Objective To construct a prognostic model of chronic heart failure (CHF) by traditional Chinese medicine (TCM) syndrome elements by machine learning method. Methods Patients with CHF admitted to the Department of Cardiology of China-Japan Friendship Hospital from January 1, 2018 to April 30, 2021 were included, and their demographic data, vital signs, co-morbidities, laboratory tests, echo-cardiographic indicators, TCM syndrome elements and treatment information were collected. The primary end point for this analysis was a model to predict cardiovascular death or hospitalization because of heart failure in one year follow-up. Least absolute shrinkage, selection operator(LASSO) regression and Cox multivariate analysis were used to screen independent risk factors that potentially affect the prognosis of CHF. A nomogram was used to establish a risk prediction model based on TCM syndrome elements. Results Totally 164 patients with an average age of (72.23±14.16) years old and 37.2% male were included in this study. The LASSO screened 9 factors from clinical variables, including coronary heart disease, hypertension, uric acid, N-terminal pro-B type natriuretic peptide (NT-proBNP), left ventricular ejection fraction (LVEF), creatine kinase-myocardial band, myoglobin, Qi deficiency and Yin deficiency. Cox multivariate regression analysis showed that Qi deficiency, hypertension, coronary heart disease, NT-proBNP and LVEF were associated with prognosis in patients with CHF. Conclusions Qi deficiency was an independent predictor of cardiovascular death or heart failure readmission in CHF patients within 1 year. The prognostic model of CHF with integrated Chinese and Western medicine has demonstrated a high accuracy.

Pallet is an important tool in transportation, and its production raw materials show a diversified development trend. In order to design the structure of discarded glass fiber reinforced plastic (GFRP) pallets, a static analysis was performed using ABAQUS. The pallet's structural parameters were used as variables, while the pallet mass and maximum stress were set as optimization objectives. An optimization experiment using DOE with ISIGHT was conducted to obtain a locally optimal solution. The DOE experimental results were fitted using a RBF model to obtain an approximate model. By applying the NSGA-II algorithm, the approximate model can be further optimized to achieve the optimal structure. The results showed that after optimizing through DOE, the pallet's mass increased by 9.2% and maximum stress decreased by 18.2%. Furthermore, after further optimization using the NSGA-II algorithm, the pallet's mass decreased by 1.2% and maximum stress decreased by 15.3%. Optimised design of the pallet structure was achieved. The function of runt-related transcription factor 3 (RUNX3) in oral cancer remains controversial. The present study aimed to determine the status of RUNX3 protein expression and its association with clinicopathological characteristics in tongue squamous cell carcinomas (SCC). The present study used three pairs of tongue SCC and non-cancerous tissues to assess RUNX3 protein expression by western blot analysis, and two tongue SCC cell lines to determine RUNX3 protein localization by immunofluorescence and immunocytochemistry. Tissue microarray immunohistochemistry was performed to detect the clinical relevance of RUNX3 in 79 patients with tongue SCC. The results demonstrated that RUNX3 protein expression was reduced in tongue SCC tissues compared with in paired non-cancerous tissues. Similarly, the expression of RUNX3 was low in SCC25 and Cal27 cells, and was predominantly localized to the cytoplasm. In the 79 patients with tongue SCC, RUNX3 protein expression was presented in different manners in carcinoma nests and tumor stroma. RUNX3 in carcinoma nests (nRUNX3) exhibited nuclear positive staining in 24/79 samples, cytoplasmic mislocalization in 41/79 samples and was undetectable in 14/79 samples. RUNX3 in stroma (sRUNX3) exhibited nuclear positive staining in 40/79 samples and nuclear negative staining in 39/79 samples. Negative nRUNX3 expression was significantly associated with lymph node metastasis (P=0.014), clinical stage (P=0.027), and overall and disease-free survival (P=0.008 and P=0.007, respectively). In addition, negative sRUNX3 expression was associated with lymph node metastasis (P=0.003) and clinical stage (P=0.003); however, not with overall survival. The findings of the present study preliminarily suggested that cytoplasmic mislocalization of RUNX3 protein may be a common event in tongue SCC, and that sRUNX3 protein expression may be a potential prognostic biomarker. When the AHP and ANP are applied to the "scenario-response" based unconventional emergency decision-making, there easily exist inconsistent data and missing data because of time constraint, incomplete decision information, limited experience and human cognitive capabilities. In this paper, a logarithmic mean induced bias matrix model is proposed to process the missing data. Specifically, the missing data is first filled with unknown variables to obtain quasi-complete matrix, then the logarithmic mean induced bias matrix model is used to build overdetermined equations or constrained nonlinear optimization problems to solve the unknown variables. An emergency simulation experiment is used to illustrate and validate the proposed model, and the simulation results show that it is effective and feasible. The proposed model is only based on the original matrix information and does not need to calculate the weight vector. The estimated solutions of unknown variables can keep the global consistency and help emergency decision makers make valid decisions. [ABSTRACT FROM AUTHOR]

