20. Viabilidad Celular in Vitro de tejido renal neonatal postmortem: Sus aplicaciones futuras.

Abstract

La importancia de los cultivos celulares humanos continúa expandiéndose, en especial en el campo de la bioingeniería de tejidos. Con el fin de establecer mejores criterios de selección en la obtención de tejido renal postmortem, se estudiaron 111 neonatos de < 3 días de edad, fallecidos en la sala de Cuidados Intensivos del Recién Nacido (CIRENA) y 22 mortinatos de la sala de partos del Hospital Universitario del Valle (HUV) en Cali, Colombia, entre enero 1 y diciembre 31 de 1995. Se consideraron los antecedentes maternos y fetales, en busca de variables que determinen una mejor viabilidad celular renal postmortem (VCRP). Además, se hizo un estudio del estado microbiológico de los neonatos postmortem y de los mortinatos mediante el examen de sangre y células para conocer la incidencia en estos de infecciones bacterianas y virales.De los 133 casos estudiados, 115 (86%) fueron de bajo peso al nacer (BPN), vgr. 500 a 2,500 gramos y los 18 (14%) restantes fueron de peso adecuado al nacer (PAN) >2,500 gramos. De 13 clases de diagnóstico registrados como antecedentes maternos, los seis más frecuentes fueron: ruptura prematura de membranas (RPM) – 20%, PRE-eclampsia/eclampsia (PE) – 13%, corioamnionitis (CA) – 11%, infección urinaria (IU) – 11%, placenta previa (PP) – 9% y vaginitis (V) – 8%. De los ocho diagnósticos registrados en los neonatos, los dos más frecuentes fueron el síndrome de dificultad respiratoria (SDR) y la enfermedad de membrana hialina (ME) -73%.Se pudo observar que la VCRP total fue de 65%, siendo mayor en los casos con BPN (69%) que en los de PAN (39%). Independientemente de los antecedentes maternos se observó una VCRP de 70% y 77% en los casos con diagnóstico de SDR y EMH, respectivamente. Por otra parte, se obtuvo la mayor VCRP en los casos con diagnóstico materno de PP (83%). Aunque por el bajo número de casos expuestos en el presente estudio estas diferencias no demostraron significancia estadística, estos resultados son consistentes con los antes publicados.Se obtuvieron 6 casos (4.5%) con cultivos bacteriológicos positivos (Klebsiella pneumoniae-2, Bacillus subtilis-2, Pseudomona aeuroginosa-1, Staphylococcus aureus-1) compatibles con el diagnóstico de sepsis neonatorum. Pruebas de tamizaje inicial con “rapid plasma reagin” (RPR) detectaron 10 (7.5%) muestras positivas que plantean la posible presencia de anticuerpos asociados con sífilis en esta población al momento del parto.Estos resultados ameritan investigar mas a fondo la prevalencia y estadios reales de sífilis en esta población de RPR positivos con pruebas más especificas como VDRL y FTA-ABS. No se detectó seroreactividad para el virus de inmunodeficiencia humana (VIH) ni de hepatitis B.Este estudio, al igual que otros anteriores, sugiere una mayor VCRP en aquellos casos con diagnósticos que pudieran asociarse con una menor madurez fetal, como BPN, EMH/SDR, PP, y PE. Se podría esperar entonces que, a menor madurez fetal, mayor posibilidad de encontrar tejidos que den una mejor viabilidad celular. Estudios continúan en curso para que nos ayuden a establecer pautas para mejorar la selección de casos que nos permitan obtener cultivos celulares con mayor viabilidad...

22. The Spatial Extent of Pain Is Associated with Pain Intensity, Catastrophizing and Some Measures of Central Sensitization in People with Frozen Shoulder

