Cystic dystrophy of the duodenal wall (CDDW) is an uncommon pathological condition that may be a cause of symptomatic duodenal mass in some patients. CDDW and closely related conditions that involve the duodenal wall and the pancreatoduodenal groove have gone by a variety names in the literature. Though their pathophysiology is not fully understood, they are thought to be due to the presence of heterotopic pancreatic tissue that undergoes a degenerative process. A multimodal imaging approach paired with biopsy is often employed to rule out malignancy in these patients. This case report provides an account of the radiological and pathological findings that helped lead to the diagnosis of CDDW. A review of relevant literature and a discussion of the presented case are provided.

Benign and malignant tumors arise in the ampulla of Vater and duodenal papilla. Depending on the origin of a tumor, a tumor presents as intraampullary mass or duodenal mass. When a tumor arises in the ampulla of Vater, the mass is within the ampulla, but when a tumor arises at the duodenal papilla, the tumor is in the duodenum.

Brunner’s gland hyperplasia (BGH) is an unusual entity that presents with large duodenal polyp or mass and rarely causes gastrointestinal bleeding. It is usually asymptomatic and often an incidental finding during the esophagoduodenoscopy (EGD). However, most of the cases are benign. We encounter a 40 years old man who presented in Emergency Room with melena. EGD revealed solitary, large duodenal polyp in second part of duodenum and later histopathological findings were consistent with BGH.

Brunner’s gland hyperplasia is a rare cause of duodenal mass and gastrointestinal hemorrhage. Imaging and esophagoduodenoscopic evaluation of this condition are frequently consistent with a duodenal malignancy often resulting in surgical resection. However, the malignant potential of these lesions is still unknown, and most are benign. We report the case of a 74-year-old man who presented with fatal gastrointestinal bleeding and esophagoduodenoscopy findings consistent with a duodenal mass and mucosal ulceration. At autopsy, histologic examination of the mass revealed Brunner’s gland hyperplasia with associated ulcer formation. In this report, we review the findings associated with this case as well the literature regarding presentation, clinical associations, and treatment of Brunner’s gland hyperplasia.

Primary small bowel tumors are uncommon and usually carry a poor prognosis. Adenocarcinoma is the predominant histological type while lymphoma is far less common. Small bowel diffuse large B-cell lymphoma (DLBCL) can mimic adenocarcinoma; thus, the distinction can be challenging before tissue examination is performed. Bowel obstruction, as well as peritoneal involvement, are often seen in gastrointestinal (GI) adenocarcinoma cases; however, these features are extremely uncommon with GI lymphomas. Herein, the authors report an unusual case of an obstructing duodenal mass with peritoneal involvement, which was highly suspicious for an advanced duodenal adenocarcinoma. Surprisingly, sampling of the tumor revealed a diffuse large B-cell lymphoma.

We report a case of a 13-year-old girl who presented with a 2-month history of intermittent abdominal pain. Laboratory examination showed hepatitis and pancreatitis. Because of persistent vomiting, computed tomography (CT) was performed, which revealed a circumferential soft tissue density in the duodenal wall, causing partial obstruction. Supportive therapy failed. Repeat CT showed no significant change from the initial study. The patient underwent upper endoscopy, which revealed a mass in the second portion of the duodenum, which occluded most parts of the lumen. The histopathological finding was consistent with an anaplastic large cell lymphoma, a rare form of small bowel neoplasm. After the third course of chemotherapy, complete resolution of the mass was noted, and her symptoms were relieved.

Intestinal malrotation is an anomaly of fetal intestinal rotation that can present with symptoms after birth or in early childhood, but is rarely diagnosed in adults. Patients who have symptomatic presentations require surgery. Other entities may mimic intestinal malrotation and respond to non-surgical management. We present 2 adult cases with the radiological diagnosis of intestinal malrotation: one with true malrotation presenting as a duodenal mass, and another with “pseudo-malrotation” due to altered anatomy. These cases illustrate the importance of recognizing and differentiating these rare adult presentations of true malrotation from “pseudo-malrotation” in regards to their acute management.

An 82-year-old man was referred for endoscopic ultrasound of an ulcerated subepithelial mass in the duodenal sweep. The mass was initially identified during upper endoscopy for coffee-ground emesis. During endoscopic ultrasound, a 21-mm hypoechoic ulcerated subepithelial mass with a duct-like structure was identified. During suction to appose the lesion against the tip of the echoendoscope, the ulceration opened into a fistulous tract with drainage of bile and stones. Subsequent abdominal imaging demonstrated that the mass-like duodenal lesion abutted the gallbladder, which had an air-fluid level. We report a cholecystoenteric fistula masquerading as a subepithelial duodenal mass.

We are presenting a case report of primary retroperitoneal seminoma, a 66 year old male patients presented with gastrointestinal bleeding, endoscopic visualization of duodenum suggested duodenal mass which was biopsied percutaneously and the pathology report proved it to be retroperitoneal seminoma, further investigations with testicular ultrasound and CT chest did not find other primary for seminoma which confirmed the retroperitoneal mass to be primary retroperitoneal seminoma.

Upper gastrointestinal bleeding is a frequent syndrome in the elderly that requires urgent attention. The main causes of melenas are peptic ulcer (gastric or duodenal) and esophageal diseases (esophagitis, esophageal varices and Mallory-Weiss syndrome). Unusually, upper gastrointestinal bleeding may be due to a duodenal tumor. Gastrointestinal stromal tumors (GIST) are included in this group. We report the case of an 81-year-old woman who presented with melenas. Gastrointestinal endoscopic studies (upper and lower) revealed no abnormalities, and a duodenal mass was found on thoracic-abdominal computed tomography scan. Urgent surgery due to a massive bleeding episode led to diagnosis of a duodenal GIST.

Posttransplant lymphoproliferative disorder (PTLD) comprises a spectrum ranging from polyclonal hyperplasia to aggressive monoclonal lymphomas. The majority of PTLDs are of B-cell origin while T-cell PTLDs and Hodgkin lymphoma-like PTLDs are uncommon. Here, we report a unique case of a 56-year-old man in whom a lymphoma with two distinct components developed as a duodenal mass seventeen years following a combined kidney-pancreas transplant. This PTLD, which has features not previously reported in the literature, consisted of one component of CD20 positive and EBV negative monomorphic diffuse large B-cell lymphoma. The other component showed anaplastic morphology, expressed some but not all T-cell markers, failed to express most B-cell markers except for PAX5, and was diffusely EBV positive. Possible etiologies for this peculiar constellation of findings are discussed and the literature reviewed for “composite-like” lymphomas late in the posttransplant setting.

A 34-year-old woman presented with epigastric pain, nausea, and dyspepsia. Contrast-enhanced computerized tomography revealed a small mass in the duodenal wall mimicking a periampullary neoplasm and, at endoscopic examination, a periampullary submucosal tumor was suspected. The diagnosis of intramural duodenal diverticulum (IDD) was made by an x-ray barium meal that showed a finger-like sac filled with barium, the so-called "windsock sign." IDD is a rare congenital abnormality caused by an anomalous process of recanalization of the primitive foregut. The intermittent filling and emptying of the IDD is responsible for epigastric pain, nausea, and vomiting. When IDD is symptomatic, surgical or endoscopic treatment is recommended.

Lipomas of the gastrointestinal tract are a rare condition, occurring during the fifth to seventh decades of life, found mainly in women. They are small and asymptomatic lesions and are usually incidentally detected. We evaluated a case of 42-year-old man presenting with an episode of melena and found to have a large duodenal mass on endoscopy, requiring surgical excision. Histopathological diagnosis revealed submucosal lipoma.
