditions. This makes it challenging to predict waves in ice attenuation, as well as sea ice breakup and dynamics. Here, we report in-situ observations of strongly modulated waves-in-ice amplitude, with a modulation period of around 12 hours. We show that simple explanations, such as changes in the incoming open water waves, or the effect of tides and currents and bathymetry, cannot explain for the observed modulation. Therefore, the significant wave height modulation observed in the ice most likely comes from a modulation of the waves-in-ice attenuation coefficient. To explain this, we conjecture that one or several waves-in-ice attenuation mechanisms are periodically modulated and switched on and off in the area of interest. We gather evidence that sea ice convergence and divergence is likely the factor driving this change in the waves in ice attenuation mechanisms and attenuation coefficient, for example by modulating the intensity of floe-floe interaction mechanisms.

Abstract

Magnetar hyperflares are the most plausible explanation for fast radio bursts (FRB) -- enigmatic powerful radio pulses with durations of several milliseconds and high brightness temperatures. The first observational evidence for this scenario was obtained in 2020 April when a FRB was detected from the direction of the Galactic magnetar and soft gamma-ray repeater SGR\,1935+2154. The FRB was preceded by two gamma-ray outburst alerts by the BAT instrument aboard the Swift satellite, which triggered follow-up observations by the High Energy Stereoscopic System (H.E.S.S.). H.E.S.S. has observed SGR\,1935+2154 for 2 hr on 2020 April 28. The observations are coincident with X-ray bursts from the magnetar detected by INTEGRAL and Fermi-GBM, thus providing the first very high energy (VHE) gamma-ray observations of a magnetar in a flaring state. High-quality data acquired during these follow-up observations allow us to perform a search for short-time transients. No significant signal at energies $E>0.6$~TeV is found and upper limits on the persistent and transient emission are derived. Abstract

We analyzed 252 hours of High Energy Stereoscopic System (H.E.S.S.) observations towards the supernova remnant (SNR) LMC N132D that were accumulated between December 2004 and March 2016 during a deep survey of the Large Magellanic Cloud, adding 104 hours of observations to the previously published data set to ensure a > 5 sigma detection. To broaden the gamma-ray spectral coverage required for modeling the spectral energy distribution, an analysis of Fermi-LAT Pass 8 data was also included. We unambiguously detect N132D at very high energies (VHE) with a significance of 5.7 sigma. We report the results of a detailed analysis of its spectrum and localization based on the extended H.E.S.S. data set. The joint analysis of the extended H.E.S.S and Fermi-LAT data results in a spectral energy distribution in the energy range from 1.7 GeV to 14.8 TeV, which suggests a high luminosity of N132D at GeV and TeV energies. We set a lower limit on a gamma-ray cutoff energy of 8 TeV with a confidence level of 95%. The new gamma-ray spectrum as well as multiwavelength observations of N132D when compared to physical models suggests a hadronic origin of the VHE gamma-ray emission. SNR N132D is a VHE gamma-ray source that shows a spectrum extending to the VHE domain without a spectral cutoff at a few TeV, unlike the younger oxygen-rich SNR Cassiopeia A. The gamma-ray properties of N132D may be affected by an interaction with a nearby molecular cloud that partially lies inside the 95% confidence region of the source position. Abstract

The High Altitude Water Cherenkov (HAWC) observatory and the High Energy Stereoscopic System (H.E.S.S.) are two leading instruments in the ground-based very-high-energy gamma-ray domain. HAWC employs the water Cherenkov detection (WCD) technique, while H.E.S.S. is an array of Imaging Atmospheric Cherenkov Telescopes (IACTs). The two facilities therefore differ in multiple aspects, including their observation strategy, the size of their field of view and their angular resolution, leading to different analysis approaches. Until now, it has been unclear if the results of observations by both types of instruments are consistent: several of the recently discovered HAWC sources have been followed up by IACTs, resulting in a confirmed detection only in a minority of cases. With this paper, we go further and try to resolve the tensions between previous results by performing a new analysis of the H.E.S.S. Galactic plane survey data, applying an analysis technique comparable between H.E.S.S. and HAWC. Events above 1 TeV are selected for both datasets, the point spread function of H.E.S.S. is broadened to approach that of HAWC, and a similar background estimation method is used. This is the first detailed comparison of the Galactic plane observed by both instruments. H.E.S.S. can confirm the gamma-ray emission of four HAWC sources among seven previously undetected by IACTs, while the three others have measured fluxes below the sensitivity of the H.E.S.S. dataset. Remaining differences in the overall gamma-ray flux can be explained by the systematic uncertainties. Therefore, we confirm a consistent view of the gamma-ray sky between WCD and IACT techniques.