Abstract

The aim of this cross-sectional study was to explore the spatial extent of pain and its association with clinical symptoms, psychological features, and pain sensitization in people with frozen shoulder (FS). Forty-eight individuals with FS completed pain drawings (PDs) and reported their clinical symptoms including pain intensity (Visual Analogue Scale) and shoulder disability (Shoulder Pain and Disability Index). Moreover, pain sensitization measurements (pressure pain thresholds, temporal summation, conditioned pain modulation, and Central Sensitization Inventory (CSI)) were assessed. Psychological features were assessed by Pain Catastrophizing Scale (PCS) and Pain Vigilance and Awareness Questionnaire. Pain frequency maps were generated, Margolis rating scale was used for pain location, and Spearman correlation coefficients were computed. The mean (SD) pain extent was 12.5% (6.7%) and the most common painful area was the anterolateral shoulder region (100%). Women presented a more widespread pain distribution compared with men. Significant positive associations were obtained between pain extent and current pain intensity (rs = 0.421, p < 0.01), PCS (rs = 0.307, p < 0.05) and CSI (rs = 0.358, p < 0.05). The anterolateral region of the shoulder was the most common painful area in people with FS. Women with FS presented more extended areas of pain; and a more widespread distribution of pain was correlated with higher levels of pain, pain catastrophizing and pain sensitization.

Abstract

[EN] Background: Foot pain is a highly prevalent health problem for which measures such as a pattern of Pressure Discomfort Threshold of the foot plantar surface can provide valuable information for orthosis design. This study aimed to describe such pattern as a tool for the assessment of painful conditions of the feet and to analyse how it modifies according to age, gender and obesity. Methods: A cross-sectional study was performed with participants allocated in: Group 1 people aged 20 to 35 years, Group 2 aged 50 to 65 years and Group 3 aged over 65. Pressure Discomfort Threshold on twelve points of the foot plantar surface was measured with an adapted manual dynamometer. Inferential analyses of the data were performed using one-way analysis of variance (ANOVA) considering foot areas, age group, gender and obesity. Findings: 36 participants were analysed. The pattern of Pressure Discomfort Threshold for all individuals showed a significantly higher threshold on the heel and external foot (P < 0.001, eta(2) = 0.124) and was statistical significantly influenced by age (P < 0.001, eta(2) = 0.17), especially in participants aged over 65; by gender, with women having higher values (P < 0.001, eta(2) = 0.13), and by obesity (P < 0.001, eta(2) = 0.19). Interpretation: A Pressure Discomfort Threshold pattern exists in the foot plantar surface. The characteristics of the discomfort pattern of the foot and its association with aging, gender and obesity may have considerable implications for orthosis and footwear design.

Abstract

[EN] Background: The purpose of this study was to evaluate the effect of low-frequency self-administered vibration therapy into myofascial trigger points in the upper trapezius and levator scapulae on patients with chronic non-specific neck pain. Methods: Twenty-eight patients with chronic non-specific neck pain were randomly assigned into a vibration group, receiving 10 self-applied sessions of vibration therapy in the upper trapezius and levator scapulae trigger points; or a control group, receiving no intervention. Self-reported neck pain and disability (Neck Disability Index) and pressure pain threshold were assessed at baseline and after the first, fifth and 10th treatment sessions. Findings: Significant differences were found in the vibration group when compared to the control group after the treatment period: the vibration group reached lower Neck Disability Index scores (F = 4.74, P = .033, eta(2) = 0.07) and greater pressure pain threshold values (F = 7.56, P = .01, eta(2) = 0.10) than the control group. The vibration group reported a significant reduction in Neck Disability Index scores (chi(2) = 19,35, P = .00, Kendall's W = 0.28) and an increase in pressure pain threshold (chi(2) = 87,10, P = .00, Kendall's W = 0.73) between the assessment times over the course of the treatment. The mean increase in pressure pain threshold in the vibration group after the 10 sessions was 8.54 N/cm2, while the mean reduction in Neck Disability Index scores was 4.53 points. Interpretation: Vibration therapy may be an effective intervention for reducing self-reported neck pain and disability and pressure pain sensitivity in patients with chronic non-specific neck pain. This tool could be recommended for people with non-specific neck pain.