As soil fertility in apple orchard with clean tillage is declined continuously, interplanting herbage in orchard, which is a new orchard management model, plays an important role in improving orchard soil conditions. By using biolog micro-plate technique, this paper studied the functional diversity of soil microbial community under four species of management model in apple orchards, including clear tillage model, interplanting white clover model, interplanting small crown flower model and interplanting cocksfoot model, and the carbon source utilization characteristics of microbial community were explored, which could provide a reference for revealing driving mechanism of ecological process of orchard soil. The results showed that the functional diversity of microbial community had a significant difference among different treatments and in the order of white clover > small crown flower > cocksfoot > clear tillage. The correlation analysis showed that the average well color development (AWCD), Shannon index, Richness index and McIntosh index were all highly significantly positively correlated with soil organic carbon, total nitrogen, microbial biomass carbon, and Shannon index was significantly positively correlated with soil pH. The principal component analysis and the fingerprints of the physiological carbon metabolism of the microbial community demonstrated that grass treatments improved carbon source metabolic ability of soil microbial community, and the soil microbes with perennial legumes (White Clover and small crown flower) had a significantly higher utilization rate in carbohydrates (N-Acetyl-D-Glucosamine, D-Mannitol, β-Methyl-D-Glucoside), amino acids (Glycyl-L-Glutamic acid, L-Serine, L-Threonine) and polymers (Tween 40, Glycogen) than the soil microbes with clear tillage. It was considered that different treatments had the unique microbial community structure and peculiar carbon source utilization characteristics.

Purpose: To explore the clinical effect of functional repair for bilateral cleft lip using modified Mulliken method., Methods: Sixty-six patients with bilateral cleft lip were selected and assigned to receive modified Mulliken method. During the operation, the prolabium was kept as narrow "tie" shape, the orbicularis oris was anatomically repositioned, and the orbicularis oris ring was re-built. Vermilion tubercle was reconstructed with the lateral red vermillion. The nasal deformity was preliminarily repaired. and the nasal columella was elongated at the same time., Results: All the patients were followed-up for 0.5-2 years, there was no "trisection upper lip". The symmetry and natural shape of Cupid's bow were obtained in more than 80% patients. The width of philtrum was similar to normal children. Full vermilion of the lips, moderate-size vermilion tubercles and good dynamic and static shape were obtained without whistle deformities. Normal width of nasal base and nostril symmetry were gained. The columella was elongated. Satisfactory contour of the nasal tip was achieved., Conclusions: Modified Mulliken method could functionally repair bilateral cleft lip and effectively correct nasolabial deformities. It is worthy of wide clinical application.

Purpose: This study evaluated the biologic response to titanium mesh and autogenous particulate bone grafts for the reconstruction of segmental mandibular defects in a dog model., Materials and Methods: Unilateral 40-mm critical-sized continuity defects of the mandible were made in five beagle dogs. Titanium mesh was shaped and fixed with titanium screws in the defects. Autogenous bone was harvested from the ablated mandible and iliac crest (the ratio of cortical bone:cancellous bone was 3:1) and used to fill the mesh. Two implants were placed into each bone graft. The animals were sacrificed after 6 months. Radiographs, histologic sections, scanning electron microscopy, and energy dispersive spectroscopy were performed to evaluate bone formation and osseointegration of the implants in the reconstructed mandibles., Results: The outline of the reconstructed mandible was satisfactory, and no bone resorption was observed in the defect area. All implants showed excellent osseointegration of the grafted bone. Furthermore, the density of bone formed around the implants was higher than that seen in control samples (intact, ungrafted dog hemimandibles)., Conclusions: Shaped titanium mesh with autogenous particulate bone graft is a predictable method for restoring critical-sized continuity defects of the mandible. Simultaneous implant placement is feasible and the structure of bone formed near implants may be optimal.

Objective: To observe the healing and bone formation of the autogenous particulate bone graft and osseointegration of the implants in shaped titanium mesh with autogenous particulate bone graft and simultaneous implant for reconstructing segmental mandibular defect in dogs., Methods: Five Beagle dogs were prepared with unilateral 40 mm segmental mandibular defect. Titanium mesh was shaped according to the mandibular defect and was fixed with titanium screws. The ablated mandibular bone and iliac bone were crashed to approximately 2 mm particulates which then filled the titanium mesh tightly (cortical bone:cancellous bone, 3:1). Two implants were placed in the autogenous particulate bone graft. The dogs were sacrificed after six months. X-ray films, histological sections, Scanning electron microscopy(SEM) and Energy disperse analysis of the reconstructed mandibles were examined to evaluate the bone formation of the bone graft and osseointegration of the implants., Results: The outline of the reconstructed mandible was satisfactory and its function was excellent. The ossific fusion among the particulate bone grafts was fine and no resorption was found. All implants showed excellent osseointegration with the bone graft. The bone near the implant was more compact than the bone away from it. The structure of the bone near implant was optimized because of simultaneous implant in shaped titanium mesh., Conclusions: The shaped titanium mesh with autogenous particulate bone graft is a good method for reconstructing segmental mandibular defect, and simultaneous implant is feasible in the reconstructed mandible in dogs.