Abstract

The identification of PeVatrons, hadronic particle accelerators reaching the knee of the cosmic ray spectrum (few $10^{15}$ eV), is crucial to understand the origin of cosmic rays in the Galaxy. We provide an update on the unidentified source HESS J1702-420, a promising PeVatron candidate. We present new observations of HESS J1702-420 made with the High Energy Stereoscopic System (H.E.S.S.), and processed using improved analysis techniques. The analysis configuration was optimized to enhance the collection area at the highest energies. We applied a three-dimensional (3D) likelihood analysis to model the source region and adjust non thermal radiative spectral models to the $\gamma$-ray data. We also analyzed archival data from the Fermi Large Area Telescope (LAT) to constrain the source spectrum at $\gamma$-ray energies >10 GeV. We report the detection of a new source component called HESS J1702-420A, that was separated from the bulk of TeV emission at a $5.4\sigma$ confidence level. The power law $\gamma$-ray spectrum of HESS J1702-420A extends with an index of $\Gamma=1.53\pm0.19_\text{stat}\pm0.20_\text{sys}$ and without curvature up to the energy band 64-113 TeV, in which it was detected by H.E.S.S. at a $4.0\sigma$ confidence level. This brings evidence for the source emission up to $100\,\text{TeV}$, which makes HESS J1702-420A a compelling candidate site for the presence of extremely high energy cosmic rays. Remarkably, in a hadronic scenario, the cut-off energy of the proton distribution powering HESS J1702-420A is found to be higher than 0.5 PeV at a 95% confidence level. HESS J1702-420A becomes therefore one of the most solid PeVatron candidates detected so far in H.E.S.S. data, altough a leptonic origin of its emission could not be ruled out either., Comment: Accepted for publication in the 2. Abstract

Cosmological $N$-body simulations show that Milky Way-sized galaxies harbor a population of unmerged dark matter subhalos. These subhalos could shine in gamma-rays and be eventually detected in gamma-ray surveys as unidentified sources. We performed a thorough selection among unidentified Fermi-LAT Objects (UFOs) to identify them as possible TeV-scale dark matter subhalo candidates. We search for very-high-energy (E $\gtrsim$ 100 GeV) gamma-ray emissions using H.E.S.S. observations towards four selected UFOs. Since no significant very-high-energy gamma-ray emission is detected in any dataset of the four observed UFOs nor in the combined UFO dataset, strong constraints are derived on the product of the velocity-weighted annihilation cross section $\langle \sigma v \rangle$ by the $J$-factor for the dark matter models. The 95% C.L. observed upper limits derived from combined H.E.S.S. observations reach $\langle \sigma v \rangle J$ values of 3.7$\times$10$^{-5}$ and 8.1$\times$10$^{-6}$ GeV$^2$cm$^{-2}$s$^{-1}$ in the $W^+W^-$ and $\tau^+\tau^-$ channels, respectively, for a 1 TeV dark matter mass. Focusing on thermal WIMPs, the H.E.S.S. constraints restrict the $J$-factors to lie in the range 6.1$\times$10$^{19}$ - 2.0$\times$10$^{21}$ GeV$^2$cm$^{-5}$, and the masses to lie between 0.2 and 6 TeV in the $W^+W^-$ channel. For the $\tau^+\tau^-$ channel, the $J$-factors lie in the range 7.0$\times$10$^{19}$ - 7.1$\times$10$^{20}$ GeV$^2$cm$^{-5}$ and the masses lie between 0.2 and 0.5 TeV. Assuming model-dependent predictions from cosmological N-body simulations on the $J$-factor distribution for Milky Way-sized galaxies, the dark matter models with masses greater than 0.3 TeV for the UFO emissions can be ruled out at high confidence level., Comment: 11 pages, 7 figures, matches accepted version in The Astrophysical Journal