35. Expanded Distribution of Pain as a Sign of Central Sensitization in Individuals With Symptomatic Knee Osteoarthritis

Abstract

Background. Expanded distribution of pain is considered a sign of central sensitization (CS). The relationship between recording of symptoms and CS in people with knee osteoarthritis (OA) has been poorly investigated. Objective. The aim of this study was to examine whether the area of pain assessed using pain drawings relates to CS and clinical symptoms in people with knee OA. Design. This was a cross-sectional study. Methods. Fifty-three people with knee OA scheduled to undergo primary total knee arthroplasty were studied. All participants completed pain drawings using a novel digital device, completed self-administration questionnaires, and were assessed by quantitative sensory testing. Pain frequency maps were generated separately for women and men. Spearman correlation coefficients were computed to reveal possible correlations between the area of pain and quantitative sensory testing and clinical symptoms. Results. El Código Penal Colombiano establece las sanciones para las lesiones no fatales, que varían en rangos de años privativos de la libertad y multa económica en función de los días de incapacidad médico-legal y secuelas que se establecen en una valoración clínica realizada por un médico forense. El Reglamento Técnico Colombiano para el Abordaje Integral de Lesiones en Clínica Forense precisa de manera general cuáles deben ser los días de incapacidad de acuerdo al tiempo estimativo de reparación de los distintos tipos de las lesiones, y determina elementos para establecer las secuelas; sin embargo, no establece gradualidades cuantitativas ni cualitativas para la determinación de las mismas. Se realizó una revisión de los lineamientos jurídicos penales, históricos, conceptuales y médicos en relación a la determinación y valoración de las secuelas en diferentes países. Se utilizaron los motores de búsqueda PubMed, Medline, Google, Science Direct y Springer journal. Se encontró que el proceso de la valoración del daño corporal, desde el ámbito médico-legal, ha evolucionado en relación a las secuelas, se propende por realizar una valoración cuantitativa a través de los procesos de baremación, con el objetivo de unificar los criterios médicos, asegurar respaldo científico y disminuir la litigiosidad de los procesos. En Colombia es deseable realizar un proceso de graduación o baremación cuantitativa de las secuelas, teniendo como base fundamental la ICF (International Classification of Functioning, Disability and Health) y la experiencia de países líderes en el tema, como España y los Estados Unidos, con el propósito de unificar los procesos para la valoración del daño y su respectiva tasación jurídica. The Colombian Penal Code establishes penalties for non-fatal injuries, varying ranges of proprietary years of freedom and fine economic in terms of days of legal medical incapacity and sequel that are established in a clinical assessment by a forensic doctor. The Colombian Technical Regulations for a Comprehensive Approach Injury Clinic Forensic generally required which must be disability days according to the estimated time of repair of different types of injuries, and determined elements to establish the aftermath; however, it does not establish quantitative or qualitative determination for the same graduations. A review of the legal, historical and medical concepts, in some countries that have made developments in the field. Search engines used were PubMed, Medline, Google, Science Direct and Springer journal. It was found that the process of evaluation of bodily harm, from the forensic field has evolved in relation to the aftermath, as each day tends more to try to make a quantitative assessment through Scale graduation, aiming to unify medical criteria, ensure scientific support and reduce litigation process. In Colombia it is desirable to perform a process of quantitative determination of the sequels, taking as a fundamental basis the ICF (International Classification of Functioning, Disability and Health) and the experience of leading countries in the subject as Spain and the United States for the purpose of unify the processes for assessment of the damage.

38. Enfermedad perinatal y viabilidad celular renal postmortem in vitro en el Hospital Universitario del Valle, Cali

Abstract

Se estudiaron 357 neonatos - 3 días de edad, fallecidos en la sala de Cuidados Intensivos del Recién Nacido (CIRENA) y 39 mortinatos de la sala de partos, del Hospital Universitario del Valle (HUV) en Cali, Colombia, para buscar posibles relaciones entre los antecedentes de enfermedad perinatal y la viabilidad celular renal postmortem (VCRP) in vitro. Se logró demostrar una VCRP total de 60%. Ésta fue significativamente mayor en mortinatos 2,500 g de peso al nacer con antecedente materno de eclampsia (26%). Además, se observó una tasa de mortalidad hospitalaria de 22% en los 3,628 neonatos admitidos a CIRENA entre octubre 1, 1992 y diciembre 31, 1994. El mayor riesgo de muerte fue para los neonatos 2,500 g and maternal history of eclampsia (26%). Abstract

The results of a surveillance study conducted by the International Nosocomial Infection Control Consortium (INICC) from January 2004 through December 2009 in 422 intensive care units (ICUs) of 36 countries in Latin America, Asia, Africa, and Europe are reported. During the 6-year study period, using Centers for Disease Control and Prevention (CDC) National Healthcare Safety Network (NHSN; formerly the National Nosocomial Infection Surveillance system [NNIS]) definitions for device-associated health care-associated infections, we gathered prospective data from 313,008 patients hospitalized in the consortium's ICUs for an aggregate of 2,194,897 ICU bed-days. Despite the fact that the use of devices in the developing countries' ICUs was remarkably similar to that reported in US ICUs in the CDC's NHSN, rates of device-associated nosocomial infection were significantly higher in the ICUs of the INICC hospitals; the pooled rate of central line-associated bloodstream infection in the INICC ICUs of 6.8 per 1,000 central line-days was more than 3-fold higher than the 2.0 per 1,000 central line-days reported in comparable US ICUs. The overall rate of ventilator-associated pneumonia also was far higher (15.8 vs 3.3 per 1,000 ventilator-days), as was the rate of catheter-associated urinary tract infection (6.3 vs. 3.3 per 1,000 catheter-days). Notably, the frequencies of resistance of Pseudomonas aeruginosa isolates to imipenem (47.2% vs 23.0%), Klebsiella pneumoniae isolates to ceftazidime (76.3% vs 27.1%), Escherichia coli isolates to ceftazidime (66.7% vs 8.1%), Staphylococcus aureus isolates to methicillin (84.4% vs 56.8%), were also higher in the consortium's ICUs, and the crude unadjusted excess mortalities of device-related infections ranged from 7.3% (for catheter-associated urinary tract infection) to 15.2% (for ventilator-associated pneumonia). Abstract

BACKGROUND. The International Nosocomial Infection Control Consortium (INICC) was established in 15 developing countries to reduce infection rates in resource-limited hospitals by focusing on education and feedback of outcome surveillance (infection rates) and process surveillance (adherence to infection control measures). We report a time-sequence analysis of the effectiveness of this approach in reducing rates of central line-associated bloodstream infection (CLABSI) and associated deaths in 86 intensive care units with a minimum of 6-month INICC membership. METHODS. Pooled CLABSI rates during the first 3 months (baseline) were compared with rates at 6-month intervals during the first 24 months in 53,719 patients (190,905 central line-days). Process surveillance results at baseline were compared with intervention period data. RESULTS. During the first 6 months, CLABSI incidence decreased by 33% (from 14.5 to 9.7 CLABSIs per 1,000 central line-days). Over the first 24 months there was a cumulative reduction from baseline of 54% (from 16.0 to 7.4 CLABSIs per 1,000 central line-days; relative risk, 0.46 [95% confidence interval, 0.33-0.63]; P < .001). The number of deaths in patients with CLABSI decreased by 58%. During the intervention period, hand hygiene adherence improved from 50% to 60% (P < .001); the percentage of intensive care units that used maximal sterile barriers at insertion increased from 45% to 85% (P < .001 ), that adopted chlorhexidine for antisepsis increased from 7% to 27% (P=.018 ), and that sought to remove unneeded catheters increased from 37% to 83% (P=.004); and the duration of central line placement decreased from 4.1 to 3.5 days (P < .001). CONCLUSIONS. Education, performance feedback, and outcome and process surveillance of CLABSI rates significantly improved infection control adherence, reducing the CLABSI incidence by 54% and the number of CLABSI-associated deaths by 58% in INICC hospitals during the first 2 years. © 2010 by The Society for Healthcare Epidemiology of America. Abstract

Introduction: The natural course of frozen shoulder (FS) remains unclear and factors like altered central pain processing and autonomic dysfunction might play a role in the disease course and incomplete recovery in a subset of patients. Psychological factors (e.g. catastrophizing and hypervigilance) might influence pain severity and measurements in patients with chronic pain and an interrelation might be present. Methods: Patients with FS completed six questionnaires and underwent quantitative sensory testing, including tactile sensitivity (allodynia), pressure pain thresholds (hyperalgesia), temporal summation and conditioned pain modulation four times (at baseline and 3, 6 and 9 months after baseline). Results: One hundred and forty-nine patients with frozen shoulder were initially recruited and 88 completed all the follow-up measurements. Pain intensity, shoulder pain and disability, catastrophizing, hypervigilance, allodynia and hyperalgesia improved over time. Fair correlation was found between pain intensity and catastrophizing and catastrophizing and hypervigilance, while poor correlation was found between pain intensity and hypervigilance, allodynia and hyperalgesia, between catastrophizing and hyperalgesia and autonomic symptoms and between hypervigilance and hyperalgesia. Discussion: Patients with FS improve in several pain variables and psychological pain related variables over the course of 9 months. This improvement might be related to each other in a small extent, due to the presence of correlations between some of these variables. Process evaluation: Data analysis is in process. Abstract