Abstract

Objective: To investigate and compare trends in incidence rates (IRs) of seropositive and seronegative rheumatoid arthritis (RA) in Denmark using various data sources for serostatus definition. Method: This nationwide population-based cohort study was based on data from Danish healthcare and clinical quality registries between 2000 and 2018. Information on anti-cyclic citrullinated peptide and immunoglobulin M rheumatoid factor was obtained, and definitions of seropositivity according to the number of applied data sources were prespecified. Annual age- and sex-standardized IRs were calculated as the number of incident seropositive and seronegative cases, divided by the number of person-years (PY) in the general population in that given year. Results: An increasing temporal trend in IR of seropositive RA and a decreasing trend in seronegative RA were observed. The IRs were higher for seropositive RA than for seronegative RA from 2009 onwards, with a widening of the IR gap between 2009 and 2016 regardless of the definition of seropositivity. When combining laboratory- and physician-reported autoantibody information and ICD-10 codes, the IR of seropositive RA in 2018 was approximately twice that of seronegative RA, at 19.0 and 9.0 per 100 000 PY, respectively. The level of antibody testing increased significantly during the study period. Conclusions: The IR of seropositive RA increased over time, whereas the IR of seronegative RA decreased. Temporal IR changes may be caused by a real change in the RA serology subtypes, an increase in autoantibody testing and availability, changes in registration practice over time, or a combination of these factors. Objectives Obesity and psoriatic arthritis (PsA) have a complicated relationship. While weight alone does not cause PsA, it is suspected to cause worse symptoms. Neutrophil gelatinase-associated lipocalin (NGAL) is secreted through various cell types. Our objective was to assess the changes and trajectories in serum NGAL and clinical outcomes in patients with PsA during 12 months of anti-inflammatory treatment. Method This exploratory prospective cohort study enrolled PsA patients initiating conventional synthetic or biological disease-modifying anti-rheumatic drugs (csDMARDs/bDMARDs). Clinical, biomarker, and patient-reported outcome measures were retrieved at baseline, and 4 and 12 months. Control groups at baseline were psoriasis (PsO) patients and apparently healthy controls. The serum NGAL concentration was quantified by a high-performance singleplex immunoassay. Results In total, 117 PsA patients started a csDMARD or bDMARD, and were compared indirectly at baseline with a cross-sectional sample of 20 PsO patients and 20 healthy controls. The trajectory in NGAL related to anti-inflammatory treatment for all included PsA patients showed an overall change of −11% from baseline to 12 months. Trajectories in NGAL for patients with PsA, divided into treatment groups, showed no clear trend in clinically significant decrease or increase following anti-inflammatory treatment. NGAL concentrations in the PsA group at baseline corresponded to the levels in the control groups. No correlation was found between changes in NGAL and changes in PsA outcomes. Abstract

ObjectiveEvidence points to a decreased breast cancer risk in systemic lupus erythematosus (SLE). We analyzed data from a large multisite SLE cohort, linked to cancer registries.MethodsInformation on age, SLE duration, cancer date, and histology was available. We analyzed information on histological type and performed multivariate logistic regression analyses of histological types according to age, SLE duration, and calendar year.ResultsWe studied 180 breast cancers in the SLE cohort. Of the 155 cases with histology information, 11 were referred to simply as 'carcinoma not otherwise specified'. In the remaining 144 breast cancers, the most common histological type was ductal carcinoma (n = 95; 66%) followed by lobular adenocarcinoma (n = 11; 8%), 15 cancers were of mixed histology, and the remaining ones were special types. In our regression analyses, the independent risk factors for lobular versus ductal carcinoma was age [odds ratio (OR) 1.07, 95% confidence interval (CI) 1.01-1.14] and for the 'special' subtypes it was age (OR 1.06, 95% CI 1.01-1.10) and SLE duration (OR 1.05, 95% CI 1.00-1.11).ConclusionsGenerally, up to 80% of breast cancers are ductal carcinomas. Though our results are not definitive, in the breast cancers that occur in SLE, there may be a slight decrease in the ductal histological type. In our analyses, age and SLE duration were independent predictors of histological status.

Abstract

Background: Chronic pain is the hallmark symptom of joint diseases. This study examined the differences in quantitative sensory testing between patients with psoriatic arthritis (PsA), hand osteoarthritis (hand‐OA) and a pain‐free control group and differences between patients with and without concomitant fibromyalgia (cFM). Methods: All patients and pain‐free controls were assessed using pressure pain thresholds (PPT), temporal summation of pain (TSP), conditioned pain modulation (CPM) and clinical pain intensities. Psychological distress was assessed with the Hospital Anxiety and Depression Scale, Pain Catastrophizing Scale, and Pittsburgh Sleep Quality Index. Disability was assessed with the Health Assessment Questionnaire and pain quality with the painDETECT questionnaire. cFM was identified using the revised 2016 American College of Rheumatology diagnostic criteria. Results: Patients with hand‐OA (n = 75) or PsA (n = 58) had statistically significant lower PPTs and CPM, greater TSP, and higher scores of psychological distress (p < 0.05) than controls (n = 20). Patients with cFM (58%) had higher scores of depression (p = 0.001), anxiety (p = 0.004), catastrophizing (p = 0.012), disability (p < 0.001), higher painDETECT score (p = 0.001), TSP (p = 0.027), and reduced sleep quality (p = 0.021) when compared to patients without cFM. Conclusion: Patients with hand‐OA and PsA exhibited signs of pain sensitization and a higher degree of psychological distress and disability than pain‐free individuals. Patients with cFM had greater TSP, painDETECT score, disability, catastrophizing, and reduced sleep quality, than patients without, indicating greater degree of pain sensitization, psychological burden, and disability. Statement of Significance: This paper shows that a significant proportion of patients with hand osteoarthritis and psoriatic arthritis with moderate pain intensity have significantly increased signs of pain sensitization and markers of psychological distress. A large proportion of these patients fulfil the criteria for concomitant fibromyalgia and these patients show even greater propensity towards pain sensitization and psychological distress. Trajectories in NGAL for patients with PsA, divided into treatment groups, showed no clear trend in clinically significant decrease or increase following anti-inflammatory treatment. The aim was to study the validity of the RA diagnosis and to estimate the completeness of relevant RA cases in each registry.Study design and setting: Patients registered for the first time in 2011 with a diagnosis of RA were identified in DANBIO and DNPR in January 2013. For DNPR, filters were applied to reduce false-positive cases. The diagnosis was verified by a review of patient records. We calculated the positive predictive values (PPVs) of the RA diagnosis registrations in DANBIO and DNPR, and estimated the registry completeness of relevant RA cases for both DANBIO and DNPR. Updated data from 2011 to 2015 from DANBIO were retrieved to identify patients with delayed registration, and the registry completeness and PPV was recalculated.Results: We identified 1,678 unique patients in DANBIO or in DNPR. The PPV (2013 dataset) was 92% in DANBIO and 79% in DNPR. PPV for DANBIO on the 2015 update was 96%. The registry completeness of relevant RA cases was 43% in DANBIO, increasing to 91% in the 2015 update and 90% in DNPR.Conclusion: DANBIO held a high proportion of true RA cases (96%) and was found to be superior to the DNPR (79%) with regard to the validity of the diagnosis. Abstract