Purpose: To evaluate the impact of country socioeconomic status and hospital type on device-associated healthcare-associated infections (DA-HAIs) in neonatal intensive care units (NICUs). Methods: Data were collected on DA-HAIs from September 2003 to February 2010 on 13,251 patients in 30 NICUs in 15 countries. DA-HAIs were defined using criteria formulated by the Centers for Disease Control and Prevention. Country socioeconomic status was defined using World Bank criteria. Results: Central-line-associated bloodstream infection (CLA-BSI) rates in NICU patients were significantly lower in private than academic hospitals (10.8 vs. 14.3 CLA-BSI per 1,000 catheter-days; p < 0.03), but not different in public and academic hospitals (14.6 vs. 14.3 CLA-BSI per 1,000 catheter-days; p = 0.86). NICU patient CLA-BSI rates were significantly higher in low-income countries than in lower-middle-income countries or upper-middle-income countries [37.0 vs. 11.9 ( p < 0.02) vs. 17.6 ( p < 0.05) CLA-BSIs per 1,000 catheter-days, respectively]. Ventilator-associated-pneumonia (VAP) rates in NICU patients were significantly higher in academic hospitals than in private or public hospitals [13.2 vs. 2.4 ( p < 0.001) vs. 4.9 ( p < 0.001) VAPs per 1,000 ventilator days, respectively]. Lower-middle-income countries had significantly higher VAP rates than low-income countries (11.8 vs. 3.8 per 1,000 ventilator-days; p < 0.001), but VAP rates were not different in low-income countries and upper-middle-income countries (3.8 vs. 6.7 per 1,000 ventilator-days; p = 0.57). When examined by hospital type, overall crude mortality for NICU patients without DA-HAIs was significantly higher in academic and public hospitals than in private hospitals (5.8 vs. 12.5%; p < 0.001). In contrast, NICU patient mortality among those with DA-HAIs was not different regardless of hospital type or country socioeconomic level. Abstract

We report the results of an International Nosocomial Infection Control Consortium (INICC) surveillance study from 2002 through 2007 in 98 intensive care units (ICUs) in Latin America, Asia, Africa, and Europe. During the 6-year study, using Centers for Disease Control and Prevention (CDC) National Nosocomial Infections Surveillance System (NNIS) definitions for device-associated health care-associated infection, we collected prospective data from 43,114 patients hospitalized in the Consortium's hospital ICUs for an aggregate of 272,279 days. Although device utilization in the INICC ICUs was remarkably similar to that reported from US ICUs in the CDC's National Healthcare Safety Network, rates of device-associated nosocomial infection were markedly higher in the ICUs of the INICC hospitals: the pooled rate of central line-associated bloodstream infections (CLABs) in the INICC ICUs, 9.2 per 1000 CL-days, is nearly 3-fold higher than the 2.4-5.3 per 1000 CL-days reported from comparable US ICUs, and the overall rate of ventilator-associated pneumonia was also far higher, 19.5 vs 1.1-3.6 per 1000 ventilator-days, as was the rate of catheter-associated urinary tract infection, 6.5 versus 3.4-5.2 per 1000 catheter-days. Most strikingly, the frequencies of resistance of Staphylococcus aureus isolates to methicillin (MRSA) (80.8% vs 48.1%), Enterobacter species to ceftriaxone (50.8% vs 17.8%), and Pseudomonas aeruginosa to fluoroquinolones (52.4% vs 29.1%) were also far higher in the Consortium's ICUs, and the crude unadjusted excess mortalities of device-related infections ranged from 14.3% (CLABs) to 27.5% (ventilator-associated pneumonia). Abstract

From July to December 2001 data were obtained from egg collections and releasing of hatched turtles at 21 conservation centers of Michoac´n State, México. The objective was to determine the hatching efficiency of collected eggs and the release of small turtles. The extracted eggs from their original nests were transported to protection centers and the recently hatched but dynamic turtles were liberated. Results show 700 thousand released turtles that comprised 78% of survival rate from the total eggs collected.