Aims:To evaluate predictors for successful biologic tapering among patients with inflammatory arthritis using baseline characteristics from the BIODOPT trial. Methods:Adult patients with rheumatoid arthritis, psoriatic arthritis or axial spondyloarthritis on stable biologic dose and in low disease activity ≥12 months were enrolled. Participants were randomized (2:1) to disease activity-guided biologic tapering or continuation of baseline biologic. Patients achieving successful tapering reduced their biologic dose by ≥50%, had no protocol deviations and were in low disease activity at 18 months. Modified Poisson regression with robust variance estimator was applied. Results:In total, 142 patients were randomized to tapering (n= 95) or control (n= 47). Successful tapering was achieved by 32 and 2%, respectively. Tapering group was the only statistically significant independent predictor for successful tapering, risk ratio (RR): 14.0 (95% confidence interval [CI]: 1.9 to 101.3,P= .009). However, higher Short Form Health Survey 36 mental component summary (SF-36 MCS) was observed to be a predictor of potential importance, RR: 1.06 (95% CI: 0.99 to 1.13,P= .097). When limiting the analyses to the tapering group only, none of the baseline variables were statistically significant independent predictors but SF-36 MCS was still considered to be of potential importance, RR: 1.05 (95% CI: 0.99 to 1.12,P= .098). Conclusion:Successful tapering is a reachable target for 1 in 3 patients with inflammatory arthritis who are interested in reducing their biological therapy. No statistically significant predictors (besides allocation to tapering) were identified. Future research on mental health and tapering is encouraged.

Abstract

The collision regulations include several qualitative terms without providing guidance as to how these terms could be understood in quantitative terms. These terms must therefore be interpreted by navigators, which poses a problem for autonomous ships. Extend the knowledge of how navigators interpret the collision regulations, with a specific focus on how they interpret the rule covering the requirement to proceed at a safe speed. Qualitative study based on interviews of a convenience sample of eight Norwegian navigators. Data was analysed with systematic text condensation. Navigators characterise safe speed as a speed in which they have control. Navigators do not look at different factors mentioned in the collision regulations in isolation, but within the context of the situation. Determining the safe speed of a vessel is more complicated than made out in the literature. As autonomous ships will have to cooperate with conventional vessels, their programming must include the knowledge of how the collision regulations are interpreted by human navigators. 31. Disease activity-guided tapering of biologics in patients with inflammatory arthritis: a pragmatic, randomized, open-label, equivalence trial.

Abstract

To evaluate whether disease activity-guided tapering of biologics compared to continuation as usual care enables a substantial dose reduction while disease activity remains equivalent. In this pragmatic, randomized, open-label, equivalence trial, adults with rheumatoid arthritis, psoriatic arthritis, or axial spondyloarthritis in low disease activity on stable-dose biologics for ≥ 12 months were randomized 2:1 into either the tapering group, i.e. disease activity-guided prolongation of the biologic dosing interval until flare or withdrawal, or the control group, i.e. maintaince of baseline biologics with a possible small interval increase at the patients request. The co-primary outcome in the intention-to-treat population was met if superiority in ≥ 50% biologic reduction at 18 months was demonstrated and disease activity was equivalent (equivalence margins ± 0.5). Ninety-five patients were randomized to tapering and 47 to control, of whom 37% (35/95) versus 2% (1/47) achieved ≥ 50% biologic reduction at 18 months. The risk difference was statistically significant [35%, 95% confidence interval (CI) 24%–45%], while disease activity remained equivalent [mean difference 0.05, 95% CI −0.12–0.29]. A statistically significant flare risk was observed [tapering 41% (39/95) vs control 21% (10/47), risk difference 20%, 95% CI 4%–35%]; but, only 1% (1/95) and 6% (3/47) had persistent flare and needed to switch to another biological drug. Disease activity-guided tapering of biologics in patients with inflammatory arthritis enabled one-third to achieve ≥ 50% biologic reduction, while disease activity between groups remained equivalent. 33. Treatment-related changes in serum neutrophil gelatinase-associated lipocalin (NGAL) in psoriatic arthritis: results from the PIPA cohort study

Obesity and psoriatic arthritis (PsA) have a complicated relationship. While weight alone does not cause PsA, it is suspected to cause worse symptoms. Neutrophil gelatinase-associated lipocalin (NGAL) is secreted through various cell types. Our objective was to assess the changes and trajectories in serum NGAL and clinical outcomes in patients with PsA during 12 months of anti-inflammatory treatment. This exploratory prospective cohort study enrolled PsA patients initiating conventional synthetic or biological disease-modifying anti-rheumatic drugs (csDMARDs/bDMARDs). Clinical, biomarker, and patient-reported outcome measures were retrieved at baseline, and 4 and 12 months. Control groups at baseline were psoriasis (PsO) patients and apparently healthy controls. The serum NGAL concentration was quantified by a high-performance singleplex immunoassay. In total, 117 PsA patients started a csDMARD or bDMARD, and were compared indirectly at baseline with a cross-sectional sample of 20 PsO patients and 20 healthy controls. The trajectory in NGAL related to anti-inflammatory treatment for all included PsA patients showed an overall change of −11% from baseline to 12 months. 42. Making Learning Support Contextually Responsive

Abstract

Research indicates that the success of inclusive education lies within the provision of adequate support for learners who experience barriers to learning in mainstream schools as well as in the changing roles of teachers and support services staff. The Western Cape Education Department (WCED) implemented a learning support model, designed to systemically deal with barriers to learning in some primary schools in the province. An evaluation of the WCED model established that this learning support model is not completely contextually responsive to the South African context with specific reference to resources, both human and physical. Although the WCED endeavours to distribute resources more even-handedly across schools in the Western Cape Province, there is still a general lack of resources and institutional capacity which hampers the successful implementation of inclusive education. This situation creates specific barriers that are responsible for the gap between current educational policies and the implementation thereof in schools.

Abstract

In South Africa, the processes of democratisation and social restructuring are inextricably linked to the debate on inclusive education. Although the debate on inclusive education originated in the disability discourse, it is increasingly viewed broader as a reform that supports and welcomes diversity. While the constitution and policy framework in the new South Africa is internationally recognised as of the most progressive regarding human rights, research shows great concerns regarding the gap between policy and implementation thereof. From a human rights and social justice perspective, education is viewed as a basic human right and the foundation for a more just and equal society. In this article I will therefore draw on Paulo Freire's (1998) notion of "pedagogy of hope" and root my argument in his definition that hope is "an ontological need that should be 'anchored in practice' in order to become 'historical concreteness'". Abstract

Objective To investigate the incidence and prevalence of rheumatoid arthritis (RA) in the adult Danish population. Method In this nationwide register-based cohort study, patients with incident RA between 1998 and the end of 2018 were identified using Danish administrative registries. The age- and sex-standardized incidence rate (IR), incidence proportion (IP), lifetime risk (LR), and point prevalence (PP) of RA were calculated. RA was defined as a first-time RA diagnosis registered in the Danish National Patient Registry combined with a redeemed prescription of a conventional synthetic disease-modifying anti-rheumatic drug in the following year. In addition, three different case definitions of RA were explored. Results The overall age- and sex-standardized IR of RA from 1998 to 2018 was 35.5 [95% confidence interval (CI) 35.1–35.9] per 100 000 person-years while the IP was 35.2 (95% CI 34.8–35.5) per 100 000 individuals. The IR was two-fold higher for women than for men. The LR of RA ranged from 2.3% to 3.4% for women and from 1.1% to 1.5% for men, depending on the RA case definition used. The overall PP of RA was 0.6% (95% CI 0.5–0.6%) in 2018: 0.8% (95% CI 0.7–0.8%) for women and 0.3% (95% CI 0.3–0.4%) for men. The prevalence increased about 1.5-fold from 2000 to 2018. Conclusion The IR and PP were approximately two-fold higher for women than for men. The prevalence of RA in Denmark increased significantly from 2000 to 2018. The RA case definition had more impact on the results than the choice of denominator.
